Costs of medical services for patients with HIV/AIDS in Khon Kaen, Thailand.

OBJECTIVE: To estimate the savings and cost of providing highly active antiretroviral therapy (HAART) to adult patients with AIDS under Universal Coverage (UC) in Khon Kaen Province, Thailand. DESIGN: Micro-costing of outpatient and inpatient services of two referral hospitals, and cost modelling. SETTING: Khon Kaen Regional Hospital and Northeast Regional Infectious Hospital. PATIENTS: Adult patients who resided in Khon Kaen and made outpatient visits at and/or those who were discharged from those hospitals from 1 December 2001 to 28 February 2002. MAIN OUTCOME MEASURE: The average cost per outpatient visit and per inpatient day. Based on these figures, the savings and cost of providing HAART to adult patients with AIDS under UC at outpatient settings in this province were estimated. RESULTS: The average cost per outpatient visit with and without antiretroviral drugs (ARV) was US$294.2 and US$26.1, respectively. The average cost per inpatient day with and without ARV drugs was US$368.1 and US$43.8, respectively. The net annual cost of HAART was estimated to be US$5 674 629. This is equivalent to 20.0% of the annual UC budget for adults in this province in 2002. Sensitivity analysis and projection to the year 2006 were conducted. CONCLUSION: A large increase in the budget would be required to provide HAART to all adult patients with AIDS under UC. However, the sensitivity analysis showed it would be an affordable policy option if low-cost antiretroviral drugs were successfully introduced. This type of analysis would be useful to assess the financial implications of providing HAART in public health systems worldwide.

Ocular findings in a Japanese family with an Arg41Trp mutation of the CRX gene.

PURPOSE: To characterize the ophthalmological features and clinical course of an autosomal dominant cone-rod dystrophy (CORD2) in a Japanese family with an Arg41Trp mutation in the CRX gene. METHODS: Mutation screening by direct sequencing was performed on 42 patients with cone-rod dystrophy. The clinical features of the patients were characterized by the visual acuity and by the findings of slit-lamp biomicroscopy, electroretinography, fluorescein angiography, and kinetic visual field testing. RESULTS: An Arg41Trp mutation in the CRX gene was identified in three members from three generations of one family with cone-rod dystrophy. Fundus examination demonstrated that the retinal dystrophy worsened with increasing age. CONCLUSIONS: A heterozygous Arg41Trp mutation in the CRX gene can produce cone-rod dystrophy in Japanese patients. Clinical examination of patients of different ages demonstrated that there was a rapid progressive worsening of the disease with increasing age.

Preliminary study of hair form of Japanese head hairs using image analysis.

The use of average curvature measurements for the forensic comparison of curly hairs has been reported, but a method, in which various types of hair form are quantitatively examined and objectively interpreted for hair comparison, has not been reported to date. In the present study, numerical data on hair form from Japanese subjects were obtained by image analysis and a morphological comparison of these head hairs was investigated. Head hairs obtained from eight Japanese males were measured for length (L), distance (D) and area (A) using a Kontron Imaging System KS400. From the three measurements mentioned above, three indexes, L/D, A/D and 2(A/L), were examined. The inter-individual variations for each value were investigated by a t-test and the availability of six values for the forensic comparison of hair form was evaluated by a stepwise linear discrimination analysis. Six values obtained from hair form by an image analysis showed large intra-individual variations. However, these six values were found to be useful for discriminating between two individuals, since the six values showed larger inter-individual variations than intra-individual variations. Discrimination on each comparison using a stepwise linear discrimination analysis was performed for some of the values and the results indicated conspicuous inter-individual variations between the two individuals. On 11 of 28 comparisons, 30 hairs from one individual could be completely distinguished from hairs of another individual, when a two-way comparison was employed. These results confirm that hair form could be quite useful in the forensic comparison of hair morphology, and suggest that numerical data obtained from hair form by image analysis are very important values for constructing a screening procedure for evidential hairs. The use of an objective measure of hair form will be especially useful for Japanese head hairs since they are generally thought to show very limited variation in morphological features.

