Prognosis and outcome of elderly hypertensives in a Japanese community: results from a long-term prospective study.

A long-term prognosis and outcome study of elderly hypertensives (aged 60 years or over) was made based on the 20-year prospective population survey conducted in a Japanese rural community (Hisayama) and the results were compared with those for younger subjects (aged 40-59 years). The risk of cardiovascular mortality related to blood pressure level increased with the elevation of either systolic or diastolic pressure in both younger and elderly groups. Cardiovascular mortality increased markedly at a systolic pressure of greater than or equal to 160 mmHg, or a diastolic pressure of greater than or equal to 100 mmHg for those aged 40-59 years. There was no cut-off level for increased risk of cardiovascular mortality for either systolic or diastolic pressures for those aged 60 years or over. Stroke mortality was seven times higher in systolic, and 10 times higher in diastolic hypertensives than in normotensives (P less than 0.01) for the group aged 40-59 years. However, deaths due to stroke and heart disease were more frequently associated with borderline (relative risk 2.3 and 2.3, respectively; P less than 0.01), systolic (relative risk 3.2 and 3.7, respectively; P less than 0.01) and diastolic hypertension (relative risk 2.1 and 4.8, respectively; P less than 0.01), compared to normotension for those aged 60 years or over. Intracerebral hemorrhage and cerebral infarction occurred more frequently in diastolic hypertensives for both young (relative risk 9.8 and 4.8, respectively; P less than 0.01) and elderly subjects (relative risk 3.4 and 1.5, respectively; P less than 0.01) than in normotensives.(ABSTRACT TRUNCATED AT 250 WORDS)

Intracerebral hemorrhage in a Japanese community, Hisayama: incidence, changing pattern during long-term follow-up, and related factors.

The incidence of intracerebral hemorrhage over 13 years is compared between two Hisayama cohorts. Among men aged 40 years or older, the annual incidence declined significantly from 3.1/1,000 in the early cohort (1961-1970) to 1.2/1,000 in the recent cohort (1974-1983). Massive ganglionic hemorrhage decreased, while small or medium-sized intracerebral hemorrhage increased in the recent cohort on pathologic or computed tomographic examination. These trends could be due to the reduced prevalence of hypertension in the Hisayama population. The association of serum total cholesterol with intracerebral hemorrhage is discussed based on the results during a 22-year follow-up period.

Purification and properties of dihydrogeodin oxidase from Aspergillus terreus.

The last step of (+)-geodin biosynthesis is a phenol oxidative coupling, which is one of the most important reactions in biosynthesis of natural products. The enzyme named dihydrogeodin oxidase catalyzes the regio- and stereospecific phenol oxidative coupling reaction to form (+)-geodin from dihydrogeodin. The enzyme was purified from the cell-free extract of Aspergillus terreus, a (+)-geodin producer, by ammonium sulfate fractionation, acid treatment, and column chromatographies on DEAE-cellulose, Hydroxyapatite, chromatofocusing, and Toyopearl HW-55S. The purified enzyme was homogeneous as judged by sodium dodecyl sulfate polyacrylamide gel electrophoresis. The molecular weight of the enzyme was estimated to be 153,000 by gel filtration on a Toyopearl HW-55S column and 76,000 by SDS-polyacrylamide gel electrophoresis, indicating that the enzyme is a dimer. The purified enzyme showed an intense blue color and had absorption maxima at 280 and 600 nm, which suggested it to be a blue copper protein. The copper content was found to be 8 atoms per subunit by atomic absorption analysis and no significant amount of other metals was detected by ICP emission spectrometry. The electron paramagnetic resonance spectrum showed the presence of type 1 and type 2 copper atoms in the enzyme molecule. Sodium azide and ethylxanthate inhibited the enzyme activity, but potassium cyanide and diethyldithiocarbamate, both known as potent copper enzyme inhibitors, were not inhibitory.

Severe disability related to cerebral stroke: incidence and risk factors observed in a Japanese community, Hisayama.

