Further studies of rimiterol and salbutamol administered by intermittent positive-pressure ventilation, and an important observation on the technique of using the Bennett ventilator.

1. Using the same technique of administering drugs by intermittent positive-pressure ventilation as used in previous studies a source of contamination of solutions nebulized was discovered. This was rectified by using a new ventilator and completely separate patient circuits for each solution nebulized. 2 Salbutamol 0.5% and 0.25% solutions achieved the same degree of bronchodilatation, but there was a significantly greater increase in heart rate produced by salbutamol 0.5%. 3 Rimiterol 0.5% and salbutamol 0.25% produced similar peak mean improvements in FEV and also induced the same degree of tachycardia, but the duration of these effects were significantly shorter in the case of rimiterol. 4 The sustained degree of bronchodilatation achieved by salbutamol 0.25% could not be mirrored by giving two doses of rimiterol 0.5%, the second dose 2 h after the first. 5 Rimiterol 0.5% induced a degree of tachycardia which was similar in peak effect to that observed after salbutamol 0.25%. However, in the controls the second dose of rimiterol, given 2 h after the first, was responsible for only a small increase in heart rate which was not significantly different than that after saline in the other three treatment groups.

Rapid carbohydrate utilization test for the identification of Neisseria gonorrhoeae.

A rapid carbohydrate utilization test for the identification of N. gonorrhoeae was investigated, with reference to its use in a routine diagnostic laboratory. The rapid test was shown to give accurate results in agreement with those of a conventional serum-free sugar medium. Because of the shorter time taken for the confirmation of an isolate, and several other advantages, it is proposed that the rapid test is an extremely useful alternative to conventional sugar tests. Immunofluorescence was also used to identify isolates of N. gonorrhoeae and the rapid carbohydrate utilization test was found to assist in differentiating between N. gonorrhoeae and N. meningitidis when equivocal or negative immunofluorescence results were obtained.

Pulmonary eosinophilia after substitution of aerosol for oral corticosteroid therapy.

Three patients with chronic asthma developed pulmonary eosinophilia while oral prednisolone was being withdrawn after the introduction of treatment with beclomethasone dipropionate aerosol. These observations suggest that the development of pulmonary eosinophilia in patients with chronic asthma can be prevented by the systemic administration of corticosteroids, but not by a corticosteroid aerosol given by inhalation. It is considered that radiological examination of the chest should be carried out at frequent intervals during and for a few months after the substitution of corticosteroid aerosol therapy for oral corticosteroid therapy in order to ensure that those patients who are liable to develop pulmonary eosinophilia will be identified at an early stage. These precautions are even more essential when there is a previous history of pulmonary eosinophilia.

Objective and subjective comparisons of terbutaline and rimiterol bronchodilator aerosols.

Double-blind studies of the bronchodilator effects of rimiterol, terbutaline and a combination of rimiterol and terbutaline were performed in patients with chronic asthma. An objective assessment in 21 patients showed no significant differences between the mean improvements in FEV1 during the first 30 minutes after drug administration. At 45 minutes and thereafter terbutaline produced a significantly greater degree of bronchodilatation than rimiterol or the combination of rimiterol and terbutaline. In a subjective study of 27 patients it was found that they were able to detect the more prolonged duration of bronchodilator activity of terbutaline but not the more rapid speed of onset of action of rimiterol.

ad and ay subtypes of hepatitis B antigen in a case of hypogammaglobulinaemia.

We describe the finding of d and y specificities of hepatitis B surface antigen (HBsAg) in a case of hypogammaglobulinaemia of the 'common variable' type treated with fresh frozen plasma infusions. Absorption studies show that the two specificities are on separate particles, suggesting dual infection. It raises important questions regarding the relationship between HBsAg persistence and the immune status of the carrier.

Misonidazole in patients receiving radical radiotherapy: pharmacokinetic effects of phenytoin, tumor response and neurotoxicity.

In 1978, a pilot study began of 29 patients with advanced tumors of the head and neck. The study showed a initial peripheral neuropathy rate of 55%, despite a dose limitation of 12g/m2 of misonidazole. Tumor response at 9 months was most encouraging. We are now able to examine tumor response and persistence of neuropathy in these patients 2 1/2 years after radical radiotherapy. The results are comparable with those obtained with hyperbaric oxygen in a clinical trial at this center during the 1970's. Neuropathy was a serious side effect but the drug phenytoin has been shown to shorten the half-life of misonidazole. We have examined the effect of phenytoin on the pharmacokinetics of misonidazole in 13 patients who received radical radiotherapy for advanced head and neck tumors or oesophageal tumors. Misonidazole was given in multiple doses, i.e. daily or weekly as it would be used in conventional therapy. Phenytoin was given either daily throughout treatment, or it was withdrawn during treatment. There were dramatic changes in the half-life of misonidazole, but the concentration at the time of irradiation was little affected. The significant changes in the half-life of misonidazole and the increased concentration of the metabolite desmethylmisonidazole are discussed.
