Differential expression of integrin subunits in DU-145/AR prostate cancer cells.

We have established a clonal DU-145 prostate cancer cell line (DU-145/AR) stably transfected with androgen receptor cDNA. We investigated the expression of integrin subunits, adhesion to extracellular matrices, the invasion of DU-145/AR prostate cancer cells. The expression of various integrin subunits and adhesion to various extracellular matrices in DU-145, DU-145/Neo and DU-145/AR cells were examined. The haptoinvasion and the haptotactic migration of these cells were investigated using a Transwell cell culture chamber assay. DU-145/AR cells exhibited lower expression of alpha6 and beta4 integrin subunits and higher expression of alpha2 and alpha5 than DU-145 cells. DU-145/AR cells showed significantly lower adhesion to fibronectin, laminin-1 and laminin-5 than DU-145/ Neo cells, whereas DU-145/AR cells showed higher adhesion to type I and type IV collagen. Haptoinvasion of DU-145/AR cells into Matrigel/fibronectin-coated filter was significantly reduced as compared with DU-145/Neo or DU-145 cells, but there was no significant difference between DU-145/AR and control cells in the haptotactic migration to fibronectin. Dihydrotestosterone (DHT) inhibited the invasive ability of DU-145/AR cells. These results indicate that androgen receptor may play a role in the regulation of adhesion to the extracellular matrices and invasion of prostate cancer cells through influencing the expression of specific integrin subunits.

[Treatment of traditional Chinese medicine for idiopathic male infertility].

Several Chinese herbal medicines have been used to treat patients with idiopathic male infertility and have been reported to improve semen quality. The clinical efficacy of these medicines was reviewed. The therapeutic effect of Hochu-ekki-to based on the pretreatment traditional diagnosis (Sho) was examined. Three months after the administration of Hochu-ekki-to, the semen count and motility significantly increased in comparison with pretreatment values. When the patients were classified into 3 categories based on "Sho", Hochu-ekki-to was effective in semen motility in patients with vacuity pattern (Kyo-Sho). Seminal plasma soluble Fas (sFas) levels before and three months after the administration of drug were analyzed. Seminal plasma sFas level elevated significantly after the administration of Hochu-ekki-to. After the administration of Hochu-ekki-to, seminal plasma sFas levels significantly correlated with sperm concentration. To make the best use of traditional medicine, it is important to give medication according to the traditional diagnosis (Sho).

Molecular interactions between STAT3 and protein inhibitor of activated STAT3, and androgen receptor.

STAT3 mainly acts as a signal transducer of IL-6 family cytokines and transcriptionally activates specific target genes. The recently discovered protein inhibitor of activated STAT3 (PIAS3) binds directly to STAT3 and blocks transcriptional activation. In our previous report, we demonstrated that PIAS3 directly interacted with androgen receptor (AR) and affected AR-mediated gene activation. Furthermore, we also showed that AR associated with STAT3 and enhanced its activity. Here, we examined molecular interactions between STAT3, PIAS3, and AR to underline the mechanism of how they regulate each other. AR activation overcame the inhibitory effect on STAT3-mediated transcription by PIAS3. Co-immunoprecipitation experiments revealed that an active form of AR relieved STAT3 from STAT3-PIAS3 complex formation. These results indicate that AR and PIAS3 regulate the STAT3-mediated transcriptional activity by their physical protein-protein competition on STAT3.

Involvement of heat-shock protein 90 in the interleukin-6-mediated signaling pathway through STAT3.

Interleukin-6 (IL-6) is a multifunctional cytokine playing roles in the immune system, hematopoiesis, and acute phase reactions. IL-6 also regulates the growth of various types of human malignant tumors. Here we demonstrate that IL-6-induced gene expression was suppressed by a specific heat-shock protein 90 (Hsp90) inhibitor, geldanamycin (GA) in human hepatoma Hep3B cells. GA also suppressed the IL-6-induced activation of signal transducer and activator of transcription 3 (STAT3) in a human embryonic kidney carcinoma 293T cells. This inhibitory effect of GA on STAT3 activation was reversed by overexpression of Hsp90. Furthermore, Hsp90 directly bound to STAT3 via its N-terminal region, which interacted with GA. We provide evidence that the action of GA on IL-6 functions was due to the inhibition of direct physical interactions between STAT3 and Hsp90, which represents a novel role of Hsp90 in the IL-6 signaling pathways.

