[Basic concept in computer assisted surgery].

OBJECTIVE: To investigate application of medical digital imaging systems and computer technologies in orthopedics. METHODS: The main computer-assisted surgery systems comprise the four following subcategories. RESULTS: (1) A collection and recording process for digital data on each patient, including preoperative images (CT scans, MRI, standard X-rays), intraoperative visualization (fluoroscopy, ultrasound), and intraoperative position and orientation of surgical instruments or bone sections (using 3D localises). Data merging based on the matching of preoperative imaging (CT scans, MRI, standard X-rays) and intraoperative visualization (anatomical landmarks, or bone surfaces digitized intraoperatively via 3D localiser; intraoperative ultrasound images processed for delineation of bone contours). (2) In cases where only intraoperative images are used for computer-assisted surgical navigation, the calibration of the intraoperative imaging system replaces the merged data system, which is then no longer necessary. (3) A system that provides aid in decision-making, so that the surgical approach is planned on basis of multimodal information: the interactive positioning of surgical instruments or bone sections transmitted via pre- or intraoperative images, display of elements to guide surgical navigation (direction, axis, orientation, length and diameter of a surgical instrument, impingement, etc. ). And (4) A system that monitors the surgical procedure, thereby ensuring that the optimal strategy defined at the preoperative stage is taken into account. CONCLUSION: It is possible that computer-assisted orthopedic surgery systems will enable surgeons to better assess the accuracy and reliability of the various operative techniques, an indispensable stage in the optimization of surgery.

[A retrospective study of one case of human infection by the highly pathogenic avian influenza A (H5N1)].

OBJECTIVE: To describe the clinical features of the infection caused by the highly pathogenic avian influenza A (H(5)N(1)). METHODS: A previously healthy 24 year old woman presented to our hospital on November 7, 2005. She was confirmed to be an H(5)N(1) infected case after death. The clinical, radiological and epidemiological data were analyzed. RESULTS: The patient had a history of direct contact with diseased and dead poultry (chicken and duck). The disease course was 10 days from onset of illness to death, and fever preceded dyspnea by 5 days. On admission, the striking characteristics were acute community-acquired pneumonia (CAP) and acute respiratory distress syndrome (ARDS), and the major radiographic abnormalities included extensive infiltration bilaterally, focal consolidation and air bronchograms. The radiographic and clinical deterioration was rapid, and the patient died in less than 3 days after hospitalization. The diagnosis of influenza A (H(5)N(1)) was confirmed by means of reverse transcriptase-polymerase chain reaction (RT-PCR) and real-time PCR on specimens of the lower respiratory tract, performed by Chinese Center for Disease Control. The postmortem examination showed bronchial hyperemia, extensive consolidation, serous cavity effusions, disseminated intravascular coagulation (DIC) and multiple organ failure (MOF). CONCLUSIONS: Human infection by the highly pathogenic avian influenza A (H(5)N(1)) is a fatal communicable disease. Information of avian influenza A (H(5)N(1)) virus, more attention to the epidemiologic data, and early intervention are critical in reducing the mortality.

[The relationship between expression of Fhit and biological behaviors of hypopharyngeal carcinoma].

OBJECTIVE: To investigate the expression of fragile histidine triad gene protein (Fhit) and the possible relationship between Fhit and biological behaviors of hypopharyngeal carcinoma. METHOD: The expression of Fhit protein in 66 cases with hypopharyngeal carcinoma were detected by rabbit antibody to human FHIT gene protein with SP immunohistochemical method. The relationship between historical grade and lymphatic metastasis were analysed. RESULT: (1) The positive expression rate of Fhit was 42.42% (28/66) in hypopharyngeal carcinoma, and 83.33% (55/66) in normal hypopharyngeal tissue, respectively. There was a significant difference in the expression of Fhit between the cancer tissue and normal tissue (P < 0.01). (2) The positive expression of Fhit in well, moderate, poor differentiated hypopharyngeal carcinoma were 65.38% (17/26), 38.10% (8/21) and 26.32% (5/19), respectively. There are significant statistic difference among them each other (P < 0.01). (3) The positive expression of Fhit was 75.00% (12/16) in the group without lymph node metastasis, and 36.00% (18/50) with lymph node metastasis. CONCLUSION: The expression of Fhit in hypopharyngeal squamous cell carcinoma lower than normal tissue. The expression of Fhit significantly associated with historical grade and lymphatic metastasis in hypopharyngeal squamous cell carcinoma. It suggested that the expression of Fhit decreased may plays an important role in the development and progression of the tumor, and thus may become a new prognostic marker in hypopharyngeal carcinoma.

