Population morbidity in a community with nitrate contamination of drinking water.

The population morbidity assessed according to ICD-9 was studied in the village of Karadzhalovo, the district of Plovdiv, where concentration of nitrates in drinking water was constantly above the tolerable level. The observation was carried out within a 5-year period (1990-94). The data were compared with those from a control village in the same district. Nonparametrical analysis was used to make a comparative analysis of the disease incidence in both communities for the observed period. Significant statistical differences were found between the morbidity in the experimental and the control villages.

The questions of medical informatics.

Each science is identified with the questions it raises with respect to its object of study. This paper discussed the formulation of the basic questions of medical informatics. From a historical point of view, it first dealt with the problems of medical computing. Thereby, three classical questions arose: How can existing computers and information technologies assist in medical activities? Which components of the mathematical apparatus of informatics can be used for solving medical problems, and how and what activities of a physician are subject to algoritmization? The present time raises a new circle of questions centered around the basic one: How is the information system of the human organism structured and how does it function? This question and others form the basis of a new trend in medical informatics.

Ethnic and religious identity and some age characteristics of alcohol use.

The sample of the study comprises 618 subjects (316 Christian-Bulgarians and 302 Muslim-Turks), selected by means of randomized cluster design, and stratified by gender, age and place of residence (town vs village). The study used a standardized questionnaire specifically developed for it. The study findings suggest that the first contact with alcohol and beginning of its regular use occur at a younger age in Bulgarians than they do in Turks. A definite trend has been found toward earlier first contact and beginning of regular use of alcohol in both ethnic groups, but it is more clearly defined in Turks. As a whole Turkish ethnic identity and identification with Muslim religious values and belief restrict the Turks in their contact with alcohol and are still significant factors in restraining alcohol use in their community.

[Standards for the determination of the physical development of newborn infants in the city of Plovdiv].

Using the method of the percentiles the author derives the referent values for the individual estimation of the newborns in the town of Plovdiv, using the following signs: height, body mass, head and chest circumferences. The referent values are derived according to sex. They can find an application for the practical work of the obstetricians and pediatricians in the town of Plovdiv.

A methodological approach for integral assessment of the immune status.

The proposed approach for assessment of the immune status includes investigation of 21 immune indices of the three effector mechanisms of the immune system--the phagocytic activity, the cellular immunity and immunoregulatory cells and the humoral immunity. Standardized reference values for the corresponding immune indices were obtained using a statistical matrix. These values take into consideration the relationships between the immune reactions, thus allowing integral assessment of immune reactivity. This method permits diagnosis of immunodeficient conditions and control of immunocorrecting therapy. It also enables screening studies to be conducted on the immune status of subjects living in ecologically endangered regions.

[A triple pregnancy following heterologous insemination and ovarian stimulation with a favorable outcome].

The larger usage of inducing ovulation drugs in the last decade brought to more frequent multiple pregnancies. The authors describe triplet pregnancy after hetero-insemination and ovaries stimulation which ended the birth (by Cesarean section) of three alive and healthy babies. The triplet pregnancy is high-risky. More often hypertension anaemia and postpartum haemorrhage are met. The most important complication anyway is the preterm delivery which shows as a result unfavourable perinatal outcome. The question about the effectiveness of the prophylactic cerclage and the prophylactic tocolysis is discussed. The general opinion is that there is a higher rate perinatal death for the second and the third fetus.

[Echographic studies of the internal genitalia in patients with gonadal dysgenesis].

The state of internal genitals of 30 patients with gonadal dysgenesis was examined dynamically by ultrasound. The results were compared with data from pneumogynecography (PGG), laparoscopy and laparotomy. In 96% of the patients ultrasound examination coincided with PFF and 100% with laparoscopy and laparotomy. Echographic examination is safe, adequately informative and could replace other more invasive depicting methods.

[Vaginal echography in extrauterine pregnancy].

Twenty eight women with suspicion for extrauterine pregnancy were examined by a vaginal transducer. Extrauterine pregnancy was diagnosed in 18 of them, which was confirmed at the operation. There were no echographic data for tubal pregnancy in 10 of the women. Their further follow-up rejected the presence of extrauterine pregnancy. Three tubal pregnancies were discovered by routine monitoring of ovarian cycles. The immediate proximity of the vaginal transducer to the examined organs in the pelvis gives a possibility for exact topical diagnosis.

[The information value of the clinical data related to the vital prognosis in severe combined trauma].

Data are reported on a study of the informative value of ten clinical and laboratory parameters, which have a bearing on the vital prognosis of severe combined trauma. The presentation is part of a more comprehensive study, aimed at creating an expert system for prognosticating this type of pathology. Case material from the Clinic of Anesthesiology, Resuscitation and Intensive Treatment at the "Pirogov" Emergency Hospital in Sofia was used (1986-1988). Included in the study were 455 patients with severe combined traumatic injuries, of whom 210 died and 245 survived. The following methods were used for quantitative assessment of the informative value: for determining the descriptive informative value--informative criterion by Mel'nikov and informative criterion by Bykhovskiĭ [correction of Bihovski]; for the distributing informative value--the method of Kulback and modification of this method, as described by Genkin-Gubler. Simultaneous use of all four methods allowed to precise the values being obtained and to perform with maximal exactness the intervals relevant to the prognosis. This furnished the opportunity to distribute the individual parameters in highly informative groups, designated for entrance to the projected expert system.

[An information model of the resuscitation activities in treating severe combined trauma].

An informative model developed by the authors for treatment of severe combined trauma is presented. Central place occupy the intellectual activities of the anesthesiologist-resuscitator, while those of the surgeon are only outlined. The construction of the informative model (presented as algorithm) takes into account the fundamental requirements for treatment of severe combined trauma. Analyzing the nature of the informative processes in the model, the authors admit that prognostication is the factor that occupies central place in the treatment of severe combined trauma. The emphasized relative value of prognosis does not underrate the importance of the other intellectual processes in the model: diagnosis, reaching a decision, choice of approach. It is considered that this informative model might be used as auxiliary means in the teaching work in postgraduate qualification courses. At the same time, it furnishes the opportunity to determine the priority in the automation of definite activities in resuscitology.