Enhancement of ultrasound-mediated gene transfection by membrane modification.

BACKGROUND: Ultrasound-mediated gene transfection (USMGT) with an echo contrast agent could be a new promising physical method of triggering localized gene delivery, but the effect is still modest. The aim of this study is to devise a method to improve efficiency of USMGT. We examined the effect of lidocaine and different temperatures on USMGT, each of which is a known membrane modifier, since the plasma membrane can be considered a site of action in USMGT. METHODS: We observed the effect of lidocaine (0.01, 0.1 or 1.0 mM) and different temperatures (7, 20, 37, 42 or 44 degrees C) on USMGT (1 MHz, 3.6 W/cm(2) (I(SATA)) and 20 s exposure) in the presence of Levovist (10 mg/ml). At 20 h after sonication, transfection efficiency was evaluated by luciferase assay. Membrane fluidity was examined by fluorescence polarization measurement. Cavitational activity was measured by ESR spin trapping with 5,5-dimethyl-1-pyrroline N-oxide. The number of cells transfected with the GFP gene was counted under a fluorescence microscope. RESULTS: Lidocaine (1 mM) and heat (42-44 degrees C) significantly increased luciferase expression approximately 18-fold and 19-fold higher than Levovist only. Both treatments were shown to increase membrane fluidity; in addition, heat enhanced a cavitational effect. It was confirmed by an experiment using the GFP gene that increase in luciferase expression was due to the increase in number of cells. CONCLUSIONS: This enhancement could be useful for ultrasound-mediated gene therapy in the future since both treatments for membrane modification could be directly applied to the living body.

Differential effect of chromogranin A fragments on invasion and growth of prostate cancer cells in vitro.

OBJECTIVES: To investigate the effect of various chromogranin A (CgA) fragments on the invasion, haptotactic migration, and growth of prostate cancer cells. METHODS: We investigated the effect of five kinds of CgA fragments (79-115, 286-301, 324-341, 344-374, and 356-374) on the invasion of PC-3 and DU-145 prostate cancer cells through a reconstituted basement membrane (matrigel) and the haptotactic migration of these cells using a Transwell cell culture chamber assay. Cell growth was assessed by the WST-1 Cell Counting Kit. RESULTS: CgA (79-115) inhibited the invasive ability of PC-3 and DU-145 cells (P = 0.035 and P = 0.037, respectively). CgA (79-115) also inhibited the haptotactic migration of these cells (P = 0.031 and P = 0.021). On the other hand, other CgA fragments had no significant effect. CgA (79-115) also inhibited the cell growth of PC-3 cells (P = 0.012) and DU-145 cells (P < 0.001). CgA (324-341), CgA (344-374), and CgA (356-374) inhibited the cell growth of DU-145 cells (P < 0.001, P < 0.001, and P < 0.001, respectively). CONCLUSIONS: These results indicate that some CgA fragments may affect the invasion and growth of prostate cancer cells.

Tissue factor expression and prognosis in patients with metastatic prostate cancer.

OBJECTIVES: To assess immunohistochemically the pattern of tissue factor (TF) expression in patients with metastatic prostate cancer, because TF is aberrantly expressed in human cancer. TF is the primary initiator of the coagulation cascade. METHODS: Seventy-three patients with untreated metastatic prostate cancer who received hormonal therapy were included in the present study. Biopsy specimens were stained with anti-human TF antibody. We evaluated the histologic grade, extent of bony metastasis, clinical response to hormonal therapy, and patient prognosis. RESULTS: TF was detected in 75.3% of the tumors of the patients with metastatic prostate cancer. TF expression showed no association with histologic grade, extent of bony metastasis, or clinical response to hormonal therapy. Patients with TF-positive tumors had a poorer cause-specific survival than those with TF-negative tumors. Multivariate analysis showed that TF expression, clinical response to hormonal therapy, and extent of bony metastasis were significant prognostic factors. CONCLUSIONS: The TF content measured using immunohistochemical staining was a useful prognostic factor for patients with metastatic prostate cancer treated with androgen withdrawal therapy.

