Three-wall orbital decompression superiority to 2-wall orbital decompression in thyroid-associated ophthalmopathy.

PURPOSE: We evaluated the results of the 2-wall and 3-wall orbital decompression in patients with Graves' disease. PATIENTS AND METHODS: In this study, we present a consecutive series of 12 patients (18 orbits) who were submitted to orbital decompression by endoscopic transnasal medial wall combined with transantral inferior wall approach and 7 patients (8 orbits) who were submitted to orbital decompression by endoscopic transnasal medial wall, transantral inferior wall combined with transcutaneous lateral wall approach. The degree of exophthalmos was evaluated with the Hertel exophthalmometer preoperatively and postoperatively in the 24th hour, and first, third, and ninth months. RESULTS: At the end of the third month, the exophthalmos decreased by a mean of 4.38 mm (range, 3 to 7 mm) with the 2-wall decompression and 7.75 mm (range, 5 to 12 mm) with the 3-wall decompression. Visual acuity maintained or improved during the follow-up period. Ocular motility disturbance occurred in 1 patient after 2-wall decompression and in 1 patient after 3-wall orbital decompression. Postoperatively, new-onset diplopia was seen in only 1 patient after 2-wall orbital decompression. CONCLUSION: The 3-wall (endoscopic transnasal medial wall, transantral inferior wall combined with transcutaneous lateral wall) approach is as safe as the 2-wall approach. Proptosis reduction is much better with the 3-wall orbital decompression.

[Aneurysmal bone cyst of the mandible: a case report].

Aneurysmal bone cyst is a benign lesion that is more commonly seen in long bones and the vertebra. Its occurrence in the craniofacial region is rare. A nine-year-old boy presented with a well-demarcated, painless, fixed mass, 3 x 3 cm in size, in the left preauricular region, with no fluctuation or crepitation. Computed tomography showed a lesion in the left mandibular ramus with a fluid-fluid level, extending to the mandibular condyle, with erosion to the adjacent cortex. Total curettage was performed via a transoral approach under general anesthesia. Histopathologic diagnosis was made as an aneurysmal bone cyst. No recurrence or complication were seen during a follow-up period of four years.

[Tracheobronchial foreign body aspiration following partial laryngectomy: a case report].

Tracheobronchial foreign body aspiration is uncommon in adults and one etiologic factor is laryngeal surgery. A 50-year-old male patient presented with cough, high fever, and severe dyspnea three months after partial laryngectomy for squamous cell carcinoma of the larynx. A computed tomography scan of the thorax showed a hyperdense mass lesion in the right main bronchus, suggesting a foreign body. Diagnostic bronchoscopy was performed under general anesthesia and a small bone fragment, 2 x 1.3 cm in size, was removed from the right bronchial lumen. The patient showed slow improvement due to prolonged atelectasis until complete clinical and radiologic recovery.

Surgical approaches to juvenile nasopharyngeal angiofibroma.

INTRODUCTION: Juvenile nasopharyngeal angiofibromas are highly vascular, non-encapsulated tumours affecting predominantly young males. These lesions are benign histologically but they may become life-threatening with excessive bleeding or intracranial extension. MATERIAL AND METHODS: The surgical approaches to 22 male patients with nasopharyngeal angiofibromas are reviewed. A modification of midfacial degloving performed without rhinoplasty incisions and lateral osteotomies is described. RESULTS: The patients' ages ranged between 9 and 26 years (mean 14.9). Three stage I tumours, 8 stage II tumours, 6 stage III tumours and 5 stage IV tumours were included into this study. All stage I lesions and one stage II lesion were treated via transnasal endoscopic approach. A modified midfacial degloving approach was used for the removal of seven other stage II lesions, all six stage III lesions, and three stage IV lesions. A combined midfacial degloving-infratemporal fossa Fisch C-transcranial approach was the route chosen for the remaining two stage IV lesions. The complications encountered in the postoperative course include temporary facial palsy in one patient (following a Fisch C infratemporal resection), mild crusting in the nasal cavity in 8 patients, and facial paraesthesia in 6 patients whose tumours were resected via midfacial degloving, and rupture of the subpetrous part of the internal carotid artery in one patient. CONCLUSION: The suggested treatment of juvenile nasopharyngeal angiofibroma consists of an endoscopic transnasal approach for early stage lesions, and a modified midfacial degloving for almost all of the advanced lesions. The latter approach is very useful considering surgical exposure, duration of surgery, cosmetic outcome, and morbidity. It can be combined with an infratemporal approach or craniotomy if necessary.

