[The diagnostic significance of anti-HCV IgM in chronic hepatitis C viral infection].

The significance of anti-HCV IgM testing in the diagnosis and evaluation of treatment of chronic hepatitis C was studied. The serum samples from 82 patients with chronic hepatitis C were tested for ALT, anti-HCV IgG and IgM and HCV RNA. All the 82 cases were positive for anti-HCV IgG and 51 (62.19%) positive for anti-HCV IgM. The emergence of anti-HCV IgM might be of four types: (1) persistent positive type, 24 (29.26%) cases; (2) intermittent positive type, 8 (9.75%) cases; (3) transient positive type, 19 (23.17%) cases; (4) persistent negative type, 31 (37.80%) cases. Usually there was progressive liver damage in type 1 and 2. If the patients were of transient positive type, they would usually recover. There was a significant correlation between the levels of serum anti-HCV IgM and ALT, while no correlation existed between the levels of serum ALT anti-HCV IgG. It is suggested that in patients with chronic hepatitis C, detection of anti-HCV IgM may be useful for estimating the extent of progressive liver damage and guiding the antiviral therapy.

[Single channel analysis of aconitine blockade of calcium channels in rat myocardiocytes].

Ventricular myocardiocytes from neonatal Wistar rats were isolated and cultured. Aconitine, Ca2+ channel blocker verapamil or Ca2+ channel activator BAY K8644 were added to the bath solution separately. Using the cell-attached configuration of the patch clamp technique, the single channel activities of L type Ca2+ channel were recorded before and after addition of all three drugs. The results showed the blocking effect of aconitine (50 micrograms.ml-1) on L type Ca2+ channels. Its mechanism may be relevant to the decrease in both open state probability and the mean open time of Ca2+ channel. The difference was statistically significant compared with control group (P < 0.01). The amplitude of Ba2+ currents, which flow through open L type Ca2+ channel was unchanged.

[Effect of intralipid on patients with acute necrotic pancreatitis: a prospective clinical study].

17 patients with acute necrotic pancreatitis were proved by CT or operation. These patients were randomly assigned to fat-glucose-based group (n = 9) and glucose-based group (n = 8). Their PN regimen consisted of nonprotein calories (NPC) 35-40KJ (146-167KJ)/kg/d and nitrogen 0.19-0.29g/kg/d. NPC was supplied by either dextrose or dextrose and intralipid. The PN duration was 2 weeks. The following measurements were performed: triglyceride, chorestrol, HDL, LDL, Apo-A, Apo-B and free fatty acids. The results showed that there was no hypertriglyceridemia developed after fat emulsion was administered for two weeks. It confirmed that fat emulsion could be rapidly oxidigzed. In our group, the patients with hyperglycemia were 64% at the admission time, and hyperglycemia was difficult to control in early period (10 days). But hyperglycemia was easly to control when patients received fat emulsion. There was no harmful effect on liver function during 2 weeks of intralipid administration. The data showed no difference between the two groups on conventional measurements, lysosomal enzymes and blood gas analysis. Intralipid infusion did not make ANP deterioration. These results indicated long-term intravenous intralipid in patients with ANP was safe and useful.

[Stability of amino acids and vitamins in total nutrient admixture].

We reported a formula of total nutrient admixture (TNA) used in our patients. During 24 hours of storage shaded from daylight at 4 degrees C and at room temperature (25 degrees C), the concentrations of 17 kinds of amino acids and vitamin A, E, B1, B2 in the TNA prepared in EVA plastic bag were not changed significantly. A patient who has been on this TNA for parenteral nutrition for more than 8.5 years is in a good nutritional status, with no clinical manifestations in difficiency of vitamins and proteins. 42 patients receiving this TNA on TPN for a period ranging from 31 to 93 days were improved significantly in their nutritional status.

[The protective effects of drugs H and X on liver cold ischemic injury].

Using isolated perfusion rat liver model (IPRL), we studied the protective effects of drugs H and X on rat's liver cold ischemic injury. After the rat's liver was preserved for 12 hours in Collins II solution with the addition of drugs H and X, the necrotic rate of sinusoidal lining cells was significantly lower than that of control group. Within certain range of the dosage of drugs H and X, the necrotic rate of sinusoidal lining cells was in the inverse ratio of the dosage in the Collins II solution. We also found that drugs H and X had obvious protective effect on liver cell redox function. But these drugs could not inhibit tissue from swelling after Collins II solution preservation. The protective mechanisms of the drugs maybe relate to the remoral of free radicals and suppression of Kupffer's cells activity.

[Changes of HDL-receptor of hepatic cell membrane in rabbit model of calcium bilirubinate cholangiolithiasis].

