[Tissue microarray and in situ hybridization study on hypoxia inducible factor 1alpha mRNA expression in epithelial ovarian tumor].

OBJECTIVE: To study the expression and significance of hypoxia inducible factor 1alpha (HIF-1alpha) in epithelial ovarian tumors. METHODS: The expression of HIF-1alpha mRNA in 295 patients with epithelial ovarian tumor was analyzed retrospectively by high-throughput tissue microarray and in situ hybridization, which was compared with 13 normal ovarian tissue samples. RESULTS: The expression rates of HIF-1alpha mRNA were 0, 13.2%, 42.1% and 81.9% in normal ovarian tissue, benign, borderline and malignant ovarian tumors. Expression rate of HIF-1alpha mRNA in borderline and invasive tumor was significantly higher than those in normal ovarian tissue and benign tumor (P < 0.001). Statistical analysis revealed that the expression of HIF-1alpha mRNA was not related to FIGO stages or histological subtypes. Close negative relation was observed between the expression of HIF-1alpha mRNA and tumor histological differentiation (P < 0.001). CONCLUSION: The overexpression of HIF-1alpha may play an important role in oncogenesis of epithelial ovarian tumor. Tissue microarray is an efficient technique of molecular biology.

[Diagnosis and treatment of seminal vesicle cyst].

OBJECTIVES: To deepen the understanding of patients with seminal vesicle cyst for correct diagnosis and treatment. METHODS: Sixteen patients with seminal vesicle cysts were treated in the period of January 1980-May 2002. Their symptoms, diagnostic results, treatment and outcomes were analyzed retrospectively. The mean age of these patients at diagnosis was 31 years (range 19 - 43). Two patients were associated with ipsilateral renal agenesis. Symptoms included hematospermia in 12 (75%) patients, urinary frequency in 8 (50%), hematuria after ejaculation in 6 (27.5%), perineal malaise in 6 (27.5%), infertility in 3 (13.7%), pain after ejaculation in 3 (13.7%), scrotal pain in 2 (12.5%) and dysuria in 1 (6.3%). Cyst was palpable in 81.3% patients on digital rectal examination. All patients underwent intravenous urography and cystoscopy. Others received ultrasonography, CT scanning, MRI, and vasovesiculography. The size of masses ranged from 3.8 cm x 3.0 cm x 2.6 cm to 9.6 cm x 5.2 cm x 5.0 cm. Final open surgery consisted of vesiculectomy (4 patients) and partial vesiculectomy (12). RESULTS: Postoperative course was uneventful except in 1 patient with epididymitis. All patients were free of symptoms after open surgery. CONCLUSIONS: Seminal vesicle cysts are rare but should be considered in men with hematospermia and otherwise inexplicable bladder irritation symptoms, perineal discomfort or other genitourinary complaints of unknown etiology. Diagnosis consists of digital rectal examination, transrectal and abdominal ultrasonography, CT scan or MRI. Vesiculectomy and partial vesiculectomy give excellent results.

[Application of a micromethod in ovarian cancer chemosensitivity testing in vitro].

OBJECTIVE: To assess the efficacy of microdetermination in the ATP-bioluminescence assay (ATP-CVA) of ovarian cancer chemosensitivity testing in vitro. METHODS: A micromethod ATP-CVA was employed in determining the chemosensitivity of 35 specimens of fresh ovarian cancer cells using Victor 2 Multilabel Counter. RESULTS: 1. By use of Victor 2 Multilabel Counter to determine ATP, in a range of 10(-9)-10(-5) mol/ml, a linear relationship was remarkable between the log ATP and the log luminescence. The equation was Y = 0.892X + 10.257, the correlation coefficient r was 0.998, P < 0.01. 2. The coefficient of variation obtained from ATP-CVA was 7.3%. 3. The sensitivity of the test was 92.0%, the specificity 70.0%, and the total predictive accuracy of ATP-CVA 87.1%. CONCLUSION: With the use of Victor 2 Multilabel Counter, ATP-CVA is fast, cost-effective, and easy to perform with only small error.

[Detection of surface EMG signal using active electrode].