A study of prognostic factors in sudden hearing loss.

We investigated the prognostic significance of the presence or absence of vertigo and tinnitus, the timing of the initiation of treatment, the type and severity of hearing loss, and age in 72 patients who had experienced sudden hearing loss. We found that the factors associated with a positive prognosis were the absence of vertigo, the presence of tinnitus, initiation of treatment within 7 days, a greater degree of hearing loss in the low frequencies, and a hearing loss of less than 45 dB. Age had no effect on prognosis.

Local control of primary oropharyngeal malignant melanomas with limited tissue excision: a report of three cases.

Mucosal malignant melanomas are rare lesions, and they have different characteristics from their cutaneous counterparts. Since extended excisions of mucosal malignant melanomas located in the oropharyngeal region cause significant morbidity, limited surgical excision comes into consideration. Three cases of extensive oropharyngeal malignant melanomas were resected with 0.5-1.5-cm healthy tissue margins. The cases were followed for local recurrences. Case 1 applied radiotherapy and chemotherapy in addition to immunotherapy, and the patient is still alive without any local recurrences 18 months after surgery. The patient in case 2 underwent radiotherapy and immunotherapy and died 6 months after surgery. The patient in case 3 received chemotherapy in addition to immunotherapy and died 12 months after surgery as a result of distant metastasis. All cases were without any local recurrences. Surgical excision with limited tumor-free tissue margins may be the surgery of choice to prevent morbidity associated with wide resection of oropharyngeal malignant melanomas if other authors also reconfirm these results with many more cases in the future.

Laryngotracheal reconstruction with a muscle-pedicle hyoid bone flap: a series of 23 patients.

We conducted a study to evaluate the effectiveness of muscle-pedicle hyoid bone flaps in the repair and reconstruction of the larynx and trachea in 23 patients who had undergone extended partial laryngectomy. Of this group, 17 patients had undergone surgery to correct laryngotracheal stenosis and 6 to remove laryngeal carcinoma. Intervention resulted in successful decannulation in 20 of the 23 patients (87.0%). Of the 3 patients who did not experience a successful outcome initially, 2 were successfully decannulated following a second intervention; the other patient required a total laryngectomy because of cancer recurrence. In light of our follow-up results, we conclude that hyoid bone appears to be an effective autograft in such cases.

Hemangioendothelioma of the frontal bone in a child.

Hemangioendothelioma is a vascular tumor of endothelial cell origin. It may involve bone or soft tissues and can behave like a benign or malignant tumor. In the literature, there are several case reports on the involvement of the head and neck region, but involvement of the frontal bone is extremely rare. This report presents the case of a 4-year-old boy with the diagnosis of spindle cell hemangioendothelioma of the frontal bone, who was treated by wide surgical resection.

Giant frontoethmoid mucocele with intracranial extension: case report.

We report the case of a 31-year-old woman who came to us with a giant frontoethmoid mass that had extended into the intracranial region and invaded the left orbit. We removed the lesion in its entirety via a combined intranasal and transcranial approach. We discuss the advantages and disadvantages of different surgical approaches, and we explain why we recommend a combined approach for the type of giant lesion that we encountered.

Spontaneous cerebrospinal fluid rhinorrhea associated with empty sella: a transnasal-transsphenoidal repair of the fistula.

