A randomized trial of weekly cardiotocography in high-risk obstetric patients.

A randomized clinical trial of once-weekly antenatal fetal heart rate monitoring in 539 high-risk patients could find no benefit of monitoring in terms of perinatal mortality, morbidity or Apgar score. The previously well-documented association between abnormal antenatal fetal heart rate traces and low Apgar score was confirmed. A detailed case review showed that in this population monitoring was irrelevant to almost all of the 13 perinatal deaths.

Immunosuppression in toxoplasmosis: studies in sheep with vaccines for chlamydial abortion and louping-ill virus.

Toxoplasma gondii infection was established in sheep seven days (acute infection) or 28 days (chronic infection) before they were given enzootic abortion (EAE) vaccine alone or simultaneously with louping-ill (LI) vaccine. Uninfected controls received the same vaccination schedule. The serological response of acute toxoplasma-infected sheep to initial LI vaccination was significantly depressed but after revaccination four weeks later these sheep demonstrated a response equal to that of the revaccinated controls. Infected sheep also gave a significantly poorer response to EAE vaccine than control animals, the effect being more marked in those with chronic infection. Revaccination evoked no clear response in any group but on three of five sampling days antibody titres of controls were significantly higher than those of infected sheep.

A controlled trial of fetal heart rate monitoring in a low-risk obstetric population.

A controlled trial of fetal heart rate (FHR) monitoring in a low-risk obstetric population was carried out in 989 patients at the Mercy Maternity Hospital and Queen Victoria Medical Centre. The use of monitoring failed to demonstrate any improvement in perinatal outcome. Monitoring was associated with an increased intervention rate and a small increase in the number of babies who remained in the isolette beyond 3 days. This study does not support the view that FHR monitoring should be used in all labors. Further study in a larger sample is recommended.

Lumbar epidural morphine as an effective analgesic following cholecystectomy.

Relief of pain with epidural morphine was evaluated in five patient subjects during two consecutive twenty-four periods after cholecystectomy. In one period, each subject received lumbar epidural morphine, first 4-6mg, and twelve hours later, 2-3 mg; in the other period, epidural placebo at the same times. Except for four hours before each injection and twenty minutes thereafter, intramuscular morphine was administered as required throughout. The experiments were double-blind. Epidural morphine, unlike epidural placebo, reduced both a visual pain analogue score (p less than 0.05) and a pain questionnaire score (p less than 0.01) twenty minutes after injection. Epidural morphine compared to placebo reduced by one-half the total amount of narcotic (epidural plus intramuscular) administered over the twenty-four hour period (p less than 0.05). Four of five subjects clearly preferred analgesia with epidural morphine over the effect of placebo plus therapeutic doses of intramuscular morphine. We conclude that epidural morphine, administered in this manner, is effective in relieving pain after cholecystectomy and that it may be preferred by patients over conventional intramuscular morphine.

Sensitivity of Neisseria gonorrhoeae to partially purified R-type pyocines and a possible approach to epidemiological typing.

Strains of Neisseria gonorrhoeae from a variety of sources were examined for sensitivity to 11 partially purified R-type pyocines from Pseudomonas aeruginosa. Selective inhibition of gonococci by pyocines of Kageyama groups R1 and R5 was observed. "Matched isolates", those from consorts or different body sites of individual patients, usually had very similar pyocine-sensitivity patterns and identical sensitivities to five antibiotics tested. This study included local isolates, strains from diverse geographic regions, and strains from disseminated gonococcal infections. It also proposed a relationship between pyocine-receptor sites in the lipopolysaccharide of Ps. aeruginosa and N. gonorrhoeae. Topics needing further evaluation are discussed.

Bone lesions in systemic acne (acne fulminans).

An 18-year-old West Indian male presented with severe sternal pain and an exacerbation of facial acne. Radiographs of the sternum revealed several lytic lesions which appeared as hot areas on successive technetium bone scans. Painful areas over the right iliac crest and left greater trochanter likewise appeared as transient hot areas on successive scans. Histology of affected bone revealed reactive changes only. High dose prednisolone provided rapid alleviation of pain, which recurred on reducing the dose to less than 10 mg daily. Auto-immune complex disease has been considered the most likely aetiological mechanism of systemic acne (acne fulminans), but lytic lesions of bone have never previously been reported in auto-immune disorders.

