Myocardium tissue ablation with high-peak-power nanosecond 1,064- and 532-nm pulsed lasers: influence of laser-induced plasma.

BACKGROUND AND OBJECTIVES: We investigated the mechanism and characteristics of porcine myocardium tissue ablation in vitro with nanosecond 1,064- and 532-nm pulsed lasers at laser intensities up to approximately 5.0 GW/cm(2). Particular attention was paid to study the influence of the laser-induced plasma on the ablation characteristics. The applicability of these two lasers to transmyocardial laser revascularization (TMLR) was discussed. STUDY DESIGN/MATERIALS AND METHODS: Porcine myocardium tissue samples were irradiated with 1,064- and 532-nm, Q-switched Nd:YAG laser pulses, and the ablation depths were measured. The temporal profiles of the laser-induced optical emissions were measured with a biplanar phototube. For the ablated tissue samples, histological analysis was performed with an optical microscope and a polarization microscope. RESULTS: The ablation efficiency at 1,064 nm was higher than that at 532 nm. The ablation threshold at 1,064 nm (approximately 0.8 GW/cm(2)) was lower than that at 532 nm (approximately 1.6 GW/cm(2)), in spite of the lower absorption coefficient being expected at 1,064 nm. For the 1,064-nm laser-ablated tissues, thermal damage was very limited, while damage presumably caused by the mechanical effect was observed in most of the cases. For the 1,064-nm laser ablation, the ablation threshold was equal to the threshold of the laser-induced optical emission (approximately 0.8 GW/cm(2)), while for the 532-nm laser ablation, the optical emission threshold ( approximately 2.4 GW/cm(2)) was higher than the ablation threshold. CONCLUSIONS: We considered that for the 1,064-nm laser ablation, the tissue removal was achieved through a photodisruption process at laser intensities of > approximately 0.8 GW/cm(2). At laser intensities of > 3.0 GW/cm(2), however, the ablation efficiency decreased; this can be attributed to the absorption of incoming laser pulses by the plasma. For the 532-nm laser ablation, the tissue removal was achieved through a photothermal process at laser intensities of > approximately 1.6 GW/cm(2). At laser intensities of > 2.4 GW/cm(2), a photodisruption process may also contribute to the tissue removal, in addition to a photothermal process. With regard to the ablation rates, the 1,064-nm laser was more suitable for TMLR than the 532-nm laser. We concluded that the 1,064-nm Q-switched Nd:YAG laser would be a potential candidate for a laser source for TMLR because of possible fiber-based beam delivery, its compact structure, cost effectiveness, and easy maintenance. Animal trials, however, have to be carried out to evaluate the influence of the tissue damage.

Spatial frequency tuning of the Ouchi illusion and its dependence on stimulus size.

This study investigated the effects of the stimulus size on the spatial frequency tuning of the Ouchi illusion, which is an illusory sliding motion perceived in a checkerboard pattern of rectangular elements that is surrounded by a checkerboard pattern of orthogonally oriented elements. Two experiments were conducted to measure the perceived strength of illusion. The optimal size of the inner pattern increased proportionally with check size. In contrast, the optimal check size increased with the size of the inner pattern but not proportionally, and the range of increase was relatively small. The optimal fundamental spatial frequency was lower for a larger stimulus both for checkerboard patterns and simpler sinusoidal grating patterns, but there were differences in the tuning curves for the two types of stimuli. These results support the idea that two processes underlie the Ouchi illusion; one computes the local motion direction, and the other integrates motion signals across space for surface segmentation. For the checkerboard and grating stimuli, the former process may be different while the latter can be shared.

Functional and morphological changes in the hippocampal neuronal circuits associated with epileptic seizures.

PURPOSE: We review what is currently known about functional and morphologic plasticity of hippocampal neuronal circuits in animal epilepsy models. RESULTS: Perforant-path kindling has been shown to enhance excitatory synaptic transmission in the dentate gyrus significantly, and this enhancement has lasted for > or =1 month after kindling. However, in our rapid kindling experiment with an interstimulus interval of 5 min, perforant-path kindling resulted in behavioral convulsions and prolonged the afterdischarge duration, but dentate synaptic transmission remained depressed during kindling. CONCLUSIONS: These results indicate that kindling-induced synaptic potentiation is not necessary for kindling development. Conversely, epileptic seizures have led to the anomalous sprouting of mossy fibers, a process thought to induce hyperexcitation of granule cells. We conclude that the sprouting of mossy fibers did not play a critical role in kindling, because we observed no significant correlation between the mossy fiber sprouting and kindling development. Epileptic seizures also have been shown to increase gamma-aminobutyric acid (GABA)A receptor-mediated inhibition during interictal periods. Although this strengthening of GABAergic inhibition is likely to be a defensive measure against seizure initiation, some studies have suggested that another type of GABAergic response facilitates seizure activity. The possibility of GABAA receptor-mediated excitation during ictal periods is theoretically examined.

