Oral disodium cromoglycate in mastocytosis.

Oral disodium cromoglycate was found useful for the treatment of gastrointestinal manifestations of mastocytosis in 2 patients with systemic disease. The urinary excretion of histamine in these 2 patients decreased following treatment. Oral disodium cromoglycate had no effect on cutaneous symptoms, either in these 2 patients nor in 2 other patients with urticaria pigmentosa without signs of systemic mastocytosis. In the latter 2 patients the urinary excretion of histamine was unaffected. Treatment was given for 4 to 6 weeks and the dose of disodium cromoglycate was 100 mg four times daily in the 3 adult patients and 100 mg three times daily in the fourth patient, an 8-year-old boy.

Oral sodium cromoglycate in chronic urticaria.

Fifteen patients were included in a double-blind cross-over trial to investigate the efficacy of oral sodium cromoglycate in the treatment of chronic urticaria. Only patients with positive oral provocation tests were entered into the study. The challenging agents were food additives or antirheumatic agents. Treatments were taken for 4 weeks and the dose of sodium cromoglycate was 200 mg four times daily. No significant differences were found between active and placebo treatment periods for diary card symptom scores. Six patients preferred the active treatment, two preferred placebo and seven had no preference. The clinician showed no preference for either treatment.

Transfer factor in mycosis fungoides: three years experience.

13 patients with clinically and histologically verified mycosis fungoides were treated with transfer factor as additional therapy to the hitherto conventional treatment after this had failed. After approximately 3 years, complete remission was present in 3 patients, 4 patients were significantly improved and registered as being in partial remission, while the condition was registered as no change in 3 patients. 1 patient was found worse, 1 patient had died after discontinuation of therapy and 1 patient was out of the study. In this case treatment was withdrawn because of the development of contact urticaria to nitrogen mustard, her basic therapy. The number of T lymphocytes, which was low prior to treatment, increased to normal values during the therapeutic period. During the first year a decrease in serum IgE was noted. The results of the clinical evaluation seem to indicate that transfer factor may be of value as an additional therapeutic agent in mycosis fungoides. Controlled investigations are needed and are in progress.

Aspects of the treatment of mycosis fungoides. A report from the Scandinavian Mycosis Fungoides Study Group.

The Scandinavian Mycosis Fungoides Study Group includes dermatologic clinics in Denmark, Norway, and Sweden. The results of the first three years of activity are presented herein. In plaque stage, topical nitrogen mustard was highly effective. Preceding intravenous tolerance induction seems to be of no value. PUVA induced equally high remission rates. Both modalities were also highly effective in cutaneous tumor stage. In advanced tumor stage and in case of extracutaneous involvement systemic chemotherapy was given. Topical treatment alone was more effective on the cutaneous lesions including tumors than systemic chemotherapy alone. Therefore a combination of topical and systemic treatment is recommended in advanced stages of mycosis fungoides.

HLA antigens in methotrexate-induced liver cirrhosis.

HLA typing was carried out in 20 psoriatics with methotrexate-induced liver cirrhosis and 44 methotrexate-treated psoriatics who at the time of investigation had not developed cirrhosis. The data were compared with results from tissue typing of 1291 healthy individuals. The antigen combination A1, B8 was found to be increased among patients with cirrhosis vis-à-vis methotrexate-treated psoriatics who did not develop liver cirrhosis. This finding is similar to data on chronic active hepatitis type A and drug-associated chronic active hepatitis. Findings of the antigen combination A1, B8 should--just as with other predisposing factors--lead to caution in liver control in psoriatics on methotrexate therapy.

Urinary histamine in severe atopic dermatitis.

Urinary histamine was studied in 11 patients with severe atopic dermatitis and 17 controls, as well as in seven atopics prior to and 6 months following hyposensitization. No statistically significant differences were found between the groups. Urinary histamine seems to be of no value as an indicator of disease activity in atopic dermatitis.

Histiocytosis X in two infants--treated with topical nitrogen mustard.

Two infants suffering from histiocytosis X were treated successfully with a topically applied nitrogen mustard. All cutaneous symptoms disappeared completely within 2 weeks in one case while the skin improved significantly in the other. No irritation or dermatitis was experienced from the treatment. The only side-effect noted was hyperpigmentation.

Changes in disease pattern in a skin clinic 1947 to 1977.

The changes in disease pattern in a Danish skin clinic during the period 1947 to 1977 is reported. The data rest upon comparisons from patient registrations taken with ten years interval. The study also comprises comparison of frequencies of positive patch tests between 1947 and 1977. The data show, that the disease pattern was far from stable. An increased frequency was registered in atopic dermatitis, allergic contact dermatitis, drug eruptions, urticaria, skin cancer, psoriasis and viral diseases. While reductions were found in all other infectious diseases, seborrheic dermatitis, and so called "non classified eczema". The changes are postulated to be of a multifactorial origine. Environment was supposed to be the main factor in regard to contact dermatitis and some infections and to be of great importance to skin cancer, while better therapeutic posibilities seem to have influenced the frequency of most of the remaining disease groups. Pronounced changes were also registered among the ten most commonly detected contact allergens. It is stated that preventive measures are extremely important within this area. The decrease in contact allergy to balsam of Peru is used as an example of the effects of good preventive medicine.