To elucidate the incidence of severe disability due to cerebral stroke and its related factors, prospective data of 1,621 Hisayama residents aged 40 and over were examined. Severe disability resulting from stroke was defined as patients who were unable to dress, take care of their toilet needs, and feed themselves without assistance, or who required a wheel chair for ambulation three months after the most recent episode. During 20 years of follow-up 255 stroke patients were observed among the sample population. The annual incidence of stroke per thousand was 9.8, and rate of severe disability was 2.8 for men and 6.4 and 2.0 for women, respectively. Of the 74 cases with severe disability, approximately 92% were attributed to cerebral infarction. Related factors to severe disability due to cerebral infarction were recurrent attacks, hypertension, changes in ocular fundi and diabetes mellitus among predispositions and quadriplegia or muscular contraction, and intelligent or mental disorders among inhibiting factors for functional recovery. Furthermore, in 59 autopsy cases with multiple cerebral infarctions, the frequency of disability increased as the number of infarcts increased. Hypertension and diabetes mellitus, as risk factors for cerebral infarction and factors inhibiting post-ictal functional recovery were discussed.

Biosynthesis of riboflavin. Enzymatic formation of 6,7-dimethyl-8-ribityllumazine from pentose phosphates.

The xylene ring of riboflavin originates by dismutation of the precursor, 6,7-dimethyl-8-ribityllumazine. The formation of the latter compound requires a 4-carbon unit as the precursor of carbon atoms 6 alpha, 6, 7, and 7 alpha of the pyrazine ring. The formation of riboflavin from GTP and ribose phosphate by cell extract from Candida guilliermondii has been observed by Logvinenko et al. (Logvinenko, E. M., Shavlovsky, G. M., Zakal'sky, A. E., and Zakhodylo, I. V. (1982) Biokhimiya 47, 931-936). We have studied this enzyme reaction in closer detail using carbohydrate phosphates as substrates and synthetic 5-amino-6-ribitylamino-2,4-(1H,3H)-pyrimidinedione or its 5'-phosphate as cosubstrates. Several pentose phosphates and pentulose phosphates can serve as substrate for the formation of riboflavin with similar efficiency. The reaction requires Mg2+. Various samples of ribulose phosphate labeled with 14C or 13C have been prepared and used as enzyme substrates. Radioactivity was efficiently incorporated into riboflavin from [1-14C]ribulose phosphate, [3,5-14C]ribulose phosphate, and [5-14C]ribulose phosphate, but not from [4-14C]ribulose phosphate. Label from [1-13C]ribose 5-phosphate was incorporated into C6 and C8 alpha of riboflavin. [2,3,5-13C]Ribose 5-phosphate yielded riboflavin containing two contiguously labeled segments of three carbon atoms, namely 5a, 9a, 9 and 8, 7, 7 alpha. 5-Amino-6-[1'-14C] ribitylamino-2,4 (1H,3H)-pyrimidinedione transferred radioactivity exclusively to the ribityl side chain of riboflavin in the enzymatic reaction. It follows that the 4-carbon unit used for the biosynthesis of 6,7-dimethyl-8-ribityllumazine consists of the pentose carbon atoms 1, 2, 3, and 5 in agreement with earlier in vivo studies.

Strategy to manage pump failure due to chronic pulmonary diseases--pathophysiology and treatment of right ventricular overload.

The pathophysiology and treatment of pump failure with right ventricular overload due to chronic pulmonary diseases were discussed. When right ventricular overload occurs hypertrophy and dilatation of right ventricle follows and the heart as a whole, including left ventricle, is altered in morphology and function. Therapeutic measures for right ventricular overload are the key to management and treatment of pump failure. Pulmonary hypertension, as an etiological factor, is discussed and is divided into two categories, that is, mild to moderate pulmonary hypertension with hypoxia due to chronic obstructive lung disease, and severe pulmonary hypertension due to pulmonary vascular disease. In each category, effects of oxygen inhalation and vaso-dilating agents were evaluated. Oxygen did not decrease pulmonary vascular resistance in the state of chronic hypoxia, though a vasodilating agent was effective. In pulmonary vascular disease, vaso-dilating agents were effective to decrease pulmonary vascular resistance and pulmonary artery pressure, though the effect was less than 30% down in resistance.

Values of electrocardiography and two-dimensional echocardiography to identify myocardial infarction due to left circumflex and right coronary artery disease.

