The homogeneity of the faecal coliform flora of normal school-girls, characterized by serological and biochemical properties.

The homogeneity of fecal coliform flora in 52 schoolgirls was studied by serotyping and biotyping 10 randomly selected colonies in one fecal culture from each child. Ninety-eight clones were identified and of these 52 were dominant and 46 were minor strains. The probability of including at least one isolate of the dominant clone in a small random sample of colonies was calculated to be 86% for one colony, 94% for 2, 97% for 3, 99% for 4, and 99.3% for 5 randomly selected colonies.

Myocardial infarction among men below age 40.

Studies were made in Göteborg over a period of 6 years of all cases of acute myocardial infarction diagnosed among men below the age of 40. Thirty-six cases were registered and 8 of these died outside hospital. Three patients died early during the hospital stay. The remaining 25 patients were compared with a random sample from the general population in Göteborg with respect to conventional risk factors. Smoking, and high plasma cholesterol values were dominating findings among the patients, whereas there was no significant differences in blood pressure levels. Coronary angiography was performed in 18 patients of whom 2 showed normal coronary arteries and left ventricular angiograms. These 2 patients were the only ones free from risk factors. Of the remaining 16 patients, 10 had only one vessel affected.

[Continuous stapedius reflex registration in low and high intensive levels (author's transl)].

To study the whole dynamic range of the stapedius reflex the intensity of the soundstimulus with fixed frequency is continuously changed from 70 to 115 dB. Thus we get results concerning the real reflex eleciting intensity level. the gradient caused by continuous intensity change and a possible limit of contraction. To investigate time resolution phenomenons the stimulating signal is pulsated in a second trial. We used this method of stapedius reflex registration with 48 normal ears and a group of patients with sensorineural hearing loss. The results and the diagnostic possibilities of this method are discussed.

Comparison of Escherichia coli from bacteriuric patients with those from feces of healthy schoolchildren.

The properties of 709 strains of Escherichia coli isolated from feces of healthy schoolchildren were compared with those of 115 strains from the urine of girls with asymptomatic bacteriuria (ABU) detected in a screening program. These fecal strains were also compared with 45 strains that caused asymptomatic reinfections and 10 that caused symptomatic reinfections in the same group of girls. Typing of O antigen was done by direct bacterial agglutination, and K typing was done with a serum agar technique. Hemolytic capacity was assessed in solid medium. Sensitivity to the bactericidal effect of normal serum and minimal inhibitory concentrations of ampicillin were also determined. The strains isolated from girls who had reinfections of ABU were found to be a random sample of the fecal flora, but the strains from children with symptomatic reinfection were not. Strains from index patients with ABU differed from the other groups in a way that was indicative of adaptive changes in the structure of cell envelopes.

Prediction of cardiovascular deaths and non-fatal reinfarctions after myocardial infarction.

The present study concerns the development and validation of a logistic multivariate prognostic function in patients followed for two years after myocardial infarction. The patients studied constituted at least 90% of all cases in a total community--the City of Göteborg, Sweden. Using a multivariate logistic function with 7 variables, based on 30 cardiovascular deaths during two years after discharge from hospital among 292 men with first infarction, breathlessness at onset of symptoms of infarction, SGOT quartile, left heart failure, relative heart size, atrial fibrillation, a history of hypertension, and AV block recorded during the hospital phase were the most important variables. The first five of these variables made significant contributions (p less than 0.01) to the predictive power. The predictive capacity was confirmed in an independent series of 195 men with first infarction, among whom 17 cardiovascular deaths occurred. Around 60% of the total cardiovascular mortality was concentrated in the highest risk quintile. Deaths from non-cardiovascular causes were predicted less efficiently. Non-fatal recurrences could not be predicted by the present model. Thus, the function can predict the excess risk of mortality but not the excess risk of reinfarction during two years among men after an initial myocardial infarction.

Coffee consumption and coronary heart disease in middle-aged Swedish men.

