[Cytokines release related to cardiopulmonary bypass in patients with prosthetic valve replacement].

To examine cytokines release related to cardiopulmonary bypass(CPB), the anthers investigated the possible differences in cytokines responses between patients undergoing prosthetic valve replacement (study group, n = 10) and those receiving closure of patent ductus arteriosus or pericardiectomy(control group, n = 9). Venous levels of interleukin-2 (IL-2), soluble interleukin-2 receptor (sIL-2R), interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-alpha), and interleukin-10 (IL-10) were measured at multiple time points before, during and after operation. As compared with pre-operative values, IL-2 levels in both groups decreased significantly (P < 0.05), and the levels of sIL-2R, IL-6, TNF-alpha, and IL-10 in both groups increased significantly at multiple time points post-operative (P < 0.01). All the values of cytokines returned to pre-operative levels at 7th post-operative day. Although there were no pre-operative differences in these cytokines between the two groups (P > 0.05), the post-operative changes of cytokines in study group was more obvious than that in control group (P < 0.01). In study group IL-10 rose to a peak value of around 620 pg/ml at the end of CPB while IL-6 and TNF-alpha levels reached their peak values of around 88 pg/ml and 52 pg/ml respectively at 1st post-operative day. IL-10 has been reported as an antiinflammatory cytokine. The preceding IL-10 peak value, as compared with the peak values of IL-6 and TNF-alpha, could be associated with the interplay and regulation of cytokine network. On the other hand, in control group the levels of IL-6, TNF-alpha, and IL-10 reached their peak values at 1st post-operative day simultaneously. The values were 34 pg/ml, 36 pg/ml, and 162 pg/ml respectively. This result suggests that besides surgical stress mediated cytokines production or suppression, CPB itself also results in obvious changes of cytokine metabolism. However further studies are needed to elucidate the underlying mechanisms and clinical value of post-operative cytokines production or suppression related to CPB in patients with prosthetic valve replacement.

[Study of macroencapsulated islet xenografts for treatment of diabetes in mice].

The aim of this study was to prevent rejection after islet xenotransplantation. Islet obtained from Wistar Furth rats were macroencapsulated with agarose and collagen before transplanted into diabetic C57BL/6, B6AF1, and BALB/c mice. The results showed that the diabetic condition was reversed in 92.3% of recipients transplanted with macroencapsulated islets. The normoglycemic state of recipients was maintained 125.8 +/- 57.9 days without immunosuppression. The glucose tolerance curves in these mice were similar to those of normal mice. The macroencapsulated islets retrieved 103 days after transplantation showed no evidence of tissue reaction or fibrosis. These indicated that the macroencapsulated islets serve both to achieve and maintain normoglycemia of recipients, as well as to protect islet xenografts from rejection.

[Immunoprotection in guinea pigs using DNA recombinant plasmid rpDJt and expressed protein P68 in L. interrogans serovar lai].

Immunoprotection against the infection by Leptospira interrogans serogroup Icterohemorrhagiae serovar lai strain 017 was demonstrated in guinea pigs vaccinated with DNA recombinant plasmid rpDJt and expressed protein P68 derived from genomic library of Leptospira strain 017. Thirty days after active immunization, each group received intraperitoneally (1/2 dose) and subcutaneously (1/2 dose) inoculum of L. interrogans serovar lai stain 017; cultures were adjusted to 5 x 10(8) cells/ml. All guinea pigs were observed for 10 days after challenge. Survival (%) of P68 group was 100(7/7); P23 group was 75(3/4); group rpDJt was 77(10/13); group lack recombinant (control) pT7-7 was 25 (3/12), and group with whole-cell inactivated vaccine was 93(13/14). Although the protective antigen in the leptospires has yet to be determined, it is evident that expressed protein P68 conferred a high degree of immunoprotection in guinea pigs.

[Cerebrospinal fluid cytology of acute viral encephalitis and meningitis].

