[Chemotherapy of non-small-cell lung cancer (NSCLC) and changes in serum sAPO-1/Fas and nitric oxide (NO) levels].

OBJECTIVE: To investigate the therapeutic efficacy, toxic side effects, and changes in serum level of sAPO-1/Fas and nitric oxide(NO) in NSCLC patients treated with VM-26, epirubicin (EPI) and cisplatin (VED regimen). METHODS: Forty-two cases of advanced NSCLC patients (squamous-cell carcinoma, 28; adenocarcinoma, 12; undifferentiated cancer, 2) were treated with 2 cycles of VED. Serum sAPO-1/Fas level was determined by ELISA method, and NO level by enzymatic method before and after treatment. RESULTS: The overall response rate was 47.6% (20/42). Serum sAPO-1/Fas level in patients with squamous-cell carcinoma who responded to treatment was significantly lower than that before treatment. The post-chemotherapy serum NO level was significantly higher than that before chemotherapy in responsive patients with squamous-cell cancer as well as in adenocarcinoma. CONCLUSION: Combination chemotherapy with VM-26, EPI and DDP can be useful in the treatment of patients with advanced NSCLC. Serum sAPO-1/Fas and NO levels could be used to monitor treatment effect.

[The significance of detecting tumor necrosis factor-alpha and its receptors in serum and pleural effusion in differentiating tuberculosis and malignant pleural effusions].

OBJECTIVE: To investigate the significance of TNF-alpha and its soluble receptors (sTNFR-I and sTNFR-II) in differentiating benign and malignant pleural disease. METHODS: TNF-alpha and its soluble receptors (sTNFR-I and sTNFR-II) were detected using ELISA method in the serum and pleural effusion of 25 cases of tuberculosis pleural effusion and 28 cases of malignant pleural effusion, and in the serum of 32 cases of normal control. RESULTS: TNF-alpha level in the serum of tuberculosis patients with pleural effusion is (16.4 +/- 6.2) ng/L, lower than that in the serum of malignant pleural effusion patients (20.5 +/- 10.9) ng/L (P < 0.01), but higher than that in the serum of normal control (11.0 +/- 2.9) ng/L (P < 0.01). TNF-alpha level in the pleural effusion of tuberculosis patients is (29.4 +/- 10.3) ng/L, significantly higher than that of malignant pleural effusion (11.7 +/- 2.0) ng/L (P < 0.01). The sTNFR-I and sTNFR-II level in the serum of malignant pleural effusion is (1,132 +/- 303) ng/L and (1,548 +/- 489) ng/L respectively, significantly higher than that in tuberculosis pleural effusion which is (979 +/- 146) ng/L and (977 +/- 93) ng/L (P < 0.01) respectively. The sTNFR-I and sTNFR-II level in malignant pleural effusion is (1,404 +/- 306) ng/L and (2,195 +/- 418) ng/L respectively, markedly higher than those in tuberculosis pleural effusion which is (1,185 +/- 215) ng/L and (1,195 +/- 300) ng/L (P < 0.01). CONCLUSION: There are highly elevated TNF-alpha in tuberculosis pleural effusion; The sTNFR-I and sTNFR-II level in the serum and pleural effusion of malignant pleural effusion patients are significantly higher than those in tuberculosis pleural effusion patients, suggesting they may play some role in differentiating benign and malignant pleural diseases.

[Detection and effect of telomerase activity in oral mucosal squamous cell carcinoma].

OBJECTIVE: To study telomerase activity in 20 oral squamous cell carcinoma(SCC), and 10 normal oral tissues and 9 oral SCC and 9 adjacent normal tissue. METHODS: A modified PCR-based telomeric repeat amplification protocal assay(TRAP) was used. RESULTS: The telomerase activity was significantly higher in SCC than that of normal mucosa, and than that of adjacent tissue as well. CONCLUSIONS: These findings indicated that telomerase activation might be a critical step in the formation and development of cancer.