Autosomal dominant macular degeneration associated with 208delG mutation in the FSCN2 gene.

OBJECTIVE: To assess the clinical and genetic characteristics of 2 Japanese families with autosomal dominant macular degeneration (ADMD) associated with a 208delG mutation in the retinal fascin (FSCN2) gene. DESIGN: Case reports with clinical findings and results of fluorescein angiography, electroretinography, kinetic visual field testing, and DNA analysis. SETTING: University medical center. RESULTS: The 208delG mutation in the FSCN2 gene was identified in 14 members of 4 Japanese families with autosomal dominant retinitis pigmentosa and in 5 members of 2 Japanese families with ADMD. The characteristic features associated with this mutation led to 2 different phenotypes, autosomal dominant retinitis pigmentosa and ADMD. CONCLUSIONS: The 208delG mutation in the FSCN2 gene produces not only autosomal dominant retinitis pigmentosa but also ADMD in the Japanese population. This mutation is relatively common in Japanese patients with autosomal dominant retinal degeneration and showed clinical variability. CLINICAL RELEVANCE: Autosomal dominant retinitis pigmentosa and ADMD can be caused by the same 208delG mutation. We suggest that mutations in the FSCN2 gene can lead to a spectrum of phenotypes.

Bilateral central retinal artery occlusion in Churg-Strauss syndrome.

PURPOSE: To describe a rare bilateral central retinal artery occlusion in a patient with Churg-Strauss syndrome. DESIGN: Observational case report. METHODS: A 68-year-old Japanese man with Churg-Strauss syndrome developed sudden vision loss in both eyes. Medical history included bronchial asthma, elevated eosinophils, and aortic regurgitation. RESULT: Central retinal artery occlusion was diagnosed by retinal whitening, a cherry-red spot, and delayed arteriovenous transit time on fluorescein angiography. Corticosteroids, cyclophosphamide, and anticoagulant therapy altered the hypereosinophilia and mononeuritis multiplex, but no visual recovery occurred during the following 2 years. CONCLUSION: Bilateral central retinal artery occlusion can occur in Churg-Strauss syndrome. Patients with this diagnosis should be carefully followed.

Politics of leprosy segregation in Japan: the emergence, transformation and abolition of the patient segregation policy.

The segregation of leprosy patients, a practice introduced early in the 20th century, was maintained in Japan after World War II. It locked in the viability of subsequent policy choices, and patients' isolation was sustained long after it was proven to be scientifically unnecessary. For leprologists and leprosarium directors, there was little opportunity to conceptualize and test the epidemiological validity and effectiveness of outpatient services as alternatives to the existing policy, since most of the patients were already hospitalized. Since leprosy was no longer a threat to the general public, bureaucratic officials, as well as legislators, lacked strong incentives to reformulate the overall policy. Within the Ministry of Health and Welfare, daily tasks were largely transferred to the section for leprosarium management, and the search for other options lost importance. For patients, long institutionalization elevated their dependency on life in leprosaria. These conditions must be emphasized as policy legacies, the results of past policies, since they posed obstacles to effective policy innovation in accordance with changing scientific knowledge. To make policies reflective of scientific knowledge, it is essential to understand and foresee the effect of policy legacy, when introducing and appraising public health policies.

Expression and functional properties of unique inward rectifier K+ channel Kir7.1 in the porcine iris and retinal pigment epithelium.

