Primary malignant melanoma of the bronchus.

A case is presented of a 55-year-old woman with fever, cough, chest pain, and hemoptysis. A chest x-ray showed a large mass in the right upper lobe of the lung. Bronchoscopy and bronchial biopsies revealed malignant melanoma of the bronchus. Careful search of all common sites for melanoma and the histological examinations of the two skin lesions failed to substantiate the possibility of an extrapulmonary origin and, thus, by deduction it can be assumed with some certainty that this lesion is primary bronchial melanoma.Total pneumonectomy, when there is no evidence of extrapulmonary extension, coupled with adjuvant chemotherapy seem to offer a chance of cure. The patient presented is the first reported case of primary malignant melanoma of the bronchus from the University College Hospital, Ibadan, and perhaps the first reported case in a Nigerian.

Scimitar syndrome with associated Morgagni hernia in a Nigerian infant.

A Nigerian infant with symptoms of moderate respiratory dysfunction since birth is described. The diagnosis of the scimitar syndrome was made intraoperatively. The presence of an associated foramen of Morgagni hernia has not been reported previously. The relevant literature is reviewed, with emphasis on the preoperative diagnostic investigations essential in planning and executing appropriate surgical therapy.

Surgery for congenital heart disease in Ibadan. I: Non-cyanotic heart defects.

Congenital heart disease represents about 5% of all cardiovascular diseases in Nigeria today. Its incidence has increased in the last decade because of improved neonatal care, increased awareness and clinical acumen of physicians, better diagnostic tools and the introduction of newer techniques in cardiac catheterization. It is now possible to make the diagnosis with certainty and to define the extent of the pathology thus making it possible to select the appropriate surgical therapy for these children. This article reviews the commonest types of noncyanotic heart disease, the indications for surgery and the types of surgical procedures available in Nigeria today. The simple extracardiac defects, such as patent ductus arteriosus and coarctation of the aorta can be corrected with little or no mortality, but the more complex intracardiac defects can only be palliated pending the introduction of open heart surgery in Nigeria.

Endocrine manifestation of pulmonary carcinoma in a Nigerian.

Bronchogenic carcinoma is an uncommon neoplasm in the Nigerian. Endocrine manifestations of pulmonary neoplasm are even more uncommon in Nigerians or elsewhere. The association of gonadotrophin activity with an anaplastic carcinoma of the lung in a 25-year-old Nigerian housewife forms the basis for this communication. The successful removal of a left lower lobe neoplasm in the patient who had manifested with menometrorrhagia and abnormal breast lactation was followed by a normal pregnancy and delivery of a normal baby boy within 12 months following the lobectomy.
