Legionnaires' disease: ultrastructural appearance of the agent in a lung biopsy specimen.

During the Legionnaires' disease epidemic that occurred in Philadelphia in 1976, we performed a transbronchial lung biopsy on a patient who suffered from Legionnaires' disease that was confirmed by serology. The biopsy was performed in an attempt to detect a causal agent for the patient's pneumonia at a time when the cause of the epidemic was a mystery. We detected pleomorphic microbial structures that were both intracellular and extracellular in location. By electron microscopy, they ranged in size from 350 nm to 2.5 mu. Some displayed cell walls and fine cytoplasmic granules that resembled ribosomes. Our findings illustrate the appearance of the Legionnaires' disease agent in acutely infected, antibiotic-treated human lung obtained from biopsy specimens.

Chronic myeloid leukemia: predictive value of sequential chromosomal surveys and their effect on survival.

A program has been initiated to evaluate the efficacy of intensive therapy based on sequential cytogenetic studies in patients diagnosed as having Ph1 positive chronic myelocytic leukemia (CML). Eighteen patients have been induced into hematological remission with busulfan, splenectomized and treated with cycle active drugs. Maintenance therapy is continued during the benign chronic phase of the disease. Intervention with cycles of aggressive therapy is predicated on the results of chromosomal surveys of bone marrow aspirates. The results of this limited study suggest that survival can be improved by such a program.

The effect of E. coli L-asparaginase on oral glucose tolerance and insulin release in man.

To study the effect of E. Coli L-asparaginase on glucose tolerance and insulin release, 6 patients with neoplastic disease were subjected to 3 hour oral glucose tolerance tests with simultaneous measurement of serum immunoreactive insulin (IRI) levels before and following the intravenous administration of 5000 I. U. L-asparaginase/day for 4 days. Five of the patients exhibited a significant deterioration in glucose tolerance; however, no change was noted in their fasting glucose and IRI levels. The deterioration in glucose tolerance was associated with a decrease in the amount of insulin secreted in the first 30 minutes after the oral glucose load. The total amount of insulin released during the 3 hour test remained unchanged. These studies suggest that L-asparaginase can cause a deterioration of glucose tolerance without accompanying fasting hyperglycaemia. This may be due, in part, to a decrease in glucose-induced insulin release during the first thirty minutes following oral glucose.

A combined assay for the rapid preliminary detection of structural retroviruses.

This report describes the use of equilibrium gradients, RNA dependent DNA polymerase assays and electron microscopy (EM) in a combined assay for the rapid preliminary detection of intact retroviruses in crude preparations. Positive combined assays of platelets from preleukemic patients corresponded with karyotypic abnormalities found in these patients. Reconstruction experiments with Rauscher Leukemia Virus added to buffer or disrupted mouse spleen demonstrated the ease of detecting 10(9) or greater particles/g crude tissue, and the effects of buffer or added protein.

Effect of busulfan on oncornavirus-like activity in platelets and chromosomes in polycythemia vera and essential thrombocythemia.

The effect of busulfan therapy on the activity of oncornavirus-like particles and chromosome patterns in patients with polycythemia vera and essential thrombocythemia was studied. Three patients had pretreatment platelet counts greater than 1 million/microliter, abnormal bone marrow karyotypes, and electron microscopic and biochemical evidence of oncornavirus. The results demonstrated that in all 3 patients virus-like particles and reverse transcriptase-like activity could no longer be found in the platelets within 2-4 weeks after the initiation of busulfan treatment. The platelet count was still elevated at this point for each patient, although the count returned to normal levels within 2-3 weeks after virus-like activity was no longer detectable. The chromosome patterns, characterized by aneuploidy (30-50%) before treatment, improved after therapy.

Low dose bleomycin and methotrexate in cervical cancer.

Twenty patients with recurrent and disseminated carcinoma of the cervix were treated with Bleomycin, 10 mg/m2 weekly and Methotrexate, 10 mg/m2 every fourth day. Twelve of the 20 (60%) had a greater than 50% shrinking of measured tumor masses lasting a median remission time of 7.5 months. The data suggest that combination protracted chemotherapy with these drugs of metastatic cervical cancer might improve the outlook of patients with this condition.

Red blood cell metabolism in AKR mice in the prelymphomatous phase.

The purpose of this study was to evaluate erythrokinetics and in vitro red blood cell (RBC) glucose utilization, 2,3-diphosphoglycerate production, and adenosine triphosphate levels following incubation in AKR mice from early in life until the onset of AKR lymphoma. Normal BALB/c mice served as controls. While hemoglobin concentration and RBC survival remained constant and similar in both groups of mice, the half disappearance time of injected radioactive 59Fe was longer and the 48-hr reappearance of 59Fe was less in AKR mice, compared with those of BALB/c mice. In vitro RBC metabolic studies indicated increased glucose utilization and 2,3-diphosphoglycerate production and decreased adenosine triphosphate levels following incubation in AKR RBC, in contrast to those in BALB/c RBC. RBC metabolic studies were also done in a small group of low leukemic C3H mice, and were similar to BALB/c mice. These differences became most marked in RBC from mice aged 15 to 30 weeks. Overt lymphoma began to occur after age 40 weeks. Hence, these erythropoietic changes occurred prior to the onset of lymphoma. The data imply a direct effect of virus infection on RBC or their precursors. The results are similar to changes in RBC metabolism noted in Rauscher-infected BALB/c mice. The broader implication of these findings in reference to viral host interactions and human leukemogenesis is discussed.

Analysis of platelets from patients with thrombocythemia for reverse transcriptase and virus-like particles.

A preliminary analysis of an RNA-directed DNA polymerase was made and a C-type virus-like particle was identified in platelets from 2 patients with the myeloproliferative disorder thrombocythemia (primary, essential, hemorrhagic, or idiopathic thrombocythemia). Platelet homogenates were centrifuged through a sucrose equilibrium density gradient. Both endogenous and exogenous DNA polymerase activity was found at a density of 1.19 g/ml. No activity was seen at comparable densities in control gradients. Electron micrographs of thin sections of these platelets revealed a particle with the morphologic characteristics of a C-type virus; however, the diameter of this particle was about 80 nm, slightly lower than that commonly found for C-type particles. Critical-point dried specimens, from the fractions of the sucrose gradient at which DNA polymerase activity was found, contained particles of the same size and morphology as those in the thin sections.

Potential roles of androgens and the anabolic steroids in the treatment of cancer - a review.

Androgenic hormones are utilized in clinical practice as a bone marrow stimulant as well as replacement therapy in certain endocrinologic disorders. In the oncologic realm breast cancer has shown some responsiveness to pharmacologic doses of androgens. However, recent work reveals that androgens are capable of stimulating the fibrinolytic mechanism and more importantly enhance the immune response. These latter properties represent a significant new role for the androgens and anabolic agents. Hence, adjunctive utilization of androgens with cancer chemotherapeutic agents may result in an effective synergistic combination in the therapy of malignant disease.
