Fourteen patients with clinically diagnosed and surgically documented polycystic ovarian (Stein-Leventhal) disease (PCOD) and 16 normal control women were studied to identify the laboratory test or tests that, from the clinician's point of view, are most likely to aid in the nonsurgical diagnosis of the disease. A single random morning blood specimen was assayed in all cases for testosterone, epitestosterone, androstenedione, FSH and LH. The mean testosterone level for the PCOD patients was significantly greater (p less than 0.001) than that for the controls, with 50% of the patients showing elevated levels. Androstenedione showed a similar pattern, but mean epitestosterone levels were not significantly different from controls. FSH was not significantly different, but LH levels were significantly higher than controls (p less than 0.005), with 10 of 13 (77%) demonstrating elevated levels. A strong positive correlation was also found between the degree of virilization and the levels of LH, testosterone and androstenedione. This study suggests that the most useful diagnostic laboratory assay from a single drawing of blood is the serum LH; the only other useful test is testosterone and/or androstenedione. These data do not support other reports of elevated levels of epitestosterone or decreased values of FSH in PCOD.

A study was made of the endometria of 16 patients with gonadal dysgenesis who had been give cyclic replacement treatment with conjugated estrogens for 2 to 10 years, and of 9 other patients who had received medroxyprogesterone in addition to the conjugated estrogens for 2 to 5 years. Seven of the former group showed varying degrees of endometrial hyperplasia, and in all of these cases the duration of estrogen therapy had exceeded 5 years. At the time of the study no cases had developed frank adenocarcinoma. Only 1 of the 9 patients who had been given medroxyprogesterone in addition to the conjugated estrogens demonstrated evidence of hyperplastic change. Progesterone may afford some protection against the development of endometrial hyperplastic activity, but its potential role in protecting against neoplasia remains to be determined. This study supports the view that estrogen replacement therapy in gonadal dysgenesis carries the risk of endometrial hyperplasia and neoplasia. Possible methods of decreasing this hazard are discussed.

Cultured cells from a human breast carcinoma (HBC) were injected subcutaneously in newborn cortisone-treated Sprague-Dawley rats (CNBR). Tumors of predominantly epithelial nature appeared in all within 30 days, and wipespread metastases occured often. Cells cultured from these tumors rapidly produced similar tumors in CNBR. Rat-to-rat passage by implanting fragments of tumor tissue was 100% successful in CNBR. The in vivo model described is simple and inexpensive.

Orbital and conjunctival involvement in multiple myeloma are rare. In most reported cases of multiple myeloma with orbital involvement proptosis has been the initial manifestation that has led to diagnosis of the disease. Our case represents and example of terminal involvement of the orbit and conjunctiva, causing proptosis. The minimal conjunctival changes were the first indication of neoplastic proliferation of plasma cells in this location and, presumably, the orbit.

Cytologic investigations of 16 proven cases of gallbladder carcinomas were analyzed in order to establish the usefulness of cytological methods, including exfoliative bile cytology, fine needle aspiration and endoscopic brushing, in the preoperative and intraoperative diagnosis and management of gallbladder carcinoma. Fine needle aspiration cytology had a sensitivity of 88% and exfoliative cytology of bile a sensitivity of 50%, but both brushings performed yielded false negatives. A preoperative diagnosis was made in 69% (9/13) of cases, and cytologic diagnosis was helpful in planning surgical management. We recommend its wider application in the preoperative diagnosis of gallbladder carcinoma.
OncoLink is a cancer information resource on the World Wide Web that provides a wide variety of information for both patients and health care providers. Introduced in March 1994, OncoLink has enjoyed a 50-fold increase in use since then, with more than 1.8 million accesses per month as of February 1997. New items are added daily, and the OncoLink Web site currently contains more than 10,000 files. During this period of rapid growth, the complexity of managing and maintaining OncoLink has increased as well. Consequently, we developed administrative procedures to handle our workload, which involves content editing, technical (or production) editing, and Web site maintenance. The new strategies have greatly reduced the need for face-to-face meetings of our editorial and production staffs. The rapid growth of OncoLink would not have been possible without these efficient new strategies for managing its daily operation.
