Some clinical results of in vitro fertilization by the Malmö Group, Sweden.

In our in vitro fertilization program, only clomiphene/human chorionic gonadotropin-stimulated cycles have been used, yielding 1.6 preovulatory oocytes/woman at follicle aspiration. The cleavage rate was significantly influenced by the percentage of abnormal spermatozoa and by the percentage of motile spermatozoa in the spermiograms. At embryo replacement, a tilted uterus obtained by a full bladder in the woman simplifies the replacement procedure. In the last 12 months, 47 embryo transfers have been carried out, resulting in five clinical pregnancies. Of these pregnancies, one has proceeded to term, one resulted in a miscarriage, and three resulted in ectopic pregnancies.

A gradual fixation method for chromosomal preparations of human oocytes.

Eleven oocytes were recovered after follicle aspiration carried out in nine women during laparoscopy or laparotomy. All oocytes were classified as preovulatory, judged by the fully dissociated cumulus. After elimination of the cumulus, ten of the oocytes exhibited the first polar body, and one was degenerated. A gradual fixation method was used for the chromosomal preparation of the oocytes. The quality of the preparations was improved, compared with the previously used air-drying technique. Nine of ten preparations from oocytes with first polar body displayed chromosomes. These sets consisted of well-spread meiotic chromosomes, and in six of them the chromosome complements of the first polar body could be seen, although quite distinct from the oocyte complement. Of eight preparations where the chromosomes could be counted, six showed 23 chromosomes, and two were abnormal, showing 21 and 16 chromosomes.

Technique to examine human sperm chromosomes using zona-free hamster eggs.

In experiments with repeated semen samples from two donors, zona-free hamster eggs were used for activation of human spermatozoa to metaphase. When mean number of spermatozoa per penetrated egg was low (1.0-3.3) after insemination in vitro, further incubation resulted in activation of spermatozoa to metaphase. High sperm/egg ratios, 4.2 and 7.5, yielded no sperm chromosomes. The preincubation and insemination intervals necessary for low sperm/egg ratios were individual.

The success rate of in vitro fertilization of human oocytes in relation to the concentrations of different hormones in follicular fluid and peripheral plasma.

Seventeen infertile patients were stimulated with clomiphene citrate and human chorionic gonadotrophin (hCG). During laparotomy or laparoscopy, follicles were aspirated. At least one fertilizable oocyte was obtained from 13 patients, who had normal preovulatory plasma 17 beta-estradiol (mean 2.2 nmol/l) in contrast to the 4 patients with infertilizable oocytes (mean 0.4 nmol/l). A close association was found between the success rate of fertilization and the increment of plasma progesterone from the day before to the day after the operation. If the concentrations of 17 beta-estradiol and progesterone in follicular fluid were below an arbitrary limit (17 beta-estradiol less than 500 nmol/l in association with progesterone less than 2,000 nmol/l), no oocytes could be fertilized. Apparently aspiration of oocytes and follicular fluid did not disturb ovarian steroid production during the luteal phase. The results suggest that plasma 17 beta-estradiol and progesterone determinations may serve as useful predictors for oocyte fertilizability in an in vitro system, following clomiphene-hCG stimulation and multiple oocyte recovery.

Granulomatous arthritis in Crohn's disease.

A case is described of non-necrotizing, epithelioid cell granulomatous synovitis of the ankle of a 22-year-old man with Crohn's disease. It appears to be the first published example of granulomatous arthritis in association with Crohn's disease.

A comparison between the hamster egg penetration test and the seminal parameters in men of infertile couples.

The zona-free hamster egg penetration test was performed on 74 random consecutive semen samples from men of infertile couples and on 7 men of proven fertility (semen donors). Of these 74 men, 31 had never before delivered a semen sample (Group I), while the remaining 43 had delivered samples before (Group II). The penetration rates were correlated with the parameters of the semen analysis. In Group I, the mean penetration rate was 59%, in Group II, 42%, and among the fertile donors, 52%. No strong correlation was found between hamster egg penetration rates and the various semen parameters. Only the proportion of motile spermatozoa in Group II correlated significantly with the egg penetration test (P less than .05). Differences in methodology make comparison with the results from different laboratories difficult. It is likely that the egg penetration test measures some separate quality of the spermatozoa that may not be clear from the routine semen analysis. The role of the hamster egg penetration test in the investigation of the causes of infertility should be evaluated further.

Verapamil, a Ca2+ channel antagonist, accelerates the in vitro penetration of zona-free hamster eggs by human spermatozoa.

Human spermatozoa were preincubated for 3.5 h with various concentrations (0-100 mumol/l) of verapamil, a Ca2+ channel antagonist, before they were mixed with zona-free hamster eggs. Control spermatozoa penetrated only 3% of the eggs, whereas the highest penetration (40.3%) was reached after preincubation with 100 mumol/l verapamil. Thus, verapamil increases the ability of human spermatozoa to penetrate zona-free hamster eggs, possibly by an acceleration of the acrosome reaction. Although it has been established that the acrosome reaction requires the entry of extracellular Ca2+ into the spermatozoa, the Ca2+ ions do not seem to enter by means of Ca2+ channels.
