[Treatment of herpes simplex keratitis with Vidarabin ointment (author's transl)].

In 20 cases of herpes simplex keratitis the efficacy of Vidarabin ointment has been tested, in 19 cases after corneal abrasion. The eyes were treated once a day and padded until the epithelial defects had closed. Thereafter the ointment was applied 4 times per day for about one week more. On the average epithelial closure had been achieved after 2.4 days, complete normalization of the epithelium after a total of 3.6 days. This therapy results in a considerable shortening of the healing process as compared to the topical use of anti-viral medication alone. From this aspect Vidarabin ointment has proved to be a valuable adjuvant.

[Erosive-ulcerative type of herpes simplex of the lids (author's transl)].

In 14 patients the diagnosis of an erosive-ulcerative type of herpes simplex of the lids was made. It is characterized by erosions of the intermarginal portion of the lid or by erosions or ulcers of the skin, especially close to the lid margin. A combination of both clinical forms is possible. Characteristic herpetic vesicles have been observed only in one case, being situated in close proximity to the erosive lesion of the lid margin. The visualisation of erosions located at the intermarginal portion of the lid can be facilitated by staining with fluorescein. The lack of vesicular eruptions impedes a proper diagnosis; hence one should search for the history of recurrent herpetic infections. A presumptive diagnosis can be confirmed by the immunofluorescence technique. For treatment, topical application of virustatic and antibiotic ointment is recommended.

[Growth hormone and diabetic retinopathy (author's transl)].

Changes in the serum level of growth hormone have been studied in 28 diabetics (6 exhibiting no diabetic retinopathy (d. r.), 10 displaying a non proliferative d. r., 6 having proliferative d. r., and 6 Brittle-diabetes) together with a control group consisting of 11 subjects (5 being totally healthy, and 6 having acromegaly without d. r.). Blood samples were obtained before and after arginine and glucagon administration and also following controlled exercise on the ergometer with 50% of the actual working capacity during 20 minutes. The presence of d. r. and its stage has been determined by fluorescein angiography. Our results demonstrate that an increased secretion of growth hormone in diabetics actually constitutes a manifestation of metabolic instability. Apparantly no correlation exists between the appearance of d. r. or its stage and the degree of growth hormone release.

[Glaucoma surgery with insertion of silicone catheters. A study based on animal experiments (author's transl)].

10 rabbits have been submitted to thermosklerostomy and insertion of a silicone catheter. The tube exhibiting an inner diameter of 0.5 mm and an outer diameter of 0.94 mm, respectively the total length measuring 5 mm, and projecting about 1.5 mm in the anterior chamber. Fixation of the tube has been accomplished at its distal end to the underlying sclera by 2 X 10-0-nylon sutures, and of the proximal end with two more sutures to the corneal lip of the wound taking a bite through total corneal thickness. Within the observation period of 2-8 months no migration of the catheter has been detected; in addition no erosion of the conjunctiva, and no inflammatory reaction or occlusion of the catheter lumen occurred. In 5 cases the intraocular pressure has been measured between the 2nd and 4th postoperative month; in all cases a reduction of the intraocular tension compared to the non operated fellow eye has been found, hypotonic values were not encountered.

Echography and histological studies in various eye conditions.

A correlation between A-scan echograms recorded clinically in vivo and histological studies of corresponding sections has been made in 42 eyes with intraocular pathological conditions. This study yielded both new and little known acoustic differential criteria for the diagnosis of retinoschisis, funnel-shaped vitreous detachments, serous as well as hemorrhagic retinal and choroidal detachments, intravitreal hemorrhages, and intraocular tumors.

[Herpes simplex of the lid margin (author's transl)].

Isolated herpes simplex affecting the lid margin can be distinguished from herpes simplex involving the entire eyelid by characteristic features. The typical intermarginal form of herpes-simplex-blepharitis presents itself as an erosion of about 3 mm in length which may be well discerned in detail after fluorescein staining. The skin adjecent to the lesion may also exhibit superficial ulceration, possibly being covered by crusts. The affected portion of the lid shows localized and moderate swelling as well as pain upon pressure. Injection of the bulbar conjunctiva will characteristically be noted mainly in the area opposing the lid lesion. The regional lymph nodes are enlarged, and possibly tender upon palpation. Local medication with anti-viral ophthalmic ointment is effective within a few days.

[Ocular involvement due to the juice of Dieffenbachia plants (author's transl)].

Upon breaking the stem or the branches of a Dieffenbachia plant juice is released in an explosive manner and may thus involve the eye. A myriad of fine calcium-oxalate crystals penetrate into all layers of the cornea, and an iritis can develop. The latter ceases within 6 days, the corneal cristals vanish within about three weeks.

[An attempt at surgical management of haemorrhagic glaucoma (author's transl)].

A surgical method for the treatment of haemorrhagic glaucoma is discussed here. The operation implies Scheie's procedure without peripheral iridectomy, and the insertion of a silicon catheter. Prior to intraocular surgery cyclocryotherapy is employed in order to improve the basic condition. The long term mode of action of this surgical technique may be due to creating a favorable outset for the formation of secondary aqueous veins.

