[Sympathetic-adrenal system in various types of anesthesia in surgical treatment of gastric and rectal cancer].

Under study was the functional activity of the sympatho-adrenal system in identical surgical interventions in 44 gastric cancer patients and 77 rectal cancer patients, operated under nitrogen monoxide-ether, nitrogen monoxide-fluothane anesthesia and under neuroleptanalgesia supplemented with nitrogen monoxide. The sympatho-adrenal activity was assessed, taking into account the content of total catecholamines (adrenaline and noradrenaline) in blood and urine prior to surgery, intraoperatively before anesthesia, during the most traumatizing moment of the operation and at 1, 3, 6 day after it. It was found that in identical operative procedures stimulation of the sympatho-adrenal system is mostly manifest in patients operated upon under either anesthesia and neuroleptanalgesia. Use of fluothane narcosis in gastric cancer patients produced no marked activation of the sympatho-adrenal system, while in rectal cancer patients it was not so significant as in either narcosis and neuroleptanalgesia.

[Transplantability and growth of sarcoma 37 under a ganglionic blockade].

The inoculation of sarcoma-37 against the background of the ganglionary blockade, provided by the intraperitoneal injection of 1 ml of pentamine, is not associated with its less take rate, but there is a manifest suppression of the tumor growth during the period up to 63 days. A preliminary ganglionary blockade completely eliminates the stimulating effect of a trauma on the tumor growth.

Amputation, Surgical↗

[Relapse symptom complex in rectal cancer after radical surgical interventions].

The analysis of 78 questionnaires of patients with recurrence of rectal cancer was performed which revealed this complication of surgery to occur in 60% of cases within a year after operation. Recurrences of the rectum cancer show a characteristic complex of symptoms with a certain structure of painful sensations as a central criterion.

[Role of nonspecific accompanying pulmonary pathology in altering the external respiratory function in lung cancer].

In 110 lung cancer patients the authors have determined the rate of separate distension of the lung and chest (DL--C), and in 60 of them radiopneumography with Xe133 was employed. All patients were subdivided into 3 groups: I--those suffering chronic concomitant lung affections, II--those with acute pulmonary diseases in the anamnesis, III--those showing no signs of concomitant pathology. It was found that the previous diseases (pneumonia, acute bronchitis) render no significant effect on the external respiration function (as evidenced by radiopneumographic findings). Whereas concomitant pathology (chronic bronchitis, chronic pneumonia, etc.) would aggravate the function considerably, that is especially pronounced in central tumor localization. In patients of the group I and II DL--C was found to be markely reduced, that may be due to asymptomatic pleurofibrosis in the group II. To estimate the possibilities of surgical treatment for lung cancer, a detailed complex diagnosis of concomitant nonspecific pulmonary pathology seems to be absolutely necessary.

[State of circulating blood volume, blood components and the function of external respiration in lung cancer].

The data concerning the changes in the circulating blood volume, globular and plasma volumes, red blood cells indices and also some indices of the external respiration function in lung cancer cases are set forth. Radiopneumography has permitted to find out some significant disorders in the regional ventilation and in the pulmonary blood flow in early postoperative period, which in case of hypoventilation is likely to cause a severe respiratory insufficiency and, therefore, requires an energetic therapeutic control.

[Effect of anesthesia on the results of the overall treatment of breast cancer].

The authors have studied the 5 years' survival of 403 cases of mammary gland cancer without development of recurrences and metastases. In 205 out of them ether-nitrous oxide anesthesia and in 198 fluoroxene-nitrous oxide anesthesia were applied. Postoperative Thiophosphamide (Thio-TEP) was administered to all the patients. The main factors characterizing the compared groups were identical in both of them. It has been proved that 5 years survival of mammary gland cancer cases rises when the patients undergo a complex treatment, and during the operation they are subjected to fluoroxene and not to ether.

Anesthesia, Inhalation↗

[Types of anesthesia and the late results of breast cancer treatment].

The author studied the remote results of the treatment in 1358 breast cancer patients, 554 of them were operated under ether-nitrogen-oxygen anesthesia and 804--under fluothane-nitrogen-oxygen anesthesia. The patients were subjected only to the Halsted mastectomy. Both groups of patients were identical in age and the degree of tumor spread. It is shown that late results of the treatment in breast cancer patients operated under fluothane narcosis are much better than those under ether narcosis.

[Anesthesia and distant results of surgical treatment in patients with stomach cancer].

Based on the study of the late results of surgical treatment in 373 patients with gastric cancer, subjected to radical subtotal gastrectomy, it was found that a group of patients operated under fluothane anesthesia shows the highest survival rate. As evidenced by the authors' findings the use of ether narcosis results in 16--27% reduction of yearly survival during the first five years. The most poor results were noted in a group of patients operated under neuroleptanalgesia.

[Hemodynamic indices and the volume of circulating blood in the surgical treatment of cancer of the esophagus and cardial portion of the stomach].

The authors report the data on changes in some indices of the hemodynamics, red blood, total blood serum protein, the circulatory volume and amount of the operative blood loss in radical surgical treatment of patients with cancer of the esophagus and cardia. Depending on the kind of anesthesia all patients were subdivided into three groups: in the first group ether-nitrous oxide-oxygen endotracheal narcosis was employed, in the second-fluothane-nitrous oxide-oxygen endotracheal narcosis and in the third-neuroleptanalgesia.

[Changes of peripheral blood flow in patients with cancer of the lungs, esophagus and cardia under the effect of anesthesia and surgery].

The authors report the data on changes in peripheral circulation, depending on the kind of anesthesia, the amount of operative blood loss, alterations in circulating blood volume and localization of tumor in patients with cancer of the lung, esophagus and cardia. Totally, 35 patients (15--lung cancer, 10--esophageal cancer, 10--cancer of the cardia) were explored. In 25 cases fluothane was used as the main anesthetic, in 10--neuroleptanalgesia. The state of peripheral circulation was estimated by the period semielimination of radioactive xenon-133 from the muscle depot. It was found that alterations in peripheral circulation were not dependent on tumor localization, the use of fluothane and neuroleptanalgesia is associated with similar changes in peripheral blood flow and, finely, an increased loss of blood results in more pronounced alterations in peripheral circulation.
