Tissue reaction to Histoacryl Blue adhesive and histological investigation of its disappearance in various organs.

The effect of the adhesive Histoacryl Blue on the abdominal organs (liver, stomach, intestines, colon, kidneys, bladder, aorta inferior vena cava and skin wounds) of 15 dogs was investigated histologically. The adhesive was applied in thin or thick layers in various ways: applied on intact or injured surfaces, or introduced into the parenchymal organs. The animals were killed 1, 2, 4, 8 weeks and 3 and 6 months after application of the adhesive and the organs containing Histoacryl Blue were processed histologically. The success of adhesion depended, irrespective of the organ, upon the technique of application and on the quantity of the adhesive used.

[Experimental surgery of the choledochus].

The authors carried out choledochal grafts in dogs with autologous parietal peritoneum and adjoining fascia of the musculus rectus abdominis. The patch was fixed with the tissue adhesive hystoacryl without suture. Additionally there was used an interior splinting with a polyethylene tube coated with paraffin. The inserted tube rubed off itself in case of a proper size. Adhesive must not get into the choledochus. 15 of the 20 experiments in dogs were successful.

Comparison of suture materials in the pancreas.

Comparative tests were performed in animal experiments with traditional and new suture materials placed into the pancreas. Dexon and polyester caused the least tissue reaction and their reaction indices were the most favourable. These threads therefore appear the most suitable for making sutures in the pancreas.

[Protective effect on the liver through premedication with radiologically detoxicated endotoxin (Tolerin) in acute myocardial infarction in dogs].

An acute myocardial infarction was produced in experiments in dogs by ligature of the ramus anterior of the left coronary artery. Cell damages in the liver showed electron microscopically 90 and 120 minutes after the begin of the experiment. Destruction or disturbance in the permeability of the liver cells was not or slight developed by pretreatment with radiologically detoxicated endotoxin. In this case no aggravation of the liver function was evident parallel to the morphological changes and the ability of the liver to detoxication of endotoxin was reserved. The experiment shows that the protective effect of Tolerin which prevented the ischemic liver cell damage is not the endotoxin tolerance. The stabilizing effect to the lysosomal membrane by Tolerin is the cause likely.

[Pressure conditions in the pancreatic duct in acute experimental pancreatitis].

The pressure conditions in the pancreatic duct of healthy dogs and of animals with experimentally induced acute serous or necrotic pancreatitis were studied with the Hess-type manometer successfully applied in biliary surgery. In acute pancreatitis hypertension was observed which was more marked in the group with necrotic pancreatitis.

Comparison of suture materials for intestinal anastomosis.

Ninety end-to-end entero-anastomoses were prepared in 45 dogs by inverted single row knotted sutures. Half of the anastomoses were carried out on the small intestine, the other half on the colon. Of the two non-absorbable materials that of the polyester type proved to be the best. Of the absorbable materials Dexon can be recommended for intestinal anastomoses, but on the colon Dexon is not reliable. Plain catgut is not recommended for this kind of anastomoses.

Preservation of pancreas for transplantation.

For preservation of the pancreas, extracellular and intracellular solutions with aprotinin light microscopy showed both solutions to be suitable for a 3-hour preservation, though the intracellular solution appeared more suitable. Preservations for 6 and 24 hours have also been attempted, but these specimens showed fairly severe histological lesions.

Experimental heterotopic pancreas segment transplantation.

Autologous and allogenic transplantations of the caudal part of the pancreas into the omentum and the spleen were performed in 18 dogs. The fate of the transplants was followed histologically. In the grafts the Langerhans-islands preserved their integrity longer than in the other parts of the organ, but in 4 to 6 weeks they, too, necrotised.

[Experimental pancreas transplantation in swine].

After surveying the problem of pancreas transplantation the results of pancreatico-duodenal transplantation attempts performed on dogs and 20 pigs are reported. Surgically, the major pancreatic duct is anastomosed to a Roux' loop using a preserved duodenal patch around the papilla. The graft is sited unto the retroperitoneal space. The animals survived for 6 to 9 months. In some cases an increasing fibrosis of the transplantate was observed. Most often, the animals died from pneumonia. The authors want to continue their research in order to obtain better results.

Experimental anastomosis of small vessels with plastic tubing.

Forty experimental anastomoses have been prepared with the help of polyethylene tubes on the hearts of 10 human corpes, 2 pigs and 16 dogs. In dogs the left internal mammary artery was anastomized with one of the coronary branches of the moving heart without extracorporal circulation. The plastic tube was moved in the vessel by means of a thread attached to the end of the tube and the tube was retrieved by the same thread before the closure of one of the suture lines. The method is illustrated on schemiatic diagrams and its advantages are pointed out.

Experimental transureteroureterostomy performed with tissue adhesive.

Transureteroureterostomy was performed on 20 experimental animals by an adhesive technique with no sutures. Sixteen cases displayed very good results, two were only partly successful, and two others proved to be failures. The longest follow-up was 3 years. Results obtained are documented by excretory urography and pathologic examinations.

Use of absorbable material for the closure of experimental longitudinal nephrotomy.

Large nephrotomy incisions made in 15 dogs were closed by sewing up the cavity system with stitches of 5/0 catgut and by sealing the renal parenchyma with tissue adhesive being pinpointed at the opened vessels. The incision was covered with a preparation of oxidized cellulose which was then likewise impregnated with tissue adhesive. This technique was found rapid and convenient and provided for adequate haemostasis. Its essential advantage lies in eliminating the deep through-and-through sutures involving a major loss of functioning renal parenchyma. All substances used were absorbed in due time.

Effect of absorbable cellulose on the renal parenchyma.

Both kidneys of 12 dogs were injured and the wounds were covered with Surgicel which was fixed by means of adhesive. The results were checked by visual inspection and histological tests. The method seems to be suitable for use in appropriate human cases.

[Management of experimental heart injuries].

Treatment of 28 stabbed heart wounds in 15 dogs was done with traditional suturing or by means of an adhesive and Surgicel. The combination of Surgicel with adhesive ensured adequate closure of the wound and healing. The method is recommended for the treatment of human heart injuries in certain cases.
