[Predictive value of detection of sentinel lymph nodes on lymphatic metastasis of non-small cell lung cancer].

BACKGROUND & OBJECTIVE: The metastasis status of regional lymph node is an important prognostic factor of non-small cell lung cancer (NSCLC). Sentinel lymph node (SLN) mapping and biopsy is a quick and high efficient technique to intraoperatively detect occult micrometastatic disease, however, its application in NSCLC is immature. This study was designed to investigate the feasibility of detecting SLN in patients with NSCLC during radical surgery, and to evaluate its accuracy of predicting metastasis status of regional lymph node. METHODS: Fifty patients with NSCLC underwent SLN detection. During radical operation, 4 ml of 1% isosulfan blue was injected into the lung tissue around the tumor at 3, 6, 9, and 12 o'clock sites. Location and number of blue dyed SLNs were recorded, and compared with pathologic results to calculate the accuracy and false negative rate of SLN detection. RESULTS: Blue dyed SLNs were seen in 33 patients with a detection rate of 66.0%. SLNs located in N1 lymph node of 24 patients (72.7%), in N2 lymph node of 6 patients (18.2%), in both N1 and N2 lymph nodes of 3 patients (9.1%). Approved by pathology, the accuracy of SLN detection was 87.9% (29/33), the sensibility was 73.3% (11/15), the false negative rate was 26.7% (4/15). CONCLUSION: SLN detection is valuable for predicting hilar and mediastinal lymph nodes metastases in NSCLC.

[Clinical value of cervical mediastinoscopy in non-small cell lung cancer of stage I].

BACKGROUND & OBJECTIVE: Whether to apply routine mediastinoscopy to non-small cell lung cancer (NSCLC) of stage I remains controversial. This study was to evaluate application of cervical mediastinoscopy in detecting mediastinal lymph nodes status of NSCLC of stage I. METHODS: The cervical mediastinoscopy was performed in 31 patients with NSCLC of stage I prior to thoracotomy from Oct. 2000 to Feb. 2004. Mediastinoscopy-positive (stage N2) patients received neoadjuvant chemotherapy. RESULTS: Of the 31 patients, 2 had disease of stage N2, and 1 had disease of stage N3 (right scalenus lymph node metastasis); positive rate of mediastinosopy was 10% (3/31), sensitivity, specificity, and accuracy of mediastinoscopy were 75%, 100%, and 97%, respectively. CONCLUSION: Mediastinoscopy might be a routine examination for patients with NSCLC of stage I.

[Relationship between cytokine gene polymorphism and susceptibility to hepatitis B virus intrauterine infection].

OBJECTIVE: To explore the possible relationship between cytokines (TNF-alpha, IFN-gamma, IL-4 and IL-10), which were expressed abnormal quantity in the peripheral blood to intrauterine HBV infectious children, gene single nucleotide polymorphism (SNP) and susceptibility to HBV intrauterine infection. METHODS: A cross sectional study on molecular epidemiology was carried out. The subjects were selected from outpatients of the hepatitis B vaccine special clinics of our hospital. According to intrant criteria, children under high risk of HBV intrauterine infection were divided into immuno-failure group (group I) and immuno-effective group (group II) while children without high risk were included in the control group. Four gene SNP sites of TNF-alpha-238, IFN-gamma + 874, IL-4-590 and IL-10-1082 region were determined by real-time quantitative fluorescent PCR. RESULTS: Significant differences of TNF-alpha-238 A allele frequency were found between group I and group II (chi(2) = 6.797, P < 0.05) as well as between group I and control group (chi(2) = 9.513, P < 0.05). No evident difference of TNF-alpha-238 A was found between group II and control group (chi(2) = 0.047, P > 0.05). Significant differences of IFN-gamma + 874 A allele frequency were found between group I and group II (chi(2) = 7.238, P < 0.05), and between group I and the controls (chi(2) = 5.199, P < 0.05) but no significant difference was found between group II and control group (chi(2) = 0.602, P > 0.05). Significant differences of IL-4-590 C/T allele frequency were not found between group I and group II (chi(2) = 0.632, P > 0.05), group I and control group (chi(2) = 0.584, P > 0.05), or between group II and control group (chi(2) = 0.004, P > 0.05) respectively. Significant differences of IL-10-1082 G allele frequency were found between group II and group I (chi(2) = 10.359, P < 0.001), and between group II and the controls (chi(2) = 35.418, P < 0.001), but not found between group I and control group (chi(2) = 1.759, P > 0.05). CONCLUSION: This study suggested the possibility that TNF-alpha-238 A allele and IFN-gamma + 874 A allele were associated with HBV intrauterine infection. There was no evident relationship between IL-4-590 C/T allele SNP and susceptibility to HBV intrauterine infection, but the IL-10-1082 G allele seemed to be associated with preventive efficacy to HBV intrauterine infection.

