Site-specific incorporation of [125I]iododeoxyuridine into DNA.

A procedure for the incorporation of [125I]IdU into specific sites in DNA is described. The approach depends upon attachment of radioiododeoxyuridine to a controlled pore glass support which is then used for automated synthesis of an oligomer. The resulting oligomer, containing a terminal 3'[125I]iododeoxyuridine, is used as a primer during DNA synthesis catalyzed by the Taq polymerase employing thermal cycling. The product formed includes the radioiodonucleotide at a single internal site determined by the length of the oligomer.

How should a seated workplace with a tiltable chair be adjusted?

For a tiltable office chair, subjective height preferences may be different to those for conventional fixed-seat chairs, due to altered biomechanics. These preferences, with a tiltable chair and associated table, have been investigated with a total of 108 subjects. To determine the shortest possible way to obtain reliable height measurements, a study (n = 10) was undertaken. It was demonstrated that two days of measurement seem necessary, the first day involving both a too-high and a too-low start position, the second day only a too-high one. Preferences were measured for each of four groups, one (n = 21) accustomed to and another (n = 35) unaccustomed to a tiltable seat; a third (n = 23) was composed of children and the fourth group (n = 19) was making large-scale drawings. When accustomed to the seat, the subjects preferred a greater seat height. The children's seat-height preference was relatively equal to that of adults, whereas their preferred relative table height was greater, most evidently so with decreasing age. The greatest relative seat height was preferred in connection with large-scale drawings. For desk work it is recommended that the seat height should be approximately 3-5 cm above popliteal height, including shoe heels, and the table height 4-6 cm above the actual elbow height.

Mathematical morphology for automatic detection of brain lesions at magnetic resonance imaging.

An original method of image analysis has been developed, using 'mathematical morphology', in order to detect brain tumors at magnetic resonance imaging of the head following injection of gadolinium-DPTA. The main steps of the analysis are as follows: 1) Spatial reduction by automatic determination of a region of interest, 2) elimination of noise by morphologic filtering (increasing alternate closing-opening sequence), 3) localization of tumor by searching for r-h maxima, and 4) extraction of tumor contours using three-dimensional structuring elements. The preliminary results of this automatic and robust method encourage further studies of the potential of tissue analysis as an aid to tumor diagnosis.

Short-term physical training in bronchial asthma.

The effect of two types of physical training on patients with perennial asthma were compared in a blind, controlled, randomized study. Eleven of 27 adults with asthma performed a physical training programme which did not change their oxygen consumption (control group). The remaining 16 asthmatics performed a physical training programme which improved their maximal oxygen consumption (training group). Both of the training programmes were performed for 1 hour, twice a week during a period of 2 months. No complications were reported during the performance of the training programmes. The doses of all medicines apart from beta 2-agonist aerosol were unchanged during the training period. The patients inhaled beta 2-agonist aerosol if their peak expiratory flow (PEF) was less than 60% of their maximal PEF. The training group decreased their use of aerosol from an average of 4.94 puffs per day to 3.41 puffs per day (P less than 0.05). The control group did not change their use of beta 2-agonist aerosol significantly. It is concluded that physical exercise which improves the maximal oxygen consumption decreases the use of beta 2-agonist spray and that heavy exercise is well tolerated by asthmatics.

Psoriatic arthritis treated with oral zinc sulphate.

Twenty-four patients suffering from psoriatic arthritis participated in a double blind cross-over trial of peroral zinc sulphate versus placebo (Trial I). Eleven patients continued an open trial of peroral zinc sulphate for an additional 24 weeks (Trial II). Remission was assessed by the disappearance of symptoms (overall condition, morning stiffness, functional capacity of the joints and joint pains), and signs (mobility and swelling of the joints). Reduction of joint pains as well as increase of mobility and decrease of swelling of several joints were observed. the clinical signs of reduced inflammation were accompanied biochemically by reduction of serum immunoglobulins and an increase of serum albumin. The need for analgesics was diminished. Severe side-effects and changes in the psoriatic skin involvement were not seen. Oral zinc sulphate seems to be valuable in the treatment of psoriatic arthritis.

An attempt to 3D reconstruct vessel morphology from X-ray projections and intravascular ultrasounds modeling and fusion.

The emergency of interventional revascularization techniques in the treatment of atheromateous vascular diseases has resulted in the need for additional valuable diagnostic information about the 3D morphology and nature of the lesion. To overcome the limitations inherent to common vascular imaging techniques, such as Digital Angiography (DA) which only gives partial information on lumen narrowing or Intravascular Ultrasound (IVUS) which provides randomly oriented transversal images, a 3D reconstruction of the vessel by fusion of X-ray and IVUS images has been developed. For that purpose, X-ray and IVUS images are acquired according to a well-defined protocol and useful information to be fused is extracted. A geometric model then leads to the determination of the unknown parameters which allow the alignment of all data in a common reference frame. The registered data are then directly introduced into a probabilistic reconstruction process using a Markovian modeling associated with a simulated annealing-based optimization algorithm. Taking into account all the information available about the vessel, the method avoids the uncertainties and ambiguities of a reconstruction based only on one modality, and the probabilistic fusion solves the possible contradictions between both acquisitions. Results of vascular lumen 3D reconstruction are shown with data acquired on an excised dog aorta. The accuracy of reconstruction of the lumen by data fusion is significantly improved compared to results obtained with separate reconstruction from angiographic or ultrasonic data. Further work will include introduction of vessel wall texture elements into the probabilistic fusion process to increase the amount of information gained by intravascular ultrasonic tissue characterization.

Successful chemotherapeutic treatment of diencephalic syndrome with continued tumor presence.

A 7-month-old infant with typical features of diencephalic syndrome (DES) associated with a hypothalamic mass, most probably a glioma, was treated with chemotherapy. The tumor showed clear shrinkage, but after more than 2 years regrowth was noted. During the treatment period the child regained normal growth and became free of symptoms. As radiation therapy, especially at a young age, has significant adverse effects and a neurosurgical approach to the diencephalic region also has the potential to cause significant sequelae, a chemotherapeutic option, when it exists, is preferred. Thus, in an infant in whom a glioma is suspected to be the cause of the DES, based on the clinical picture and the neuroimaging appearance, chemotherapy should be considered the primary therapeutic modality. Even if its effect is temporary, its use is well justified. The most appropriate treatment protocol still needs to be determined.
