The association between pharmacy closures and prescription drug use: a retrospective analysis of Medicaid prescription claims in Iowa.

OBJECTIVES: The objectives of this study were 2-fold: (1) to examine the association of pharmacy closures with prescription drug use by Medicaid recipients in Iowa; and (2) to evaluate how drug utilization patterns differ between patients whose pharmacies closed and patients whose pharmacies remained open. DESIGN: A 2-group pretest-posttest study of Medicaid enrollees who may have been affected by pharmacy closures. Prescription medication use during the periods preceding and after pharmacy closures was compared. A comparison group was used to account for extraneous factors. PATIENTS AND METHODS: Sixteen community pharmacies were selected from a pool of pharmacies that closed during 1994; 1092 patients were identified as the main users of these pharmacies, and a comparison group of 3491 patients whose main pharmacies had not closed also was identified. The average number of each patient's prescription claims for the 6 months preceding closing and the 6 months after closing was computed. Multiple regression analysis was conducted to determine whether any association existed between pharmacy closures and the use of prescription drugs. RESULTS: Patients whose pharmacies closed during 1994 had fewer prescription claims after the closings than before the closings. In contrast, patients whose pharmacies remained open had more prescription claims. This difference remains statistically significant after controlling for other factors, such as patient demographics and health status. CONCLUSIONS: A decrease in prescription drug use was associated with pharmacy closures. Attention should be directed to patient access to prescription medications in rural areas, as relatively more pharmacies close in rural areas.

Hepatotropism of nonenveloped DNA virus in rhesus monkey infected by transfusion-transmitted virus.

OBJECTIVE: To study wether the nonenveloped DNA virus transmitted via blood transfusion is hepatotropic. METHODS: Total DNA was extracted from tissues of 5 experimentally infected Rhesus monkeys. A dot hybridization was done with virus double DNA strand probe or single antisense strand probe. RESULTS: Both single- and double-strand probes were hybridized with DNA of the liver, spleen, stomach, small intestine and colon. The virus was conformed present in most of all the organs when double-strand probe was used. The positive was noted only in the liver and small intestine when single-strand antisense probe was used, which showed that in liver and small intestine might have replicative intermediates of the virus. CONCLUSION: It suggests that nonenveloped DNA virus replicate in the liver and small intestine, so it might be hepatotropic.

[Analysis on the causes of burn among 17,339 patients].

OBJECTIVE: To find out the common causes of burn and to develop preventive measures to prevent or reduce the injury of burn. METHODS: Descriptive analysis was used to review the burn-cases in our Burn Department from 1970 to 1998 and to summarize the epidemic features and common causes of the injuries. RESULTS: From 1970 to 1998, a total number of 17 339 inpatients were seen in our Burn Department. According to the causes of injury, burn patients were categorized as heat-injured, electric-injured, chemical-injured, etc. Most patients were injured with heat, taking up 89.11% of the total patients (P < 0.01). Most of the heat-injured patients were scalded with hot water (49.31%, P < 0.01). Children being burnt, accounted for 32.82% of the total patients (P < 0.01), whom should not be ignored. CONCLUSION: Burn happens frequently in daily life, so the preventive methods should be taken seriously.

[Effects of phoxim and fenvalerate on TTX-S and TTX-R sodium channels in the DRG neurons of adult rat].

