Folliculocentesis: a novel research technique to investigate the intrafollicular endocrine microenvironment.

During development of the dominant follicle, the avascular granulosa cells and oocyte are exposed to the follicular fluid endocrine microenvironment. An alteration in the endocrine characteristics of follicular fluid affects follicular steroidogenesis, oocyte maturation, ovulation and subsequent corpus luteum function. In-vitro studies on pooled follicular fluid from ovarian specimens lacked temporal precision between menstrual and follicular endocrine events. We have established a new technique, termed folliculocentesis (FC), to sample follicular fluid from the dominant ovarian follicle without compromising its growth or function during the mid- to late follicular phase. A total of 38 subjects with regular ovulatory cycles each underwent two identical cycles of hormone and follicle growth monitoring: one cycle served as the control, and FC was performed during the second cycle. During all cycles, plasma luteinizing hormone (LH), oestradiol and ultrasound monitoring of follicle growth were commenced on day 7 and continued until after ovulation. During FC cycles, 200 microliters of follicular fluid were aspirated from the dominant follicle using transvaginal ultrasound guidance when the follicle diameter reached > or = 10 mm. Six subjects were excluded from the study because of incomplete or invalid endocrine data. In all, 32 subjects completed both the FC and control cycles. The follicle growth pattern, maximum follicle diameter, plasma oestradiol, oestradiol peak, plasma LH, LH surge and follicular phase length were similar during FC and control cycles. A total of 50 valid follicular fluid samples were obtained when the dominant follicle was sampled once, twice or three times during the same cycle and from the same follicle in 15, 16 and one subjects respectively. The follicular fluid samples contained steroid concentrations consistent with those of the mid- to late follicular phase. We conclude that the FC procedure is safe, easy to perform and does not affect follicle growth or hormone dynamics. Analysis of the follicular fluid samples is expected to provide us with valuable in-vivo information about ovarian endocrinology.

A time course study of capacitation and the acrosome reaction in human spermatozoa using a revised chlortetracycline pattern classification.

OBJECTIVE: To study the time course of capacitation, spontaneous, and A23187-induced acrosome reaction of human spermatozoa during 8 hours incubation in vitro using the chlortetracycline (CTC) assay with a revised fluorescent pattern classification. DESIGN: Fertile donor spermatozoa were isolated by direct swim-up and incubated in Earle's balanced salt solution for up to 8 hours. At hourly intervals, spermatozoa were stained with CTC before and after the addition of A23187 to induce the acrosome reaction. SETTING: The University Clinic, Jessop Hospital for Women, Sheffield, United Kingdom. PATIENTS: Donors participating in the Donor Insemination Program. MAIN OUTCOME MEASURES: Eight fluorescent patterns identified by the CTC assay and acrosome-reacted spermatozoa detected by indirect immunofluorescence using 18.6 monoclonal antibody. RESULTS: Using a statistical model defined by analysis of deviance allowed rationalization of the CTC pattern classification by grouping together patterns that showed a similar and significant change over time. In addition, spontaneous and A23187-induced acrosome-reacted spermatozoa identified by the CTC assay were shown to be correlated significantly to those identified by indirect immunofluorescence. CONCLUSION: The CTC assay using a revised pattern classification offers a more precise description of human spermatozoa capacitation in vitro. Also, CTC-identified acrosome reaction (both spontaneous and A23187 induced) was confirmed independently by indirect immunofluorescence.

Implantation and the endometrium.

Our knowledge relating implantation and the endometrium is largely descriptive at present. In the absence of a unifying hypothesis, I describe endometrial receptivity for implantation. Tissue component data are described, first relating to whole tissue, then isolated glands and stroma, vessels, the luminal epithelium, and infiltrating leukocytes. Finally, I consider trophoblast-endometrial interaction. Stimulatory and inhibitory factors impinging on a proteolytic cascade must be related to the process of invasion of the trophoblast. Only early data are available, as the description of this fascinating process unfolds.

