[Diagnosis and treatment of pulmonary hamartoma].

OBJECTIVE: To summarize the characteristics of pathogenesis, image, treatment and prognosis of pulmonary hamartoma. METHODS: 39 cases with hamartoma confirmed by pathology received surgical operation during 1971-1998 period. Their clinical data including symptoms, disease courses, image characteristics and surgery types were investigated. The prognostic results of 31 cases were collected by a follow-up study. RESULTS: The average age of morbidity was 44 years old and the mean disease course of hamartoma was 13 months. 21 cases without symptoms were determinated by chance and 18 cases with some extent of cough, expectoration, fever, chest distress, and short of breath. The hamartomas of 21 cases were in right lung, 18 in the left. Image characteristics presented as popcom-shaped tumor with clear border and without satellite focus. 31 cases underwent enucleation, 4 wedge-shaped resection, 3 lobectomy and 1 pneumonectomy. No recurrence or malignant change was demonstrated by follow-up study. CONCLUSIONS: Pulmonary hamartoma is a kind of neoplasms with satisfactory prognosis. Intraluminal hamartoma should be differentiated from hilar lung cancer. Localized resection should be performed for the treatment of the disease.

[Effect of tongbiling on synoviocyte function in adjuvant arthritis rats].

OBJECTIVE: To study the effect of Tongbiling (TBL) on the proliferation of synovial fibroblast and interleukin-1 (IL-1), tumor necrosis factor-alpha (TNF-alpha) and postaglandin E2(PGE2) secretion by synoviocytes in adjuvant arthritis (AA) rats. METHODS: Synovial fibroblasts was derived from culture of tissue piece. The effect of primary synoviocyte culture supernatants on the fibroblast proliferation were assayed and IL-1, TNF-alpha bioactivity and PGE2 content of supernatants of cultured synoviocytes were measured. RESULTS: TBL could inhibit significantly the synovial fibroblast proliferation (P < 0.001), and down-regulated IL-1, TNF-alpha and PGE2 productions (P < 0.01); indomethacin could promote obviously the synovial fibroblast proliferation (P < 0.001). It inhibited significantly PGE2 production, but further up-regulated the IL-1 and TNF-alpha secretion by synoviocytes (P < 0.01). CONCLUSION: The therapeutical effect of TBL on AA might be associated with its down-regulating the secretory function of synoviocyte, then restoring the abnormal proliferation of fibroblast to normal levels.

[Effects of the anti-LCAMs monoclonal antibodies on proliferation of pulmonary fibrosis in vitro].

OBJECTIVE: To explore the effects of the antibodies to LCAMs on proliferation of pulmonary fibroblast (L929 cell line). METHODS: Proliferation of fibroblasto and level of collagen production were determined by (3)H-TDR incorporation and ELISA. RESULTS: The supernate of AMs cultured with anti-LCAMs could inhibit the proliferation of fibroblast. The presence of anti-CD(11)B (125 microg/ml) could significantly inhibit the proliferation of fibroblast (P < 0.05), and reduce collagen secretion by 29.7%. When anti-CD(18) and anti-CD(54) at the concentration of 25 microg/ml, 5 microg/ml respectively, the inhibition rate of fibroblast proliferation differed from that in control group (P < 0.05). moreover combination of the two antibodies resulted in more significant inhibition effect than that seen by the two antibodies separately. CONCLUSIONS: The increased expression of CAMs on AM might play an important role in the pathogenesis of the pulmonary fibrosis and MAbs to CD(11)B/CD(18), CD(54) possess a potential regiment for intervention of pulmonary fibrosis.

[In vitro studies on the bioactive materials released by lung fibroblasts in experimental pulmonary fibrosis].