Using cytological examination, the cerebrospinal fluid(CSF) of 98 patients with acute viral encephalitis was investigated. We found 81.4% activated lymphocytes and 5.8% plasma cells in CSF in the first week. In addition, various lymphoid features were disclosed in association: mitosis, binucleation, abnormal nuclear lobulation, Russell bodies, and morula in plasma cells. These cellular changes specifically existed in CSF of acute viral encephalitis, which were helpful to distinguish the said disease from tuberculous melitis and multiple sclerosis.

[The effect of ectopic HCG on microvessel density in gastric carcinoma].

OBJECTIVES: To study the effect of ectopic HCG on microvessel density in gastric carcinoma using immunohistochemical staining with anti-beta-HCG polyclonal antibody and anti-factor VIII related antigen(FVIII RAg)antibody. METHODS: A total of 40 specimens resected from patients with gastric carcinoma was investigated by immunohistochemical staining with a anti-beta-HCG polyclonal antibody and a monoclonal antibody against FVIII RAg. The expression of beta-HCG and the microvessel density(the average number of microvessel in three areas of highest vascular density at 400 x magnification) in various regions of the histologic specimens were studied. RESULTS: Fifteen (37.5%) of the 40 gastric carcinomas was positive for beta-HCG protein in tumor tissue. beta-HCG protein was negative in the tumor-adjacent and in normal tissue. The mean microvessel density was 27.4 +/- 7.1 in the tumor, 11.9 +/- 5.4 in tumor-adjacent and 4.8 +/- 1.5 in normal tissues (P < 0.05-0.01). There was a close correlation with beta-HCG positivity and microvessel density (MVD). There was no correlation between the degree of tumor cell differentiation and beta-HCG expression, nor was a correlation between it and MVD. High expression level of beta-HCG and MVD was significantly associated with hematogenous metastases. CONCLUSION: Ectopic HCG expression may have angiogenic effect which, in turn, may facilitate hematogenous dissemination of gastric cancer.

[c-erbB2 gene amplification in human primary epithelial ovarian cancer and its clinical significance].

OBJECTIVE: To study c-erbB2 gene amplification in human primary epithelial ovarian cancer and its clinical significance. METHODS: One hundred forty nine samples of primary epithelial ovarian tumors, including 34 beign tumors, 7 borderline tumors and 108 carcinoma, were studied for c-erbB2 gene amplification by Southern blot analysis. RESULTS: c-erbB2 gene amplification was detected in 35 (32.4%) of the 108 cases with primaey epithelial ovarian carcinoma, while all cases with beign tumors and borderline lesions were negative (P < 0.001). There was a strong correlation between c-erbB2 gene amplification and FIGO stage of human ovarian carcinoma (P = 0.026), degree of cell differentiation (P = 0.011) and the size of residual tumor (P < 0.001). Univariate and multivariate analysis showed that c-erbB2 gene amplification was an independent prognostic factor. The average 3-year survival rate was 28.4% in patients with gene amplification and 57.7% in patients without gene amplification(P = 0.016). CONCLUSION: c-erbB2 gene amplification occurs almost in 1/3 of the patients with ovarian carcinoma examined. Ovarian carcinoma with c-erbB2 gene amplification is highly malignant and associated with poor prognosis.

[Normal-sized ovary carcinoma syndrome--clinical and pathological analysis of 11 cases].

OBJECTIVE: To evaluate clinical-pathologic features, diagnostic criteria, effective treatment and prognosis of a clinically rare "normal-sized ovary carcinoma syndrome". METHODS: Histologic slides of 11 cases from 1984 to 1993 of normal-sized ovary carcinoma syndrome were retrospectively reviewed. Seven of the 11 cases were extraovarian peritoneal serous papillary carcinoma (EPSPC). The remaining 4 cases were serous adenocarcinoma of the ovary (2 cases), malignant mesothelioma (1 case) and metastatic poorly differentiated adenocarcinoma of unknown origin (1 case). All patients received relatively thorough surgical resection combined with multiple courses of adjuvant chemotherapy. RESULTS: The patient with malignant mesothelioma survived for 90 months, one case with EPSPC survived for 87 months up to Dec 1997. The average survival time of 9 patients who died of their diseases was 17.8 months, being longer than that reported in the literature. CONCLUSION: Following accurate diagnosis of normal-sized ovary carcinoma syndrome, radical tumor resection is the first treatment of choice. For residual tumor, postoperative chemotherapy or radiotherapy is essential to obtain better prognosis.