The more accurate timing of insemination with regard to ovulation does not create a significant improvement in pregnancy rates in a donor insemination program.

OBJECTIVE: To determine if the accurate prediction of ovulation before artificial insemination of frozen donor semen was justified by increased pregnancy rates. DESIGN: A retrospective study of inseminations, over a 2-year period, administered to women who were enrolled on a donor insemination program and who were monitored using one of four ovulation timing regimens. Also, a study of the significance of follicle status at the time of insemination with relevance to pregnancy was carried out. SETTING: A university teaching hospital donor insemination program. PATIENTS: Patients entered in this study were undergoing insemination with thawed, cryopreserved, donor semen. MAIN OUTCOME MEASURES: Pregnancy and follicle status at the time of insemination. RESULTS: There was no statistically significant difference in pregnancy rates between the four methods of ovulation detection most commonly used for artificial insemination. Inseminations several hours either side of ovulation did not result in a significantly different pregnancy rate than inseminations carried out closer to ovulation. CONCLUSION: More accurate and expensive methods of detecting changes in ovulation result in no significant increase in fecundity than other less expensive timing techniques.

The sequential effects of human cervical mucus, oviductal fluid, and follicular fluid on sperm function.

OBJECTIVE: To compare the sequential effects of human cervical mucus (CM), oviductal fluid, and follicular fluid (FF) on sperm motility, hyperactivation, and the acrosome reaction during 9 hours of incubation in vitro. DESIGN: Spermatozoa from a fertile donor were allowed to penetrate human CM in vitro and then were recovered and incubated in either 30% human oviductal fluid or a simple salt solution for 3 hours before challenge with 20% FF. Thus, the sequential effects of the fluids collected from the female reproductive tract on sperm hyperactivation and acrosome reaction were examined. SETTING: The donor insemination program at the University Clinic within the Jessop Hospital for Women, Sheffield, United Kingdom. PATIENTS: All donors used in this study were involved in the donor insemination program. MAIN OUTCOME MEASURES: Sperm motility, hyperactivation, and percentage of acrosome-reacted spermatozoa were measured. RESULTS: This study used an in vitro technique to mimic sperm transport in vivo. Spermatozoa sequentially incubated with female reproductive tract secretions in vitro showed a maintenance in the level of FF-induced hyperactivated motility and a decreased response to the FF-induced acrosome reaction when compared with sperm challenged with FF in the absence of prior incubation with human oviductal fluid. CONCLUSION: It is suggested that human oviductal fluid may serve to delay the FF-induced acrosome reaction.

Endometrial differentiation in the peri-implantation phase of women with recurrent miscarriage: a morphological and immunohistochemical study.

OBJECTIVES: To study endometrial differentiation in the peri-implantation phase of women with recurrent miscarriage and to compare the results with endometrium of normal fertile women. DESIGN: A prospective study of endometrial specimens precisely timed from the LH surge, using traditional histologic dating (Noyes' criteria), quantitative histologic measurement (morphometric analysis), and immunohistochemical techniques. RESULTS: Fifteen of 25 (60%) subjects in the recurrent miscarriage group had retarded endometrial development in the peri-implantation period as monitored by morphometry. The recurrent miscarriage group showed reduced levels of four mucin-related secretory epitopes, and greater reductions were associated with morphological retardation. Normal differentiation was observed in all of the 14 subjects in the control group. CONCLUSIONS: Women with idiopathic recurrent pregnancy loss may be divided into two distinct subgroups on the basis of their endometrial response in the peri-implantation period. Precisely timed endometrial biopsy should be incorporated in the investigation of recurrent miscarriage.

The significance of elevated basal follicle stimulating hormone in regularly menstruating infertile women.