Research of surface electromyogram(EMG) signal is important in rehabilitation medicine, sport medicine and clinical diagnosis, accurate detection of signal is the base of quantitative analysis of surface EMG signal. In this article were discussed how to reduce possible noise in the detection of surface EMG. Considerations on the design of electrode unit were presented. Instrumentation amplifier AD620 was employed to design a bipolar active electrode for use in surface EMG detection. The experiments showed that active electrode could be used to improve signal/noise ratio, reduce noise and detect surface EMG signal effectively.

[A study of aspermia-related genes by genchips and analysis of the RAP1A gene].

OBJECTIVE: To study the differential gene expression profiles between the normal and aspermia human testes by genechips. METHODS: Probes were prepared from mRNA extracted from both normal and aspermia testes and employed on Biostar H-40s genechips to detect the differential gene expression profiles. A distinctly up-regulated gene RAP1A was analyzed by bibliogrphic retrieval. RESULTS: Six hundred and twenty-three differential expressed genes were found, among which the distinctly up-regulated gene RAP1A was closely related to human sperm regulation. CONCLUSIONS: Screening the differential gene expression profiles between the normal and aspermia human testes by genechips can be used in the study of aspermia-related genes.

[Protective effect of nitric oxide synthase inhibitor (L-NAME) on germ cell apoptosis in experimentally cryptorchid rats].

OBJECTIVE: To explore the protective effect of nitric oxide synthase inhibitor (L-NAME) on the germ cell apoptosis in the rat cryptorchid. METHODS: Immature rats (22 day-old Sprague Dawley) were subjected to unilateral cryptorchid. Thirty rats were divided into three groups: sham operation group (testes still in the scrotum after operation); operation group; operation + L-NAME group(given L-NAME 10 mg/kg after operation, dip). Seven days after operation germ cell apoptosis was detected by terminal-deoxynucleotidyl transferase mediated-dUTP nick end labeling(TUNEL). Biochemical parameters (NO, NOS) were evaluated with spectrophotometric determination. RESULTS: At the 7th day after the operation, compared with the control, the number of apoptotic germ cells in the cryptorchid testis was increased significantly, but the testis weight was decreased predominantly(P < 0.01). The levels of NO and NOS in the cryptorchid were significantly higher than the control. CONCLUSIONS: The levels of NO and NOS might be involved in the germ cell apoptosis in the cryptorchid; L-NAME could protect the germ cell from apoptosis in experimentally cryptorchid rats by reducing the activity of NOS and reducing the level of NO in the testis.

[The biological characteristics of beta glucuronidase cDNA transfected renal cancer cell line GRC 1/betaG].

AIM: To establish a renal carcinoma cell line which can highly express beta-glucuronidase(betaG), and to observe the biological characteristics of the transfected cells. METHODS: Recombinant eukaryotic expression vector pcDNA3.1-betaG was constructed. It was transfected into renal cancer cells GRC-1 via liposome. The transcription and expression of betaG gene were detected by dot blot and Western blot. The biological characteristics of the betaG gene transfected cells was observed under light microscope, transmission electron microscope and flow cytometry. RESULTS: Dot blot and Western blot detection confirmed that the betaG gene had been stably integrated into the genomic DNA of the GRC-1 cells and was highly expressed. Transmission electron microscope observation showed that the lysosomes and endoplasmic reticulum were abundant, the number of microvili and process was significantly increased in the transfected cells, but growth condition and cell cycle of GRC-1 cells had no notable difference before and after transfection. CONCLUSION: A renal carcinoma cell line that can highly express betaG gene was established, which lays the foundation for further study on gene therapy.

[Typing for HLA-DR DNA by polymerase chain reaction with sequence-specific primers].

AIM: To type the HLA-DR DNA for renal transplantation by PCR with sequence specific primers (PCR-SSP). METHODS: According to nucleotide sequences of HLA-DR, 16 pairs of specific primers and a pair of positive control primers were designed and synthesized for PCR-SSP. Then HLA-DR sites of 52 donors and recipients for renal transplantation were typed by the PCR-SSP. RESULTS: All the samples were successfully typed by PCR-SSP with the synthesized primers. The results were available within 3 hours after sampling and the accuracy and reproducibility were 100%. CONCLUSION: Genotyping for HLA-DR sites by PCR-SSP with primers reported herein was a simple and accurate technique suitable for clinical application.

[Gene therapy for ovarian cancer by adenovirus-mediated transduction of cytosine deaminase gene in vitro].

