Cutaneous metastasis from hepatocellular carcinoma resembling granuloma teleangiectaticum.

We reported a case of cutaneous metastasis originated from hepatocellular carcinoma, which appeared as a nodule resembling granuloma teleangiectaticum. The nodule, which was composed of tumor cells and intervening capillaries, showed characteristics similar to those of primary hepatocellular carcinoma. On immunohistochemical staining, the tumor cells were positive not only for AFP (alpha-fetoprotein), but also for CEA (carcinoembryonic antigen), which is negative in most cases of primary hepatocellular carcinoma. We discussed this case with a review of previous reports of skin metastasis from liver carcinoma.

[Colon bladder replacement after total cystectomy].

Although ileal conduit diversion is widely accepted in the treatment of the patients undergoing radical cystectomy, many patients would prefer other alternatives which allow continence. and urination through the urethra. We describe a new procedure in which a segment of detuburalized right colon is used as a continent reservoir. Eight patients, 7 after radical cystectomy for bladder cancer and one after total exenteration for rectal cancer, have undergone colon bladder replacement. New created bladder had a capacity of 300 to 600 ml. All patients could pass urine through the urethra but one is on self-catheterization. Five of the 8 patients had no residual urine. Three months after operations 4 were totally continent and 3 were satisfactorily dry during daytime but slightly enuretic. Excretory urography showed no abnormalities in their upper urinary tract. Considering the "quality of life' of a patient, this procedure can be an ideal option for selected patients.

[Clinical statistics of the patients admitted to the Department of Urology, Kobe University Hospital, 1985-1987].

Between 1985 and 1987, a total number of 791 inpatients were treated at our department and 683 operations were performed. Urogenital malignant tumors (42.0%) were most frequently treated. Male infertility (14.8%) and urolithiasis (8.5%) followed. Bladder tumor was the most frequent disease in our hospital and transurethral resection of bladder tumor was the most frequent operation. Clearly endourological procedures have replaced so-called open surgeries.

[Tissue culture of human skin--a review of experimental methods and applications to dermatologic problems].

The usual methods of skin culture composed of organ culture, explant culture and cell culture were described. In organ culture of normal human skin, some blister diseases models have been used for the study of the mechanisms for producing the skin damage in pemphigus, bullous pemphigoid and epidermolysis bullosa hereditaria. Recent advances in epidermal cell culture system have furnished a potent tool for the study of keratinization at the molecular level. In the present status on tissue culture of human skin, these applications to clinical and laboratory investigations were discussed.

[A study of the use of methotrexate, vincristine, cis-platinum, cyclophosphamide, adriamycin, bleomycin (MVP-CAB) in metastatic transitional cell carcinoma].

Eight patients with metastatic transitional cell carcinoma of the urinary tract were treated with the MVP-CAB regimen. All of them had bidimensionally measurable lesions. The MVP-CAB regimen consisted of cyclophosphamide 500 mg/m2, methotrexate 20 mg/m2, adriamycin 20 mg/m2, bleomycin 30 mg/body, and vincristine 1 mg/body on day 1, cis-platinum 50 mg/m2 on day 2, and prednisolone 20 mg/body on days 1-3, given every 3-4 weeks. A partial response was seen in five patients, minor response in one patient and no change in two patients. The response rate was 63% (5/8). The main toxic effect of the MVP-CAB regimen was leucopenia. In 75% of the patients there was a decrease in white blood cell count by not more than 2,000/mm3, but no severe complication was noted. In addition, mild nausea, vomiting, mild anorexia, alopecia and fever were found. However, these symptoms were transient. One patient died of pulmonary fibrosis induced by bleomycin after 3 cycles.

[Treatment of stage D prostate cancer patients with hormones in combination with cyclophosphamide].

Eleven patients with newly diagnosed stage D prostate cancer between June, 1984 and June, 1985, were administered 300 mg/day of Honvan in 3 divided portions, 4 capsules/day of Estracyt in 2 divided portions, or 100 mg/day of Prostal in 2 divided portions was administered orally, concomitantly with large-dose intermittent treatment of 1 g/m2/3 weeks of cyclophosphamide. According to the NPCP criteria, of the 5 cases given Honvan 1 case was PR and 4 cases were stable: all 3 cases given Estracyt were stable: and in 3 cases given Prostal, 1 case was stable, 1 case had progression and 1 case dropped out. The response duration of this combination therapy was 3 to 16 months (average: 9.9 months), and obvious improvements in the subjective symptoms and performance status were noted in the cases that responded to the treatment. As for side effects, gastrointestinal symptoms were observed in 9 cases, leucopenia in 3 cases, and thrombocytopenia in 1 case. All these cases, however, were slight and transient. In 1 case, jaundice and an elevation in GOT and GPT were detected, but these changes were also temporary.