Laparoscopic ablation of peripelvic renal cysts.

Recently, laparoscopic surgery has been reported for symptomatic renal cysts. A 60-year-old female was referred to Toyama Medical and Pharmaceutical University Hospital with a chief complaint of general fatigue and left back pain. CT demonstrated bilateral peripelvic renal cysts, and DIP demonstrated left hydronephrosis and a medical shift of the left renal pelvis. Laparoscopic ablation of bilateral peripelvic renal cysts was performed under general anesthesia and a round excision was made in the cyst wall via the peritoneum. After surgery, the left back pain disappeared and CT demonstrated resolution of peripelvic renal cysts. Laparoscopic ablation of peripelvic renal cyst is a highly effective and minimally morbid procedure.

Changes in male reproductive function after high altitude mountaineering.

The male reproductive functions of the members of the Masherbrum (7821 m) Expedition in 1999 were examined via semen analyses and endocrine tests. Specimens were collected from three subjects who had stayed above 5100 m for 21 to 24 days and above 6700 m for 4 to 5 days before departure and 1 month, 3 months, and 2 yr after returning from the expedition. Semen analyses showed no change in the semen volume. Sperm counts decreased after 1 month and had not recovered after 3 months, but they had recovered after 2 yr in all subjects. An increase in abnormally shaped sperm was also observed after 1 month, but had nearly recovered to the preexpedition state after 3 months. Endocrine tests revealed slightly decreased testosterone in the blood after 1 month, which had decreased still further after 3 months. The tests were completely normal after 2 yr. We suggest that a high altitude sojourn may induce reversible spermatogenic and Leydig cell dysfunction.

Prognostic significance of changes in short-term prostate volume and serum prostate-specific antigen after androgen withdrawal in men with metastatic prostate cancer.

OBJECTIVE: Endocrine therapy is the standard treatment for metastatic prostate cancer although progression to androgen independence is inevitable. To evaluate prognostic factors in metastatic prostate cancer, patients who had been treated with endocrine therapy were investigated especially for the change in prostate volume. METHODS: Fifty-nine patients with untreated metastatic prostate cancer who received endocrine therapy were included in the present study. Blood chemistry, histological grade, extent of bony metastasis, clinical response to hormone therapy including the short-term change in prostate volume and serum prostate-specific antigen (PSA), and prognosis of the patients were evaluated. RESULTS: With univariate analysis, hemoglobin concentration, serum alkaline phosphatase, lactate dehydrogenase (LDH), histological grade, extent of bony disease, the short-term change of prostate volume and response of PSA at 3 months were shown to be significant prognostic factors. Response of PSA, LDH and the change in prostate volume were significant for predicting prognosis with multivariate analyses. Five-year survival rate in patients whose prostate had regressed 20% or more at 1 month and whose PSA had been normalized at 3 months was 67%, whereas that in patients whose prostate had regressed less than 20% and whose PSA had not been normalized was 0%. CONCLUSIONS: The patients in whom PSA had not been normalized at 3 months and the prostate volume had regressed less than 20% at 1 month were in the high-risk group. New or more aggressive treatment should be considered.

Snap-Gauge band compared to RigiScan Plus in a nocturnal penile tumescence study for evaluation of erectile dysfunction.

OBJECTIVE: The aim of this prospective study was to examine the ability of the Snap-Gauge band to differentiate organic from psychogenic erectile dysfunction (ED). PATIENTS AND METHODS: Nocturnal penile tumescence testing was performed on 12 patients using a Snap-Gauge band and RigiScan Plus. A total of 29 nightly examinations were evaluated. The results obtained with the two methods were compared. RESULTS: The average tumescence, average rigidity, tumescence activity units and rigidity activity units in the positive group (the group of cases in which one, two or three films of the Snap-Gauge band were broken) were each significantly greater than that in the negative group (the group of cases in which no film was broken). The Snap-Gauge band correctly diagnosed 90% of the patients with a sensitivity of 100% and specificity of 75% with respect to results obtained using the RigiScan Plus. CONCLUSIONS: The Snap-Gauge band is inexpensive compared to the RigiScan Plus and is relatively reliable. We conclude that the Snap-Gauge band can play a role in ED assessment and can function as a screening device in evaluation of ED.

