Peripheral vascular disease in a middle-aged population sample. The Jerusalem Lipid Research Clinic Prevalence Study.

Peripheral artery status and prevalence of peripheral vascular disease were studied in a sample of 1,036 men and 556 women, residents of Jerusalem. Ankle blood pressures determined by the Doppler ultrasound technique (Parks Electronic Lab, USA), were higher in men than in women and increased with age. In a multivariable regression analysis, systolic blood pressure and plasma cholesterol levels were significantly and inversely related to the ankle/arm blood pressure in men, and alcohol intake was inversely associated in women. The prevalence of peripheral vascular disease, as determined by symptoms of intermittent claudication, was 1.3% in men and 1.8% in women; absent pedal pulses were found in 1.1% of the men and in 2.0% of the women; leg pain on the treadmill was a reason for termination of exercise in 3.7% of the men and in 5.4% of the women and an ankle/arm blood pressure ratio of less than 0.90 in 4.2% of the men and in 5.4% of the women. The overlapping of the diagnostic criteria was low. No consistent determinants of the various measures of peripheral vascular status were found. In multivariable logistic models, cigarette smoking was significantly associated with intermittent claudication [Odd ratio (OR) = 3.03, 95% confidence interval (CI) 1.27 to 7.26], age was associated with leg pain on the treadmill (OR = 1.91, 95% CI 1.08 to 3.40 in men and OR 3.52, 95% CI 1.52 to 8.16 in women), body mass index was associated with absent and diminished pulses in women (OR = 1.61, 95% CI 1.07 to 2.42, systolic blood pressure was associated with an ankle/arm ratio of less than 0.90 in men (OR = 1.35, 95% CI 1.01 to 1.81), and glucose was associated with absent and diminished pulses in women (OR = 1.20, 95% CI 1.00 to 1.44).

Short-term effect of nifedipine on effort tolerance in patients with angina pectoris.

The short-term effect of oral nifedipine on effort tolerance was tested in 10 patients with effort angina pectoris and a positive effort test (GXT). The patients had four symptom-limited GXTs, using the Bruce protocol, on each day of the study at 0800, 1000, 1400, and 1800 hours. They received four doses of 10 mg oral nifedipine on one day and four doses of placebo on the other, each dose given half an hour prior to each GXT. Values with nifedipine were compared to values with placebo at the same time during each day. Nifedipine improved effort tolerance by 0.5 +/- 0.6 min (p = NS) on the first GXT (mean +/- SEM), by 1.2 +/- 0.6 min (p = NS) on the second GXT, by 1.0 +/- 0.3 min (p less than 0.01) on third GXT, and by 1.3 +/- 0.3 min (p less than 0.01) on the fourth GXT. Improvement of effort tolerance was associated with a fall in resting blood pressure and less ST depression; these changes were statistically significant only on the fourth GXT, which may indicate a cumulative effect of subsequent doses of nifedipine.

Effect of sustained release isosorbide dinitrate on exercise performance.

The duration of action and hemodynamic mechanisms of isosorbide dinitrate in a sustained release formula (ISDSR) were examined over a 24-hour period. 20 patients with effort angina pectoris were divided randomly into two groups, one group received a single dose of 40 mg ISDSR (at 07.30 h) and the other, two doses of 40 mg ISDSR (at 07.30 and 13.30 h). Every patient had four exercise tests on the first (placebo) day and second (ISDSR) day and a single test on the third (placebo) day. Effort tolerance was improved significantly (p less than 0.05) with ISDSR and this lasted for more than 10 h. The double product was compared to ISDSR. Improvement was achieved by a decrease in blood pressure and an increase in heart rate. There were no signs of increased ischemia (ST segment depression) with the prolongation of exercise time. The antianginal and hemodynamic effects of ISDSR were more pronounced in the patients who received two doses of ISDSR. Thus, ISDSR improved effort tolerance for a prolonged period by inducing a sustained decrease of blood pressure and increase in heart rate in patients with effort-induced angina pectoris.

Treadmill exercise testing in a middle-aged Jerusalem population sample. The Jerusalem Lipid Research Clinic Prevalence Study.

We studied determinants of the exercising physical capacity of 1,640 subjects (1,043 men and 597 women) using the Bruce protocol as modified by Sheffield. These participants represented a sample of parents of 17-year-old adolescents living in Jerusalem. Apparently healthy participants underwent a graded treadmill exercise test (88% of the men and 87% of the women). Exercise was terminated when the subject achieved a heart rate of 85 to 90% of the predicted maximal heart rate or could not continue for other reasons, mainly fatigue or weakness. Exercise performance was assessed according to time on the treadmill. On univariate analysis, duration of exercise was related in both sexes to age, country of origin, reported physical exercise, resting heart rate, systolic and diastolic blood pressure, body mass index and plasma lipids and lipoproteins. In multivariate analysis, time was related to age, body mass index, heart rate and habitual physical exercise in both men and women, and to systolic and diastolic blood pressure in men only. Cigarette smoking and plasma lipid levels were not independently related to time on the treadmill in either sex. Ethnic differences were observed.