[The prevention of primary liver cancer by selenium in high risk populations].

OBJECTIVE: To study the preventive effects of selenium on primary liver cancer. METHODS: After screening of blood samples in 18,000 males from 20 to 65 years-old in Qidong, Jiangsu province (a high risk area for liver cancer), 2,065 cases of HBsAg positive, AFP negative and normal liver function (normal ALT values) were found. The subjects were randomly divided into two groups, based on their residence areas; 1,112 subjects (experimental group) received one tablet of sodium selenite (0.5 mg Se) every day and 953 subjects (control group) received one placebo tablet every day. RESULTS: During three years of intervention and follow up, the blood selenium concentration and glutathione peroxidase activity of the subjects in the experimental group were increased and had significant difference as compared with those of the control group (P < 0.01). At the same time, the prevalence rate of micronucleus cells in peripheral blood lymphocytes in the experimental group was significantly lower than that of the control group (P < 0.01), and the incidence of new liver cancer in the experimental group (3 057.55/10(6), 34 cases out of 1,112 subjects) was significantly lower than the control group (5 981.11/10(6); 57 cases out of 953 subjects) (P < 0.01). CONCLUSION: The results confirms that selenium supplementation in general populations lived in high risk is effective in the prevention of liver cancer and the using of selenium tablets is simple and feasible.

[Expression of proliferation and apoptosis related genes in colorectal adenoma with malignant transformation].

OBJECTIVE: The effect of proliferation and apoptosis related genes of colorectal adenoma with malignant transformation and its clinicopathologic significance was studied. METHODS: p53, bcl-2, Fas, bax expression were detected immunohistochemically in 61 cases of colorectal adenoma with either mild or severe dysplasia and carcinoma. The rate of proliferation was evaluated with Ki-67 and apoptosis with TUNEL. RESULTS: The positive rates for p53 and bcl-2 protein in carcinomatous area, adenoma with severe and mild dysplasia were 67.2%, 57.4%, 19.7% and 77.1%, 80.3%, 49.2% respectively. The mean Ki-67 index and PCD index were 38.4%, 36.5%, 16.1% and 2.2%, 1.9%, 0.5% respectively. The difference of the expression of p53 protein, bcl-2 protein and Ki-67 index, PCD index in carcinomatous region and adenoma with severe dysplasia compared with mild dysplasia was statistically significant (P < 0.05). CONCLUSION: It is apparent that both apoptosis inhibition and abnormal proliferation play an important role in tumor formation. Adenoma with severe dysplasia is a lesion very close to carcinoma in nature, which possesses active tendency to become fully malignant. Detection of these proteins is important in the early diagnosis of malignant transformation of an adenoma.

[A study of the immune in formation of calcium bilirubinate gallstones in different rabbit models--the changes of the immunoglobulins in serum and bile and the immunoglobulins forming cells in the gallbladder mucoderm].

The changes of Ig-forming cells in the gallbladder mucoderm and the immunoglobulins in serum and bile were studied in the rabbit models. One hundred rabbits were randomly divided into the control group (Con, n = 10), simple biliary obstruction group (BO, n = 45) and biliary obstruction and infection group (BOI, n = 45). The results showed that the concentrations of serum immunoglobulin A (IgA) in BO and BOI groups increased remarkably in all phases (P < 0.001), The concentrations of serum IgG in both groups increased with the formation of gallstones. The IgA and IgG contents of bile samples in BO and BOI groups with negative bacterial culture were much higher than those of control group (P < 0.05), but the Ig content of bile with positive culture was slightly lower than that of the control group. Only a few Ig-forming cells were found in the gallbladder mucoderm of normal rabbits. The counts of Ig-forming cells in the mucoderm in BO group remained unchanged, but increased much higher in BOI group (P < 0.001), especially in IgG formation. This experiment demonstrates the immunoglobulins of serum and bile change significantly during the formation of gallstones. The Ig content of bile has a relationship with bacterial infection. IgA plays an important role in gallstone formation. The gallbladder of rabbit may be an important place of Ig-formation. The quantity of Ig forming cells in biliary tract may have a close relationship with the gallstone formation.