OBJECTIVE: To study the value of adenovirus mediated expression of the Escherichia coli cytosine deaminase (CD) gene with 5-fluorocytosine (5-FC) in the gene therapy of ovarian cancer cells. METHODS: Ovarian cancer cells were transduced CD gene by adenovirus vector and exposed to varying concentration of 5-FC. After 5 days of incubation, MTT assays were performed to measure the cell viability. RESULTS: It was shown that tumor cells transduced with CD gene had a high sensitivity to 5-FC. Mixed cellular assay revealed that CD-postive cells had a neighbor cell killing effect on CD-negative cells when exposed to 5-FC. 20% CD positive cells killed more than 80% mixed cells. CONCLUSION: CD/5-FC gene therapy approach is an effective anti-tumor strategy in the treatment of ovarian cancer.

[Expression and correlation of C-met and estrogen receptor in endometrial carcinomas].

OBJECTIVE: To investigate the expression and correlation of C-met and Estrogen receptor (ER) in endometrial carcinomas. METHODS: We examined 55 samples of endometrial carcinomas using specific polyclonal endometrial carcinomas. METHODS: antibody and monoclonal antibody correspondent to the human C-met and ER respectively. The histochemical score was estimated according to the formula H = (i + 1) x pi, with a criterion of equal or more than 70 as considered to be positive in expression. RESULTS: The expression of C-met was correlated with both histological grade and clinical stage of endometrial carcinomas. The H scores of carcinomas in grade 3 was higher than in grade 1 and grade 2, while higher H scores were found in carcinomas in stage III and II compared to stage I (P < 0.05). The higher of the grade and stage were, the weaker the ER were expressed in endometrial carcinomas (P < 0.05). There was an inverse correlation between the expression of ER and C-met (r = -0.5842 P < 0.05). CONCLUSIONS: The expression of C-met and ER might be a useful predictor for the prognosis of endometrial carcinomas and an indicator to evaluate the effectiveness of the hormone intervention.

Inflammation, infection, and pulmonary function in infants and young children with cystic fibrosis.

Our aim was to study the effect of lower airway infection on clinical parameters, pulmonary function tests, and inflammation in clinically stable infants and young children with cystic fibrosis (CF). To accomplish this goal, a prospective cohort of screened CF patients under 4 years of age were studied, using elective anesthesia and intubation for: passive respiratory mechanics (single breath occlusion passive deflation) and lung volumes (nitrogen washout), under neuromuscular blockade; and bronchoalveolar lavage (BAL) of 3 main bronchi for cytology, cytokine interleukin (IL)-8, and quantitative microbiology. There were 22 children studied, with a mean age of 23.2 months (6.7-44 months). A greater relative risk of lower airway pathogens was associated with prior respiratory admission (3.60, 95% confidence interval [CI] 2.87-4.51), history of asthma (1.75, 95% CI 1.52-2.03), and chronic symptoms (1.50, 95% CI 1.23-1.83), especially wheeze (1.88, 95% CI 1.61-2.19). Lower respiratory pathogens (> or = 10 cfu/ml BAL) were found in 14 out of 22, and greater than 10(5) cfu/ml in 8 out of 22 subjects. The level of pathogens in BAL (log10 cfu/ml) explained 78% of the variability in percent neutrophils and 34% of the variability in IL-8 levels. Pathogen level also correlated with pulmonary function tests of specific respiratory system compliance (r -0.49, p = 0.02) and functional residual capacity over total lung capacity (r 0.49, p = 0.03). We conclude that the presence of pathogens in the lower airways correlated with levels of inflammation, respiratory system compliance, and degree of air trapping.

Relationship between sputum inflammatory markers, lung function, and lung pathology on high-resolution computed tomography in children with cystic fibrosis.

