Resected case of a double cancer, a hepatocellular carcinoma and a cholangiocellular carcinoma, and their spread to the skin.

Combined hepatocellular and cholangiocellular carcinomas are rare. Moreover, double cancer cases of hepatocellular carcinoma and cholangiocellular carcinoma are very rare. This report describes a patient with double cancer. A correct clinical diagnosis was made with successful resection, and cutaneous metastases occurred near the exit site of an abdominal drain after the resection of the tumor. The patient, a 66-year-old man with chronic hepatitis C, was admitted to our hospital because he was suspected of having primary liver cancer. Two liver masses in the anteroinferior segment were detected by using angiography, computed tomography during angiography, and computed tomography during arterioportography. These clinical findings indicated that the tumor in the right lobe was hepatocellular carcinoma. A resection of the S5 subsegmentectomy was performed. One mass was diagnosed histologically as hepatocellular carcinoma, and the other mass was diagnosed as cholangiocellular carcinoma. One year after the operation, the patient palpated a hard subcutaneous nodule 4.0 cm in diameter in the right lower abdominal wall. A subcutaneous tumor was excised, and a histological examination revealed moderately differentiated hepatocellular carcinoma. The patient is currently doing well without further recurrence of hepatocellular carcinoma or cholangiocellular carcinoma, 18 months after subsegmentectomy and six months after excision of the subcutaneous tumor.

Carcinoid of the papilla of Vater; a case report.

A 68-year-old Japanese man, without any symptoms, was found to have a carcinoid tumor of the Ampulla of Vater. A physical examination indicated no anemia or jaundice and no abnormal findings at all in the chest or abdomen. Except for glucose intolerance, the routine laboratory data were normal. An endoscopic biopsy was performed that suggested malignant tumor cells. There were no signs of carcinoid syndrome. A pylorus-preserving pancreatoduodenectomy with extensive lymph node dissection was performed. Histological and immunohistochemical studies resulted in the diagnosis of a carcinoid of the papilla of Vater, without regional lymph node metastases. Although postoperative, an anastomotic leakage of pancreaticogastrostomy was noted; the pancreatic fistula was closed seven weeks later to use the somatostatin analogue.

Long-term survival after hepatectomy for large metastatic breast cancer: a case report.

Long-term survival after the resection for liver metastasis from breast cancer is rare. We report on a 49-year-old woman who has been alive and well for 12 years following hepatic resection for metastatic breast cancer. Fourteen years before, the patient underwent a radical mastectomy for cancer of the right breast. One year after the operation the patient presented with right upper quadrant pain and a large isolated right lobe liver metastasis was found. Initially, it was too large to be treated by hepatectomy. Thus, intra-arterial one-shot injection of 20 mg of mitomycin C mixed with 4 mL of lipiodol was given three times at 1-month intervals and was followed by 5-fluorouracil (250 mg) and mitomycin C (2 mg) administered 9 times at 2-week intervals through the implanted port. Additionally, 150 mg of 5-fluorouracil and 1200 mg of medroxiprogesterone acetate were orally administered for 5 months. These therapies led to a remarkable reduction in tumor size and the metastasis was safely resected by an extended right hepatic lobectomy. The specimen was completely necrotic and histologically no viable tumor cell was observed. In the present case, the aggressive preoperative chemo- and hormonal therapies might be significant in making a contribution to longer survival.

Correlation of 18-F-fluorodeoxyglucose (FDG) accumulation with glucose transporter (Glut-1) expression in esophageal squamous cell carcinoma.