To investigate the value of the 12-lead ECG and two-dimensional echocardiography (2DE) in the distinction of left circumflex (LCX) from right coronary artery (RCA) disease, we analyzed the location of Q waves, infarct lesions, and coronary artery narrowings in 26 patients with angiographically documented single-vessel disease. Q waves in leads II, III, and aVF were associated with the posterior wall (PW) lesions at the papillary muscle level. Extensive lesions from the PW to the posterior septum (PS) identified RCA disease, while extension to the lateral wall (LW) identified LCX disease. Eleven of 12 patients with high posterior infarction (tall R wave in V1) were found to have extensive LW lesions and 10 of these had coronary narrowings in or proximal to the obtuse marginal branch of LCX. All 6 patients with high posterior infarction and high lateral infarction (Q in I or aVL) had infarct lesions extending from the LW to the anterior wall (AW) and were associated with LCX disease with a large obtuse marginal branch. Of 10 patients with Q waves in V6, the apical LW and PW were involved in 7 and either segment in 3. Nine of these 10 patients had LCX disease. It is concluded that the location of Q waves in inferior infarction could aid in recognizing infarct extension and underlying coronary artery disease.

The induction of intestinal metaplasia in rats by pyloroplasty or pyloroplasty plus vagotomy.

The influence of increased gastric pH on induction of intestinal metaplasia in the stomach of male JCL/SD rats was examined following surgical procedures of pyloroplasty or pyloroplasty and vagotomy. Twelve months after pyloroplasty plus vagotomy serum gastric concentrations were significantly higher (group II) than in rats receiving only pyloroplasty (group I) or in sham operated animals (group III). Additionally, trehalase activity was also higher in group II than in groups I and III. The incidence of intestinal metaplasia was significantly higher in groups I and II compared with the sham operated control animals (group III). This study indicates that intestinal metaplasia could be induced by surgical procedures such as pyloroplasty with or without vagotomy. Furthermore, elevation of the pH on gastric mucosa by such procedures may play a significant role in the subsequent development of metaplasia in the stomach.

Blood pressure tracking in Japanese adolescents. Five-year follow-up in Hisayama, Japan.

Blood pressures (BPs) were measured with standardized sphygmomanometers in 434 Japanese boys and girls living in the town of Hisayama. Simultaneously, data on pulse rate, weight and height were obtained. Out of the original 434 subjects, data were obtained repeatedly for 5 years in 280 subjects. BP levels were significantly correlated with weight in those aged 14-15 and also 19-20 years, but correlation coefficients were small. During the 5-year period, the mean systolic and diastolic blood pressure (SBP & DBP) increased significantly in both sexes, but the increments were greater in boys. Both SBP and DBP at 14-15 years of age were significantly correlated with data taken 5 years later for both sexes, and subjects with a higher initial BP (more than 90th percentile of the distribution) tended to have a higher BP after 5 years. SBPs after 5 years were independently correlated with initial SBP levels and changes in QI (D-QI) in both sexes. On the other hand, DBPs after 5 years were independently correlated with initial DBP levels and height for boys, and initial DBP levels and D-QI for girls.

[Abnormal left ventricular endocardial structures detected by two-dimensional echocardiography in systemic lupus erythematosus].

This is a report of abnormal endocardial structures on the left ventricular wall as visualized by two-dimensional echocardiography (2-D echo) in patients with systemic lupus erythematosus. Abnormal endocardial structures, arising from the left ventricular endocardium and appearing to proliferate into the cavity, were found in 11 (20%) of 54 patients. The most frequently involved site was posterobasal left ventricular wall in the parasternal long-axis view, and the posterolateral segment of the left ventricle including the anterior and posterior papillary muscles in the parasternal short-axis view. M-mode echocardiograms, simultaneously recorded with 2-D echo, revealed abnormal structures of increased intensity adjacent to the endocardium with thicknesses of 4 approximately 7 mm. Most of the abnormal endocardial structures were observed in patients in the active phase by 2-D echo, and they appeared to regress with steroid therapy. In one autopsy case, fibrous endocardial thickening suggesting healed endocarditis was present in the anterior papillary muscle as observed by 2-D echo. Their location and response to steroid therapy suggests that these abnormal echoes could represent Libman-Sacks endocarditis.

Natural history of borderline hypertension in the Hisayama community, Japan--I. The relative prognostic importance of transient variability in blood pressure.

Long-term prognosis of borderline hypertensives was studied in a prospective population survey carried on since 1961 in the town of Hisayama, Japan. Five consecutive BP recordings on 1621 subjects aged 40 and over were obtained at entry, and the variability in BP between the first and fifth readings was taken into account when classifying the subject into categorical groups. Even with an estimated variability in BP in several measurements on one occasion, a large fluctuation in BP was observed in both the borderline hypertensives and the normotensives. Borderline hypertensives with a transient elevation in BP more frequently died from cardiovascular disease than did those without BP elevation, as estimated by the long-term cumulative mortality. However, there was no difference in the frequency of hypertension-related organ damage between these two groups at entry.