[Bone morphing system for ligament balancing in total knee arthroplasty].

OBJECTIVE: To investigate effectiveness of applying the Bone Morphing based image-free computer-assisted system for the ligament balancing management in the total knee arthroplasty (TKA). METHODS: Between November 2002 and June 2003, twenty-one posterior stabilized total knee prostheses (Craver, France) were implanted in 21 patients using the Bone Morphing based image-free Ceravision system. This cohort included 5 men and 16 women with an average age of 72.4 years, two undergoing high tibial osteotomy and 1 undergoing distal femoral osteotomy before. The preoperative deviation was measured by the full-length AP X-rays. The knees were in varus deviation in 14 patients and in valgus deviation in 7 patients, with an average of 2.36 degrees (varus 13 degrees-valgus 13 degrees). The frontal X-rays of the knee were assessed, the mean value of the varus force-stress test was 8.47 degrees (varus 2 degrees-varus 20 degrees), and the mean value of the valgus force-stress test was 3. 63 degrees (varus 7 degrees-valgus 12 degrees). RESULTS: With the Ceravision-recorded data, the intraoperative alignment was assessed, the mean lower limb axis was 3.33 degrees (varus 12 degrees-valgus 10 degrees), and compared with the preoperative data, the difference was significant (P < 0.05); the mean value of the varus force-stress test was 6. 47 degrees (varus 0 degree - varus 24 degrees), the mean value of the valgus force-stress test was 4.32 degrees (varus 8 degrees-valgus 15 degrees), and compared with the preoperative data, the difference was significant (P < 0.05). The post-prosthetic alignment on Ceravision with a deviation of 0.175 degrees (varus 2 degrees-valgus 3 degrees) was compared with the postoperative alignment by the full-length AP X-rays, with a deviation of 0.3 degrees (varus 3.5 degrees-valgus 1.5 degrees), the difference wasn't significant (P > 0.05). The clinical check-up performed 3 months after operation showed that the average range of movement (ROM) was 115 degrees (105-130 degrees), the mean frontal laxity was 0.27 mm (0.2-0.5 mm). The femoral and tibial components were implanted in the satisfactory 3 dimensional position without ligament imbalance in all the patients, and there were no instability or patella complications. CONCLUSION: Utilization of the Bone Morphing based image-free computer-assisted system can achieve an accurate component 3 dimensional alignment, optimal bone resection, optimal control of surgical decision in releasing the soft tissues, rotating the femoral component to gain an extension/flexion rectangular gap, and managing the ligament balancing so as to achieve a satisfactory initial clinical outcome. This system can be routinely used in the TKA.

[Self hearing recovery in infants with hearing loss after universal newborn hearing screening system].