BACKGROUND: We previously reported that positron emission tomography (PET) with 18-F-fluorodeoxyglucose (FDG) might be a useful tool for evaluating the stage of esophageal squamous cell carcinoma (SCC), and that FDG-PET shows greater accuracy in the diagnosis of lymph node metastasis than computed tomography. Further, we elucidated the relationships among FDG-PET performance, glucose transporter (Glut)-1 expression and serum levels of the tumor markers carcinoembryonic antigen (CEA), cytokeratin 19 fragment (CYFRA) and squamous cell carcinoma antigen (SCC-Ag) in esophageal SCC. PATIENTS AND METHODS: We studied 44 patients with thoracic esophageal SCC who had undergone radical esophagectomy. Immunohistochemical analysis was used to detect the expression of Glut-1 in resected specimens and FDG accumulation was assessed by FDG-PET scan. RESULTS: FDG uptake in the primary tumor was found in 34 out of 44 (77.3%) patients. No significant correlation was observed between SUVs and the tumor markers CEA, CYFRA and SCC-Ag. The survival rate in patients with high FDG uptake (SUV > 3) was significantly lower than in cases with low FDG uptake (SUV < 3) (p < 0.01). A significant correlation was observed between SUV and Glut-1 expression (p < 0.05). The prognosis in patients with both low Glut-1 expression and low FDG uptake tended to be more favorable than in patients with high Glut-1 expression and/or high FDG uptake. CONCLUSION: Glut-1 expression was related to FDG uptake, and assessment of both FDG uptake and Glut-1 expression might be useful for providing prognostic information in patients with esophageal SCC.

Efficacy and toxicity of nedaplatin and 5-FU with radiation treatment for advanced esophageal carcinomas.

BACKGROUND: Nedaplatin, a new analog of cisplatin, has been developed and marketed in recent years in Japan. In this study, we evaluated the efficacy and toxicity of a regimen of nedaplatin and 5-FU with radiation in patients with advanced esophageal carcinomas. MATERIALS AND METHODS: Between June 1999 and December 2002, 42 patients with advanced and unresectable thoracic esophageal squamous cell carcinomas (SCCs) were enrolled into this study. Twenty-five of these 42 patients were considered to have inoperable tumors due to distant organ metastasis, distant lymph node metastasis, severe organ dysfunction and rejection of surgery by the patients. The remaining 17 patients received neoadjuvant treatment followed by surgery. The clinical responses of the primary tumors were evaluated. RESULTS: Response and toxicity could be assessed in 22 of the 25 patients with unresectable esophageal SCCs. Two of these 22 achieved a complete response (CR), 15 had a partial response (PR) and 5 showed no change (NC). The overall response rate (CR + PR) was 77%. The one- and 2-year survival rates were 30.7% and 10.2%, respectively, with a median survival time (MST) of 10.1 months. Of the 17 patients who received neoadjuvant treatment, 14 underwent subsequent surgery. Preoperatively, 2 of these 17 patients achieved a CR, 10 had a PR and 5 showed NC. The overall response rate for all 39 patients was 74%. The pathological complete response (pCR) rate of the 14 patients who underwent surgery was 28.6%. Their one- and 2-year survival rates were 48.2% and 12.1%, respectively, with an MST of 12.2 months. Toxicity was evaluated in the 39 patients whose responses were assessed. Toxicity higher than grade 3 occurred in 8 out of 39 (20.5%) patients, and 6 (15.4%), 3 (7.7%) and one (2.6%) had grade 3 leukopenia, thrombocytopenia and anemia, respectively. CONCLUSION: In conclusion, this study indicates that combination chemoradiotherapy with nedaplatin and 5-FU is safe and efficacious for patients with advanced esophageal carcinomas. We recommend further evaluation of this treatment regimen in such patients.

Predictive value of uric acid in gastric mucosal damage in hemodialysis patients.