Effects of atypical antipsychotic drugs vs. haloperidol on expression of heat shock protein in the discrete brain regions of phencyclidine-treated rats.

Haloperidol augmented a trend of an increase in the heat shock protein (hsp70) mRNA levels induced by phencyclidine (PCP) in rat medial prefrontal cortex, nucleus accumbens and striatum, while the atypical antipsychotic drugs such as clozapine, olanzapine and risperidone decreased it. When administered alone, clozapine, but not haloperidol, decreased hsp70 mRNA levels. Haloperidol and the atypical antipsychotic drugs may thus have differential effects on hsp70 expression in some brain regions of PCP-treated rats.

The events relating to lanthanide ions enhanced permeability of human erythrocyte membrane: binding, conformational change, phase transition, perforation and ion transport.

The binding and uptake of Gd3+ ions by human erythrocytes in vitro were studied by determining the Gd contents in membrane and in cytosol by means of particle-induced X-ray emission (PIXE) spectrometry. Results obtained from varied incubation time revealed that the Gd3+ ions bind to the membrane proteins and lipids at first. Gd3+ binding to the membrane lipids and proteins lasts 0 approximately 20 and 20 approximately 100 ms respectively, as shown by the stopped-flow studies. Then a fraction of Gd3+ ions diffuses through the membrane. The kinetics of Gd3+ binding indicates that the binding to phospholipids is prior to that to the membrane proteins, but a portion of the lipid-bound Gd3+ redistributed later to the proteins. PIXE studies showed that the entry of Gd3+ increased the influx of Ca2+ and Cl-. By monitoring the changes in fluorescence of proteins and that of the Ln3+, the uptake of La3+, Eu3+, Gd3+ and Tb3+ was shown to be a process comprising a series of events. Binding to the membrane molecules induces the phase transition of lipid bilayer and conformational changes and aggregation of membrane proteins. Conformational changes of the proteins were characterized by Fourier transform IR spectroscopy (FT-IR) deconvolved spectra, i.e. alpha-helix content decreases while beta-sheet increases. ESR spectra of MSL-labeled proteins reflect the aggregation state related with the conformational change. [31P]NMR spectra of membrane lipid bilayer revealed the Ln3+ ions induced hexagonal (H(II)) phase formation. Phase transition and aggregation of membrane proteins cause the formation of domain structure and perforation in the membrane. These alterations in membrane structure are responsible for the Ln3+ enhanced membrane permeability. Thus the previous Ln3+ binding will facilitate the across-membrane transport of other Ln3+ ions through the membrane.

Cerebral blood flow threshold and regional heterogeneity of heat shock protein 72 induction following transient forebrain ischemia in rats.

Heat shock proteins (HSPs) induced by brain ischemia may play an important role in neuroprotection from neuronal degeneration. In this study, we examined the cerebral blood flow (CBF) threshold to produce regional differences in HSP72 induction after transient forebrain ischemia in spontaneously hypertensive rats (SHRs). Female SHRs were subjected to 20 min of cerebral ischemia induced by bilateral carotid artery occlusion. The CBF was measured by laser Doppler flowmetry. At forty-eight hours after cerebral ischemia and reperfusion, the rats were decapitated and the brains were removed. Specific areas (hippocampal CA1, CA2-3, dentate gyrus, dorsolateral and ventromedial striatum, and parietal cortex) were thereafter dissected from the brain. The amounts of HSP72 in these samples were determined using Western blot analysis. In the hippocampus, HSP72 was induced when the CBF decreased to less than 18-25% of the resting level. The mean values of HSP72 produced in the CA1 area, CA2-3 area, and the dentate gyrus following ischemia and reperfusion treatment were 4.44 +/- 1.43 (+/-SD) ng/microg protein, 3.51 +/- 0.72 ng/microg protein and 3.77 +/- 1.05 ng/microg protein, respectively. In the parietal cortex, the amount of HSP72 induction was less pronounced (2.55 +/- 0.40 ng/microg protein), while HSP72 was hardly detected at all in the striatum, even under conditions of very severe CBF reduction and reperfusion. We demonstrated the existence of both a CBF threshold (i.e., approximately 20% of the resting level) for HSP72 induction and regional heterogeneity for the induction of HSP72 protein.