OBJECTIVE: To analyze the reasons of hearing recovery of newborns and infants which were diagnosed hearing loss within 3 months old by universal newborn hearing screening (UNHS) system. METHOD: Newborns and infants born from Nov. 2001 to Jun. 2005 in Shanghai who failed UNHS were studied. Audiometric evaluations were performed in Shanghai Children's Hearing and Speech Center. Those with hearing loss were diagnosed within 3 months old and were followed-up at least 6 months. Analysis was carried out targeting for hearing recovery cases. RESULT: Six hundred and eighty-one cases failed the UNHS and were confirmed hearing loss within 3 months old. During the 6-24 months period of followed-up, 94 (13.8%) of 681 infants caught normal hearing. Among these 94 infants(115 ears), otitis media with effusion was found in 56 cases (59.6%), no reasonable cause was found in the other 38 cases (40.4%). Among the 115 ears which recovered to normal, there were 105 ears (91.3%) with primary diagnosis of mild hearing loss, 8 ears (7.0%) with moderate hearing loss and 2 ears (1.7%) with moderate-severe hearing loss. No recovery was found in the cases with primary diagnosis of severe and profound hearing loss. CONCLUSION: Newborns and infants with hearing loss diagnosed within 3 months old have possibility to recover as they are growing. The critical reasons might be self-recovery from otitis media with effusion and outcomes of central auditory system development.

[ABR and CNAP combined monitoring during the lateral skull base surgeries].

OBJECTIVE: Through the application of ABRs and CNAPs monitoring during the lateral skull base surgeries, we intended to detect the causes of intraoperative hearing damage and estimate the value of the techniques. METHOD: Intraoperative auditory monitoring were performed in 14 lateral skull base surgeries in which hearing preservation were attempted. ABRs was recorded in all the steps of the procedure, while CNAPs was recorded after opening the dura. All patients' hearing levels were followed up. RESULT: All the patients had auditory evoked potentials (AEPs) change during the procedure, especially after the step of drilling [prolonged (0.26 +/- 0.19) ms] and during the encephalic manipulations [prolonged (0.68 +/- 0.38) ms]. Some patients' AEPs recruited in the end of procedure. CONCLUSION: The intraoperative auditory monitoring could play a role in preventing the hearing damage in the procedure. Drilling may cause the hearing damage. After relieving the manipulation inducement, the hearing damage would partially recruit. The prognosis of hearing is related to the prolongation of wave latencies in the procedure. A combination use of the ABR and CNAPs may be a better method of intraoperative auditory monitoring.

[Expression and localization of merlin in vestibular schwannoma].

OBJECTIVE: To clarify the expression and subcellular localization of merlin in vestibular schwannoma. METHODS: Fifty four paraffin embedded vestibular schwannoma samples confirmed by pathology after resection were included in the study. The expression of merlin in vestibular schwannoma was analyzed by immunohistochemistry. Nerve tissues that were resected during surgical treatment for trigeminal neuralgia and Meniere's disease were used as control. Western blotting was used to analyze the electrophoresis migration of merlin in the acoustic neuroma. Image analysis was used to calculate the positive expression percentage of merlin in each individual. The expression percentage of merlin in the tumor tissue was compared with age and gender of the patients, clinical course of the tumor, tumor growth index, tumor diameter and clinical stage. RESULTS: Merlin was expressed in 0 to 87.5% of the cells in vestibular schwannoma tissue with a mean of (46.66 +/- 5.75)%. There was a negative correlation between merlin expression percentage and tumor growth index. There were no correlations between merlin expression percentage and the age, gender, tumor diameter and clinical stage. There exists a difference for the location of merlin, mainly in the nucleus and perinucleus. There was also a cytoplasmic location. Merlin in the tumor tissue was shown by western blot to be in 65000 and 125000 positions. CONCLUSIONS: Merlin was expressed in vestibular schwannoma tissue, with a different intra-cellular location. Merlin might also exist as a complex with other proteins in the tumor tissue.

[Intratympanic dexamethasone medication for sudden sensorineural hearing loss: clinical evaluation].

