Solitary gastric melanotic schwannoma: sonographic findings.

Solitary gastric schwannoma is rare, and solitary melanotic schwannoma is even rarer, posing a dilemma in diagnosis and treatment. We report the case of a 69-year-old woman with gastric melanotic schwannoma who presented with nausea, vomiting, and abdominal pain. Abdominal sonographic examination revealed a 5-cm hypoechoic mass in the epigastric area that was confirmed to be a gastric submucosal tumor on endoscopic examination. The diagnosis of melanotic schwannoma was confirmed via sonographically guided percutaneous core biopsy. The tumor was resected, and no recurrence has occurred in a 3-year follow-up.

Hemoclip-assisted polypectomy of large duodenal Brunner's gland hamartoma.

Brunner's gland hamartoma, also known as Brunneroma or Brunner's gland adenoma, is a rare tumor of the duodenum and an uncommon cause of gastrointestinal bleeding. In symptomatic patients, treatment requires either surgical resection or endoscopic polypectomy. We report a case of upper gastrointestinal bleeding from a pedunculated Brunner's gland hamartoma in the duodenal bulb. A new technique using hemoclip-assisted polypectomy is described to remove a large pedunculated Brunner's gland hamartoma.

Heterotopic pancreas presenting as gastric submucosal cyst on endoscopic sonography.

A 50-year-old Chinese woman with vague epigastric discomfort underwent upper gastrointestinal endoscopy that revealed a saddle-shaped submucosal tumor in the antrum along the greater curvature. Endoscopic sonography showed that the tumor consisted of two cystic lesions. Because of persisting symptoms, the tumor was resected 8 months later. Pathological examination revealed scattered heterotopic pancreatic acini and ducts in the submucosa and muscularis propria. We suggest that heterotopic pancreas, though rare, should be considered in the differential diagnosis of cystic lesions of the gastric wall.

Endoscopic ultrasonographic appearance of gastric emphysema.

Emphysematous gastritis (or phlegmonous gastritis) and gastric emphysema (or gastric pneumatosis) are variations of conditions associated with the presence of intramural air in the stomach. The presence of air in the gastric wall is a very rare clinical condition, associated with bacterial infection, increased intragastric pressure from gastric outlet obstruction, gastric mucosal disruption or air dissection from the mediastinum. In adults, this can occur in the setting of instrumentation-related injury, gastric outlet obstruction by gastric, duodenal or pancreatic malignancies or bowel ischemia. Here we describe a case of gastric emphysema related to repeated biliary stenting and partial duodenal obstruction in a patient with inoperable periampullary cancer, and provide the first description of the endoscopic ultrasonographic findings of gastric emphysema in the literature. In our case, endoscopic ultrasound showed a band of bright echogenicity arising from the submucosa layer, representing air in the gastric wall.