Decreased plasma tryptophan associated with deep white matter lesions in elderly subjects.

The aim was to identify potentially treatable risk factors for cerebral white matter lesions often found on MRI in elderly persons. findings were assessed on 1.0 T MRI of 178 subjects living in the community and aged 60 years or older. Participants underwent standardised evaluations including standard questionnaires, a physical and neurological examination, cognitive function tests, electrocardiogram, a complete blood chemistry panel, and plasma amino acid measurements. Brain MRI infarcts, deep white matter lesions (DWMLs), and periventricular hyperintensities were found in 26%, 43%, and 29% of the 178 participants, respectively. Subjects with DWMLs were significantly older and had a higher frequency of hypertension, higher systolic blood pressure, and more brain infarcts, but lower plasma concentrations of tryptophan. In the multivariate model, greater age and lower plasma tryptophan concentrations were independently associated with DWMLs. Tryptophan concentrations were inversely related to DWML grading, whereas hypertension and brain infarction were more common in subjects with higher extents of DWMLs. The present study suggests that greater age and lower plasma tryptophan concentrations were important in producing DWMLs in elderly subjects.

Immunolocalization of CRHSP28 in exocrine digestive glands and gastrointestinal tissues of the rat.

The 28-kDa (on SDS-PAGE) Ca2+-regulated heat stable protein (CRHSP28) was recently purified as novel phosphoprotein in exocrine pancreas, since it undergoes an immediate increase in serine phosphorylation when acini are stimulated with Ca2+-mobilizing agonists. Examination of CRHSP28 protein expression in rat revealed that most was highly expressed in pancreas and other morphologically related exocrine tissues, including the parotid, lacrimal, and submandibular glands. Immunofluorescence staining in pancreas indicated that CRHSP28 was specifically concentrated in zymogen granule-rich areas in the apical cytoplasm of acinar cells. Lack of colocalization with pancreatic lipase in dual immunofluorescence studies confirmed localization of CRHSP28 to the area immediately surrounding the granules. Western analysis of pancreatic zymogen granule membrane proteins indicated CRHSP28 was not associated with the granules following their purification. A similar pattern of apical cytoplasmic secretory granule staining was noted in lacrimal and submandibular glands. CRHSP28 protein was also expressed at relatively high levels in mucosal epithelial cells of the stomach and small intestine. CRHSP28 was found in the supranuclear apical cytoplasm of cells lining the small intestinal crypts, including Paneth cells, and was abundant in the cytoplasm of goblet cells. In the stomach, strong CRHSP28 staining was seen in mucus-secreting cells in the upper portion of the gastric glands and in the apical, granule-rich cytoplasm of chief cells located in the lower portions of the glands. Dual labeling with anti-H+-K+-ATPase demonstrated a comparatively lower expression of CRHSP28 in parietal cells. Collectively, the high relative expression of CRHSP28 in various secretory cell types within the digestive system, together with its intracellular localization surrounding the acinar cell secretory granules, strongly supports a role for CRHSP28 in Ca2+-mediated exocrine secretion.

Intracellular pH regulation of CA1 neurons in Na(+)/H(+) isoform 1 mutant mice.