PURPOSE: To investigate the membrane functional properties of porcine iris pigment epithelial cells (IPE), and compare the characters of inward rectifier potassium (Kir) channel in the IPE with those in the retinal pigment epithelial cells (RPE). METHODS: IPE and RPE were acutely dissociated from porcine eyes. Functional properties of Kir channels were characterized using whole cell patch clamp recording techniques. Expression of Kir7.1 mRNA in both cells was detected by reverse transcription-polymerase chain reaction (RT-PCR). RESULTS: Whole cell current in the IPE exhibited a mild inward K+ rectification, and showed little dependence on [K+]o. Unusual high (7.04 +/- 1.7) Rb+ to K+ inward conductance ratio indicated that Kir7.1 subunit was expressed in the IPE as the same as RPE cells. Also, Kir7.1 mRNA was detected in both porcine IPE and RPE by RT-PCR. However, functional expression of Kir conductance in IPE cells (21.7 S/F) was much smaller than that in RPE cells (205.6 S/F). CONCLUSIONS: The Kir7.1 subunit was predominantly expressed in the acutely dissociated porcine IPE and its functional properties are similar to those in the RPE. However, the current density seems too small to fulfill the task of the Kir function of RPE.

Monoclonal antibodies against blood group A secretors and nonsecretors saliva.

To obtain monoclonal antibodies (MAbs) that distinguish secretor and nonsecretor from their saliva in forensic casework, two (K7405 and K7422) and one (K7516) MAbs reacting to blood group A antigen were produced by immunization of mice with salivary mucin obtained from blood group A secretors and nonsecretors, respectively. K7405, produced by immunization with salivary mucin obtained from A secretor, reacted with the A substances bound to the carrier protein but not with the A substance separated from the carrier protein. On the other hand, the K7422 and K7516 were reactive to the A substance separated from carrier protein. From these results, we conclude that K7405 recognizes the A substances clustered on the carrier protein and K7422 and K7516 recognize the isolated A substance. In the forensic blood typing of body fluids, A secretors and A nonsecretors can be clearly discriminated by the combined application of two MAbs (K7405 and K7516), which react differently against saliva samples.

Assessment of urinary cotinine as a marker of nicotine absorption from tobacco leaves: a study on tobacco farmers in Malaysia.

To assess dermal absorption of nicotine from tobacco leaves in relation to Green Tobacco Sickness (GTS), urinary cotinine concentrations were measured in 80 male tobacco-growing farmers and in 40 healthy males (controls) who did not handle wet tobacco leaves in Kelantan, Malaysia. Among non-smokers, urinary cotinine levels in farmers were significantly higher than those of controls; farmers with urinary cotinine of 50 ng/ml/m2 or above showed eye symptoms more frequently than those below this level (p<0.05). Farmers who did not wear protective equipment had subjective symptoms more frequently than those who used the equipment (p<0.05); some of these symptoms were seen more frequently in organophosphate (Tamaron) users than in non-users. As tobacco farmers evidence a risk of nicotine poisoning from tobacco leaves, assessment including GTS together with effects of pesticides will be necessary.

Increase of olfactory threshold in plating factory workers exposed to chromium in Korea.

To disclose the effects of chromium (Cr) on olfactory function, olfactory threshold tests were conducted on 27 male plating workers (Cr workers) with signs and symptoms of olfactory irritation but without nasal septum perforation or ulcer and on 34 male control subjects in Korean plating factories. The Cr workers had been exposed to Cr fume for 0.9 to 18.2 (mean 7.9) years; their blood Cr concentrations (0.16-3.69, mean 1.29 microg/dl) were significantly higher than those of the 34 control subjects (0.04-1.95, mean 0.55 microg/dl). Scores on recognition thresholds among the Cr workers were significantly higher than those of the control subjects (p < 0.05) and related positively and significantly to the exposure periods of the 27 Cr workers (p < 0.05). Olfactory thresholds were not significantly different between the Cr workers with and without nasal signs or symptoms, except that the scores on the recognition threshold were significantly higher in those experiencing difficulty with smell (p < 0.05). It is suggested that olfactory threshold is affected by Cr without development of nasal septum perforation or ulceration.

Variant alleles on the penta E locus in the PowerPlex 16 kit.