OBJECTIVE: To study the joint neurotoxic effects of phoxim (Pho) and fenvalerate (Fen) on tetrodotoxin-sensitive (TTX-S) and tetrodotoxin-resistant (TTX-R) Na(+) currents in dorsal root ganglion (DRG) neurons of adult rat. METHODS: Whole cell patch clamp technique was used to test the effects of Pho and Fen on TTX-S and TTX-R sodium currents in DRG neurons. RESULTS: The inactivation of TTX-R sodium channel was obviously slowed down by Fen. The tau(Na) of peak currents at doses of 10, 50 and 100 micromol/L Fen and control groups were (8.10 +/- 2.41) ms, (11.78 +/- 2.76) ms, P < 0.01, (8.76 +/-1.94) ms, P < 0.05 and (6.41 +/- 1.32) ms respectively. The inactivation of TTX-R sodium channel tail currents was also significantly delayed by Fen. The tau(Na) of the tail currents at doses of 10, 50, 100 micromol/L Fen and control groups were 6.11 +/- 0.52 (P < 0.05), 7.82 +/- 0.82 (P < 0.05), 7.23 +/- 1.09 (P < 0.05) and (4.91 +/- 0.97) ms separately. As compared with TTX-R sodium channel, the TTX-S sodium channel was less responsive to Fen exposure, which only led to slowly decay TTX-S sodium tail currents. There was no any effect of Pho on the TTX-S and TTX-R sodium channels. The mixed treatment of a Pho and Fen did not show joint effect on the sodium currents. CONCLUSION: Both the peak and tail currents are changed by Fen, however, Fen has more remarkable effects on TTX-R than on TTX-S sodium channel. The combined exposure to Pho and Fen shows no joint effect on the sodium channel.

[Cell proliferation in endolymphatic sac during the immune response of inner ear].

OBJECTIVE: To study the cell proliferation in endolymphatic sac(ES) during the secondary immune response in inner ear. METHODS: Fifteen healthy, female SD rats were employed in the experiment. Sensitized systematically with keyhole limpet hemocyanin (KLH), the animals were inoculated with the same antigen through cochlea basal turn into the labyrinth. Administrated 5-bromo-2'-deoxyuridine (BrdUrd) intraperitoneally, the rats were sacrificed and the temporal bones were harvested at 3, 7, 14 day after labyrinth vaccination respectively. The frozen sections of the decalcified samples were dealt with H-E staining and immunohistochemical methods to investigate the cellular infiltration, BrdUrd and IgG positive cells in the ES. RESULTS: While the ES lumen and perisaccular region were infiltrated with mononuclear-phagocyte at the 3rd day post-vaccination, plasmocyte and lymphatic cells become the predominant infiltrating cells in the ES at the 7th day. The KLH in the ES lumen were phagocytized at the 3-7th day post-vaccination. S-phase cells and IgG positive cells in ES increased markedly in the 3rd day and 7-14 days post-vaccination respectively with similar localization. CONCLUSION: The activity of cell proliferation in the ES enhance and local proliferated cells may differentiate in situ into immunocompetent cells during the secondary immune response against TD antigen in the inner ear. This indicates that ES plays an important role in immune response of inner ear.

HIV-1 tat transcriptional activity is regulated by acetylation.

The human immunodeficiency virus (HIV) trans- activator protein, Tat, stimulates transcription from the viral long-terminal repeats (LTR) through an RNA hairpin element, trans-activation responsive region (TAR). We and others have shown that trans-activator protein (Tat)-associated histone acetyltransferases (TAHs), p300 and p300/CBP-associating factor (PCAF), assist functionally in the activation of chromosomally integrated HIV-1 LTR. Here, we show that p300 and PCAF also directly acetylate Tat. We defined two sites of acetylation located in different functional domains of Tat. p300 acetylated Lys50 in the TAR RNA binding domain, while PCAF acetylated Lys28 in the activation domain of Tat. In support of a functional role for acetylation in vivo, histone deacetylase inhibitor (trichostatin A) synergized with Tat in transcriptional activation of the HIV-1 LTR. Synergism was TAR-dependent and required the intact presence of both Lys28 and Lys50. Mechanistically, acetylation at Lys28 by PCAF enhanced Tat binding to the Tat-associated kinase, CDK9/P-TEFb, while acetylation by p300 at Lys50 of Tat promoted the dissociation of Tat from TAR RNA that occurs during early transcription elongation. These data suggest that acetylation of Tat regulates two discrete and functionally critical steps in transcription, binding to an RNAP II CTD-kinase and release of Tat from TAR RNA.

Synergetic Effect of Cationic Polymer Microparticles and Anionic Polymer on Fine Clay Flocculation.