[Vascular endothelial growth factor expression and microvessel density in stage I-II non-small cell lung cancer and their prognostic significances].

BACKGROUND & OBJECTIVE: TNM staging system is used widely to predict prognosis of non-small cell lung carcinoma (NSCLC) patients, but patients with the same stage may have very different survivals; better prognostic index is needed. Angiogenesis is considered to be essential for tumor development, progression, and metastasis, but the prognostic impacts of vascular endothelial growth factor (VEGF) and microvessel density (MVD) in NSCLC is controversial. This study was to evaluate the prognostic value of VEGF and MVD in NSCLC. METHODS: VEGF and MVD in 214 specimens of stageI-II NSCLC (20 in stage IA, 137 in stage IB, and 57 in stage IIB) were detected by tissue chip and SP immunohistochemistry. No patient underwent postoperative antitumor treatment. RESULTS: VEGF expression didn't relate to gender, age, blood type, pathologic type, and TNM stage (P0.05). MVD correlated with age and pathologic type (P0.05), but did not relate to gender, blood type, and TNM stage (P0.05). The mean value of MVD was 65.8+/-5.2 in VEGF-low patients, and 67.5+/-2.5 in VEGF-high patients (P0.05). The 5-year survival rate was significantly lower in MVD-high patients than in MVD-low patients (34.5% vs. 60.0%, P=0.013). Furthermore, multivariate Cox regression analysis showed that MVD (P=0.000) was an independent prognostic factor of NSCLC. CONCLUSION: High MVD closely relates to poor prognosis of stageI-II NSCLC.

[Experimental study of excitation of peripheral nerves in transverse electric field stimulation].

The classical cable function has been used to represent the response of peripheral nerves stimulated by external parallel electric field. Experiments show that peripheral nerves can be activated by perpendicular electric field induced by magnetic pulses, indicating that the activation mechanism needs to be thoroughly investigated. Several excitation properties of peripheral nerves in transverse-field were explored in the present paper. With a human's median nerve in vivo, stimulation threshold, excitation position and the relation between excitation threshold and fiber's radius were studied. The relation between stimulation threshold and stimulation duration was researched with sciatic nerves from toad in vitro. The experimental results verify that the modified cable function is much efficient than the classical cable function. The research will improve the nerve magnetic stimulating technique and be beneficial to further application.

[Modeling for activating peripheral nerves by transverse electric field].

The classical cable function has been used to represent the response of peripheral nerves stimulated by external parallel electric field. It can not describe the excitation of peripheral nerves stimulated by perpendicular electric field. In this paper, responses of the nodes of Ranvier to transverse-field are deeply investigated by mathematic simulation and in-vitro experiments. The paper demonstrates that, under perpendicular electric field stimulation, the responses evoke a two-stage process including an initial polarization and the actual change of the transmembrane potential. It is the net inward current along a radial direction of the node of Ranvier that causes the peripheral nerve excitation. Based on the two-stage process, a novel model is introduced to describe peripheral nerves stimulated by transverse-field, and the classical cable function is modified. The new model and the improved cable function are verified by several in-vitro experiments. They can be used to represent peripheral nerves responses by arbitrary electric field stimulation.

Decolorization of azo dyes with high salt concentration by salt-tolerant mixed cultures under anaerobic conditions.

Because the lack of detailed study of biological decolorization in high salt dye wastewater, it is still difficult to evaluate the biological treatment on high-salinity dye wastewater. The experiments were carried out to study the salt-tolerant bacteria, which is useful in the treatment of high-salinity colored wastewater. Simulated wastewater containing 5-150 g/L salt (NaCl) and 50-2000 mg/L Reactive Brilliant Red K-2BP was treated with three salt-tolerant mixed cultures (CAS, TAS, DSAS), which were under a gradually acclimated procedure. With the increase of concentrations of salt and dye, the decolorization became low. The abilities of decolorization of dyes wastewater by three mixed cultures (CAS, TAS, DSAS) were studied, CAS and DSAS mixed cultures showed more active for the treatment of high-salinity colored wastewater than TAS mixed cultures. The results suggested that there might be a simple process for the high salt wastewater treatment, which could be incorporated into conventional activated sludge plants.