Spontaneous cerebrospinal rhinorrhea is a rare clinical condition. More commonly it develops due to cerebral tumors or hydrocephalus; it is seldom seen in association with empty sella syndrome. A sixty-three-year-old woman who presented with left nasal discharge was hospitalized on the suspicion of CSF rhinorrhea. Analysis of the nasal fluid revealed a clear color, a positive Pandy's test, and a positive result for glucose (72%). On magnetic resonance scans, herniation of the suprasellar cistern was observed into the sellar cavity, and the left sphenoid sinus was filled with contrast medium. The repair of the fistula was performed via a transnasal and transsphenoidal approach. The patient was followed-up for five years without any symptoms or recurrences.

Arteriovenous malformation of the maxillary sinus and mandible: a case report.

Vascular tumors are rarely encountered in the paranasal sinuses. When they do arise there, they can be life-threatening because of the risk of bleeding. Embolization alone and in combination with surgical resection has been the mainstay of treatment. We report the case of a prepubertal girl who experienced two recurrences of arteriovenous malformation, one in the maxillary sinus and the other in the mandible. To our knowledge, this is the first report in the literature of recurrences arising in these two locations.

Tracheal reconstruction with autogenous composite nasal septal graft.

We describe lateral resection of tracheal tumors and repair of the defect with a composite graft of nasal septal mucosa and cartilage. The preliminary experience with this technique in 3 patients was satisfactory. The procedure can be used for patients who are not candidates for standard resection and end-to-end anastomosis.

Aneurysmal bone cyst of the sphenoid bone.

Aneurysmal bone cyst (ABC) is an uncommon benign lesion that rarely presents in the craniofacial region. Aneurysmal bone cysts represent nearly 1.4% of all bone tumors, and among those, only 3% are located in the cranium. In this study, we report on an ABC located in the sphenoid bone with superior nasal cavity and ethmoid extension. The presenting symptom of our patient was headache, followed by diplopia, loss of visual accuracy, and abduction restriction. We successfully resected the lesion by a combined subcranial-midfacial degloving approach without any complications or recurrence.

Tracheal reconstruction with free composite cartilage graft: a case report.

Tracheal resection followed by an end-to-end anastomosis has been the method of choice for the treatment of tracheal neoplasms. We hereby report a 40-year-old female patient in whom we performed reconstruction with composite nasal septal cartilage graft following one-staged tracheal resection. Histopathologic diagnosis was well-differentiated adenoid cystic carcinoma. Fiberoptic and biopsy examinations made within the first postoperative month showed no residual or recurrent tumor at the resection site. The patient received radiotherapy postoperatively. She has been free of disease for 18 months. To our knowledge, the use of composite nasal septal cartilage graft in the reconstruction of tracheal tumors has not been previously reported.

[Orbital decompression by endoscopic transnasal and transantral approach in thyroid-associated ophthalmopathy].

OBJECTIVES: We evaluated the results of orbital decompression in patients with Graves' disease. PATIENTS AND METHODS: Orbital decompression was performed by endoscopic transnasal medial wall approach and transantral inferior wall approach in 18 eyes of 12 patients (8 males, 4 females; mean age 41 years; range 24 to 65 years). The degree of exophthalmos was evaluated with the use of the Hertel exophthalmometer preoperatively, and postoperatively in the 24th hour, first and third months. RESULTS: At the end of the third month, exophthalmos decreased by a mean of 4.4 mm (range 3 to 7 mm). Visual acuity and field defects improved considerably in all patients. Complications occurred in three cases (25%) including diplopia, displacement of one eye inferiorly, and maxillary sinusitis caused by the encroachment of the adipose tissue upon the ostium, respectively. CONCLUSION: With the increasing ease of surgical procedures and enhanced experience on the part of surgeons, successful treatment of exophthalmos has become possible with decreased complication rates.