Elevated plasma follicle stimulating hormone (FSH) during the reproductive life is an early manifestation of ovarian ageing. The presence of elevated basal FSH in young, regularly menstruating women may represent a stage of menopausal transition consequent on premature ovarian failure. A total of 48 regularly menstruating, infertile women aged < 40 years, with high FSH and aged-matched controls with normal FSH underwent detailed monitoring of endocrine and follicle growth during one complete menstrual cycle. During the same cycle, detailed immunological screening was performed and the epidemiological features of all subjects were also reviewed. Subjects in the high FSH group had significantly higher basal FSH, luteinizing hormone (LH) and follicular phase LH concentrations. Despite their normal preovulatory oestradiol production, the high FSH group showed significantly slower follicular growth, smaller follicle diameter and lower luteal phase salivary progesterone. All these features have been described in older women during their menopausal transition. In addition, the prevalence of autoimmune antibodies was significantly higher in the high FSH group. This study suggests that infertile women with elevated FSH are in their perimenopause despite having regular ovulatory and apparently normal cycles. An autoimmune basis is suggested as a factor underlying their premature ovarian failure. Further endocrinological and autoimmunological follow-up is recommended.

Falloposcopic comparison of unilateral and bilateral proximal tubal occlusive disease.

The purpose of this study was to compare the appearance of the entire length of the oviductal canal in women with unilateral versus bilateral proximal tubal occlusion. Eleven women had apparent unilateral disease and 18 had apparent bilateral occlusion. Proximal occlusion was confirmed both by hysterosalpingography and laparoscopy. Falloposcopic examination was performed by retrograde visualization using the linear eversion catheter, in office. Bilateral apparent proximal occlusion was found to have a significantly higher incidence of actual proximal occlusion and distal intralumenal abnormalities. However, five proximal segments and six distal segments were found to be abnormal where the pre-examination diagnosis was unilateral occlusion. Furthermore, the contralateral oviductal canal was found to be abnormal in four women with apparent unilateral disease, indicating that apparent unilateral proximal tubal occlusion is associated with actual pathology. These data demonstrate the value of intralumenal assessment of apparent proximal occlusion.

Placental protein 14 in cycles with normal and retarded endometrial differentiation.

The purpose of this study was to investigate whether endometrium with retarded development differs, functionally, from endometrium with normal 'in-phase' development. Precisely timed endometrial biopsies were obtained from 24 women suffering from unexplained infertility at 4, 7, 10 and 13 days following the luteinizing hormone (LH) surge. Frozen sections were labelled with an anti-placental protein (PP) 14 monoclonal antibody using an avidin-biotin peroxidase technique and semi-quantification of endometrial PP14 was performed using a Quantimet 970 image analyser. Serum PP14 and saliva progesterone were measured for each patient. Data were analysed using one- and two-way analysis of variance. Normal and retarded endometrium were identified in 16 (group I) and eight (group II) women respectively. Both groups demonstrated a significant increase of the area of precipitate measured for PP14 from day LH + 4 to LH + 13. However, two-way analysis of variance showed that endometrial PP14 was significantly (P < 0.05) lower in the retarded endometrium group at LH + 10 and LH + 13. Serum PP14 was also significantly lower (P < 0.01) in women with retarded endometrial development at LH + 13. Women with normal endometrial development had a significantly higher (P < 0.05) concentration of cumulative saliva progesterone from LH + 3 to LH + 5. This study indicates that there are functional differences between normal and retarded endometrium. These differences may adversely affect uterine receptivity during implantation and the early placentation stage.

The artificial cycle as an effective treatment of persistently retarded endometrium in the luteal phase.