High-resolution computed tomography (HRCT) is a sensitive technique for early visualisation and location of cystic fibrosis (CF) bronchopathology, and has been shown to detect acute reversible and chronic changes. It would be expected to correlate with markers of the underlying pathological processes, such as sputum cytokines and cytology, as well as with pulmonary function tests (PFTs). Our aim was to study the relationship between PFTs, sputum cytology, and sputum cytokine interleukin-8 (IL-8) and HRCT in CF patients. Prospective standardized collection of sputum samples was performed at the time of routine annual high-resolution CT scans. Forced expired volume in 1 sec (FEV(1)) and forced vital capacity (FVC) were recorded. Sputum processing was selective, with dispersal by the three-enzyme technique. IL-8 measurements were by kit assay. HRCT scans were scored by a pediatric radiologist, blinded to clinical condition, using a modified Bhalla score.Forty-three CT scans were performed on 34 children with CF between March 1998 and April 2000. Mean age was 12.3 years (range, 6-21 years), FEV(1) (% predicted) was 67% (range, 23-120%), and mean modified Bhalla score was 11.2 (range, 0-22). Sputum IL-8 concentration (mean, 86; range, 4-150 ng/mL) and total cell count (mean, 31.9 x 10(6)/mL; range, 21.8-42.0 x 10(6)/mL) were high. FEV(1) and FVC correlated with modified Bhalla score (r = -0.66, P < 0.0001 for both), and most individual components of the score, especially mosaic perfusion (r = -0.64, r = -0.61 respectively, P < 0.0001) and extent of bronchiectasis (r = -0.61, P < 0.0001 for both). The combination of these two predicted 58% of the variability in FEV(1) on analysis of variance (P < 0.0001). Sputum total cell count correlated weakly with modified Bhalla score (r = 0.38, P < 0.05) and with FEV(1) and FVC (r = -0.36, P < 0.05; and r = -0.46, P < 0.01). Differential cell counts, cell viability, and IL-8 did not correlate with modified Bhalla scores, or with reversible components such as mucus plugging, centrilobular nodules, or peribronchial thickening. In conclusion, pathological changes on HRCT correlated with lung function but not with sputum markers of inflammation.

Interleukin-16 in tracheal aspirate fluids of newborn infants.

BACKGROUND: It is currently unknown if interleukin (IL)-16 exists in the lungs of ventilated infants, and because the predominant cells in the airways of infants with CLD are CD4+ macrophages, we hypothesized that IL-16 plays a role as a pro-inflammatory mediator in lung inflammation. AIMS: To examine if IL-16, a chemoattractant for CD4+ cells, is detectable in airway secretions of ventilated newborns. Its presence may be associated with lung inflammatory responses. STUDY DESIGN: Cohort cross-sectional study. SUBJECTS: Thirty-four mechanically ventilated newborn infants. MAIN OUTCOME MEASURES: Tracheal fluid (TF) specimens collected during the first month of life were examined for cell differentials determined from cytospin slides and supernatant was analyzed by ELISA for IL-16. RESULTS: Eighty-three cross-sectional tracheal fluid (TF) specimens were analyzed. Eleven of the 27 preterm but none of the 7 term infants developed chronic lung disease (CLD). IL-16, ranging from 203 to 42,073 pg/ml, was detected in 16 of the 46 specimens obtained from CLD infants, 1 of the 30 specimens from 16 non-CLD preterm and 2 of the 7 specimens from 7 term infants (p<0.001). Leukocyte counts (median 16.6 vs. 2.0 x 10(-9)/l, p<0.0001) and percentage neutrophils (median 93% vs. 73%, p<0.001) were higher in IL-16 positive specimens. CONCLUSION: IL-16 is detectable within the airway secretions of ventilated newborn infants and its presence is associated with a neutrophilic infiltration. Further studies are required to investigate its role in chronic inflammation in CLD.

Diagnosis and treatment of renal trauma in 298 patients.

OBJECTIVE: To investigate the diagnosis and treatment of renal trauma. METHODS: Retrospective analysis of 298 patients with renal trauma was carried out. Among them, 272 (91.3%) had blunt renal injuries and 26 (8.7%) had penetrating injuries; 123 (41.3%) had multiple complicated intraabdominal injuries and 56 (18.8%) had concomitant shock. Normal-dose-IVU examination was used in 39 patients and double-dose-IVU in 44 patients, ultrasonography in 109 patients, and CT in 45 patients. Conservative and supportive therapy was done in 193 patients (64.8%) and operation in 105 patients (35.2%). RESULTS: The positive rate was 48.7% by the normal-dose-I VU examination and 90.9% by double-dose-IVU, 78.8% by ultrasonography, and 95.6% by CT. One hundred and eighty-three patients were cured by conservative therapy and 101 by operation. Fourteen patients died. CONCLUSIONS: B-ultrasound can be conveniently used for primary assessment of renal injuries, while CT shows rapid, accurate and proper condition of a renal trauma patient. The treatment depends on the severity of the injury. The conservative therapy is employed in most cases which present slight or moderate injury and no evident massive bleeding. Severe injury requires surgical exploration. The operative approach is by using a transabdominal incision, which makes it relatively easy to explore intraabdominal organs and control the injured kidney. It is also very important to control shock and prevent other severe complications in the early stage of the treatment.