Agarose-bound horse-liver alcohol dehydrogenase. Dependence of molecular properties and activity on coupling conditions.

1. Spectroscopic methods for protein and active-site determination with the same sample of immobilised horse liver alcohol dehydrogenase have been developed. 2. The influence of pH, active-site protection of the soluble enzyme and protein concentration on coupling of alcohol dehydrogenase with cyanogen-bromide-activated Sepharose has been investigated. In phosphate buffer (pH 8.0) products with over 90% active-site retention have been synthesized. The binary complex alcohol-dehydrogenase . NADH gives a preparation with the same active-site content but a lower apparent specific activity compared to the unprotected enzyme. Increase in protein concentration yields products with the same active-site content relative to bound protein but the apparent specific activity is decreased. 3. The great similarity in spectroscopic properties of soluble and immobilised enzyme, as well as of their ternary complexes, shows that no significant conformational change has taken place during immobilisation. 4. Exchange of the non-catalytic Zn2+ against Co2+ yields a hybrid Sepharose--Co2Zn2-alcohol-dehydrogenase with over 90% active-site retention during metal exchange. The absorption spectra of the soluble and immobilised hybrid are identical.

Continuous measurement of tissue pH in the human fetal scalp.

A method for continuous measurement of scalp tissue pH is described. The tissue pH probe was found to be robust and values for tissue pH were close to those for scalp blood pH. Combining the pH and fetal heart fate (FHR) electrodes in the one mechanical assembly facilitated application to the fetal scalp in early labour but the combined assembly electrode was found to have some disadvantages and manufacture of a separate tissue PH electrode is recommended. Continuous monitoring of scalp tissue pH enables closer study of the physiological basis of changes in fetal acid base status and should prove useful to the obstetrician in management of high risk pregnancies during labour.

The reduction of the intrapartum still birth rate at the Queen Victoria Memorial Hospital.

A significant reduction in intrapartum anoxic fetal deaths has occurred at the Queen Victoria Memorial Hospital over the last ten years. The possible explanations for this include a variety of factors, which may either improve the health of the population presenting to the hospital, or improve obstetric care. Epidemiological evidence suggests that the use of fetal diagnostic techniques has contributed to the reduction of intrapartum anoxic still births.

Controlled trial of fetal intensive care.

A fetal intensive care unit was formed at the Queen Victoria Memorial Hospital in 1972. Because of some doubt concerning the value of fetal intensive care, a controlled clinical trial including all high-risk patients was performed. The trial clearly showed that intensive care is associated with improved neurologic and biochemical status of the neonate; however, it is possible that this improvement results from the use of fetal diagnostic tests or some other factor associated with intensive care. Sufficient evidence was gathered to warrant the continuation of fetal intensive care in this hospital, but in other contries, where funding is difficult to obtain, a controlled trial would appear justified.

Community fieldwork collaboration between medical and social work students.

This article describes an educational demonstration in interdisciplinary community field experiences between social work and medical students at the University of Kentucky Medical school, the hospital social service department, the school of social work, and community social and health agencies. The pilot program in which students from different professions lived for 6 weeks in outling rural communities served by the medical center, provided an opportunity: (a) to study the feasibility of combining a borad community health study experimence with casework services in a hospital-based educational program and (b) to assess the benefits of early interdisciplinary community and clinical work between future doctors and social workers. Students saw the advantages of cooperative teamwork in studying community problems, and were able to apply classroom theory about community organization to real community situations. The organizational structure and staff resources required to carry out such a demonstration are described, and the implications of the training project are discussed.

Technique for determining nickel in blood by flameless atomic absorption spectrophotometry.

We describe a technique for determining nickel in serum by flameless atomic absorption, with use of dimethylglyoxime. The procedure permits a greater number of analyses per technician at a relatively low cost, yet provides acceptable accuracy and reliability. After the sample is evaporated and dry ashed in a muffle furnace at 560 degrees C for 5 h, the residual salts are dissolved in hydrochloric acid (1 mol/liter). Sodium citrate and dimethylglyoxime are then added for complexation of iron and nickel, respectively. The pH is adjusted to 9 with ammonia and the nickel dimethylglyoxime is extracted with methyl isobutyl ketone. The determination is carried out by injecting 50 mul of the organic phase into the graphite furnace, where the sample is dried at 120 degrees C and ashed at 1200 degrees C, respectively.