[Ectopic ureter without urinary incontinence despite ureteric orifice in the vestibulum: report of one case].

UNLABELLED: This is one case of ectopic ureter which lacked urinary incontinence despite ureteric orifice in the vestibulum. CASE: A 20-year-old woman complained of miction pain. On examination, we found a small orifice in the vestibulum. DIP revealed slight hydronephrosis in the left contracted kidney and normal pyelogram in the right kidney. On cystoscopy, the bilateral ureteral orifices were normal, but we found the head of a catheter placed in the orifice of the vestibulum was passing under the mucosa of the bladder. The abnormal lumen was recognized by introducing an opaque medium through a catheter placed into the orifice of the vestibulum. The diagnosis was an abnormal left ureteral ectopic opening into the vestibulum with left complete duplication of renal pelvis and ureter. Left heminephro-ureterectomy was performed. Some discussions about the ectopic ureteral orifice without incontinence were conducted.

The regeneration of cholinesterase-positive structures in the process of burn wound healing in the skin of the guinea-pig.

The regeneration of acetylcholinesterase-positive structures was studied during the process of third-degree burn wound healing in the skin of guinea-pigs. The procedure in this study generally followed the histochemical thiocholine technique of Karnovsky and Roots (1964) with slight modification. Acetylcholinesterase activity in the skin was found to disappear from the burn wound including its margin within 14 days post burn. True regeneration of acetylcholinesterase activity became visible after 21 days post burn. The cholinergic fibres reinnervating arterioles became visible on the twenty-first day, and thereafter gradually increased in number and staining intensity at the deep region of scar margins. The cholinergic fibres detected at the upper region of the scar centre on the twenty-first day disappeared on the twenty-eighth day. Such denervation of the cholinergic fibres may be elicited by the absence of the target organs or by a dense collagenous scar which induces degenerative changes in regenerated nerves. The denervation of cholinergic nerve fibres occurred in association with degeneration of the target organs. On the other hand, the regeneration of the cholinergic nerve fibres was not always associated with the regeneration of the target organs, but occurred later than that of the target organs.

The regeneration of the sympathetic catecholaminergic nerve fibers in the process of burn wound healing in guinea pigs.

In the present study, morphological alterations of cutaneous catecholaminergic nerve fibers in the process of burn wound healing have been investigated by using glyoxylic acid induced fluorescence histochemistry. The sympathetic denervation is detected within 48 hr after burns as well as in various excisions. The regenerating sympathetic fibers are observed on the 7th day after burns. These regenerating fluorescent fibers seem to develop in accordance with the regeneration of blood vessels in the wound margins at various stages of healing. Regenerating sympathetic fibers gradually increase in number and the maximum density of these fibers is found approximately at 2 weeks after burns. Thereafter, the density of the fibers gradually decreases to less than that of sympathetic innervation of intact skin. On the 14th day after burns, dense regenerating fibers are observed around the regenerated arterial vessels in the deep granulation tissue of the wound margins. These fibers may correlate with the degeneration of regenerated blood vessels in the center of the wound because of contracting the blood vessel of the wound margins. Moreover, dense collateral sprouting of regenerating fibers is observed around the degenerative blood vessel in the center of the wound on the 16 day after burns. This sprouting occurs not because of searching after a new target organ following the degeneration of the target organ but because of playing important roles in certain functions during wound healing.

An unusual form of pemphigus vegetans.

Numerous vegetating plaques developed in a 63-year-old man followed up for the management of pemphigus vulgaris of 5 years' duration. These lesions occurred over the trunk and right upper arm with no involvement in the intertriginous area, and were controlled with systemic steroid and ODT. Direct immunofluorescence test showed heavy deposition of fibrinogen in the papillary dermis. To our knowledge, the occurrence of pemphigus vegetans in the non-intertriginous area is rare.