BACKGROUND/AIMS: Uric acid, which has an antioxidant effect, is present at very high serum levels in hemodialysis patients, but its effect has not yet been clarified. In this study we investigated whether or not serum uric acid levels could be associated with gastric mucosal damage in hemodialysis patients. METHODOLOGY: We performed endoscopy in 53 patients who were on dialysis for more than two months and determined the relationship between serum uric acid levels, clinical features and gastric mucosal damage. RESULTS: Of 53 patients, gastroduodenal ulcer was found in 13 patients (Ulcer group), gastritis lesions in 25 patients (Gastritis group) and no pathological findings in 15 patients (Normal group). Serum uric acid levels before the dialytic session were remarkably higher in the Ulcer group than in the Normal (P < 0.01) or Gastritis group (P < 0.05). There was also a significant difference between the Gastritis and Normal groups in the serum uric acid levels (P < 0.05). Alcohol intake and noncompliance resulted in a significant increase in both gastric mucosal damage (P = 0.009 and P = 0.010) and serum uric acid levels (P < 0.05 and P < 0.01). Serum uric acid levels were markedly decreased after treatment with a proton pump inhibitor or H2 blocker. CONCLUSIONS: In hemodialysis patients alcohol intake and noncompliance contribute to the increase in serum uric acid levels and gastric mucosal damage, and serum uric acid levels have predictive values in gastric mucosal damage.

Helicobacter pylori infection in hemodialysis patients.

BACKGROUND/AIMS: Studies on the relationship between high serum urea nitrogen, creatinine and Helicobacter pylori infection in hemodialysis patients still give conflicting results. In the present study we investigated the prevalence of Helicobacter pylori positivity in patients with hemodialysis [HD(+)] and without hemodialysis [HD(-)] and assessed the relationship between clinical factors, serum urea nitrogen, creatinine levels and Helicobacter pylori prevalence in these patients. METHODOLOGY: 117 patients with dyspeptic complaints were included in the study. They consisted of 36 HD(+) patients (31%) and 81 HD(-) patients (69%). Endoscopy was performed and gastric antral biopsies were obtained for immunohistochemical analysis for Helicobacter pylori in all patients. RESULTS: Helicobacter pylori was positive in 53(45%) of 117 patients [Hp(+)]. In univariate analysis Hp(+) patients received hemodialysis therapy significantly less often (P = 0.002) and had lower serum urea nitrogen (P = 0.0008) and creatinine (P = 0.003) levels than Hp(-) patients. There was no significant difference in age, gender, endoscopic findings or comorbid conditions (hypertension or diabetes mellitus) between these groups. Multivariate logistic regression analysis revealed that only the serum urea nitrogen level was significantly associated with Helicobacter pylori prevalence (P = 0.008). CONCLUSIONS: These results indicate that high serum urea nitrogen seems to correlate with a low prevalence of Helicobacter pylori infection and hemodialysis patients with high serum urea nitrogen may be protected against Helicobacter pylori infection.

Mucosa-associated lymphoid tissue lymphoma of the esophagus: case report and review of the literature.

We report the histomorphologic and immunohistochemical features of another case of mucosa-associated lymphoid tissue (MALT) lymphoma arising from the esophagus and discuss the problems of differential diagnosis. The patient was a 49-year-old man, who had no gastrointestinal symptoms. On endoscopy, a smooth-surfaced, semibulbous lesion was found 36 cm from the incisors. We performed radical resection of this submucosal tumor with video-assisted thoracoscopic surgery for the purpose of diagnosis and treatment. The immunophenotype of the centrocyte-like-cells was CD20+, BCL2+, CD5-, CD10-, CD23- CD45RO- and cyclin D1-. Diffuse immunostaining of bcl-2 was detected in the nuclei of the tumor cells without lymph follicles. Southern blotting analyses of the IgH gene detected a single dominant band indicative of a clonal IgH rearrangement. From the pathological, immunohistochemical, and molecular biological features we concluded that the tumor was a MALT lymphoma. Only three cases of primary esophageal MALT lymphoma have been reported to date. On the basis of the present case and the three previously reported cases, we suggest that MALT lymphoma of the esophagus is usually an elevated type. The spectrum of sites in which gastrointestinal MALT lymphoma occurs should be expanded to include the esophagus.

Inverse correlation between expression levels of p27 and the ubiquitin ligase subunit Skp2 in early esophageal squamous cell carcinoma.