OBJECTIVES: The purpose of the present study was to clarify the consecutive changes and roles of hyaluronan (HA), interleukin-8 (IL-8) and collagen production by lung fibroblasts in the course of pulmonary fibrosis. METHODS: Quantitative and comparative assessments of the HA, IL-8 and collagen (presented by hydroproline; HYP) levels in lung fibroblast-conditioned media were made at various stages during the development of bleomycin-induced pulmonary fibrosis in rats. RESULTS: In lung fibroblast-conditioned media of bleomycin-treated animals: (1) The HA levels increased significantly on day 1, peaked on day 3, and then gradually declined and returned to control values on days 14 - 28. (2) The IL-8 levels strikingly increased on day 1, reached the peak values on day 7, thereafter, gradually decreased, regained control values by day 28. (3) The collagen levels increased significantly on day 7, peaked on day 14, and then gradually declined, However, remained significantly above normal values on day 28. (4) In bleomycin group, the HA and IL-8 levels both significantly correlated to cell components in BALF. CONCLUSIONS: The lung fibroblasts were activated, produced increased HA and IL-8, and were the main source of HA and IL-8 within the lung in the early stage of pulmonary fibrosis; increased HA and IL-8 synthesis of lung fibroblasts might reflect the intensity of alveolitis and the disease activity; The excessive collagen deposition within the lung primarily occurred at the middle stage of pulmonary fibrosis.

[The effect of anti-transforming growth factor-beta1 antibody on fibroblast proliferation in vitro].

OBJECTIVE: Using anti-transforming growth factor-beta(1) antibody binding to transforming growth factor-beta(1) (TGF-beta(1)) in conditioned supernatant obtained from alveolar macrophages (AM) in broncho-alveolar lavage fluid (BALF) in bleomycin-induced rats, we investigated the effect of binding of TGF beta(1) to anti-TGF beta(1) antibody on proliferation and synthesis of collagen by fibroblast in pulmonary fibrosis. METHODS: Bleomycin (0.75 mg/100 BW) was instilled intratracheally into wistar rats, then broncho-alveolar lavage (BAL) was performed 7 days later. AMs (5 x 10(5)/ml) were cultured in 0.2% FCS RPMI-1640 medium in vitro for 24 hours and conditioned medium was obtained. L929 fibroblast (7 x 10(3)/well) was cultured in 96-well microtiter plate with conditioned medium and in different doses of anti- TGF beta(1) antibody (1, 5, 10, 20 microg/ml) and IgG for 24 hours respectively. (3)H-Tdr was added 1 microCi per well for 6 hours before culture finished. The proliferation of fibroblast was studied by (3)H-Tdr incorporation rate and synthesis of collagen type IV was tested by ELISA. RESULTS: (1) Fibroblast proliferation which was induced by AM conditioned medium could be significantly depressed by anti-TGF beta(1) antibody in vitro (P < 0.01), and it appeared in a dose-dependent manner. (2) Synthesis of collagen type IV was depressed about 32% by 10 microg/ml anti-TGF beta(1) antibody. CONCLUSIONS: Fibroblast proliferation and collagen synthesis could be depressed by anti-TGF beta(1) antibody in vitro. It seems possible to provide a new way for the treatment of pulmonary fibrosis.

[Multivariate analysis of prognostic factors after hepatectomy for primary liver cancer].

OBJECTIVE: To determine the prognostic factors for primary liver cancer (PLC) after hepatectomy. METHODS: 182 patients who underwent hepatectomy for PLC from 1988 to 1996 were included for univariate and Cox multivariate analysis. 20 factors contributed to the long-term survival rate (SR) and the disease-free SR were analysed. RESULTS: The cumulative SR (n = 182) in 5 years was 43.3%. The 5-year SR and disease-free SR in the curative resection (CR) group (n = 156) were 48.0%, 24.9%, respectively. Multivariate analysis revealed that classification of CR and differentiation grade were significant factors for the long-term SR, and classification of CR and Child-Pugh's classification were significant factors for the disease-free SR. The 5-year SR and disease-free SR for pathological CR, clinical CR and non-CR (palliative resection) groups were 61.1%, 20.4%, 0 and 28.8%, 15.5%, 0, respectively. CONCLUSIONS: The classification of CR is the determining prognostic factor. Pathological CR is the first option for patients with requirements. To control liver disease and improve adjuvant therapy are essential to decreasing the recurrence and metastasis rates.

[The expressions of Fas and Fas-L in human breast invasive ductal carcinomas and their surrounding tissues].

OBJECTIVE: To study the relationship of the expression of Fas and Fas-L with the pathogenesis and differentiation of breast invasive ductal carcinomas. METHODS: Both carcinoma tissues and their surrounding tissues from 50 patients with breast invasive ductal carcinomas were studied for the expression of Fas and Fas-L by immunohistochemical staining. RESULTS: The expression level of Fas in breast ductal carcinoma was significantly lower than that of tissues surrounding carcinomas, and the expression level of Fas-L in breast ductal carcinoma was significantly higher than that of tissues surrounding carcinomas. There was no significant difference in the expression of Fas and Fas-L in carcinomas with different differentiation grades. CONCLUSIONS: The expression of Fas is downregulated and the expression of Fas-L is upregulated in human breast invasive ductal carcinomas. Human breast invasive ductal carcinomas may enhance their development through downregulating the expression of Fas and upregulating the expression of Fas-L. The expression level of Fas and Fas-L has nothing to do with the differentiation of human breast invasive ductal carcinomas.