OBJECTIVE: To investigate clinical efficacy of intratympanic dexamethasone injection for sudden sensorineural hearing loss. METHOD: Subjects received an course of at least 7-day dexamethasone [5 g/(L x d)] intratympanic injection. Pure-tone averages (PTA) calculated using air conduction thresholds at 4 -frequencies (500, 1000, 2000, 4000 Hz) at pre-injection and post-injection were compared. The change in PTA greater than 10 dB HL was considered to be effective. RESULT: PTA at pre-injection and post-injection were (65.65 +/- 24.73) dB HL and (50.25 +/- 25.59)dB HL respectively in 21 patients with sudden sensorineural hearing loss. There was significant improvement in PTA (P < 0.01). Improvement in hearing was found in 47.6% patients. Promising results of intratympanic dexamethasone medication were found in cases with shorter duration or/and without vertigo. No unexpected adverse events such as otitis media or perforated tympanic membrane or worsening in hearing occurred during the injection or follow-up period. CONCLUSION: Intratympanic dexamethasone medication can be beneficial in treating patient with sudden sensorineural hearing loss. Duration and vertigo are two important factors for prognosis.

METHODS: Between March 1993 and May 2005, 32 patients with jugular foramen region tumors were operated. The mastoid and neck approach, the infratemporal fossa approach with and without rerouting the facial nerve, the combined transcochlear and infratemporal fossa approach were respectively used according to the tumor type, size, auditory, facial and lower cranial nerve functions. The cranial nerve monitoring was used and a postoperative CT or MRI scanning was taken in all cases. RESULTS: In this series of 32 patients, 13 were paragangliomas, 10 schwannomas,2 meningiomas, 2 giant cell tumors, 1 chondrosarcoma, 1 mucochondrosarcoma, 1 adenoid cystic carcinoma, 1 rhabdomyosarcoma and 1 case cholesterol Granuloma. The follow-up period was 3-60 months. Total removals were achieved in 26 cases, near-total removal in 5 cases, and subtotal removal in one case. One patient died after surgery because of intracranial hemorrhage. Another patient died two year after operation because of recurrence. Postoperative cerebrospinal fluid leakage was occurred in 5 cases and meningitis in two cases. They were all treated conservatively. The facial function one week after operation was grade 1-2 in 13 cases, grade 3-4 in 12 cases and grade 5-6 in 7 cases. It changed into grade 1-2 in 23 cases, grade 3-4 in 7 cases and grade 5-6 in 2 cases. The postoperative hearing was improved in 4 cases, stable in 10 cases, deteriorated in 11 cases and totally lost in 7 cases. The lower cranial nerve function was normal in 9 cases, transient palsy in 11 cases, permanent paralysis with compensation in 10 cases and without compensation in 2 cases. CONCLUSION: The jugular foramen region tumor could be removed with maximal function preservation by using various surgical approaches. The postoperative life quality was acceptable in most cases. Surgical techniques and postoperative complications management were fundamental in achieving a good result.

[Expression of glutathione-S-transferase fusion protein and human CCL3L1 protein].

OBJECTIVE: To clone human CCL3L1 cDNA and to express and purify the glutathione-S-transferase (GST) fusion protein and human CCL3L1 protein. METHODS: Total RNA was isolated from breast cancer cell line MCF7. CCL3L1 cDNA including open reading frame was obtained by RT-PCR. PCR product was digested with EcoR I and cloned into the pGEX-4T-1 vector. The plasmids from positive clone was prepared and sequenced to confirm the CCL3L1 in correct fusion form. pGEX-4T-CCL3L1 was transfected to BL21 E. coli via isopropyl-beta-D-thiogalactoside (IPTG) induction to produce GST-CCL3L1 fusion protein, which was further detected by SDS-PAGE and Western blotting. RESULTS: As shown and confirmed by restriction endonuclease digestion analysis, CCL3L1 was correctly inserted into pGEX-4T-1 vector. The expressed fusion protein had a relative molecular weight of approximately 34 kD. CONCLUSION: GST-CCL3L1 fusion protein can be successfully expressed using appropriate vector.

[Isolation, in vitro culture and identification of cardiac stem cells from neonatal SD rats].