[The behaviour of plastic catheters within the anterior segment of rabbit's eyes (author's transl)].

The location and tolerance of plastic tubes (2 polyvinyl and 7 silicone respectively) within the anterior segment of the eyes of nine rabbits were observed for periods of 3 to 9 months. In 3 animals the tube was held in place with a single (scleral) suture: the catheter was ultimately totally displaced into the anterior chamber. In the remaining 6 cases a second (corneal) suture was applied. As a result only a limited migration of the silicone catheters was observed. The suprascleral as well as the intracameral portions of the tubes were well tolerated. Even the catheters that had migrated completely into the anterior chamber did not cause any inflammatory reaction.

[Pathology of exogenous mycotic endophthalmitis (author's transl)].

The histological examination of five bulbi with exogenous mycotic endophthalmitis reveals invasion of fungi confined to the anterior segment including the anterior vitreous body. Hyphae are found within abscesses, and granulomas respectively. The posterior eye remains relatively free, however a posterior vitreous detachment always exists together with a massive retraction of the vitreous gel. These findings should be kept in mind if intravitreal injections of amphotericin-B, or a vitrectomy are considered. After intravitreal injections of amphotericin-B no retinal lesions could be revealed.

[Treatment of the diabetic retinopathy with doxium-clofibrate (author's transl)].

28 patients with non proliferating exudative diabetic retinopathy were treated with 750 mg Doxium and 1500 mg Clofibrate daily during 8.6 months on an average. Before beginning--in the middle of the period of observation--and after finishing the treatment, a retinal fluorescein angiography was performed. The waxy exudates decreased in 11 patients (39.29%) and increased in 8 patients (28.57%). In 9 patients (32.14%) no change of the amount of the hard exudates was observed. The treatment was almost only in those angiopathies successful, in which the fluorescein angiography showed a preponderance of the hyperpermeability over the obliterating process of retinal capillaries.

[Changes in the ocular muscles in progressive scleroderma (author's transl)].

One patient out of 13 with generalized scleroderma displayed related changes of ocular motility. Histologic examination of the external ocular muscles of one of our patients who had died, revealed a massive increase of the interstitial collagen, and various stages of muscular degeneration. These changes correspond closely with the histopathological findings in the myocardium.

[Congenital grouped pigmentation of the retina (author's transl)].

In this disease entity we usually encounter black, sharply demarcated pigmented patches, occurring in groups and increasing in size toward the periphery. In the majority of cases the anomaly is unilateral (84%). The pigmentation may be found in one single sector (59%), in several separate sectors (27%) or distributed over the entire fundus (11%). Histology reveals pigment epithelial cells with hypertrophy and distinct irregularity in size and shape being arranged in one or two layers. Hyperpigmentation also exists with pigment granules of various size and distribution.

[Treatment of malignant lid tumours by intrapalpebral injection of urea (author's transl)].

36 malignant tumours of the eyelids (basaliomas, basal-cell carcinomas and squamous-cell carcinomas) have been treated by intrapalpebral injections of a 5% to 25% solution of urea as recommended by E.D. Danopoulos. 1.5 to 3.0 ml of the solutions were injected around the tumours twice per week. Six malignant tumours disappeared completely after a treatment of eleven months; their largest initial diameter averaged 0.53 cm. Considering the protracted treatment, the uncertainty of success, without histological examination of the excised scar tissue, and finally because of the low rate of verified success, injections of dissolved urea around malignant tumours of the eye lids cannot replace radical excision.

Atypical cotton-wool spots.

Atypical cotton-wool spots in four selected cases of diabetic retinopathy are discussed. The most striking characteristic distinguishing them from typical cotton-wool spots is their size, which ranges from about 2 to 4 disc diameters. They develop after stenosis or a complete obstruction of a first order arteriole at the point at which it emerges from the parent arterial branch. The size of the lesion corresponds with the size of the affected arteriole. Restoration of local circulation is attempted by neighbouring arterioles and venules, the latter carrying a reversed blood flow. Arterio-venous communications along the border of the infarcted area are a prominent feature, most likely attributable to the necessity of free drainage of the arterial influx. The same haemodynamic principle applies to the coarsening of the adjacent capillary bed. Secondary changes of the bordering venous branches include aneurysm formation and the staining of the venous wall.

[Secotrial diabetic retinopathy in a case of partial atrophy of the optic nerve (author's transl)].

In a 35-year-old man diabetic patient with atrophy of the upper half of the right optic nerve and loss of the lower half of the visual field, no signs of diabetic retinopathy developed in the upper half of the eyeground. The diabetic retinopathy affecting the inferior half of the fundus was strictly confined to the inferior half of the eyeground; in addition the retinopathy took a more benign cours compared with the left eye showing a normal disc. After photocoagulation in the left fundus, the retinopathy improved to a grade comparable to the influence of the partial optic atrophy upon the course of the diabetic retinopathy in the right eye.