The objective of this study was to examine the results of two hormonal treatment modalities on subjects who had persistently abnormal endometrial development in the luteal phase. A prospective study design was used to investigate 14 women who had persistently retarded endometrium associated with infertility (n = 11) or recurrent miscarriage (n = 3). Treatment A consisted of progesterone supplementation in the form of i.m. progesterone at a daily dose of 25-50 mg starting on day luteinizing hormone (LH) + 1 for 14 days. Treatment B consisted of artificial cycles produced after down-regulation of the hypothalamic-pituitary-ovarian axis with Goserelin (3.6 mg s.c.) followed by the administration of a standard hormone replacement therapy. Endometrial biopsy was taken on day 19 of the artificial cycle or days LH + 5 to +7 in the progesterone supplementation cycle. A histological study was made of the endometrial specimens by standard dating criteria and morphometry. The artificial cycle resulted in normal development in all subjects (n = 11), whereas progesterone supplementation restored normal endometrial development in only seven of 11 (64%) subjects. We conclude that persistently retarded endometrium could be treated more effectively with the artificial cycle than with progesterone supplementation.

Computer-assisted measurement of hyperactivation in human spermatozoa: differences between European and American versions of the Hamilton-Thorn motility analyser.

Spermatozoa from healthy donors of proven fertility were analysed for percentage hyperactivation, mean curvilinear velocity (VCL), mean progressive velocity (VSL), amplitude of lateral head displacement (ALH) and percentage linearity (LIN) using two versions of the Hamilton-Thorn motility analyser (HTMA) running at different frame rates. The sort fraction criteria developed using a machine utilizing 30 Hz gave comparable figures for the percentage spermatozoa displaying motility patterns concomitant with hyperactivation when compared to that developed for a machine using a frame rate of 25 Hz. Statistically significant differences were observed when criteria developed using 30 Hz machines were transferred to those using 25 Hz. The frame rate dependency of VSL and VCL are thought to account for these differences; values for ALH and LIN were largely unaffected by frame rate. These results have implications for workers using the European version of the HTMA (at 25 Hz) with sort criteria, containing frame rate-dependent variables, which have been developed by American colleagues using the machines of a higher frame rate. More appropriate sort criteria for workers using European (25 Hz) machines are suggested.

Lymphoid tissue in the endometrium of women with unexplained infertility: morphometric and immunohistochemical aspects.

The purpose of this study was to investigate whether the endometrium of women with unexplained infertility differs in some immunological aspects from the endometrium of normal fertile women. Endometrial biopsies were obtained from 24 normal fertile women (group I) and 24 women suffering from unexplained infertility (group II) at 4, 7, 10 and 13 days following the luteinizing hormone (LH) surge. Endometrial granulated lymphocytes were assessed morphometrically in 2 microns resin sections. A panel of 11 monoclonal antibodies was employed to characterize the leukocyte subsets in frozen sections. Semi-quantification was performed with a Quantimet 970 image analyser. Data were analysed using one- and two-way analysis of variance. Compared with fertile controls, women with unexplained infertility had significantly lower numbers of CD8+ (T suppressor/cytotoxic) cells at each post-LH date. In contrast, the number of CD4+ (T helper/inducer) cells was significantly higher in group II. Throughout the luteal phase, infertile women had fewer CD56+ cells than normal fertile controls. The volume fraction of endometrium occupied by the nuclei of endometrial granulated lymphocytes did not alter with the cycle stage but the mean nuclear diameter and axial ratio decreased from LH+7 to LH+13. The differences observed in endometrial leukocytic subpopulations between fertile and infertile women may contribute to unexplained infertility probably by affecting the embryonic maternal dialogue during the implantation and early placentation period.

Keratan sulphate as a secretory product of human endometrium: cyclic expression in normal women.

The distribution of keratan sulphate (KS) in normal endometrium has been studied by immunohistochemistry using monoclonal antibody 5D4. KS is present in association with glandular epithelium throughout the normal cycle. Production is hormonally regulated, as indicated by the significant increase observed in the secretory phase of the cycle. Intracellular immunoreactivity increases to a maximum 3 days after the luteinizing hormone (LH) peak. This is followed by an increase in secreted KS from 4 days after the LH peak, and essentially all endometrial glands showed immunoreactive deposits by 5 days after the LH peak. Beginning on day 7 after the LH peak, a fraction of glands was observed to have lost the epitope. Nevertheless, KS remained in a significant proportion of the gland lumens 9 days after the LH peak. The data suggest that analysis of secretory KS in women gives an index of hormonally regulated epithelial differentiation in the peri-implantation phase.

