[Effect of the use of lidocaine on patients with acute myocardial infarct].

In 411 patients with acute myocardial infarction (AMI) the effect of intravenous lidocaineeee administration was studied in a suboptimal dose - under 1,5 mg/min (to 1979) and in the optimal dose - over 1,5 mg/min, minimum 2 mg/min during the first 12 hours, with an average 24 h dose, 3,0 g in the course of 48 hours. The patients were grouped into three prognostic groups, depending on their initial assessment according to the coronaryprognostic index of M. R. Norris (1969). The indices, via which the drug effect was determined; were 13, the more significant ones beind: ventricular arrhythmias, early lethality rate, postinfarction angina pectoris and regression fo lesion-ischemic ST-T alterations. It was established, after the analysis of the data obtained, that the prophylactic and therapeutic use of lidocain, considerably reduced the incidence of ventricular arrhythmias in patients with AMI. Lidocain, in an optimal dose, reduces the incidence, justifying its routine administration. Lidocain effect on ventricular arrhythmias, the early lethality rate and certain complications (postinfarction angina pectoris, early cardiac insufficiency, regression of ST-T alterations) in AMI, depend on the initial prognosis and the stage of the hemodynamic disorders.

[Clinical and hemodynamic effect of molsidomine in patients with unstable angina pectoris].

In 30 patients with a grave form of ischemic heart disease--unstable angina pectoris, peroral treatment with molsidomin (Corvaton) was carried out in a dose I table of 20 mg, three times daily, in the course of 33 days at an average. The incidence, intensity and duration of stenocardiac paroxysms were followed up as well as the nitroglycerine need- and electrocardiographic changes. The main hemodynamic indices, prior to and post treatment, were also determined. The stenocardiac paroxysms were established to disappear after molsidomin treatment in 30 per cent of the cases as well as reduction of anginose symptomatics in more than 50 per cent in 50 per cent of the patients, reduction of stenocardiac symptomatics to 50 per cent in 3.3 per cent of the cases and no effect in 16.7 per cent of the patients. The hemodynamic molsidomin effect was conditioned by the increase of the volume of the venous vascular system, due to which the venous flow to the heart was diminished--a reduction of the overloaded myocardium and its oxygen needs occurred.

[Clinical and hemodynamic characteristics of the decompensation syndrome in patients with congestive cardiomyopathy].

Thirty seven patients were clinically and hemodynamically studied under hospital conditions. The stage of cardiomegaly, the stasis in the pulmonary and systemic circulations were followed up as well as the presence of manifestations of pulse deficit, incidence of gallop rhythm manifestation. It was established that in the patients with II-A stage of decompensation of circulation, a decreased cardiac output was found, enhanced peripheral resistance and normovolemia. In the patients with II-B stage of circulation decompensation, further decrease of stroke and minute volume was found as well as moderately slowed down blood flow in the pulmonary and systemic circulation and normal peripheral vascular resistance. Conclusions were drawn, on the base of the data obtained, about the optimal treatment depending on the circulation decompensation stage.

[Urinary catecholamine excretion and cardiac output in hypertensive patients].

Cardiac output was determined to 70 patients with hypertonic disease I, II and III stage by dye-dilution method and urine excretion of adrenaline and noradrenaline was determined by fluorescence method. The extent of stroke and minute volume was determined depending on the stage of the disease, degree of arterial hypertension and patients' age. Noradrenaline excretion was normal in 75.8 per cent from the whole group of the patients examined and decreased in 24.2 per cent. Adrenaline excretion was increased in 7.1 per cent and with normal values--in 92.9 per cent of the patients. No discrepancies in adrenaline and noradrenaline excretion were found in the patients with hypertonic disease, depending both on the degree of arterial hypertension and patients' age. A moderate positive correlation was found between noradrenaline excretion and cardiac index and stroke index.

[Peroral use of a lidocaine preparation].

The authors studied the therapeutic effect of lidocainee peroral forms in case of rhythm disorders, accompanying with various cardiac diseases. The results obtained revealed that in 76 per cent of the cases with ventricular extrasystoles, the treatment result was very good, with complete disappearing of extrasystoles. Good effect was found in 18 per cent, with the recording of some rare, single extrasystoles towards the end of the treatment, and the treatment had no effect on 6 per cent of the patients. The drug proved to be effective in the cases with supraventricular extrasystoles. Lasting result was attained in 89.36 per cent of the patients treated. Due to the favourable therapeutic effect and good tolerance the authors recommended lidocain peroral form for wide application in the treatment of rhythm disorders.

[201T1 study of regional myocardial perfusion using a gamma counter and the EC1020 computer].

Myocardial perfusion was followed up after 500-1000 muCi 201TI-chloride injection at a gamma camera Pho-Gamma HP, Nuclar, Chicago with 4060 channel analyser and the images, according to the programmes of the authors, were processed with a computer type EC 1020. Some of the patients were examined by removing the detector, aiming at magnifying of the image and enhancing the differentiation ability under I cm. In some borderline cases, the patients were examined under physical loading and at rest for the differentiation of the normal from the disperfusion myocardial zones. Ninety patients were examined with myocardial infarction, obs. myocardial infarction, stenocardia and left bundle branch block. New additional data were obtained, in all the patients as: infarction zones, hypoperfusion foci, mosaic image of myocardium, vizualization of microinfarction, etc. The processing of the images of the regional myocardial perfusion with 201TI from gamma camera by computer, is a new, considerably higher stage of radioisotope diagnostics.

[Clinical dynamic ECG observations of the physical load tolerance of myocardial infarct patients].