Prognostic significance of serum soluble Fas level and its change during regression and progression of advanced prostate cancer.

To evaluate the clinical usefulness of serum soluble Fas (sFas) and sFas ligand as a prognostic factor and for monitoring the regression and progression of metastatic prostate cancer treated with endocrine therapy, sFas and sFas ligand were measured in sera collected from 30 patients with untreated metastatic prostate cancer. sFas levels were measured sequentially in 16 patients who had progressed to a state of elevated prostate-specific antigen (PSA) and 5 patients who were in regression following endocrine therapy. Serum sFas levels in patients with metastatic prostate cancer were found to be significantly higher than that of control patients with benign prostate hyperplasia. Patients with low levels of serum sFas had a higher cause-specific survival rate and a higher PSA progression-free rate compared with patients with high levels of serum sFas. In patients who suffered PSA progression following endocrine therapy, serum sFas increased in parallel to the increase in PSA levels although the magnitude of change in sFas was very small. In patients whose tumor regressed following therapy, sFas levels did not change. sFas ligand levels were very low or below measurable levels in all specimens. sFas levels might be associated with poor prognosis in metastatic prostate cancer. Serum sFas ligand appears to have limited clinical relevance.

Evaluation of tissue factor antigen level in human seminal plasma.

We measured the seminal plasma levels of tissue factor (TF) and interleukin-6 (IL-6) in men and examined their relationship with sperm concentration and motility. The study comprised 71 patients in three groups: an infertile group with ( n=11) and without ( n=50) leukocytospermia and a fertile group ( n=10). The seminal plasma levels of TF were significantly higher in the infertile patients than in the fertile ones. The seminal plasma levels of both TF and IL-6 were significantly higher in the infertile patients with leukocytospermia than in those without leukocytospermia. In 54 nonazoopermic cases the seminal plasma levels of TF were significantly correlated with the sperm concentration and sperm motility. Further studies are necessary to clarify the role of TF in human fertilization.

Vasoactive intestinal peptide (VIP) enhances the cell motility of androgen receptor-transfected DU-145 prostate cancer cells (DU-145/AR).

We established a clonal DU-145 prostate cancer cell line (DU-145/AR) stably transfected with androgen receptor (AR) cDNA and investigated the expression of type 1 vasoactive intestinal peptide (VIP) receptor (VIP1R) and type 2 VIP receptor (VIP2R) mRNA in these cells by reverse transcriptase-polymerase chain reaction analysis and the effect of VIP on the invasion and the haptotactic migration of these cells. DU-145/AR cells constitutively expressed both VIP1R and VIP2R mRNA, but the parent DU-145 cells did not. VIP increased the invasive capacity of DU-145/AR cells. VIP also enhanced the haptotactic migration of these cells to fibronectin. However, the growth of these tumor cells was not affected by VIP at any concentrations used in this study. These results indicate that VIP may play a role in the regulation of the invasion of prostate cancer.

Preoperative parameters to predict tumor volume in Japanese patients with nonpalpable prostate cancer.