BACKGROUND: In esophageal squamous cell carcinoma (SCC), the relationship between expression of the cyclin-dependent kinase inhibitor p27 and tumor aggression and prognosis is still controversial. Moreover, the expression of S-phase kinase-interacting protein 2 (Skp2), the ubiquitin ligase subunit required for the ubiquitin-dependent degradation of p27, remains unknown. MATERIALS AND METHODS: We used immunohistochemistry to analyze Skp2 and p27 expression in surgical specimens obtained from 32 patients with early esophageal SCC. We also used Western blotting to characterize the expression of Skp2 and p27 in 7 cell lines derived from esophageal SCC. RESULTS: Expression of Skp2 showed an inverse topographic distribution and correlation to that of p27 in many esophageal carcinomas. Of the 7 cell lines, 6 showed an inverse relationship between Skp2 and p27 expression. Patients without an inverse correlation between Skp2 and p27 expression had a significantly unfavorable prognosis in the early stage (p=0.0493). CONCLUSION: These findings suggest that the target substrate of Skp2 in early esophageal SCC is mainly p27, and that failure of Skp2-induced degradation of p27 may influence tumor progression and lead to a poor prognosis.

A localized primary sclerosing cholangitis preoperatively diagnosed as hilar cholangiocarcinoma; report of a case.

The authors report herein a case of primary sclerosing cholangitis localized to the hepatic hilum which occurred in a 67-year-old male. The direct cholangiography revealed bile duct stenosis from the common hepatic duct to bilateral hepatic ducts. We could not confirm bile duct malignancy, however, hilar cholangiocarcinoma was most suspicious. We performed right trisegmentectomy of the liver with caudate lobectomy and lymph node dissection 3 weeks after right portal embolization. Pathological findings confirmed localized primary sclerosing cholangitis. Surgical resection of the affected bile duct is most effective in localized primary sclerosing cholangitis, because the prognosis of the disease is poor and secondary carcinogenesis in primary sclerosing cholangitis has high incidence.

Arteriovenous malformation of the jejunum detected by arterial-phase enhanced helical CT, a case report.

This is a report on a case of arteriovenous malformation of the jejunum, which is a relatively rare occurrence that can be detected by helical computed tomography (helical CT). The patient was a 63-year-old man who was admitted to our hospital because of a tarry stool. He developed hypotension as a result of losing a large amount of bright-red blood through rectal bleeding and was given fluids and blood transfusions. Subsequently, a helical CT scan was performed at 10-mm slice thickness. A small high-density area in the jejunum was identified on the arterial phase, which was scanned 30 sec after an intravenous infusion of 100 mL of iopamidol at a rate 2.5 mL/sec. An emergency operation, based on the diagnosis of bleeding from the jejunum, was performed because the patient had developed hypotension once again. A small, elevated lesion, which coincided with the high-density area detected by helical CT, was found in the jejunum. A segmental resection and end-to-end anastomosis were done. The pathological diagnosis revealed an arteriovenous malformation of the jejunum. In conclusion, helical CT may be an effective noninvasive method for detecting the cause and site of gastrointestinal bleeding.

Liver metastases of a minute rectal carcinoid less than 5mm in diameter: a case report.

We report a case of liver metastases of a minute rectal carcinoid less than 5mm in diameter, which was found during the postoperative follow-up course of a stomach cancer patient. For the early stomach cancer, laparoscope-assisted distal gastrectomy with lymph node dissection was performed on August 26, 1998. Later, abdominal CT revealed space-occupying lesions in the liver (S2). Metastatic tumors of the stomach cancer were suspected, but further examination revealed that the lesions were metastatic tumors due to a rectal carcinoid tumor. Ten months later, metastatic carcinoid tumors were found in the liver (S1, S5, S6, S7, S8). Subsegmentectomy of the liver (S7) and microwave coagulation therapy (S1, S5, S6, S8) were performed. Carcinoid tumors metastatic to the lymph nodes, liver, and other areas have been reported, but all were larger than 20mm in diameter. In this case, the primary tumor was less than 5mm in diameter, which is extremely rare. This patient was successfully treated with lateral segmentectomy, subsegmentectomy (S7), microwave coagulation therapy of the liver, and transanal extirpation. She is presently in a good condition and has had no recurrence of the carcinoid tumor from ten months after the last hepatectomy.