[A novel rotavirus causing large scale of adult diarrhea in Shi Jiazhuang].

An outbreak of adult diarrhea causing more than 1,000 hospitalized cases among graduates in a university in Shi Jiazhuang city (Hebei province), occurred in 10-28, April, 1997. The non-bacterial pathogenic agent is confirmed as Rotavirus first by PAGE. Forteen samples have been demonstrated sharing the same nucleic acid electrophoresis pattern of 4-2-1-1-1-1-1. The positive rate was 47% (thirty samples have been checked) with no other RV pattern. Dada showed that RV with this kind of nucleic acid electrophoresis pattern were responsible for the outbreaks acute diarrhea in adults. The novel RV and known ADRV was RT-PCR with end-primers 5, 9 of typical ADRV, the positive control typical ADRV's results are positive, the novel RV's are negative. Experimental results showed that the novel RV did not belong to group B RV, totally different from all the known ARDV. This case was the first reported one in Hebei province with an outbreak of adult diarrhea not by any known ADRV.

[Relationship between p53 gene expression and radiation effect in laryngeal carcinoma].

The purpose of this study was to determine the relationship between the biologic character of laryngeal carcinoma and the effects of radiation therapy. Using monoclonal antibody for p53 gene protein, 73 cases of laryngeal carcinoma were stained by the immunohistochemical DACO CSA System method. Results of the stain can be divided into intense, moderate and negative expression. There is a significant relationship between the gene expression and radiation effect. We found that samples which had higher level of staining due to increased gene expression, were affected more by the radiation therapy. The p53 gene expression can be useful in predicting the outcome of laryngeal carcinoma and be helpful in clinical therapy.

Growth regulation of ovarian cancer cell line HO-8910 by transforming growth factor beta 1 in vitro.

OBJECTIVE: To further understand the role of growth regulation of human ovarian cancer cells by transforming growth factor (TGF) beta 1. METHODS: The cell proliferation, cAMP synthesis, gene expression, and induction of programmed cell death (PCD) in human epithelial ovarian cancer cell line HO-8910 cells exposed to TGF beta 1 in vitro were studied. RESULTS: TGF beta 1 inhibited cell growth and DNA synthesis, and induced G0/G1 arrest in cell cycle. It could also trigger PCD in cells. This induction of PCD may occur within G0/G1 phase. Meanwhile, the assay also showed that TGF beta 1 could inhibit the mRNA expression of c-myc, EGFR and TGF beta 1 genes in cells. CONCLUSIONS: TGF beta 1 can not only act as an autocrine to inhibit cell proliferation, but also trigger PCD in HO-8910 cells. These functions may be fulfilled through some specific signal transduction pathways.

Hypertension control improved through patient education. Chinese PEP Investigators.

OBJECTIVE: To evaluate the effects of patient education for hypertension on hypertension control. METHODS: Of 169 eligible patients (systolic blood pressure > or = 160 mmHg and/or diastolic blood pressure > or = 95 mmHg), 60 were assigned to educational group (group E, antihypertensive drug treatment with an addition of patient education) and 109 to routine group (group R, antihypertensive drug treatment alone). The average follow-up approximated to 3 years. RESULTS: The blood pressure was reduced from at baseline and sustained in the three-year follow-up by 20/13 mmHg in group E and by 22/13 mmHg in group R. For this similar blood pressure reduction, about 10 mg less of metoprolol and 6 mg less of nitrendipine were used in group E than in group R. The percentage of the patients in whom goal blood pressure (under 160/90 mmHg) was achieved during follow-up was higher and progressively increased in group E (1st year: 65%, 2nd year: 72%) in comparison with in group R (1st year: 45%; 2nd year: 55%). Body weight was significantly reduced by 1.36 and 1.81 kg from at baseline to at the 1st and 2nd year repeated measurements in group E. The significant reduction from at baseline to at the 2nd year was significantly different from that in group R (P = 0.02). For 24-hour urinary sodium excretion, it was decreased in the group E, whereas it was increased in the group R. The cumulative rates of hypertension-related complications were 4.43% in group E and 7.02% in group R (absolute difference = 2.59%, P = 0.48). The rate of missed appointments was somewhat higher in group R (10%) than in group E (7%) during the first year but lower in the 2nd and 3rd year (R vs E: 10% vs 2% in the 2nd year; 8% vs 2% in the 3rd year). Four patients lost to follow-up in group R (6.87%) and 1 patient in group E (1.74%, P = 0.08). CONCLUSION: The findings of this study suggest that patient education is of some benefits to the hypertension control.

