Intravitreal gnathostomiasis and review of literature.

Intravitreal gnathostomiasis is a rare disease. To our knowledge, only five case reports are available in the literature wherein the parasites were surgically removed from the vitreous cavity and the species were identified. In this report, we document the clinical features of intravitreal gnathostomiasis along with the route of entry and parasitic migration pattern in ocular tissue, surgical management, and electron microscopic features of the retrieved advanced third-stage larva of Gnathostoma spinigerum (aL3) for a 48-year-old Indian woman with excellent postsurgical visual recovery.

Visual performance: Comparison of foldable intraocular lenses.

PURPOSE: To compare best distance visual acuity, color perception, and contrast sensitivity between eyes with an AcrySof Natural intraocular lens (IOL) (Alcon Laboratories Inc.) and fellow eye with an AcrySof IOL and to compare results in the IOL groups to those in normal, similarly aged patients with clear phakic eyes. SETTING: Sri Sankaradeva Nethralaya, Guwahati, India. METHODS: This prospective randomized parallel group single-center patient-masked examiner-masked study comprised 26 eyes of 13 healthy patients with bilateral age-related cataract. All patients had cataract surgery by phacoemulsification in both eyes; 1 eye received an AcrySof Natural IOL (SN60AT) and the fellow eye, an AcrySof (SA60AT) IOL. Final postoperative examination was performed at the end of 18 months. In addition to other findings, visual acuity, color perception, and contrast sensitivity were recorded. A group of normal, similarly aged phakic individuals with clear lenses was examined for contrast sensitivity. The records thus obtained were analyzed statistically. RESULTS: The AcrySof and AcrySof Natural IOL groups had a statistically significant improvement in visual acuity and color perception in comparison to preoperative status, and this was maintained until the last postoperative follow-up at 18 months. There was no statistically significant difference in the findings between the 2 IOL groups. The AcrySof and the AcrySof Natural IOL groups showed significant improvement in contrast sensitivity from the preoperative to the postoperative period (both P < .001). However, the AcrySof Natural IOL group was better than the AcrySof IOL group when each group was compared separately to similarly aged phakic individuals with clear lenses. CONCLUSIONS: Pseudophakia following AcrySof Natural SN60AT and AcrySof SA60AT IOL implantation led to equivalent visual acuity and color perception. But the contrast sensitivity in the AcrySof Natural group was better and near normal in reference to a healthy, age-matched control group.

Management of posteriorly dislocated endocapsular tension ring and intraocular lens complex.

We report a case of late posteriorly dislocated endocapsular tension ring (ECR) and intraocular lens (IOL) complex into the vitreous cavity that behaved as a single 13 mm disc. A 3-port pars plana vitrectomy was performed, and perfluorocarbon liquid was used to retrieve the ECR-IOL complex to the retropupillary area. A 3-point scleral fixation was performed to reposition the inseparable ECR-IOL complex.

Effect of mitomycin C on ostium in dacryocystorhinostomy.

BACKGROUND: To observe the effect of intraoperative mitomycin C of varying concentration on postoperative ostium size after external dacryocystorhinostomy (DCR). METHODS: In this prospective controlled study, 60 cases of DCR were taken up irrespective of age and sex. Cases were divided into three groups. Group 1 was taken as a control group and operated without mitomycin C; in experimental group 2 and group 3, mitomycin C at a concentration of 0.05 mg/mL and 0.4 mg/mL was applied, respectively, for 2 min. Each group consisted of 20 cases. Half of the cases in each group underwent single-flap DCR (SFDCR) and half underwent double-flap DCR (DFDCR) surgery. Nasal endoscopic evaluation was performed on the first postoperative day, at 2 weeks and at 6 months after surgery. A Student's t-test was used to compare the osteotomy size of each group. RESULTS: At the end of the final follow up, average size of the ostium in group 1 was 3.6+/-2.36 mm2 in SFDCR and 4.5+/-3.59 mm2 in DFDCR, in group 2 was 4.8+/-4.82 mm2 in SFDCR and 4.9+/-4.12 mm2 in DFDCR, and in group 3 was 16.6+/-6.80 mm2 in SFDCR and 17.5+/-9.07 mm2 in DFDCR. The ostium size in group 3 was found to be significantly bigger in comparison with group 1 (P=0.0001 in SFDCR, P=0.001 in DFDCR) and with group 2 (P=0.0009 in SFDCR, P=0.001 in DFDCR). No statistically significant difference of ostium size was found in SFDCR and DFDCR. CONCLUSION: Mitomycin C in appropriate dose minimizes postoperative fibrosis and granulations, thereby maintaining a bigger postoperative ostium throughout the postoperative observation period.

Management of a posteriorly dislocated endocapsular tension ring and a foldable acrylic intraocular lens.

We report a rare case of late spontaneous extension of a posterior capsule tear from lateral traction of an endocapsular ring, resulting in dislocation of the capsular tension ring and acrylic intraocular lens (IOL) into the vitreous cavity. A 3-port pars plana vitrectomy was performed to explant the prostheses; the eye was made pseudophakic by placement of a scleral-fixated posterior chamber IOL. The combined triple procedure was safe and effective.

Chloroma of the orbit in a non-leukemic adult: A case report.

Granulocytic sarcoma has been reported as an isolated lesion in non-leukemic patients. However, the majority of these subjects develop acute leukemia within a mean interval of 10.5 months from the time of diagnosis. We present a case of granulocytic sarcoma of the orbit in a non-leukemic patient who was treated with chemotherapy and is doing well after a 2-year period. Patients who do not develop leukemia have a better prognosis. Appropriate treatment is delayed in most cases because of a high rate of misdiagnosis.
