Pulmonary sarcoma complicated with neurofibromatosis type 1 (NF1).

We report here a case of primary pulmonary sarcoma complicated with neurofibromatosis type 1 (NF1). A 60-year-old man was urgently admitted to Tokai University Hospital, Japan, due to severe right chest pain. Skin inspection showed subcutaneous tumor lesions and café-au-lait spots, and therefore a diagnosis of neurofibromatosis type 1 (NF1) was made. Chest X-ray showed marked fluid accumulation in the right pleural cavity, and he was diagnosed as having empyema by thoracic drainage. Chest X-ray showed a large round mass lesion (80 mm in diameter) in the right lower lung field. Percutaneous needle biopsy of the tumor revealed that the empyema was caused by actinomycotic infection, and then he underwent right middle and lower lobectomy. Histopathological examination of the surgical specimens revealed features of spindle cell sarcoma. Several postoperative systemic examinations showed no other lesions except in the right lung. We concluded that this was a complicated case of primary pulmonary sarcoma with NF1.

INI-1, a partner gene of ALL-1, is highly conserved in human acute leukemia.

Recently, much interest have been focused on the role of INI-1, a partner gene of transfusion protein All-1 for leukemogenesis of acute leukemia cases. We found the presence of a mutation at codon 152 in INI-1 that resulted in a conservative amino acid change in cDNA and genomic DNA from an AML patient. We also found the known polymorphic nucleotide change at the third letter of codon 299 in exon 7 of INI-1 in 3 of 31 ALL cases and in 4 of 30 AML cases. In conclusion, INI-1, a partner gene for ALL-1, is considered to be highly conserved in acute leukemia cases.

Interleukin-10 expression is closely correlated with the expression of granulocyte-macrophage colony-stimulating factor in non-small cell lung cancer.

It is unclear how the cytokine network contributes to the expression of granulocyte-macrophage colony-stimulating factor (GM-CSF) in non-small cell lung cancer (NSCLC). We examined the gene expression of GM-CSF and interleukin (IL)-10 in 78 surgical specimens and 16 xenografts of NSCLC by reverse transcription-polymerase chain reaction. Forty-four of the 78 (56.4%) surgical specimens and 11 of the 16 (68.8%) xenografts of NSCLC showed GM-CSF gene expression. Forty-one of the 44 (93.2%) GM-CSF-positive NSCLC specimens showed IL-10 expression, and GM-CSF expression was significantly correlated with IL-10 expression (p < 0.0171, Mann-Whitney's U-test). These results suggested that IL-10 is one of the cytokines up-regulating the GM-CSF expression in NSCLC.

Circadian dynamics of heart rate and physical activity in patients with heart failure.

The present study was designed to develop a method to continuously measure Holter electrocardiogram (ECG) and physical activity in terms of metabolic costs to examine circadian dynamics of RR intervals and physical activity in patients with heart failure. A total of 7 healthy subjects and 3 heart failure patients performed cardiopulmonary exercise test using four-stage graded treadmill walking at 0% grade to examine whether the acceleration signals in the vertical direction could reflect actual body energy expenditure during physical activity. Then, using this new method, 24-hr monitorings of ECG and physical activity were performed in 24 inpatients with heart failure while they were allowed to walk around freely. Our results showed the integral of rectified acceleration signals was closely correlated with actual metabolic cost in all subjects. Instantaneous changes in heart rate were quite concordant with physical activity. As compared with the asymptomatic patients (n = 12), the symptomatic patients (n = 12) had lower energy expenditure during 8-hr daytime periods but higher mean heart rate. Furthermore, a more prominent ultradian rhythm of circadian changes in heart rate and physical activity was found in 50% of all subjects studied. The simultaneous analysis of Holter ECG and physical activity as the same time series revealed that in patients with heart failure, sympathovagal balance shifted toward sympathotonic conditions and their physical activity could become subject to intrinsic ultradian dynamics of body's homeostasis.