[The model of piggyback orthotopic liver transplantation in pigs].

To establish the pig model of piggyback orthotopic liver transplantation(PBOLT), twelve cases of pig PBOLT were performed. The result showed that ten pigs were survived after operation, and the operative successful rate was 83%. The mean operation time and mean anhepatic-phase time were 7 hours and 34 minutes, respectively. The mean bleeding amount was 1200 ml. The appearance and tissue structure of liver in every stage under optic microscope were normal. The results indicate that the pig model of PBOLT is feasible and that it is important to decrease the anhepatic-phase time for a successful liver transplantation.

[Piggyback allogenic orthotopic liver transplantation: one case report].

Piggyback allogenic orthotopic liver transplantation was performed successfully on one patient with primary liver carcinoma. The operation took 11 h and 30 min. The resection of the liver in the patient was initiated by the ligation and transection of the veins within the third hepatic portal. The hepatic venous system was reconstructed with the end of the superhepatic inferior vena cava anastomosed to the side of the inferior vena cava at the angle of 30 degrees-45 degrees in the recipient. The hemodynamics was steadfast intraoperatively, without extracorporeal venous bypass. The patient had a good post-operative recovery. The patient died of respiratory failure caused by metastasis.

Effects of high temperature and noise on erythrocyte membrane ATPase activity in pilots during flight.

Objective. To determine the effect of heat and noise on erythrocyte membrane ATPase activities in pilots during flying. Method. Twenty-four pilots performing bombing for 3 h (45-53 degrees C, 122-97 dB in the cabin) served as the subjects. 21 ground personnel served as control (27 degrees C in the room). Blood samples were taken from both groups before flying (6:00 a.m.), and immediately (12:00 a.m.) and 8 h (8:00 p.m.) after flying. Na(+)-K+ ATPase, and Ca2(+)-Mg2+ ATPase activities in erythrocyte membrane were determined with colorimetry. Result. The Na(+)-K+ ATPase activity in erythrocyte membrane at 6:00 a.m. in pilots was higher than that in control group at the same time (P<0.01). The Ca2(+)-Mg2+ ATPase activities in erythrocyte membrane at 12:00 a.m. and 8:00 p.m. in pilots were significantly higher, compared with those in control group at the same time (P<0.01). Conclusion. The ATPase values obtained in our study were all within normal range, and the daytime variation of both groups are the same. Exposure of human body to heat and noise for long time may be harmful, the higher ATPase activity is, the more catabolism of ATP will be. ATP exhaustion will lead to Ca2+ overload in erythrocyte thus stiffen the red cell membrane.

[Establishment of animal model for ischemic facial palsy in rat].

OBJECTIVE: In order to establish animal model for ischemic facial nerve palsy in rats and to observe the intrinsic vascular network of facial nerve in the fallopian canal. METHOD: Selective vascular embolization of the external carotid artery was performed in rats. The animals were killed at 1 and 30 days after operation respectively. 10% gelatin solution with fluorescent dye were injected into their left ventricles. The changes intrinsic vascular network were observed under inverted fluorescent microscopy. RESULT: Facial palsy was presented within 5 to 15 min following arterial embolization, the intrinsic capillaries of facial nerve appeared to be thinner and some of them were blocked by microspheres, especially in the labyrinthine segment. Facial palsy almost recovered after 30 days postoperatively. The capillaries returned to be almost normal shape and only few of them were thinner. CONCLUSION: The establishment of animal model for ischemic facial palsy will make contribution to experimental research and clinical treatment for facial palsy.

[Analysis of the binding of GATA factors to the regulatory elements of human beta-globin gene in hydroxyurea-induced and uninduced HEL cells].

