Intramural hematoma of the duodenum.

Five new cases of intramural duodenal hematoma are reviewed with 174 cases derived from the literature. Operative therapy with simple evacuation of the hematoma is advocated as the procedure of choice. Bypass procedures and resection should be reserved for selected cases.

Reconstruction of defects of the mandible.

A series of 14 mandibular reconstruction in 13 patients with chips of autogenous, cancellous bone and marrow supported by a titanium mesh tray is presented. The mandibular defects were the results either of operation for carcinoma of the tongue and the floor of the mouth or trauma. Satisfactory rehabilitation was accomplished in 11 patients. The advantages of this technique are ease of accomplishment, ability to replace long and awkardly shaped segments, less shrinkage due to bone resorption, and shorter intermaxillary fixation.

Effect of norepinephrine on gastric wound healing.

Lavage of the stomach with norepinephrine has been suggested as a temporary means of controlling upper gastrointestinal tract bleeding. The effect of norepinephrine on the healing of a standard gastric incision in the rat was studied using irrigation topically. When compared with those rats in the control study that were irrigated with a saline solution alone, no significant difference was found in the tensile strength, local inflammatory response or synthesis of new collagen, as determined by the hydroxyproline-proline ratio. There would appear to be no adverse effect on gastric wound healing as a result of preliminary irrigation of the gastric mucosa with norepinephrine given topically in a concentration of 16 milligrams per liter.

Parathyroid cysts: Functional and mediastinal.

A report of a functioning mediastinal parathyroid cyst is presented with a review of the world literature. This represents the tenth functional, eleventh mediastinal, and third functioning mediastinal parathyroid cyst reported. The predilection of functional cysts for aberrant locations would suggest on hypothesis of their occurrence by degeneration of pre-existing adenomas based on an inadequacy of their blood supply. The criteria for ascribing functional activity to a parathyroid cyst are presented and discussed.

Physiologic anatomy of the palmar circulation in 200 normal hands.

The functional capacity of the Superficial Palmar Arch (SPA) was evaluated in 200 normal hands by a non-invasive narrow beam ultrasonic technique. Peak systolic velocity of flow with radial artery occlusion increased in all SPAs except for 22 (11%) which were considered incomplete. Ulnar artery occlusion resulted in a more variable response including 89 (45%) SPAs with bidirectional flow. SPA blood flow is complex but understandable in terms of hand blood flow and vascular bed resistance. The use of tobacco does not increase the incidence of incomplete SPAs.