[What is bioinformatics?].

Yet bioinformatics has neither clear definition nor proper Japanese equivalent for this word. Bioinformatics is mainly composed of two fields or disciplines; one is computer and information technology as a tool for analyzing massive data such as genome and proteome, and the other one is research for the integrated biology based on the knowledge of genome. Classical and narrow-sense bioinformatics corresponds to the former. The latter called new bioinformatics aims to understand living thing as a whole, where both genome information and mathematical model play complimentary roles. From now the postgenome area on, this new bioinformatics may give novel perspective about living thing, biology and human.

Transcranial measurement of diffuse light reflectance from cold-injured brains in rats.

Diagnosis of brain edema is important to manage severe head injuries and diseases, but there is no method of noninvasive bedside monitoring of brain edema. We speculate that brain edema changes the optical properties of brain tissue. To verify this speculation, we perform transcranial measurement of diffuse light reflectance from cold-injured brains in rats. To induce edema, a liquid nitrogen-cooled copper probe of 3 mm diameter was applied to the right parietal bone for 60 s under anesthesia. The rat skull was irradiated with 633-nm HeNe laser light and 532-nm Nd:YVO4 laser light, which were used to investigate changes in light scattering and blood flow, respectively. Reflectance intensity was measured as functions of time and position on the skull. We found that reflectance intensities were significantly decreased around the cold injury at both 633 and 532 nm, while reflectance was increased in the cold injury at 532 nm, indicating that cerebral blood volume was decreased in the cold injury. Under the condition of decreased cerebral blood volume, the decrease in reflectance intensity around the injury suggests that the scattering coefficient of brain tissue was reduced due to edema formation in this area.

Measurement of burn depths in rats using multiwavelength photoacoustic depth profiling.

We perform measurement of photoacoustic (PA) signals for burned skin in rats in the spectral range of 500 to 650 nm. The wavelength dependence of PA signal amplitude shows characteristics similar to those of the absorption spectrum of hemoglobin, suggesting that the PA signal originates from blood in the uninjured skin tissue under the injured tissue layer. High-contrast signals are obtained in the spectral range of 532 to 580 nm. At 550 nm, a PA detector is scanned on the wounds and PA tomograms are obtained. The tomograms clearly show the zones of stasis, demonstrating that a 2-D PA measurement is useful for burn depth assessment.

In vitro gene transfer to mammalian cells by the use of laser-induced stress waves: effects of stress wave parameters, ambient temperature, and cell type.

Laser-mediated gene transfection has received much attention as a new method for targeted gene therapy because of the high spatial controllability of laser energy. We previously demonstrated both in vivo and in vitro that plasmid DNA can be transfected by applying nanosecond pulsed laser-induced stress waves (LISWs). In the present study, we investigated the dependence of transfection efficiency on the laser irradiation conditions and hence stress wave conditions in vitro. We measured characteristics of LISWs used for gene transfection. For NIH 3T3 cells, transfection efficiency was evaluated as functions of laser fluence and number of pulses. The effect of ambient temperature was also investigated, and it was found that change in ambient temperature in a specific range resulted in drastic change in transfection efficiency for NIH 3T3 cells. Gene transfection of different types of cell lines were also demonstrated, where cellular heating increased transfection efficiency for nonmalignant cells, while heating decreased transfection efficiency for malignant cells.

Effect of distal splenorenal shunt plus splenopancreatic disconnection on glucose and amino acid metabolism.

BACKGROUND/AIMS: The Warren-Zeppa distal selective splenorenal shunt (DSRS) is aimed at decompressing esophageal varices by a distal splenorenal shunt, at the same time ligaturing left gastric, gastroepiploic and umbilical veins. In the long-term follow-up the loss of shunt selectivity was observed in several cases. Therefore, Inokuti and Warren reported splenopancreatic disconnection (SPD) to prevent splenic collaterals from stealing portal venous blood. METHODOLOGY: This report presents metabolic data (K-glucose, IRI in iv-GTT, Fischer ratio) of 10 cirrhotic patients operated with DSRS plus SPD. RESULTS: K-glucose of iv-GTT was improved from 2.3 +/- 0.2 to 2.5 +/- 0.2. Fischer ratio was also improved from 1.58 to 2.0 after the operation. IRI behavior tends to be lower after operation as a result of devascularlization of the collateral circulation by the selective shunt. CONCLUSIONS: From these results it seems that DSRS+SPD has favorable effects on glucose and amino acid metabolism.
