Histological and needle-biopsy studies of juvenile mastopathy.

The cytomorphological and histological aspects of benign breast tumours found in the course of 1000 needle-biopsies in 119 young females (under 20 years old) has been outlined. According to the author epithelial-proliferation which not only appears alarming at this stage but is also on the edge of malignancy is quite frequent at this age. However, based on thorough follow-up examinations he warns of too quick and unfavourable judgement regarding epithelial-proliferation as none of the 119 cases cited have become malignant through the years observed. The found the diagnostic value of needle-biopsy especially useful in younger patients and suggests its more habitual use in everyday practice.

[Clinical symptoms and aspiration biopsy cytology of chronic mastitis (author's transl)].

The clinical aspects of chronic mastitis and the author's results achieved by means of aspiration biopsy cytology are dealt with from the oncological point of view. It is stated that chronic mastitis occurs more rarely in these days than a few years ago. In the course of 1000 breast examinations made by means of aspiration biopsy cytology in 1967--1979, there were 17 cases of chronic, nonspecific mastitis, 1 tuberculous mastitis, 2 plasmocytic mastitis, and 1 case in which the simultaneous occurrence of tuberculosis and carcinoma were diagnosed with the help of this method. In the above diseases the accuracy of aspiration cytology is high. The procedure is fast, accurate, and can be performed on outpatients, too. Its application is recommended in all places where there are adequate conditions.

Fine-needle biopsy of sarcomas of the breast.

From 1967 through 1978, fine-needle biopsy of the breast was performed in a total of 612 cases including 415 benign and 197 malignant mammary lesions. In 16 cases the diagnosis was sarcoma, namely primary tumour in 12 cases (fusocellular, 2; round-cell, 1; chondromyxosarcoma, 1; leiomyosarcoma, 1; melanoblastoma, 1; angiosarcoma, 2; malignant cystosarcoma phylloides, 4), and metastatic sarcomas in 4 cases (round-cell, 1; reticulum-cell, 1; melanoblastoma, 2). The cytomorphology, the histology and the diagnostic problems of mesenchymal malignant neoplasms are discussed in detail, and the practical value in the pre-operative diagnosis of mammary sarcomas by fine-needle biopsy is stressed. Clinical experience has proved that this procedure is harmless, therefore its routine application is recommended.

[The effect of alcohol and alcohol withdrawal therapy on the concentration of carbohydrate products of metabolism and electrolytes (Na, K, Mg, Cl) in the blood and cerebrospinal fluid--changes in the oxidation-reduction in the brain].

Both ethanol and ethanol-disulfiram reaction result in a lactacidosis of CSF. The Disulfiram produces hyperglycorhachia, the ethanol causes a shift of the oxidation-reduction processes in the direction to reduction. The Apomorphine has no side effects. The various treatments of alcoholics do not influence the electrolyte concentrations in the CSF.

Needle-biopsy cytology in the preoperative diagnostics of breast diseases.

The author has performed 1000 breast needle-biopsies during a period of 20 years. Based on his own experiences and on the data of the literature the method is evaluated as a useful, informative and complementary tool in the preoperative diagnosis of breast diseases. The method is reliable, quick, painless and performable on out-patients, it causes no dissipation of the tumour, therefore, it is recommended as a routine diagnostic method.

[Fibroadenoma in an accessory breast. A case of polythelia and fibroadenoma in the left breast region and a perivulvar accessory breast].

The case of a 32-year-old female patient with multiple malformations (hare-lip, polythelia, fibroadenoma in an accessory mammary gland) and independent of these, another fibroadenoma in the breast is reported. The fibroadenoma developing in the accessory breast around the vulva, diagnosed by means of aspiration biopsy cytology, should be payed special attention. This case also confirms the applicability in preoperative diagnostics of aspiration biopsy cytology, a method which has proved to be effective for years.

Adenofibroma↗

[Aspiration biopsy cytology of the diseases of the mammary gland (author's transl)].

A detailed report is given on the results of aspiration cytology used in 612 patients. On the basis of his own experiences he speaks high of cytodiagnostics in the complex of cyto-radio-clinical triplet, where its value may reach 98-100% to the contrary of 60-88% of the other two components. It is rapid, trustful, undangerous, in everyday practice is suggested at every place, where the personal and objective conditions are guaranteed.

[Significance of the exfoliative cytodiagnosis in cases of Paget's disease of the nipple (author's transl)].

8 cases of Paget's disease of the nipple were diagnosed by means of cytological examination: in 6 patients by exfoliative, in 2 patients by aspiration biopsy cytology. The diagnoses were verified histologically. Early exfoliative cytology is recommended for early diagnosis in all cases of Paget's mamillae. This method is quick, safe, easily to be carried into effect and to apply repeatedly; but, if the process is suspected of malignancy, a trial-excision has to be carried out. A well-trained cytologist rarely needs the latter. This simple method will reduce the development of advanced Paget's disease.

[Fine needle puncture of mesenchymal malignant growths of the mammary gland].

From 1967 to 1978 the author made 612 fine needle biopsies of the mamma (415 benignant, 197 malignant cases). Among malignant cases there were 11 primary (2 fusocellular, 1 rotundocellular sarcomas, 1 chondromyxosarcoma, 1 leiomyosarcoma, 1 melanoblastoma, 1 angiosarcoma, 4 malignant cystosarcomas phylloides) and 4 metastatic (1 rotundocellular, 1 reticulocellular, 2 melanoblastomas) sarcomas. The author gives a detailed report on cytomorphology, histology and diagnostical problems of mesenchymal malignomas. He thinks the practical value of fine needle biopsy important in the preoperative diagnostics of sarcomas in the mamma. On the basis of his experiences obtained during years, he considers it undangerous and recommends its daily use in practice.

[(Gynecomastia as precancerous lesion: aspiration biopsy cytology and its histology)].

In 19 of 39 patients suffering from gynecomastia, between 1967 and 1979 aspiration biopsy cytology and surgery was performed by the author. Atypia and precancerous lesions accompanied by an advanced stage of epithelial proliferation were found in two cases, in two other ones, in addition to the above mentioned lesions, breast cancer was discovered. The myoepithelial elements observed cytologically and histologically are important pathognomonic signs in theses patients. In spite of the few cases, the precancerous nature of gynecomastia, primarily in its proliferative forms, manifests itself.

[Effect of bilateral and unilateral electroconvulsive therapy (ECT) on the composition of the cerebrospinal fluid (CSF). A possibility to calculate the intracellular redox changes of the brain in humans (author's transl)].

The changes in the composition of CSF and venous blood under the effect of bilateral and unilateral ECT were studied. The intracellular redox changes during the therapy were calculated on the basis of experimental data. Of the patients, 27 were treated with bilateral, another 27 with unilateral ECT. Just before the electroshock, the patients were paralyzed with intravenous succinylcholine chloride and were artificially ventilated with O2. The characteristic change we found was the augmentation of pyruvate concentration in CSF. This change and the calculated shift of NADH/NAD+ ratio in the direction of oxidation refer to the disturbance of the function of enzymes which play a role in the oxidation-reduction processes.