[Anticancer effects of cytosine deaminase gene/5-fluorocytosine therapies to ovarian cancer in vivo].

OBJECTIVE: To evaluate the anticancer effects of cytosine deaminase (CD) gene/5-fluorocytosine (5-FC) to human ovarian papillary serous cystadenocarcinoma in nude mice. METHODS: Replication-deficient adenovirus containing CD gene was used to transfect human embryonal kidney cell line 293, and the adenovirus's concentration was up to 1 x 10(10) pfu (plaque forming unit)/ml after purification. Ten female BULB/C nude mice were implanted subcutaneously with approximate 0.08 g tissue of ovarian papillary serous cystadenocarcinoma from the parental generation, and they were divided into test group and control group. While the tumors grew with diameter of approximate 3mm, test group received intratumoral injections of adenovirus-CD at 1 x 10(9) pfu in a 100 microl volume on day 1, 3, 5. These animals were administered 5-FC at doses of 400 mg/kg twice daily by intraperitoneal injection from day 1 to day 7. Normal saline was used in control group with the same volume and by the same methods. Tumor volumes were measured in following days. RESULTS: (1) Compared with control group, the growth of tumors in test group was significantly suppressed. Twenty days after the therapies, the tumor volumes in test group and control group were (91 +/- 68) mm(3) and (238 +/- 122) mm(3) respectively, there was significant difference. This difference lasted for approximately 30 days (P = 0.045 - 0.019). (2) The mean time of tumor volume doubling in test group and control group were (8.1 +/- 0.7) days and (6.4 +/- 0.7) days, respectively (P < 0.05). CONCLUSIONS: This research has shown the significant effects of CD gene/5-FC on human ovarian cancer in nude mice model. The results may be the important basis of clinical trials. They suggest that this toxic gene/prodrug therapy system may play an important role in ovarian cancer therapy in the future.

Tissue array technique for p53 expression in human ovarian neoplasms.

OBJECTIVE: To investigate the expression of p53 in human ovarian neoplasms by tissue array technique. METHODS: The expression of p53 protein in various ovarian tissues was studied by tissue array and immunohistochemistry. RESULTS: The expression rate of p53 was 33% in ovarian cancers. There were no expressions in normal ovarian tissues, benign ovarian neoplasms and borderline ovarian neoplasms (P < 0.005). p53 expression was not associated with age of patients and grade of differentiation (P > 0.05) while p53 expression was associated with tissue types (P < 0.05). CONCLUSION: The expression rates of p53 in ovarian cancers were obviously increased.

[Development of research for combined gene therapy and radiotherapy for cancer patients].

Cancer treatment is one of the most important fields in medical research. All strategies such as radiotherapy, chemotherapy, surgery, and gene-based therapy have their own advantage and disadvantages. Nowadays, a novel method which combined traditional therapy with gene therapy plays an important role in the field of cancer research. This review described the current state of combined therapies of gene-based and radiate radiotherapy. Meanwhile, we evaluated the prospect of this combined treatment.

[The relationships of PSA, FPSAR, clinical and pathological stage in patients with prostate cancer].

OBJECTIVES: To investigate the relationship between clinical and pathological stage, serum prostate specific antigen (PSA) concentration and free-to-total PSA ratio (FPSAR) in patients with prostate cancer. METHODS: Clinical and pathological stage were determined on the basis of pathological examination and clinic material in 42 prostate cancer patients treated by prostatectomy. PSA and FPSAR were measured before the operation. Spearman rank correlation was applied to evaluate the relationship between clinical and pathological stage, serum PSA concentration and FPSAR. RESULTS: Serum PSA concentration was significantly positively correlated with pathological stage(P < 0.05) but not correlated with clinical stage (P > 0.05) in prostate cancer patients. FPSAR was significantly correlated with pathological stage and negatively correlated with clinical stage in prostate cancer patients (P < 0.05). CONCLUSIONS: FPSAR is a more powerful predictor of clinical stage, pathological stage and prognosis than PSA.