[Determination of hydrophobic pharmaceutical: tablet of reserpine compound by capillary zone electrophoresis].

Tablet reserpine compound is a kind of hydrophobic pharmaceutical that can not be separated by CZE with salt-water system. We solved this problem by adding organic component into the buffer. Thus a satisfactory separation of reserpine and hydrochlorothiazide was achieved by using a 24% (V/V) acetonitrile in the buffer solution of 10 mmol/L phosphate with a pH 9.0. The capillary column of 50 microns i.d. and 50 cm total length with effective length(the distance from the inlet to the detector) of 35 cm was used during all analytical procedure. The factors which influenced resolution such as the pH, acetonitrile content and phosphate concentration have been investigated. The CZE separation conditions of reserpine and hydrochlorothiazide using the standard mixture solution, are also showed. In the quantitative analysis, the reproducibility of CZE determination is illustrated. The regression coefficients of calibration graphs and the detection limits are given. The results of the study of recovery and quantitative results of these components using external standard method are given.

[Effect of hypoxia on proliferation and alpha 1 (I) procollagen gene expression by human fetal lung fibroblasts].

OBJECTIVE: The effect of 2% hypoxia on the proliferation and alpha 1(I) procollagen gene expression by human fetal lung fibroblast was studied. METHODS: Cell proliferation was determined by cell counting and nonradioactive cell proliferation assay method. alpha 1(I) procollagen gene expression was detected by Northern blot hybridization analysis. RESULTS: Cell number and 490 nm OD expressing low oxygen values were markedly increased by the 24, 48 and 72 h exposure to 2% hypoxia respectively. An obvious increase of the alpha 1(I) procollagen mRNA expression was detected after 24 h exposure to 2% hypoxia. The increase of gene expression could be inhibited by Ligustrazin (30 micrograms/ml, 10 micrograms/ml). CONCLUSIONS: 2% hypoxia plays an important role in the outcome of wound healing and pulmonary vascular remodeling by affecting the function of human fetal lung fibroblasts.

[Observation on inhibitory effect of Coptis alone and its combination with Scutellaria and Liquorice on the growth of Staphylococcus aureus].

Using MIC as relative index, an observation was made on the growth inhibition of Staphylococcus aureus by Coptis alone or in combination with Scutellaria and or liquorice. The result shows that when Coptis is combined with Scutellaria, the bacteriostatic effect gets weaker for Scutellaria, but remains unchanged for Coptis, and when Scutellaria takes greater percentages in the combination, the effect of Coptis seems to be stronger. When Coptis is combined with liquorice, whatever the respective percentages may be, the effect tends to get weaker. When the three drugs are used in combination, the effect remains unchanged or even gets stronger for Coptis or liquorice, but tends to decrease for Scutellaria. Coptis used singly or in combination with Scutellaria or liquorice is inferior in bacteriostatic effect to the combination of the three drugs. The mechanism is yet to be further studied.

[Human tongue carcinoma model established in nude mice by orthotopic transplantation and studies on its major biological properties].