OBJECTIVE: To isolate and culture cardiac stem cells (CSCs) in vitro and evaluate their potential of differentiation into functional cardiac myocytes. METHODS: Myocardial tissues obtained from neonatal SD rats were cut into pieces of 0.5-1.0 mm(3), and digested twice for 5 min at 37 degrees C; with 0.2% trypsin and 0.1% collagenase II. The remaining tissues were cultured in complete explant culture medium (CEM) at 37 degrees C; in the presence of 5% CO(2). About a week later, a layer of fibroblast-like cells was generated from the adherent explants. These cells were passaged and seeded at about 1x10(6) cells/ml in poly-D-lysine-coated multi-well plates in cardiosphere-growing medium. When beating of the cultured cells was observed (at week 2), flow cytometry and immunohistochemistry were performed for identification of the primary and passaged cells. RESULTS: The primary cells were successfully cultured from the digested myocardial tissue, and flow cytometry demonstrated the phenotype of c-kit(+)CD31(+)CD34(-)CD45(-)CTnT(-). After cell passage for about two weeks, single beating cells and cell clusters with synchronized contraction were seen microscopically, and their phenotype was converted to c-kit(+)CD31(-)CD34(-)CD45(-) CTnT(+). Immunohistochemistry staining identified CTnT expression in the passaged cells but not in the primary cells. CONCLUSIONS: A cell population with the phenotype c-kit(+)CD31(+)CD34(-)CD45(-)CTnT(-) has been obtained from neonatal SD rat heart, which possesses the potential to differentiate in vitro into beating cardiac myocytes and express CTnT protein.

[Current researching situation of mucosal irritant compontents in Araceae family plants].

Plants in Acaceae family are often considered as ornamental and medicines. However many of them have irritation properties. As medicinal plants some of them are recorded in Chinese Pharmacopoeia and they are figured as poisonous. Through investigating the domestic and overseas studied paper, the needle-like calcium oxalate crystal exits in the plants of Acaceae family could be thought as irritation components of them. This conclusion is same with the studied conclusion of our study group in the medicines plant of Pinellia ternate belonging to the Acaceae family and our studies showed that the needle-like calcium oxalate crystal was the main irritation component of raw P. ternate. The irritated mechanism of raphides is relevant to its special shape, the protein enzyme adhering to it and idioblasts in plants.

[Cavernous hemangioma in internal auditory canal: case report and review of the literature].

OBJECTIVE: To discuss the diagnosis and treatment of cavernous hemangioma in internal auditory canal (IAC). METHOD: One case of cavernous hemangioma in IAC was diagnosed and treated. The clinical data including clinical manifestation, hearing tests, imaging findings and pathology diagnosis were reviewed. RESULT: The tumor was totally removed by translabyrinthine approach without any complication, such as facial paralysis or cerebrospinal fluid otorrhea. CONCLUSION: Cavernous hemangioma of the IAC have arisen from vascular plexuses surrounding Scarpa's ganglion. It is a rare occupying lesion in IAC of benign nature. Early diagnosis depends on clinical manifestation and imaging findings. Subtle radiographic findings may help in differentiating these lesions from schwannomas. The ultimate diagnosis is made on pathologic examination of the tissues after surgical extirpation.

[Replantation of amputated ear with microtechnique].

OBJECTIVE: To report replantation of amputated ear with microtechnique in 5 cases. METHODS: From 2002 to 2005, 5 totally amputated ears were replanted and 4 obediently, 1 retrogressively. In every case, 1-2 arteries and 1-3 veins were anastomosed. Seven vessels were detective and 1-6 cm vessels were transplanted from forearm or dorsal side of hand. Amputated ears obtained blood supply again in about 6-10 hours after injury and exploration was enforced for venous crisis in 2 cases, and bloodletting in 1 case. RESULTS: All 5 cases survived. Auricles possess perfect shape, no pigmentation, slight atrophy and perfect sensation. CONCLUSIONS: Amputated ear should be replanted as long as no obvious contusion occurred, and the keys to prevent vascular crisis are cutting off unhealthy vessels, grafting superficial veins for bridging, and the high quality of vascular anastomosis.
