Synchronous bilateral oncocytoma of the parotid glands. A case report.

A case of synchronous bilateral oncocytoma arising in the parotid salivary gland is presented and the literature is reviewed. The oncocytoma forms a small group of neoplastic diseases in the parotid gland and synchronous bilateral tumors are relatively rare. According to the available literature this is the fourth case reported.

Rectal mucosal biopsy in aganglionosis and allied conditions.

Rectal mucosal biopsies were performed in 146 patients suspected of having Hirschsprung's disease. The biopsies were serially sectioned and screened for the presence of ganglion cells. In the 101 cases in which ganglion cells were found, the diagnosis of Hirschsprung's disease was ruled out. In 45 patients, histologic examination revealed the typical features of Hirschsprung's disease, i.e., the absence of ganglion cells and the presence of numerous hypertrophied nerve bundles. The varied morphologic features of Meissner's plexus in patients of different ages and the diagnostic pitfalls are described in detail. Serial frozen sections stained with hematoxylin-eosin were examined in eight cases of neonatal intestinal obstruction and enterocolitis. This method was found to be useful and reliable in emergencies. In three of these cases ganglion cells were found and major surgical procedures were avoided. In 73 cases, staining for acetylcholinesterase activity was performed. The results matched the microscopic findings in all but three false-positive cases.

Mycobacterium fortuitum granulomatous synovitis caused by a dog bite.

Mycobacterium fortuitum was isolated from the synovial tissues of the dorsum of the hand after a dog bite. Caseating granulomas with Langhans' type giant cells were demonstrated microscopically. Healing followed prolonged treatment with antituberculosis drugs. It is suggested that the organism was forced into the patient's tissues by the dog's bite.

Myelofibrosis in young adults.

3 young adults with myelofibrosis, a rare disease in this age-group, are presented. Their clinical course was distinguished by a virtually asymptomatic state at time of diagnosis and a stable, or very slowly progressive course over a 4- to 5-year follow-up. Myelofibrosis in young adults may pursue a more benign course than that usually noted in older populations.

Pelvic abscess associated with a Lippes loop. An unusual case.

An unusual case occurred of Lippes-Loop-associated pelvic abscess, characterized by a relatively mild clinical course and an unusual localization in the pelvis. This case emphasizes the importance of considering the association between intrauterine devices and pelvic abscess.

Methyldopa-induced immune hemolytic anemia and chronic active hepatitis.

Immune hemolytic anemia and chronic active hepatitis developed concurrently in a 76-year-old man after prolonged therapy with methyldopa. Both complications disappeared following cessation of treatment with the offending drug and a short course of a high-dose corticosteroid (prednisone). Inhibition of T-suppressor lymphocyte function by methyldopa may be instrumental in the emergence of autoimmune complications following usage of the drug.

Tumour of the nose as a presenting feature of leukaemia.

A skin nodule on the external nose, as a presenting feature of acute myeloid leukemia, is described. The child presented with a lesion on the tip of the nose together with the clinical and haematologic features of leukaemia. The histological finding of the nodule was found to be a characteristic leukaemia cutis with leukaemia cell infiltrate in the skin. Epistaxis and external ear infection were also additional features of E.N.T. system involvement, as we find in other leukaemic patients. Since starting chemotherapy the patient has been well, in complete remission, and the lesion on the nose has almost disappeared.

Severe poisoning in a child by the mushroom Inocybe tristis, Malençon and Bertault.

Following ingestion of the mushroom Inocybe tristis, a 2 1/2-yr-old girl had sudden onset of diarrhea, vomiting, stupor and myosis. Subsequently, she developed excessive hypertension, a purpuric rash and transient hepatic, muscular, renal and cutaneous damage. The patient completely recovered after supportive therapy. This is an instance of severe multisystemic toxicity caused by the mushroom Inocybe tristis, Malençon and Bertault, previously unreported.

Primary malignant melanoma of the small intestine and the APUD cell concept.

A primary melanoma of the small intestine is reported. A possible explanation of its origin according to the APUD cell concept is suggested. This concept may lessen the doubts and confusion whenever an intestinal melanoma is discovered and a primary tumor at one of the more common sites can reasonably be excluded.