Penta E in the PowerPlex 16 kit is a pentanucleotide tandem repeat marker located on Chromosome 15, containing an AAAGA repeat motif. Variant alleles (18.4 and 19.4) were found in the Japanese population. A sequence analysis revealed that both the variant alleles had a partial repeat motif of AAAA, resulting in one-base-shorter alleles compared to known alleles. Despite the relatively large amplicon sizes (379 to 474 bp) of Penta E, an accurate allele assignment can be reliably made by capillary electrophoresis. However, alleles differing in size by only one base (e.g., 18.4 and 19) were not separated and appeared as a single broad peak. The Genotyper software assigned one of the component alleles to this peak. Therefore, such broad peaks require careful interpretation so as to not overlook the other component allele contained by the peak. As an index to recognize a peak containing two alleles, the ratio of peak area to peak height was found to be useful.

Real-time analysis of ligand-induced cell surface and intracellular reactions of living mast cells using a surface plasmon resonance-based biosensor.

Surface plasmon resonance (SPR)-based sensors have been used to detect the binding between interactive molecules. We applied the SPR technology to the analysis of interactions between living cells and molecules reactive to the cells, using mast cells and mast cell-reactive antigens. The exposure of dinitrophenol-human serum albumin (DNP-HSA), an antigen that stimulates mast cells, to IgE-sensitized mast cells induced a robust and long-lasting SPR signal in a dose-dependent manner. The maximal increase in SPR signal induced by 100 ng/ml DNP-HSA was 0.200 +/- 0.120 angle (mean +/- SD, n = 37), about 1000 times larger than the theoretically expected increase for the simple binding of DNP-HSA to Fc(epsilon)RI, the high-affinity IgE receptor. A small, but similarly prolonged signal was observed when the cells were stimulated by an agonist of the adenosine A3 receptor. The signal induced by DNP-HSA was abolished by genistein, and partially inhibited by phorbol 12-myristate 13-acetate and wortmannin. Interestingly, the signal induced by DNP-HSA was only weakly inhibited by DNP-lysine, suggesting that DNP-lysine manifests its action not by inhibiting, but by modulating the crosslinking of Fc(epsilon)RI. We concluded that SPR sensors can detect biologically significant signals in a real-time manner from the interactions between cells and molecules reactive to the cells.

Particulate matter and particle-attached polycyclic aromatic hydrocarbons in the indoor and outdoor air of Tokyo measured with personal monitors.

Using two types of personal monitors for suspended particulate matter of diameter under 10 microm (PM-10) and for particles of diameter under 1 microm with attached polycyclic aromatic hydrocarbons (PPAH), we measured the PM-10 and PPAH concentrations in the indoor and outdoor air in various locations in the Tokyo area. The major findings were as follows. (1) The PPAH concentrations in a clean living room increased rapidly within several minutes after one cigarette was smoked. (2) Using the average indoor concentrations of PM-10 and PPAH in a department store as control concentrations, respectively, where the average indoor PM-10 concentration was closest to an annual average outdoor concentration in Japan, the mean value for indoor air concentrations of PM-10 by location ranged from 2.2 to 6.2 times the control concentration, and the mean value for indoor air concentrations of PPAH by location ranged from 1.0 to 32.2 times the control concentration. (3) Using the same control concentrations, the mean value of outdoor concentrations of PM-10 by location ranged from 1.6 to 8.5 times the control concentration, while the mean value of outdoor concentrations of PPAH by location were up to 353.7 times the control concentration. The major polluted places were main traffic roads, highways, and street tunnels. (4) The correlation coefficient between the PM and the PPAH concentrations in the total monitoring time was 0.014, which was not significant (P>0.05).

Pleiotrophin/heparin-binding growth-associated molecule as a mitogen of rat hepatocytes and its role in regeneration and development of liver.