HEL cells, a human erythroleukemia cell line, expressing mainly the gamma-globin genes, small amount of epsilon-globin gene, but not beta-globin gene. Our previous studies demonstrated that beta-globin gene could be expressed in HEL cells induced by hydroxyurea. However, the molecular mechenism is still unknown. Here the binding patterns of GATA factors (GATA-1 and GATA-2) to the regulatory elements of human beta-globin gene were examined with the nuclear extracts from hydroxyurea-induced and uninduced HEL cells. Our results showed in EMSA assay that GATA factors could bind to the core sequence of HS2(-10681 to -10971 bp), the 3' flanking sequence of HS2 core(-10323 to -10680 bp) and the promoter of human beta-globin gene(+20 to -112 bp). However, the binding patterns between hydroxyurea-induced and uninduced HEL cells were different. Furthermore, by using Western-blot analysis, our data showed that the amount of GATA-2 was decreased in hydroxyurea-induced HEL cells. In contrast to GATA-2, the amount of GATA-1 was increased in hydroxyurea-induced HEL cells. These results showed that the different members of GATA family might play different roles during the differentiation of erythrocytes. GATA-1 may stimulate the differentiation of HEL cells, while GATA-2 can probably inhibit the differentiation of HEL cells.

[Separation of enantiomeric labetalol by reversed-phase high performance liquid chromatography].

A reversed-phase high performance liquid chromatographic method for the separation of labetalol enantiomers was developed. In the method, 2, 3, 4, 6-tetra-O-acetyl-beta-D-glucopyranosyl isothiocyanate (GITC) was chosen as the reagent for pre-column chiral derivatization of labetalol to give four diastereomeric thiourea derivatives. These derivatives were efficiently separated on a Nova-Pak C18 column using V(MeOH):V(0.01 mol.L-1(pH 7.00) H2PO4(-)-HPO(4)2- buffer) = 51:49 as the mobile phase and detected by UV detector at a wavelength of 250 nm or fluorescence detector at lambda ex = 340 nm and lambda em = 440 nm. The effects of the pH of mobile phase on the retention and fluorescence absorbance are also discussed.

Establishment of immunoglobulin M(IgM)-immunosorbent agglutination assay (ISAGA) for diagnosis of toxoplasmosis.

AIM: To establish an immunosorbent agglutination assay (ISAGA) for detection of IgM antibodies against Toxoplasma gondii. METHODS: In the ISAGA, wells of microtiter plates were coated with anti-human IgM antibodies and sealed with 1% bovine serum albumin. After the test sera were added and incubated, the plates were washed, T. gondii tachyzoite antigen suspension was added, and incubated overnight at 37 degrees C. The ISAGA results were evaluated by comparing with those detected by Danish ISAGA and ELISA and those detected by slide enzyme immunoassay (S-EIA). RESULTS: Forty-four sera from Danish pregnant women were tested by the IgM-ISAGA, 41(93.2%) were consistent with the Danish results. Sixty-seven sera from Danish and Shanghai pregnant women were detected by IgM-IgM-ISAGA and S-EIA, the total consistency rate was 92.5%. A significant correlation was found between the titers of the ISAGA and S-EIA, with a Pearson correlation coefficient of 0.589(P < 0.001). The titers of ISAGA were eighteen times higher than those of S-EIA. This method enables the detection of IgM antibodies as low as approximately 0.08 IU/ml. CONCLUSION: The IgM-ISAGA is therefore sensitive, specific, easy to perform, and is useful for mass screening and diagnosing recent Toxoplasma infection or reactivation.

[Functional analysis of human A gamma-globin gene promoter with-173 T-->C mutation].

OBJECTIVE: To investigate the effect of the human A gamma-globin gene-173 T-->C mutation on the binding of transacting factors to the promoter and the activity of the promoter. METHODS: Electrophoretic mobility shift assay and transient transfection assay were used in this study. RESULTS: The A gamma-globin gene-173 T-->C mutation decreased the affinity of GATA-1 to the mutant promoter fragment (-201(-)-158) 96% (P < 0.01) and the binding of Oct-1 to the same DNA fragment 55% (P < 0.05). The activity of the mutant promoter was 2-fold (P < 0.05) as strong as that of the normal one in MELGM979 cells. The mutant and the normal promoters exhibited basically same activities in K562 and Hela cells. CONCLUSIONS: The -173 T-->C mutation decreased dramatically the binding of GATA-1, implicating that GA-TA-1 may act as a negative regulator of A gamma-globin gene in adults. The -173 T-->C mutation may enhance the activity of A gamma-globin gene promoter in the adult erythroid cell environment.