Comparison of ampullary assessment by falloposcopy and salpingoscopy.

The purpose of this study was to compare and contrast falloposcopic and salpingoscopic ampullary assessments in a series of 20 women undergoing tubal microsurgery for distal tubal or peritubal disease. Four women had an extrinsic cause for their peritubal adhesions, and would have been expected to exhibit a normal oviductal canal. All of the falloposcopic examinations were performed as an outpatient procedure. Salpingoscopic examinations were undertaken at the time of microsurgery. The endoscopic examinations were undertaken by two clinicians, who were blinded to each other's assessments and to the indication for surgery. Of the 31 Fallopian tubes which were examined, 24 were found to be abnormal by salpingoscopy and 23 were found to be abnormal by falloposcopy. Total score and scores for epithelial appearance, vascularity, intraluminal adhesions and dilatation were found to be significantly associated. Furthermore, falloposcopy predicted salpingoscopic status correctly in 84% of cases. These data suggest that falloposcopy is a useful method of assessing ampullary condition.

Detection of antibodies to ovarian antigens in women with premature ovarian failure.

Premature ovarian failure is a common condition of uncertain aetiology in most cases, although autoimmunity is thought to play a role in a proportion of cases. The frequency of ovarian antibodies, which may be markers for an autoimmune aetiology in this condition, remains unclear. To define this further, we have examined the sera of 45 women with premature ovarian failure (five with iatrogenic ovarian failure, nine with an associated autoimmune disease, and 27 with idiopathic ovarian failure), as well as four women with infertility due to Turner's syndrome and 41 pre- and post-menopausal controls. Using two human ovarian antigen preparations, 24% and 60% of the ovarian failure patients reacted in an ELISA (P < 0.05 and P < 0.001 compared with controls), but frequent cross-reactivity was found with fallopian tube antigens. The apparent aetiology of ovarian failure did not correlate with the presence of ovarian antibodies. Using bovine ovary as an antigen, there was a significant overall increase in binding by the ovarian failure patients, but this was almost identical to binding in an ELISA with bovine fallopian tube. In contrast to a previous report, there was no significant increase of binding to soluble or Triton-extracted membrane fractions of bovine corpora lutea containing the LH/hCG receptor by the patients with ovarian failure. These results suggest that ovarian antibodies are common in premature ovarian failure, but their specificity and pathogenic role are questionable.

Hypothalamic-pituitary ageing: progressive increase in FSH and LH concentrations throughout the reproductive life in regularly menstruating women.

OBJECTIVE: Reproductive ageing is associated with reduced fertility and endocrine changes that become more pronounced during the perimenopausal period. We aimed to assess changes in gonadotrophin concentrations and the onset of these changes during the reproductive life prior to the perimenopause. SUBJECTS: Ovarian and pituitary hormones were measured and follicle growth monitored in 500 regularly cycling infertile women aged 20-44 years. MEASUREMENTS: Serum levels of FSH and LH were determined during the follicular phase and ultrasonography was used to monitor follicle growth and ovulation. The luteal phase was assessed by salivary progesterone measurements. Changes in basal FSH and LH with age during ovulatory cycles were assessed. RESULTS: There was a significant progressive increase in FSH levels as early as age 29-30 years which was continued throughout the 30s and became more marked in the early 40s. LH levels showed significant increase at the age of 35-36 years which was maintained till the age of 40 years and followed by a further increase in women aged > 40 years. CONCLUSION: An increase in FSH and LH concentrations can be detected in women with regular ovulatory cycles quite early during the reproductive life. The increase in basal FSH (and later LH) may represent the earliest endocrine marker of reproductive ageing. These age-related increases in FSH and LH can be used as hormonal markers to counsel patients as to the likelihood of their reproductive potential.