Chemoenzymatic synthesis of CMP-sialic acid derivatives by a one-pot two-enzyme system: comparison of substrate flexibility of three microbial CMP-sialic acid synthetases.

Three C terminal His6-tagged recombinant microbial CMP-sialic acid synthetases [EC 2.7.7.43] cloned from Neisseria meningitidis group B, Streptococcus agalactiae serotype V, and Escherichia coli K1, respectively, were evaluated for their ability in the synthesis of CMP-sialic acid derivatives in a one-pot two-enzyme system. In this system, N-acetylmannosamine or mannose analogs were condensed with pyruvate, catalyzed by a recombinant sialic acid aldolase [EC 4.1.3.3] cloned from E. coli K12 to provide sialic acid analogs as substrates for the CMP-sialic acid synthetases. The substrate flexibility and the reaction efficiency of the three recombinant CMP-sialic acid synthetases were compared, first by qualitative screening using thin layer chromatography, and then by quantitative analysis using high performance liquid chromatography. The N. meningitidis synthetase was shown to have the highest expression level, the most flexible substrate specificity, and the highest catalytic efficiency among the three synthetases. Finally, eight sugar nucleotides, including cytidine 5'-monophosphate N-acetylneuraminic acid (CMP-Neu5Ac) and its derivatives with substitutions at carbon-5, carbon-8, or carbon-9 of Neu5Ac, were synthesized in a preparative (100-200 mg) scale from their 5- or 6-carbon sugar precursors using the N. meningitidis synthetase and the aldolase.

Self-protection: the advantage of radical oligomeric mixtures in organic synthesis.

Atom-transfer radical oligomers of allyl iodoacetates were converted to 4-pentenoic acids upon treatment with zinc. Reactions of the radical oligomers of various omega-alkenyl iodoacetates with Grignard reagents afforded the corresponding substituted tetrahydrofuran derivatives. These results indicated that radical oligomeric mixtures not only serve as versatile intermediates in organic synthesis, but also exhibit unique advantages in that the oligomeric mixtures are self-protected and the deoligomerization functions as the simultaneous deprotection.

Learnability-based further prediction of gene functions in Gene Ontology.

Currently the functional annotations of many genes are not specific enough, limiting their further application in biology and medicine. It is necessary to push the gene functional annotations deeper in Gene Ontology (GO), or to predict further annotated genes with more specific GO terms. A framework of learnability-based further prediction of gene functions in GO is proposed in this paper. Local classifiers are constructed in local classification spaces rooted at qualified parent nodes in GO, and their classification performances are evaluated with the averaged Tanimoto index (ATI). Classification spaces with higher ATIs are selected out, and genes annotated only to the parent classes are predicted to child classes. Through learnability-based further predicting, the functional annotations of annotated genes are made more specific. Experiments on the fibroblast serum response dataset reported further functional predictions for several human genes and also gave interesting clues to the varied learnability between classes of different GO ontologies, different levels, and different numbers of child classes.

[Treatment of unresectable esophageal carcinoma by stenting with or without radiochemotherapy].

OBJECTIVE: To evaluate the benefits of post-stenting radiotherapy and/or chemotherapy for unresectable esophageal carcinoma. METHODS: Fifty-three patients with unresectable esophageal carcinoma were randomly divided into two groups: patients in group A (n = 27) were treated with stenting alone, and those in group B (n = 26) were treated with stenting followed by radiotherapy and/or chemotherapy. Comparison was made by assessing their survival time, quality of life (QOL), degree of dysphagia, and stenting-related morbidity, respectively. RESULTS: There was no statistically significant improvement in dysphagia and QOL between the two groups. Although there was no difference in the frequency of stenting-related complications, re-stenosis occurred much less frequently in group B patients (P = 0.007). The mean survival time of patients was 245 +/- 41 days and 262 +/- 43 days in group A and group B, respectively. There was no significant difference between the two groups (P = 0.813). CONCLUSION: Besides decreased recurrence of stenosis, post-stenting radiotherapy and/or chemotherapy does not provide additional benefits for survival.

[Association of interferon-gamma + 874 gene single nucleotide polymorphism with susceptibility to intrauterine HBV infection].