The possible association between coffee and myocardial infarction (MI) has been studied both prospectively in a random population sample of Swedish men aged 50 years (n=834) and with case control methodology in non-selected male patients surviving a MI (age 40-57 years, n=230). Coffee consumption was significantly associated with two other important risk factors for MI, namely smoking and alcoholic intemperance, but weak (non-significant) relationships were found with serum cholesterol, serum triglycerides, systolic or diastolic BP, and dyspnoea on exertion. In the prospective study there was no significant relationship between coffee and MI, either with univariate or multivariate analysis. The retrospectively reported coffee consumption of MI patients was higher than of those who later suffered a MI (the population sample). With the aid of non-parametric multivariate analysis of the combined population sample and the series of MI patients, a significant association was found between coffee consumption and MI. The experience of having had a MI may have affected the patients' rating of coffee consumption, or their consumption may have really increased during some months or a few years before the MI.

Comparison between the use of oral contraceptives and the incidence of surgically confirmed gallstone disease.

A causal relationship between the use of oral contraceptives and gallstone disease has been proposed. If this is true, it is reasonable that the incidence of cholecystectomy in women of exposed ages should increase. In the present study a statistical analysis of the change in the incidence of cholecystectomy in women between 1961, i.e. 4 years before, and 1971, i.e. 6 years after, the introduction of oral contraceptives in Sweden was performed. To delimit uncontrolled factors, such as diet and attitude towards surgery, the change in incidence of cholecystectomy in women was compared with the corresponding change in men during these years. The results indicate an increased incidence of cholecystectomy in women in age groups exposed to oral contraceptives. These findings support a causal relationship between the use of oral contraceptives and the incidence of gallstones in the population.

Primary risk factors in patients with myocardial infarction.

A predicted probability of suffering myocardial infarction based on a multiple risk function involving serum cholesterol, systolic blood pressure, and tobacco consumption, was allocated retrospectively to 270 men who survived a primary myocardial infarction. The infarction patients were representative of all surviving, diagnosed cases of primary infarction in men in certain age groups in Göteborg, Sweden, during the years 1968-70. The patients were divided into three groups-low, moderate, and high risk. A large number of patients had suffered infarction despite relatively low risk, but the patients showed a tendency toward higher risk in comparison with the risk distribution in a representative population sample. In order to study whether other variables, not included in the risk function, could "explain" the infarction in patients with relatiely low risk, the different risk groups were compared. A high degree of mental stress, diabetes mellitus, and dyspnea on exertion, and possibly also raised triglycerides, contributed to "explain" the infarctions in the low-risk group. Low physical activity during leisure time was probably also of importance.

[Clinical significance of kidney imaging in bone scintigraphy].

The results of the examination of 62 patients who had undergone skeletal scintigraphy with 18-F and a urologic examination, were evaluated to determine whether the 18-F incorporation in the kidneys might indicate the presence of renal disease. Patients with and without renal disease demonstrated a similar 18-F incorporation in the kidneys. Our data indicate that a side different 18-F renal activity is without clinical significance.

The reaction of salivary substances with bacteria.

Reactions between selected strains of oral streptococci and saliva were studied spectrophotometrically. The curves of absorbance (700 nm) versus those of time were sigmoidal for aggregating saliva-bacteria mixtures. Whtn the logarithm of the ratio of the change in absorbance to the remaining absorbance was plotted against the reaction time, straight lines were obtained. At a fixed concentration of bacteria, the slope of these lines for a series of dilutions of a selected saliva sample was a parabolic function of the volume of saliva in the reaction mixtures. A model for the aggregation reaction was developed which explained the shape of these parabolic curves. The parameters of the model can be calculated from the experimental data and can be used to estimate the concentration of aggregation-inducing substances in undiluted saliva. A method for calculating the specific rate constant for the binding of free cells to aggregates was developed. This rate constant is a characteristic feature of a given aggregation reaction and can, in principle, be used to distinguish between aggregation-inducing substances of different molecular structures.