HDL-receptor of the hepatic cell membrane and Fch level of the hepatic tissue in the rabbit model of calcium bilirubinate cholangiolithiasis were examined. The results showed: (1)the HDL-receptors' Bmax of the biliary obstruction and infection (BOI) and biliary obstruction (BO) groups decreased during the 1st week after operation (P < 0.01); they were only 69.09% and 49.09% of that in the control group respectively, and the lowered level continued until the 6th week; the Kd values of BOI and BO groups were not different from that of the control group during the 1st week, but they were significantly higher in the 6th week after operation (P < 0.01); (2)the Fch levels of the hepatic tissue were slightly decreased in all experimental groups (P < 0.05). We concluded that under the conditions of biliary obstruction and infection, there was a disorder of the HDL receptors of hepatic cell membrane. At the early stage of biliary obstruction and infection, the receptors decreased in number, and at the later stage, the affinity changed and less HDL entered into the hepatic cell. Then the precurser cholesterol pool for BA biosynthesis narrowed. Thus, BA secreted from the liver cells into genetic elements possibly promote the formation of the calcium bilirubinate gallstones.

A study on oliguric standard in the elderly.

Water deprivation test was done in 44 healthy elderly persons, 26 were male, and 18 were female. They were deprived of the intake of water for 25 hours. The results showed that the maximal renal concentration function both in the elderly males and females was 887 +/- 91 mmol/L and 888 +/- 127 mmol/L respectively. The renal excretion of waste products in 24 hours both in the elderly males and females was 815 + 170 mmol/d and 620 + 132 mmol/d respectively. On the basis of point estimation, the minimal essential urine volume per day in the elderly males and females was 926 ml/d and 708 ml/d respectively, if the daily urine volume of the elderly is less than this level, then, the metabolic products generated per day can not be excreted by the kidney, and it would result in disorders of internal environment. Therefore, the oliguric standard in elderly males and females must be less than 926 ml/d and 708 ml per day respectively.

[The analysis of GPI in chimeric mice of ES cells (MESPU 13)].

To estimate the differential potentiality of ES (Embryonic Stem) cell line MESPU 13, the heart, liver, spleen, lung, kidney, pancreas, gonad, muscle and blood of 19 chimeric mice were analyzed for GPI (Glucose Phosphate Isomeras) marker, in these studies, type A band from the ES cells, appeared parallel to the coat color chimerism of the mice. When coat color chimerism is below 40%, type A band was not seen except in the kidney of a few mice. Type A band was detected in nearly all the organs and tissues, when coat color chimerism was over 85%, indicated the ES cell has a fairly high potential to differentiate into cells of endoderm, mesoderm and ectoderm. In addition, only type A band was observed in the muscles of 6 mice which coat color chimerism is over 85%, the results indicated that these muscles were differentiated only from ES cells.

[Surgical treatment of undifferentiated esophageal carcinoma].

From 1961 through 1983, a total of 3,804 cases with carcinoma of esophagus underwent surgical resection of cancer at Shanghai Chest Hospital. Thirty-nine of them were primary undifferentiated esophageal carcinoma (UEC) with an incidence of 1%. In 97% of the cases, the lesions were located at the mid- and lower-esophagus. All cases underwent tumor resection and esophagogastrostomy, with an operative complication rate of 10%, and mortality rate of 13%. Follow-up until December 1994, all patients were dead. 42% of the cases died within 6 months, another 42% of the cases died within 7-24 months, with a median survival period of 7 months only. The postoperative 5-year survival rate was 15%. To improve the therapeutic results, preoperative radiotherapy and postoperative adjuvant therapy are needed.

[Construction of a retroviral vector carrying human growth hormone gene and its introduction and expression in 3T3 and ES cells].

In order to study the expression of growth hormone gene in heterologous cells, the retroviral vector pINS was used to construct the recombinant plasmid carrying the human growth hormone gene (hGH), and then it was introduced into mouse fibroblast cell line 3T3 and mouse embryonic stem cell line CCE. Levels of accumulation of hGH secreted into medium was quantified by a radioimmunoassay. Expression level of hGH gene varied considerably from one clone to another. Several 3T3 clones expressing hGH gene at high level were obtained, such as GHSNC 20, in which hGH concentration in medium was 3784ng/ml, Integration of exogenous gene was stable. Southern blot result showed that such kind of difference was not related to the number of integrated copies of the foreign gene. The expression of ES clones transfected with pINS-GH was undetectable, and integration of the foreign gene was unstable. Southern blot result showed that hGH gene really integrated into the cellular genome, and no rearrangement of foreign gene is found at present.

[A primary observation on the effect of cell fusion on metastatic potential of tumor cells].