OBJECTIVE: To establish clinically relevant animal model of human tongue carcinoma. METHODS: Cell suspensions of human tongue squamous cell carcinoma cell line (Tca8113) were inoculated orthotopically into nude mice and the growing tumors were serially passaged by orthotopic transplantation. The oral cavity, mandible and neck of the nude mice were sectioned and examined histologically. The ultrastructure and chromosome of the tumors were also investigated. RESULTS: 1. The success rate of transplant was 90%. 2. The mouse-growing tumors exhibited the same identical properties as the donor tumor. 3. Human tongue cancer cells could grow in the homologous oral cavity site of nude mice, extended and metastasized to adjacent tissues and lymph node of the neck. CONCLUSION: The model can be useful for evaluating biological behavior and new therapeutics for tongue cancer.

[Effect of various methods of tissue storage on split-skin viability at 4 degrees C].

OBJECTIVE: To study various tissues and organs storage solutions (liver, kidney and corneal) at 4 degrees C for the storage of skin and the viability of skins stored in these solutions at correspondent condition. METHOD: Five experiment groups investigated for storing porcine split-skin at 4 degrees C were MEM, UW, MEM plus CS, UW-R and MEM-R. A new assay, WST-1, was applied to evaluate skin viability at 1, 3, 5, 7, 10 and 14 day during storage respectively. RESULT: Skin viability can all be protected by these three solutions. The effect of UW solution was superior to MEM. UW-R and MEM-R were superior to MEM and UW. CONCLUSION: The method of UW-R which replaces medium routinely may be an optimal method of storing skin at 4 degrees C.

[The determination of neuron-specific enolase of serum in the diagnosis and supervision of retinoblastoma].

OBJECTIVE: To investigate the value of determining quantity of neuron-specific enolase (NSE) in the diagnosis and supervision in patients with retinoblastoma (Rb). METHOD: We used rapid electrophoretic and the fluorometer method in the 35 serum specimens from 35 patients with Rb during surgery, after enucleation, respectively, at 3, 6, 12, >or= 18 months and 31 serum specimens from 31 control cases to determine the quantity of NSE and enolase isoenzymes. RESULTS: There were 207 +/- 65.9 u/L of all isoenzymes and 98.4 +/- 40.3 u/L of NSE from 35 patients with Rb. In comparison with the levels of control cases (respectively, 140.7 +/- 46.0 U//L of all isoenzymes, 41 +/- 10.9 u/L of NSE), both serum levels in patients with Rb increased obviously, the difference being significant. At 6, 12, and >or= 18 months after surgery, the determinations of all isoenzymes and NSE from 35 patients with Rb decreased step by step, the difference being significant in comparison with the values before the surgery. After surgery >or= 18 months, the levels of NSE and all enolase decreased to that of NSE and all enolase from control cases. CONCLUSION: The determination of enolase level, especially NSE of serum, is valuable for the diagnosis of patients with Rb.

[Longitudinal surveillance of sensitivity of Plasmodium falciparum to pyronaridine in south Yunnan].

AIM: To understand the changes in the sensitivity of Plasmodium falciparum to pyronaridine in the south of Yunnan Province. METHODS: WHO standard in vivo and in vitro microtest was used. RESULTS: In 1984-1985 and 1995, 36 and 39 cases of acute falciparum malaria were observed, the average defervescence time being 32.7 +/- 16.0 h and 56.2 +/- 27.4 h, the mean asexual parasite clearance time, 64.2 +/- 22.9 h and 55.3 +/- 11.8 h, and the recrudescence rate of patients followed up for 28 days, 15.2%(5/33) and 37.5%(9/24), respectively. The resistance rates of resistant isolates by in vitro microtest were 0(0/24), 8.8%(3/34), 13.8%(4/29) and 36.4%(4/11) in 1988, 1990, 1992 and 1995, respectively, while the corresponding ID50 were 13.0, 20.0, 32.0 and 40.0 nmol/L, ID95 were 48.0, 136.0, 325.0 and 330.0 nmol/L, the mean concentrations for complete inhibition of schizont formation were 32.6, 99.5, 148.2 and 190.1 nmol/L, respectively. CONCLUSION: The sensitivity of Plasmodium falciparum to pyronaridine was found to have a tendency of continuous dropping in south Yunnan.
