Dissociation between the direct stimulatory and inhibitory effects of a gonadotropin-releasing hormone analog on ovarian functions.

The paradoxical effects of gonadotropin-releasing hormone (GnRH) on the ovary have hitherto been believed to result from different regimens of administration; an acute treatment was shown to stimulate the ovary while chronic administration of the hormone inhibited LH-induced responses. In the present report we demonstrate that a single injection of a GnRH analog (D-Ala6)des-Gly10-GnRH-N-ethylamide (GnRHa, 2 micrograms/rat) is sufficient to obtain a significant inhibition (75%) of hCG-induced ovulation in PMSG-primed, either intact or hypophysectomized, immature rats. Inhibition of ovarian development, in terms of growth and ovulation, by multiple injections with GnRHa (2 micrograms/rat, twice daily for 3 days) could be obtained only upon administration of the hormone at early stages of follicular development, i.e. concomitantly with the PMSG injection. When administered after PMSG, GnRHa could not inhibit the ovary but rather induced ovulation by itself in the absence of hCG. A 12-24 h delay in initiation of GnRHa treatment triggered 65% of the rats to ovulate while a delay of 48 h resulted in 100% ovulation. Under both regimes of GnRHa administration, either the inhibitory or the stimulatory, the oocytes of the treated rats were induced to resume meiotic maturation. Since under the inhibitory regime ovulation did not occur, maturation was followed by a massive degeneration of the oocytes trapped within their follicles. These findings demonstrate that the follicular stage of development rather than the dose and/or duration of GnRHa administration determines whether GnRHa inhibits ovarian growth and ovulation, while the competence of the oocytes to respond to the GnRHa stimulus and mature is independent of hormonal priming.

Unilateral tubal pregnancy of twins.

A case of unilateral extrauterine twin pregnancy is presented. Even though this is a histopathological diagnosis, one might suspect it when facing an early rupture of tubal pregnancy preceeded by high beta-subunit HCG levels.

Oxytocin-stressed and unstressed cardiotocograms for the prediction of fetal compromise.

The results of unstressed and oxytocin-stressed cardiotocograms recorded in 222 high-risk pregnancies within 48 h of delivery were compared with signs of fetal distress in labour and Apgar scores. Thirty-nine (18%) developed a late deceleration pattern in labour and 18 newborn (8%) had Apgar scores of less than 7 at 1 min. A direct comparison of the predictive value of unstressed and stressed cardiotocograms, using Fisher's exact test, showed a very significant difference in favour of the stressed cardiotocogram for the prediction of both fetal compromise and fetal well-being. The exclusion, or very selective use, of the oxytocin-stressed cardiotocogram and reliance on the unstressed cardiotocogram for the antepartum investigation of high-risk pregnancies sacrifices accuracy for convenience.

Ultrasonography in the diagnosis of cervical incompetence in pregnancy-a new diagnostic approach.

Ultrasonography was used to measure the width of the internal os in 24 patients, for whom a McDonald operation was planned because they were considered to have cervical incompetence and in 19 women with normal obstetric histories. The mean widths of the internal os were 2.57 cm +/- 0.36 and 1.67 cm +/- 0.23, respectively (p less than 0.001). This new and objective procedure, which is used during pregnancy, provides an additional method for the diagnosis of cervical incompetence.

The influence of progesterone treatment on the course and outcome of pregnancies following suture of the cervix for cervical incompetence.

In this work, we have been studying the effects of Progesterone treatment on the outcome of pregnancy and its influence on the newborn, in a group of women who have undergone cerclage as a treatment of cervical insufficiency, in comparison with a group of women who have had cerclage and have not been treated with Progesterone. The results may be summarized as follows: 1) Preventive Progesterone treatment after cerclage does not influence abortion rate, but reduces significantly the hospitalization due to threatened abortion (uterine contractions). 2) In the group treated with Progesterone, lower newborn birth weight and Apgar score were observed.

Biphasic change in serum potassium concentration following a single dose of succinylcholine chloride.

Serum potassium concentration during the thiopentone-succinylcholine induction of endotracheal anesthesia was examined in 53 patients undergoing elective surgery. There was no change in serum potassium after thiopentone. During muscle fasciculation serum potassium fell by 0.37 meq/L and returned to the initial level by the end of fasciculation. During muscle relaxation there was a further rise of 0.41 meq/L. The expected increase in serum potassium following the administration of succinylcholine is shown to be preceded by a transient decrease.

Serum potassium in mother and fetus during normal delivery and cesarean section.

The influence of i.v. succinylcholine in term pregnant women on serum potassium of the fetus and of their own blood samples of two groups were taken to estimate the potassium level of women and their fetuses: 34 women delivering spontaneously and 24 by cesarean section using general endotracheal anesthesia. No significant differences were found in the newborns of serum potassium level of both groups, a finding which teaches, that succinylcholine given to the mother, has no significant influence on the serum potassium level of the newborn. Likewise, no significant change was foreseen in the serum potassium level in women after introduction of succinylcholine during cesarean section, in contrast to an increase of the serum potassium reported in the literature.

Elective cervical suture in preventing premature delivery in multiple pregnancies.

Of 133 pregnancies conceived after ovulation induced by menotropins and chorionic gonadotropins, a group of 20 women with multiple gestations--15 twins, three triplets and two quadruplets-had elective cervical suture during the 12th to 15th weeks of pregnancy. A comparable group 20 women with multiple pregnancies without cervical suture served as the control for comparing the mean duration of pregnancy, fetal survival rate and mean length of hospitalization. Duration of pregnancy was significantly longer in the sutured group (P less than 0.05). The perinatal loss was significantly lower--16 of 47 infants in the sutured patients, as compared with 25 of 47 infants in the control group. The mean hospitalization period was shorter in women who underwent cervical suture (P less than 0.025).