The role of insulin-like growth factor 1 and insulin-like growth factor binding protein 3 in human esophageal cancer.

BACKGROUND: Insulin-like growth factors (1GF-1) and insulin-like growth factor binding protein-3 (IGFBP-3) play important roles in cell growth and differentiation. The aim of this work was to investigate the roles of IGF-1 and IGFBP-3 in esophageal cancer. MATERIALS AND METHODS: We examined the circulating IGF-1 and IGFBP-3 concentration in 18 healthy controls and 66 esophageal cancer patients by ELISA and a ligand capture immunoassay. Immunohistochemistry for IGF-1 was performed on surgical specimens obtained from 93 patients with esophageal cancer. RESULTS: The serum IGF-1 and IGFBP-3 levels were significantly elevated in patients compared with healthy subjects and there was a positive correlation between IGF-1 and IGFBP-3. There was a significant correlation between IGF-1 level and depth of invasion and pathological stage. Poor prognosis was significantly correlated with increasing IGF-1 levels. The survival rates of high IGF-1 expression immunohistochemical study patients were poorer than those of low expression patients. CONCLUSION: Elevated serum IGF-1 levels may be an important predictor of risk for esophageal cancer. IGF-1 related to the progression of esophageal cancer may depend on an autocrine function of IGF-1.

Transthoracal partial resection or local excision of small esophageal carcinoma with a radical mediastinal lymphadenectomy--proposal of limited operation for postoperative quality of life.

BACKGROUND/AIMS: Controversy remains regarding which treatment should be employed for superficial esophageal cancer. The treatment modalities for such superficial cancers vary from a local excision, including an endoscopic mucosal resection (EMR) to an extended radical esophagectomy (three-field lymphadenectomy). In the current report, we proposed the use of either a transthoracal partial resection or local excision of the esophagus with a radical mediastinal lymphadenectomy for the patients with small superficial esophageal squamous cell carcinoma, in order to perform effective radical cancer surgery and maintain the quality of life (QOL) of the patients. METHODOLOGY: Surgical procedures: After thoracotomy and detaching of the esophagus from the surrounding tissues, the proximal and distal resection lines were determined by careful palpation of the clips within the esophagus which had been placed during the preoperative endoscopic examination. Thereafter, a mediastinal lymph node dissection, including the bilateral recurrent nerve nodes and paratracheal nodes, was performed. After this procedure, intrathoracic esophageal end-to-end anastomosis was performed in the ordinary manner. We performed this procedure on three patients with small localized esophageal cancer. RESULTS: Operations were safely performed on these three patients and postoperative quality of life was almost satisfactory. CONCLUSIONS: Transthoracal partial resection or local excision with a radical mediastinal lymphadenectomy was useful for the patients with small localized esophageal carcinoma.

Clinicopathological characteristics of poorly differentiated adenocarcinoma of the colon and rectum.

BACKGROUND/AIMS: The clinicopathological significance of poorly differentiated adenocarcinoma (Por) of the colorectum remains controversial. The purpose of this study was to investigate the clinicopathological characteristics of Por by comparison with well (Well) and moderately differentiated adenocarcinoma (Mod) of the colorectum and by subclassification of Por into three types according to the amount of fibrous stroma in tumor tissue. METHODOLOGY: Eleven patients with Por, 64 with Well and 254 with Mod were clinicopathologically compared. RESULTS: Por was located predominantly in the right side of the colon. The maximal size of the tumors in Por (72.3+/-25.0 mm) was significantly larger than in Well (42.2+/-26.8 mm) and Mod (52.2+/-22.8 mm) (p=0.0009, 0.0047). The frequency of lymph node metastasis in Por was significantly higher than in Well (p=0.0009). The five-year survival rate for patients with Por was 45.5%, for Well was 71.4% and for Mod was 59.5% (NS). The medullary type of Por had a good prognosis. CONCLUSIONS: Por proliferated and metastasized more rapidly than Well. The subclassification of Por according to the amount of fibrous stroma in tumor tissue could play an important role in the clinicopathological study of colorectal carcinoma.

