[Transvaginal sclerosing therapy of cystic ovarian lesions with alcohol].

AIM: Description of puncture technique for conservative treatment of cystic ovarian lesions. METHOD: The cyst should be unilocular, sonolucent, with a smooth inner wall of the capsule, without septa and neovascularisation on transvaginal colour Doppler. Serum CA-125 levels must be lower than 35 U/ml. The capsule of the cyst was punctured with a 18 gauge needle, sonographically guided with a 5 MHz transvaginal probe. After complete emptying of the cyst, (cytological examination of the cystic fluid was negative), we injected an amount of sterile 96 % alcohol equalling 50 % of the volume of the cyst. The alcohol remained in the cyst for 5 minutes and was then aspirated completely. RESULTS: 152 patients aged 18 to 60 with an ovarian cyst volume between 40 and 140 ml were treated. Five of the patients (3.3 %) had intensive pelvic pain and therefore received intravenous analgesics. After three months, relapse appeared in 8 patients (5.3 %), in most of whom the volume of the cyst had been more than 100 ml. CONCLUSION: The proposed puncture technique is simple and easily performed. Treatment with 96 % alcohol showed good results. In cases where the cyst volume exceeds 100 ml, we advise surgical treatment by laparoscopy or laparotomy.

Ultrasonic evaluation of benign and malignant nodules in echographically multinodular thyroids.

In 165 patients with ultrasound findings of multinodular thyroids in whom thyroid resection was performed, sonographic features and pathohistologic findings of removed nodules were analyzed. Of 426 nodules removed, 70 were carcinomas and 356 benign. Carcinomas are more often hypoechogenic (p < 0.01) and contain nodular calcifications (p < 0.01), while benign nodules are more often iso-hyperechogenic (p < 0.01), showing intranodular cystic degenerative changes (p < 0.01) and perinodular hypoechogenic rim (p < 0.01). Mean diameter of carcinomatous nodules is lower than in benign nodules (p = 0.022). The relative proportion of malignant nodules is highest in the upper halves of thyroid lobes (p < 0.01). Although certain sonographic signs increase the likelihood of a given lesion being malignant or benign, the lack of absolute specificity in the ultrasound evaluation of thyroid nodules was confirmed.

Renal vascular resistance in diabetic nephropathy: duplex Doppler US evaluation.

PURPOSE: To evaluate duplex Doppler sonography to assess renal vascular resistance (RVR) in diabetic patients. MATERIALS AND METHODS: Resistive indexes (RIs) and pulsatility indexes (PIs) were measured in intrarenal arteries of 144 patients with diabetes mellitus and 61 control subjects. RIs and PIs were compared for control subjects and patients with diabetic nephropathy and correlated with laboratory and clinical findings. RESULTS: In control subjects, the mean RI was 0.595 +/- 0.033 (standard deviation) and mean PI was 1.001 +/- 0.105. Elevated RIs and PIs accompanied progression of nephropathy. Statistically significant (P < .001) correlations were observed between both RI and PI and serum creatinine level, creatinine clearance rate, systolic and diastolic blood pressure measurements, and patient age. CONCLUSION: Doppler indexes reflect increased RVR in diabetic nephropathy and correlate with laboratory and clinical parameters, but RI and PI measurements offer no advantages over these parameters to predict disease progress or in patient care.

Intrarenal duplex Doppler sonographic evaluation of unilateral native kidney obstruction.

RIs were measured in intrarenal arteries in 66 kidneys of 33 examinees without renal impairment and in 42 kidneys of 21 patients with unilateral urinary obstruction. The mean RI in normal kidneys was 0.593 +/- 0.040. Patients with unilateral obstruction had a mean RI of 0.709 +/- 0.039 in obstructed kidneys and a mean RI of 0.591 +/- 0.033 in contralateral nonobstructed kidneys. Statistically significant differences have been noticed in the groups of normal versus obstructed kidneys (P < 0.001) and of obstructed versus contralateral nonobstructed kidneys (P < 0.001). The mean dRI was 0.118 +/- 0.034 in patients with unilateral obstruction, and it was 0.014 +/- 0.012 in examinees without renal impairment (P < 0.001). A comparison of RI values between the right and left kidneys in a patient with unilateral obstruction proved more useful than using a 0.7 RI cutoff value in a Doppler sonographic diagnosis of unilateral obstruction.

Two cases of lethal complications following ultrasound-guided percutaneous fine-needle biopsy of the liver.

Two cases with lethal complications are reported among 1750 ultrasound (US)-guided percutaneous fine-needle liver biopsies performed in our department. The first patient had angiosarcoma of the liver which was not suspected after computed tomography (CT) and US studies had been performed. The other patient had hepatocellular carcinoma in advanced hepatic cirrhosis. Death was due to bleeding in both cases. Pre-procedure laboratory tests did not reveal the existence of major bleeding disorders in either case. Normal liver tissue was interposed in the needle track between the liver capsule and the lesions which were targeted.