Mouse spleen cells transfected with pSV2 neo by CaPO4 precipitation were fused with highly metastatic cell clone (PLA801-D95) from human large cell lung cancer cell line. Hybrid cell clone PMS-2 was obtained after G418(400/ml)selection. After injection of 7 x 10(6) PMS-2 cells into nude mice, there was a tumor nodule developed, but the metastatic foci could not be found while 3 x 10(6) PLA801-D95 cells would metastasize to lung and lymph nodes after they were injected into nude mice. It might indicate that sometime mouse spleen cells could not suppress tumor formation but the metastatic potential could be suppressed by the fusion of mouse spleen cells with the lung cancer cells. The results of growth curve, serum independence and incorporation rates of H-thymidine all showed that the growth rates of parental cells were higher than those of PMS-2. Our data suggest that suppression of tumorigenicity and metastatic potential could be controlled by different kinds of genes, and the cloning of metastasis suppressor gene by subtractive hybridization is ongoing in our laboratory.

[Characterization of platelet activation induced by CD9 McAbs].

The platelet activation induced by two anti-human platelet P24/CD9 McAbs was investigated. The results showed that the pathways of platelet aggregation induced by the two McAbs (HI117 and SJ9A4) were different and HI117 and SJ9A4 induced the phosphorylation of platelet proteins (40,000 and 20,000) when platelets were activated. However, HI117 did not cause a rise in intracellular Ca2+ concentration in activated platelets in comparison with SJ9A4 and the epitope recognized by HI117 and SJ9A4 was different and this might explain why the two CD9 McAbs played different roles in platelet activation in addition, McAbs HI117 and SJ9A4 could not promote associations of other proteins (eg: GP II b/III a) with P24/CD9 on activated human platelets. All these results indicate that the mechanisms of platelet activation induced by HI117 or SJ9A4 are different from each other. It suggests that CD9 antigen plays an importent and complex role in platelet activation.

[Vena iliaca externa thrombosis after open operation of the bladder and prostate].

Since 1986, we have performed total cystectomy or partial cystectomy with ureteroneocystostomy in 64 cases and prostatectomy in 182 cases. 5 cases developed vena iliaca external thrombosis (VIET) or thrombophlebitis postoperatively. Three of them recovered after treatment, and 2 died of pulmonary embolism 7 and 16 days after operations. VIET is a very serious complication. Once develops pulmonary embolism, the patient may die soon. We suggested that patients be encouraged to increase limbs activities, be given medicines of antilipemic and antiatherogenic preoperatively. And during the surgical operation, patients' blood pressure should be kept steady, and hypopietic shock should be avoided as long as possible. The vena iliaca external should never be constricted when a deep hook is used for exposure or gauze is used to stop bleeding. After operation, the patient should be encouraged to exercise on bed or around bed as early as possible, so as to increase venous flow. The veins of lower limbs should be avoided by using infusion. Styptic should not be applied abusively. Infusion of dextranum-40 and orally taking of persantinum may also be used to prevent this kind of complication.

[Hemodynamic and renal effects of strong diuretic atrial natriuretic peptide in patients with acute myocardial infarction].

The hemodynamic and renal effects of strong diuretic atrial natriuretic peptide (ANP) were investigated in 12 patients with acute myocardial infarction. Strong diuretic ANP was administered as a bolus injection of 100 micrograms and followed by a continuous infusion of 2 micrograms/kg within 10 minutes into the right atrium via a catheter. We found that strong diuretic ANP induced an increase in cardiac index and stroke volume index. Right atrial pressure, pulmonary arterial pressure, pulmonary capillary wedge pressure, systemic vascular resistance and pulmonary vascular resistance were decreased after ANP infusion. The maximal responses were observed immediately after the infusion was completed. No significant change of arterial blood pressure and heart rate was observed. Furthermore, strong diuretic ANP also increased the urine volume, urinary sodium, urinary chloride and urinary potassium. These results suggest that strong diuretic ANP infusion may induce significant changes of hemodynamics and may be used to prevent the development of severe circulatory volume overload in patients with AMI.

[Hoffa disease: diagnosis and arthroscopic treatment].

The diagnosis of Hoffa disease (syndrome of impigement of the infrapatellar fat pad) was established in 20 patients with anterior knee pain by clinical physical examination, magnetic resonance imaging and arthroscopic findings. Arthroscopic partial or total resections of the infrapatellar fat pad were performed after arthroscopic examination. According to the Larson's rating score for knee function, 18 cases had a satisfactory result at an average follow-up of 36 months. The results of follow-up also suggested there was no significant difference between the partial and total resections of the fat pad.

[Arthroscopic evaluation and treatment of the disorder resulted from open meniscectomy of the knee].

Between 1989 and 1994, twenty four cases of knee disorders caused by open meniscectomy had undergone the second operation by arthroscopic procedures. The results of retrospective study indicated that main reasons of early disorders following meniscectomy were locking of remanent posterior horn, tears of remanent meniscus and injuries of fat pad and the cause of late disorders were single compartment osteoarthritis and genu varum/valgum caused by meniscectomy. In early cases, the arthroscopic reshape or removal of meniscal remanents and partial resection of fat pads could get satisfactory results. In late cases of osteoarthritis with genu varum/valgum, arthroscopic debridement, high osteotomy of tibia or knee replacement should be carried out as means of treatment.