Considerable multiple esophageal carcinoma. Implications of a new clinical entity.

BACKGROUND/AIMS: The multiple occurrence of primary squamous cell carcinoma of the esophagus is often observed, and most such occurrences are double cancers. There have also been some cases with three or more intra-esophageal cancers, however, no detailed clinicopathologic study has yet been performed in the literature. METHODOLOGY: Two hundred and fifty patients of primary esophageal squamous cell carcinoma without preoperative treatment that underwent esophageal resection were re-evaluated by serial histopathologic investigations and we analyzed the data of ten patients with three or more intraesophageal cancers. RESULTS: The clinical and histopathologic characteristics were as follows; 1) all but one of the cases were male, 2) all patients had a history of both heavy smoking and drinking but only one case had a family history of esophageal cancers among their siblings, 3) the depth of invasion in the carcinomas was restricted to within the submucosal layer of the esophageal wall, which was defined as superficial esophageal carcinoma, almost all (90%) of the cases accompanied esophageal squamous epithelial dysplasia. CONCLUSIONS: Based on these prominent characteristics of considerable multiple intra-esophageal cancers, a new clinical entity of "esophageal field cancers" could thus be suggested.

Appraisal of the simultaneous right-thoracic and abdominal approach with intrathoracic reconstruction after esophageal resection for patients with intrathoracic esophageal cancer.

BACKGROUND/AIMS: This study evaluated the surgical procedure with the simultaneous right-thoracic and abdominal approaches with intrathoracic reconstruction after performing a macroscopically curable esophageal resection and a mediastio-abdominal extended lymphadenectomy as the standard operation for patients with intrathoracic esophageal cancer. METHODOLOGY: Forty-seven patients with thoracic esophageal cancer were operated on at the Department of Surgery I, Gunma University Hospital from 1995 to 1999. Before the end of 1997, all 21 cases underwent subtotal esophagectomy with three-field lymphadenectomy. After 1998, however, 16 cases underwent the same procedure while the remaining 10 underwent a simultaneous right-thoracic and abdominal approach with intrathoracic reconstruction with lymphadenectomy based on our criteria (Group I). Postoperative factors including the data of cytokines were compared between Group I and conventional subtotal (10 cases) esophagectomy with three-field lymphadenectomy (Group II, 37 cases). RESULTS: None of the patients in Group I has shown a recurrence of cancer. The CRP levels at 2 POD and the serum IL 6 levels at 3 POD were significantly lower in Group I than in Group II (p<0.05), and the postoperative duration of mechanical ventilation was also significantly shorter in Group I than in Group II (p<0.001). The postoperative body weight gain at 12 months after surgery was also higher in Group I than in Group II. CONCLUSIONS: This procedure is indicated for patients with esophageal cancer, located in the middle- or lower-thoracic esophagus, limited to within the esophageal wall, and without either upper mediastinal or neck lymph node metastasis, in order to not only cure the cancer but also in order to improve the postoperative quality of life.

Long myotomy of the esophagus and gastric cardia with a complete fundic patch procedure for diffuse esophageal spasm.

Under the hypothesis that the surgical management of diffuse esophageal spasm requires the elimination or reduction of episodes of dysphagia and chest pain and prevention of postoperative gastroesophageal reflux, long esophageal myotomy and fundoplication had been performed. However, there have been some cases with unsatisfactory results. We describe herein a new surgical procedure of long myotomy of the esophagus and gastric cardia with a complete fundic patch operation for the patient with diffuse esophageal spasm. The advantages of this procedure are to preserve the separation of each myotomized edge and to reinforce the wall of the surface of the myotomized mucosa in order to avoid the postoperative problems. Postoperative course of the patient with this procedure was satisfactory.