Dynamic ECG observations are reported after a full day interval motor programme, applied with four motor regimes on patients with myocardial infarction under clinical conditions. The motor programme was used for the treatment of 241 patients, distributed into 2 groups. Observation on the effect of peak physical loading at the end of each motor regime was also reported. The authors concluded that the tolerance to physical loading was adequate, being confirmed by the absence of significant ECG dynamics even with the peak loadings, performed at the end of all motor regimes.

[Retrospective assessment of the preinfarction period].

The prodromal symptoms were retrospectively assessed in 100 patients with acute infarction of myocardium. The authors found preinfarction state in 48 per cent of the patients. In 95 per cent of the cases pain paroxysm was the basic manifestation. In 60,42 per cent it was accompanied by rhythm disturbances and to a lesser degree by some other symptoms: sense of lack of air, unusual fatigability, conductivity disturbances, vegetative manifestations, etc. In 43,75 per cent of the patients, electrocardiograms were recorded and in 80,95 per cent a lowered, bi-phase or negative T-wave was found and in 47,62 per cent--S--T depression.

[Intracardiac hemodynamic characteristics in high-degree mitral stenosis and an assessment of the indications for its surgical treatment].

The data from the right cardiac catheterization, echocardiography M-type and radiocardiography were juxtaposed in 54 patients with "pure" or predominating severe mitral stenosis (MS)(mitral valvular opening under 1 cm2)--confirmed at operation, in the determination of their functional and hemodynamic characteristics. The patients were grouped into three groups depending on the degree of pulmonary--capillary pressure increase (PC)--greater than 2.666, greater than 3.999 and 4.132 kPa(greater than 20, greater than 30 and less than 31 mmHg). The first group covering 28 per cent of the patients with a mean PC--2,399 +/- 0.257 kPa = (18 +/- 1.93 mmHg) are of particular interest. A light to moderate increase of pulmonary arterial pressure, elevated TPVR, absence of right ventricular insufficiency with reduced M. V SI resp. were established in them. It was assessed as a group with hypovolemia and proper preoperation preparation but requiring attention as regards the eventual hypotension, intra- or post operation. The third group covers 22 per cent of the patients and is characterized with increased TPVR, high hypertension in pulmonary artery, elevated telediastolic pressure in right ventricle and normal or lightly increased MO, assessed as a group with an inadequate diuretic therapy pre-operatively. The second group is with typical hemodynamic characteristics of severe MS and includes 50 per cent of the patients. The analysis forces the conclusion, that for an adequate evaluation of hemodynamics and MS severity, the PC pressure is necessary to be always discussed in connection with blood volume and flow, as well as with the other hemodynamic indices, in order to avoid the omissions as regards MS operability.

[Causes of early mortality in myocardial infarct].

The causes of lethality were studied in 49 patients with acute myocardial infarction of an average age 71.24 +/- 12.12. that died within the first 6 hours after their admission to hospital in the period 972-1979. They represent 15.46 per cent of all patients with infarction that died during that period. The anterior localization of the infarction (extent of necrosis), the repeated myocardial infarction and existing cardiac insufficiency are of significance for the lethality out of the factors studied. The acute rhythm and conductivity disturbances are the cause for the lethality in 38.78 per cent of the patients, acute cardiovascular failure (shock and/or pulmonary edema)--52.02 per cent, whereas rupture of the ventricular wall and pulmonary embolism are rare--8.16 per cent and 2.04 per cent respectively. During the first hour of hospital admission--30.64 per cent of the patients died, whereas during the rest of the hours up to the sixth hour the lethality was respectively--18.38%, 12.24%, 20.4%, 8.16% and 10.20% of the patients. Some of the other factors of significance for the early lethality were also analyzed and some measures for their prophylaxis or reduction of intensity of their effect were recommended.

[Comparative evaluation of the radiocardiographic and the dye dilution methods of determining cardiac output and circulating blood volume].

The authors compared the radiocardiographic and dye-dilution methods for hemodynamics determination. Cardiac capacity and volume of circulating blood were determined in 42 patients with atherosclerotic myocardiosclerosis and acquired valvular defects. A very high correlation dependence was established between the two methods. The correlation coefficient for minute volume is r = 0.93, for cardiac index it is r = 0.91 and for beat volume--r = 0.95 (P less than 0.001). A high correlation coefficient was established for the volume of the circulating blood--r = 0.86 (P less than 0.001). The authors admit that both methods are equally accurate and completely interchangable.

[Changes in myocardial contractile function and in the hemodynamic indices in patients with myocardial infarct before and after beta-blockader treatment].

The changes in the basic hemdynamic indices and in the phase structure of the cardiac cycle in patients with acute infarction prior to and post treatment with beta-blockers (Obsidan and Trasicor) were studied. Manifested changes in the hemodynamics were established, namly: pulse rate and cardiac flow decrease, the total peripheral resistance grows; veloqcilty of blood flow is slowed down. The preparations used have a negative effect upon the phase structure of left ventricle as well. The authors underline that beta-blockers administration in acute myocardial infarction should be evaluated very carefully and in the presence of clinical signs for cardiac insufficiency, it should be carried out on the background of cardiotonic treatment.

[Analysis of hospital mortality in myocardial infarct].

Hospital lethality was studied in myocardial infarction which was 24.9 per cent for the period 1973--1976 with a decreasing tendency during the last two years. The study covers 134 deceased of myocardial infarction at an average age of 70,78, their pathologoanatomical findings in the coronary arteries and localization of the nectrotic zone. In 49,26 per cent the case was that of repeated myocardial infarction, thus raising the lethality in the first 24 hours after hospital admission with 70,9 dead up to the 7th day. The analysis of the lethality causes revealed that according to the relative share they were as follows: acute cardiovascular failure, cardiac rupture, acute rhythm and conductivity disturbances, pulmonary embolism, inflammatory lung diseases, etc. No correlation was found between arterial hypertension, cigarette smoking and diabetes mellitus and the leading lethality cause.