The effect of nifedipine on normal and ischemic left ventricular function during atrial pacing stress.

We studied 44 patients (group I: 9 with insignificant disease and group II: 35 with significant coronary artery disease) by nuclear angiography during the controlled increase in heart rate by atrial pacing to evaluate the effect of 20 mg of sublingual nifedipine on left ventricular function. Left ventricular volumes were calculated using count-derived changes: end-diastolic (EDE), end-systolic (ESE), stroke volume (SVE) and cardiac output equivalents (COE) and ejection fraction (EF) and ejection rate (ER) were calculated during basal sinus rhythm (C) and at the maximal atrial pacing (AP) rate with and without nifedipine. In normal subjects, AP decreased ventricular volumes. The administration of nifedipine decreased blood pressure but had no further effect on left ventricular volume. In the ischemic ventricle, AP produced reversible ventricular wall paresis with a smaller decrease in EDE, an increase in ESE and a significant fall in EF. Nifedipine induced a striking improvement in EF and ER. This was probably a consequence of afterload reduction and increased coronary blood flow.

[Diagnosis of a cyst wall hypernephroma].

The article reports on a 56-year old patient with the very rare preoperative diagnosis of a cyst wall hypernephroma. This diagnosis was confirmed both surgically and histopathologically. Diagnosis with the help of roentgenography showed that the diagnosis which had been suspected on the basis of selective arteriography, received a high measure of certainty by the additional use of non-invasive sonography and computerized tomography, thus finally justifying surgical approach to lay open the kidney.

Thermographic analysis of skin test reaction using AGA thermovision.

The course of infrared thermography including isothermograms on the skin surface was investigated considering blood flow, redness of the skin and permeability of blood vessel, in the following skin reactions: 1. Intracutaneous injection of histamine and histamine liberator compound 48/80 increased the heat radiation. Local application of antihistamine externa which decreased the development of the urticarial histamine reaction, increased the infrared radiation of the skin surface. Combined injection of histamine or histamine liberators with antihistamines in a sufficient dosis (1:1 respectively 4:1) diminished also the heat radiation in addition to the urticarial reaction. 2. The Pyrexal reaction of the skin with early erythema and later papule development shows an equivalent picture in the AGA Thermovision. The pretreatment shows an equivalent picture in the AGA Thermovision. The pretreatment of the skin with corticosteroid ointments shows a corresponding lowering of the erythema, of papule development as well as of heat radiation. The blanching of corticosteroids after occlusive dressing is difficult to recognize by the isotherms of AGA Thermovision. 3. Allergic reactions of the immediate type show, corresponding to the wheal eruption, a marked increased of heat radiation combined with a projection of the enlarged veins on the skin surface. 4. Allergic reactions of the delayed type are combined with a definite elevation of heat radiation of the skin. The area of a positive skin test with allergic eczematous reaction shows a distinct elevation of ann infrared radiation. Although the allergic skin area which was substantiated by a positive skin test was no longer visible, a distinct infrared radiation could be detected. Preventive treatment of the test area of skin patch-testing with corticosteroids inhibits the heat radiation even if the allergic eczematous reaction occurs faintly. The thermographic analysis of the different skin test reactions complied with the morphological aspects of the reaction.

Blood parasites of some Jamaican birds.

The prevalence of avian hematozoa in 1791 birds of 80 species and 22 families from Jamaica was determined. Species of Haemoproteus were the most common hematozoan encountered. Species of Leucocytozoon were diagnosed only in three over-wintering North American migrants and the genus was absent in native Jamaican birds. The low prevalence of infection (7.4%) of blood parasites in Jamaican birds was closely similar to that seen in Neotropical birds and far below that noted for the Nearctic avifauna of the same families. Prevalence in adult and young birds was virtually identical; transmission occurred primarily during the period February-April.

[Proposals for inhalation therapy with ultrasonic nebulizers (author's transl)].

With aerosols active substances (drugs) can be transported for treatment. Their effect depends on the choice of apparata, the drugs used, and the procedure of inhalation. The use of electronic nebulizers is effective because a sufficient volume of liquid substances can be nebulized by this way. Bronchospasmolytics, secretolytics, antiphlogistics, and antibiotics may be used. The right connexion of the patient, who should inhale in a closed system is important for an effective treatment. The patient is exposed to some hazards by contamination with inhaled bacteria, by hyperventilation, bronchospasm and by allergic reactions.