OBJECTIVE: To explore the susceptibility of children to develop intrauterine hepatitis B virus (HBV) infection through studying the association between interferon gamma (IFN-gamma) + 874 single nucleotide polymorphism (SNP) and intrauterine HBV infection. METHODS: The subjects were selected from outpatients who were in our hepatitis B (HB) vaccine following-up clinics. The subjects whose mothers were HBV carriers were inoculated with HB vaccine or HB vaccine and hepatitis B immunoglobulin (HBIg). Intrauterine HBV infection was defined as peripheral blood HBsAg and/or HBV-DNA positive at birth and lasting for six months (group I). Normal immune children were defined as peripheral blood negative for HBV marker since birth and afterwards HBsAb titers were above protective level (group II). The subjects were composed of the following two groups. Group I consisted of 46 children with intrauterine HBV infection. Group II was composed of 73 normal children. A Taqman fluorescence polymerase chain reaction for the IFN-gamma + 874 SNP was performed for both groups. RESULTS: IFN-gamma + 874 SNP was tested successfully for every subject. Frequencies of AA, AT and TT genotype were 67.4%, 19.6% and 13.0% in the intrauterine HBV infection group, and 45.2%, 30.1% and 24.7% in the normal immune children group. A significant difference was found in the frequency distribution of IFN-gamma + 874 genotype between the two groups (chi(2) = 5.102, P = 0.02389). In the intrauterine HBV infection group the AA genotype was more common than in normal immune group. CONCLUSION: There is an association between IFN-gamma + 874 SNP and intrauterine HBV infection. This study suggested the possibility that IFN-gamma + 874 SNP might be important in determining an individual's susceptibility to development of intrauterine HBV infection.

Rapid natural resolution of intracranial hematoma.

OBJECTIVE: To investigate the clinical characteristics of intracranial hematoma and the mechanism involved in its rapid natural resolution. METHODS: Seventeen cases of intracranial hematoma with typical clinical and CT manifestations were retrospectively studied. RESULTS: Intracranial hematoma was found obviously decreased in size within 72 h after its occurrence in 8 cases. The rest 9 cases presented complete resolution. CONCLUSIONS: Rapid natural resolution of acute epidural hematoma is mostly found in teenagers and the resolution is correlated with cranial fracture at the hematoma site. As for acute subdural hematoma, its rapid resolution is associated with the transfer of cerebrospinal fluid toward subdural space, the lavage effect, and the compression caused by the increased intracranial pressure or the space left resulting from redistribution of the hematoma in brain atrophy.

[Simulation of biodegradation of petroleum contaminants in natural waters of the Yellow River].

Petroleum contaminants is one of the major pollutants in the Yellow River. Laboratory simulation experiments were carried out to study the natural biodegradation of the petroleum contaminants. When adding 10 mg/L petroleum contaminants to the Yellow River water samples containing 0 or 0.5 g/L sediment, the petroleum contaminants-degrading bacteria increased gradually after about one week of acclimatization. With the sediment content of 0.5 g/L, about 85% petroleum contaminants with the initial concentration of 11.64 mg/L in river water could be degraded within 63 days at 20 degrees C. The biodegradation rate was greatly influenced by the sediment content and the initial concentration of petroleum contaminants, and such effect was different from one degradation stage to another. In addition, the existence of sediment affected the biodegradation kinetics of petroleum contaminants.

[Relationship between genetic polymorphism of tumor necrosis factor-alpha and susceptibility to intrauterine HBV infection].

OBJECTIVE: To study the possible relationship between tumor necrosis factor (TNF)-alpha-238G/A gene polymorphism and the susceptibility to intrauterine HBV infection. METHODS: Two hundred and fifty-six children, including 130 infants born to HBsAg positive mothers were divided into two groups: forty-five children with intrauterine HBV infection (group I) and 85 children without intrauterine HBV infection (group II), with a control group of 126. TNF-alpha-238G/A gene polymorphism was examined in all 256 children, by means of real-time quantitative fluorescent PCR. RESULTS: A significant difference of TNF-alpha-238A allele frequency was found between group I and group II (x2=6.797, P=0.009), and between group I and the controls group (x2=0.047, P=0.002), but there was no significant difference between group II and the control groups (x2=0.047, p=0.828). CONCLUSION: This study found that genetic polymorphism of tumor necrosis factor-a was associated with intrauterine HBV infection