Previously pleiotrophin (PTN) was identified among proteins secreted by Swiss 3T3 cells as a mitogen for cultured adult rat hepatocytes. The present study showed that the growth of rat hepatocytes was enhanced when cultured with rat hepatic stellate cells (HSCs). HSCs expressed PTN mRNA and secreted its protein in the co-cultures. Recombinant PTN enhanced the growth of hepatocytes in culture, suggesting that HSCs stimulate the growth of hepatocytes through the action of PTN. To know the biological role of PTN in the growth of hepatocytes in vivo, we examined the expression of PTN in four regeneration models of adult liver and embryonic liver of rat. The expression of PTN mRNA in the liver was markedly up-regulated by the treatment with D-galactosamine (GalN) or with acetylaminofluorene followed by partial hepatectomy. HSCs expressed PTN mRNA in response to GalN treatment and its protein was found on hepatocytes. The mRNA expression of N-syndecan, a PTN receptor, was up-regulated in GalN-treated hepatocytes. The mesenchymal cells in the septum transversum enclosing the embryonic liver, but not embryonic HSCs, expressed PTN mRNA. We suggest that PTN is secreted from activated adult HSCs and embryonic mesenchymal cells as a mitogen of parenchymal cells in adult and embryonic liver, respectively.

Effectiveness of combined blood conservation measures in thoracic aortic operations with deep hypothermic circulatory arrest.

BACKGROUND: The effectiveness of blood conservation measures for thoracic aortic operations with deep hypothermic circulatory arrest has not yet been documented. METHODS: From July 1997 to December 2000, 148 thoracic aortic operations were performed in our department. Sixty-one cases involving patients who underwent elective thoracic aortic operation with deep hypothermic circulatory arrest were reviewed retrospectively. RESULTS: Seventeen patients did not meet the criteria for the blood conservation program and were excluded from the present study. Therefore, 44 patients were analyzed in this study. Overall, 50% of patients did not require operative homologous blood transfusion (HBT) and 43% did not require in-hospital HBT. Smaller amounts of autologous donation, greater blood loss, and a longer operation time were independent risk factors for HBT requirement. Among 16 patients who had made an autologous donation of 1,600 mL or greater, 75% did not require intraoperative HBT and 69% did not require in-hospital HBT. The overall perioperative mortality rate was 4.5%. As for postoperative complications, prolonged intubation and postoperative infection were significantly more frequent among patients who required in-hospital HBT. CONCLUSIONS: Our combined blood conservation measures were effective in avoiding HBT during major thoracic aortic operations with deep hypothermic circulatory arrest and may have reduced postoperative complications. The amount of the autologous donation was a strong predictor for avoiding HBT.

Statistical evaluation of morphological data of Japanese head hair and the screening of evidential hair samples by cluster analysis.

Intra-individual and inter-individual variations in Japanese head hair morphology were reevaluated and the screening of evidential hair samples using a statistical method was investigated. A quantification of the morphological data was used as a system in which morphological features obtained by macroscopic and microscopic observations can be objectively processed in the form of numerical information. For confirming the availability of morphological features for hair comparison, the inter-individual comparison of ten male Japanese was investigated by discrimination analysis using 18 variables including five original numerical morphological features (variables) and 13 variables obtained from six morphological features by quantification. The majority of the comparisons showed a high level of discrimination between two individuals. For morphologically narrowing down the number of evidential hair samples collected from a crime scene, a screening method was investigated by cluster analysis using 18 variables. In an experimental model of evidential hair samples derived from 20 individuals, hair samples from 13 individuals were successfully discriminated using the cluster analysis described in the present investigation.

Thymoma associated with pure red-cell aplasia: clinical features and prognosis.

As information on the clinical features and prognosis of thymoma complicated by pure red-cell aplasia is limited, follow-up data on thymoma patients who had a thymectomy between 1954 and 1999 were analyzed retrospectively. Six of 166 cases were complicated by pure red-cell aplasia. In 3 of these, the pure red-cell aplasia appeared after surgical intervention. Remission was observed in 2 patients who underwent extended thymectomy. The other 4 patients subsequently died from pure red-cell aplasia. The outcome in patients with pure red-cell aplasia was poorer than that in the entire group of patients with thymoma and in those with thymoma complicated by myasthenia gravis. The possible onset of pure red-cell aplasia after thymectomy should be kept in mind during follow-up.