To understand the role of Na(+)/H(+) exchanger 1 (NHE1) in intracellular pH (pH(i)) regulation and neuronal function, we took advantage of natural knockout mice lacking NHE1, the most ubiquitously and densely expressed NHE isoform in the central nervous system (CNS). CA1 neurons from both wild-type (WT) and NHE1 mutant mice were studied by continuous monitoring of pH(i), using the fluorescent indicator carboxy-seminaphthorhodafluor-1 (SNARF-1) and confocal microscopy. In the nominal absence of CO(2)/HCO(3)(-), steady-state pH(i) was higher in WT neurons than in mutant neurons. Using the NH(4)Cl prepulse technique, we also show that H(+) flux in WT neurons was much greater than in mutant neurons. The recovery from acid load was blocked in WT neurons, but not in mutant neurons, by removal of Na(+) from the extracellular solution or by using 100 microM 3-(methylsulfonyl-4-piperidino-benzoyl)-guanidine methanesulfonate (HOE 694) in HEPES buffer. Surprisingly, in the presence of CO(2)/HCO(3)(-), the difference in H(+) flux between WT and mutant mice was even more exaggerated, with a difference of more than 250 microM/s between them at pH 6.6. H(+) flux in CO(2)/HCO(3)(-) was responsive to diisothiocyanato-stilbene-2, 2'-disulfonate (DIDS) in the WT but not in the mutant. We conclude that (a) the absence of NHE1 in the mutant neurons tended to cause lower steady-state pH(i) and, perhaps more importantly, markedly reduced the rate of recovery from an acid load; and (b) this difference in the rate of recovery between mutant and WT neurons was surprisingly larger in the presence, rather than in the absence, of HCO(3)(-), indicating that the presence of NHE1 is essential for the regulation and/or functional expression of both HCO(3)(-)-dependent and -independent transporters in neurons.

Somatostatin activation of mitogen-activated protein kinase via somatostatin receptor 1 (SSTR1).

Hormones and growth factors regulate cell growth via the mitogen-activated protein (MAP) kinase cascade. Here we examine the actions of the hormone somatostatin on the MAP kinase cascade through one of its two major receptor subtypes, the somatostatin receptor 1 (SSTR1) stably expressed in CHO-K1 cells. Somatostatin antagonizes the proliferative effects of fibroblast growth factor in CHO-SSTR1 cells via the SSTR1 receptor. However, in these cells, somatostatin robustly activates MAP kinase (also called extracellular signal regulated kinase; ERK) and augments fibroblast growth factor-stimulated ERK activity. We show that the activation of ERK via SSTR1 is pertussis toxin sensitive and requires the small G protein Ras, phosphatidylinositol 3-kinase, the serine/threonine kinase Raf-1, and the protein tyrosine phosphatase SHP-2. The activation of ERK by SSTR1 increased the expression of the cyclin-dependent protein kinase inhibitor p21(cip1/WAF1). Previous studies have suggested that somatostatin-stimulated protein tyrosine phosphatase activity mediates the growth effects of somatostatin. Our data suggest that SHP-2 stimulation by SSTR1 may mediate some of these effects through the activation of the MAP kinase cascade and the expression of p21(cip1/WAF1).

[Emergency coronary artery bypass grafting for ischemic heart disease].

Between June, 1988 and May 1998, 524 patients underwent isolated coronary artery bypass grafting (CABG) utilizing cardiopulmonary bypass in our institution. The 74 patients who underwent CABG on emergency surgery were subjected in this study. Age was ranged 33 to 89 years (mean 65 +/- 1.1; 50 male, 24 female). Of 74 cases, 52 cases underwent emergency CABG due to UAP, and 22 cases were AMI. The hospital mortality rate in 74 cases was 19% (15/74) and 2.4% (11/450) in patients who had scheduled CABG (p < 0.0001). Mortality rate in AMI group was 41% (9/22) and 9.6% (5/52) in UAP (p < 0.005). Number of patients aged 75 years or elder was 14 (19%) in emergency CABG, and 43 (8.9%) in scheduled CABG (p < 0.05). To improve the operative results of emergency CABG, it is important to reduce the number of emergency CABG in elderly, by the careful diagnosis and aggressive surgical treatment as same as in younger patient.
