The challenge of childhood diabetes mellitus in India.

The prevalence of DM is about 0.4/1000 children with a lower incidence in the rural areas. Children comprise 3-5% of the total diabetics. A study of 55 pediatric cases of DM (1980-84) showed that only 22 (40%) had ketoacidosis on admission. Ten (18.2%) had onset of illness before 4 years of age. HLA antigen studies in childhood IDDM have shown a positive linkage disequilibrium with Bw21 (RR-12.7), and DR3 (RR = 16.6). Prevalence of islet cell antibodies (ICA) was 30.9% (n = 110) as compared with 0.8% in controls. Antibodies against Coxsackie B2 virus were increased (75.5% vs 46.4% in controls). The C-peptide content was substantially low. Malnutrition related DM occurs in adolescents in some parts of India. It is characterized by moderate hyperglycemia, low serum glycerol, relative insulin insensitivity, and pancreatic malformation/calcification in about 1/4 of subjects. There is no association with HLA antigens or ICA, and the precise etiology is unclear. Mortality was 3.6% in patients admitted in our hospital but is higher in other regions due to poverty and relative lack of health care facilities.

Hormonal therapy in undescended testes.

A clinical study trial of HCG therapy in 78 selected prepubertal cryptorchids (40 unilateral and 38 bilateral) having a total of 116 undescended gonads revealed that complete descent occurred in 12.5% gonads (13% in unilateral and 12% in bilateral group). Good response was observed in only those gonads which were located in the inguinal canal and at the external ring, irrespective of it being right or left sided gonad. In patients with impalpable testes, though there was no complete descent, yet partial descent was noted in 18% (21/116 gonads). The response was better in the older children.

Nomograms of fetal nuchal fold thickness for early detection of Down syndrome.

A prospective study was undertaken in North India to find the normative data on the nuchal fold thickness of randomly selected 150 fetuses between 16-22nd week of gestation. The data revealed that the width of nuchal fold thickness ranged between 2-5 mm. The mean for 16-18, > 18-20, and > 20-22 week's gestation was 2.6 +/- 0.2, 3.8 +/- 0.9, and 4.0 +/- 0.8 mm respectively. In one fetus nuchal fold thickness was 6mm at 18 weeks (> 2SD above the normal value) and thus Down Syndrome was suspected. This was later confirmed by amniocentesis and karyotyping.

Down Syndrome↗

Optimal care for children with Down syndrome in India.

Medical problems which arise in Down Syndrome are reviewed. Guidelines for management including early diagnosis of congenital heart defects, gastrointestinal anomalies, eye problems, hearing defects, thyroid dysfunction, at lanto-axial instability, prognosis for mental retardation, and role of early stimulation are provided. Prenatal diagnosis in high risk pregnancies is recommended.