BACKGROUND: Serum prostate-specific antigen (PSA) determination has led to the increased detection of nonpalpable prostate cancer (T1c cancer). Some T1c cancers are small and may pose no threat. To examine the characteristics of patients with T1c cancer in Japan, patients treated with radical prostatectomy were reviewed. To predict potentially insignificant cancer, which is a candidate for delayed treatment, patients whose biopsy specimen showed relatively favorable features were also examined. METHODS: In 40 patients with T1c cancers treated with radical prostatectomy, the pretreatment parameters of serum PSA, PSA density, biopsy Gleason score, and number of cancer-positive cores were determined and compared with the histological features in the surgical specimens. Pretreatment parameters were also determined in patients whose biopsy Gleason score was 6 or less, and whose biopsy specimen contained one or two cancer-positive cores, with 50% or less cancer involvement in any cancer-positive cores. RESULTS: At the time of radical prostatectomy, 73% of patients had organ-confined disease. When insignificant cancer was defined as a volume of less than 0.5 cm3 and a Gleason score of 6 or less, in 9 of the 40 (23%) patients with clinical T1c disease, the cancer was clinically insignificant. Multiple regression analysis of the pretreatment variables, including pretreatment PSA level, PSA density, and number of cancer-positive cores demonstrated that the pretreatment PSA level and number of cancer-positive cores were independent factors that predicted tumor volume. From the biopsy features, 19 patients were assessed as having clinically insignificant disease. Nine (47%) of them had tumors of less than 0.5 cm3, whereas 2 (11%) had tumors of more than 2 cm3. The values for pretreatment PSA density and PSA density adjusted for the transition zone volume for those with a cancer volume of 0.5 cm3 or more were significantly higher than the values for those patients with a cancer volume of less than 0.5 cm3. CONCLUSION: T1c cancers in Japanese patients included various cancers, from clinically insignificant to locally advanced ones. Pretreatment PSA level and the number of cancer-positive cores are useful parameters with which to predict cancer volume in the surgical specimen.

Serum pro-gastrin-releasing peptide (31-98) in benign prostatic hyperplasia and prostatic carcinoma.

OBJECTIVES: To clarify whether serum levels of pro-gastrin-releasing peptide (ProGRP) (31-98) could be a useful marker in patients with prostatic carcinoma. GRP is produced and secreted by prostatic neuroendocrine cells. METHODS: Serum levels of ProGRP(31-98) were measured by enzyme-linked immunosorbent assay in 20 patients with benign prostatic hyperplasia and 107 patients with prostatic carcinoma. RESULTS: The mean serum levels of ProGRP(31-98) in patients with distant metastasis and hormone-resistant prostate cancer were significantly elevated compared with those in patients with organ-confined disease. Significantly elevated levels of ProGRP(31-98) were detected in 9 patients with prostatic carcinoma before any treatment. During hormone-resistant prostate cancer progression, ProGRP(31-98) levels were elevated in 9 patients (23%). Of the 9 patients with Stage D3 and elevated serum ProGRP, 4 had a normal serum prostate-specific antigen level. CONCLUSIONS: ProGRP may be a potential tumor marker for prostate cancer. Additional studies in large groups of patients are needed to define the clinical value of ProGRP.

Percutaneous endopyelotomy for ureteropelvic junction obstruction.

Percutaneous endopyelotomy was performed 13 times on 11 patients with primary and secondary UPJ obstruction between 1994 and 2002. Excretory urogram revealed improvement in ten of eleven patients. One of the ten successfully treated patients required repeated endopyelotomy. Endopyelotomy failed in one patient, who had secondary UPJ obstruction that had been stenosed by granuloma caused by a ureteral stone. As the patient had UPJ obstruction of high insertion type with thinned renal parenchyma, nephrectomy was performed after repeated endopyelotomy. Compared with open pyeloplasty, percutaneous endopyelotomy is less invasive and is cosmetically advantageous.

Low serum testosterone level predicts worse response to endocrine therapy in Japanese patients with metastatic prostate cancer.

Patients with prostate cancer generally respond to androgen withdrawal therapy, but progression to androgen-independence is frequently observed later. To examine whether pretreatment serum androgen status could predict disease progression in metastatic prostate cancer, pretreatment serum testosterone, histological grade, extent of bony metastasis, serum prostate-specific antigen (PSA) response to hormone therapy, and prognosis of the 40 patients with untreated metastatic prostate cancer who received endocrine therapy were evaluated. Although there were no differences in age, pretreatment PSA level, extent of bony disease and histological grade between patients with normal testosterone and those with low testosterone, PSA response after endocrine therapy was better in normal testosterone group. There was a significantly longer interval to disease progression in patients with normal testosterone than in those with low testosterone. The patients with metastatic prostate cancer with low serum testosterone were in the high risk group of worse response to endocrine therapy. Additional therapy might be considered in those patients.