Although much research has been done into flocculation systems involving anionic microparticles, few systematic investigations employing cationic polymer microparticles (CPM) have been reported. In the present work, emulsifier-free polymerization was conducted via copolymerization of styrene with three types of cationic monomer. This technique allowed cationic polymer microparticles with well-defined structures to be prepared. Dynamic flocculation experiments showed that the effectiveness of CPM alone as a flocculant for fine clay particles is affected by CPM charge density and by particle size, in the range 0.2 to 0.4 µm. A strong synergetic effect between CPM and anionic polyacrylamide with high molecular weight and low charge density was observed. This significantly reduced the dosage of polymeric flocculant required for effective flocculation. The best flocculation occurred at a value of the anionic polymer:CPM ratio of 1:8 (wt), with anionic polymer dosage approximately 0.06 wt% on clay. Electrostatic interaction between CPM and anionic polymer appears to be an important driving force for flocculation. Copyright 1999 Academic Press.

[Preliminary study on ToRCH epidemic laws in Wuhan region].

OBJECTIVE: To investigate the ToRCH epidemic law in Wuhan region. METHODS: The ToRCH infection in the normal populations of Wuhan region was inspected through a whole year (from Mar. 1997 to Feb. 1998) by detecting the specific IgM antibodies to ToRCH in sera. RESULTS: The results showed that in the whole year, the average positive rates of IgM aitibodies to Toxo, RuV, CMV and HSV were 5.8%, 7.5%, 4.2% and 4.9% respectively. Three characteristics of the ToRCH epidemic in the region were concluded: (1) The epidemic of RuV obviously changed with seasons. Its infection rates were the highest (8.5%, 19.5%, 14.0%) during spring (March, April and May). However, the changes of Toxo, CMV and HSV were not obvious with seasons; (2). The adult infections were mainly recessive, which had no apparent clinical symptoms; (3) The cross infections of the four pathogens were common, the cross-reaction rate of the surveyed year was 2.6%. CONCLUSIONS: RuV infection of pregnant woman may result in congenital malformation of the emlbryo. Accoroding to the seasonal epidemic characteristics of RuV, women in pregnancy had better not to go to the public places during the sping seasons (March, April, May) as far as possible.

Alternative-splicing of serotonin receptor isoforms in the pharynx and muscle of the parasitic nematode, Ascaris suum.

Pharyngeal pumping is essential for nematode feeding and survival and is dramatically stimulated by serotonin (5-HT). In the present study, a cDNA pool was prepared from poly A + RNA isolated from pharynxes dissected from the parasitic nematode, Ascaris suum, and was used as a template for RT-PCR with degenerate primers designed from sequences conserved in 5-HT receptors from a variety of sources. A putative 5-HT receptor cDNA (AS1) was identified which exhibited most identity to the 5-HT2 family of receptors. AS1 was 1925 nucleotides, did not appear to be trans-spliced and contained a 3' untranslated region of 127 nucleotides with a polyadenylation signal (ATTAAA) and a short poly A+ tail. The coding region predicted a protein of 532 amino acids with a molecular weight of 60 176. When AS1 was transiently expressed in COS-7 cells, isolated membranes exhibited the high affinity, saturable binding of [125I]LSD. More importantly, [125I]LSD binding was inhibited by 5-HT, but not other biogenic amines, supporting the identification of AS1 as a 5-HT receptor. Additional cDNAs identical, in part, to AS1 were also identified. AS1deltaIV lacked a predicted 42 amino acids at the carboxy terminus of the third intracellular loop, while AS2 and AS3 contained different COOH-termini, regions implicated in G-protein coupling in other heptahelical receptors. A portion of the gene (5htn) encoding AS1 also was cloned and sequenced. This genomic fragment was about 10 kb, contained the entire AS1 open reading frame and included eight exons and seven introns. From this analysis, it appears that these different AS cDNAs were generated by alternative-splicing, AS1deltaIV from the deletion of exon IV, and AS2 and AS3 from the use of alternative sites within exon VII as 5' splice acceptor sites for exon VIII. Using RT-PCR and primers specific for each of the isoforms, AS1 -3 appeared to be expressed in pharynx, while only AS1 and AS2 were present in body wall muscle. More importantly, the deletion of exon IV appeared to be associated exclusively with AS1 in pharynx and AS2 in muscle.
