Kinetic analysis of prostatic volume and prostate specific antigen (PSA) in patients with advanced prostatic cancer treated by castration.

The clinical usefulness of the kinetic analysis of prostate specific antigen (PSA) in patients with advanced prostatic cancer treated by castration has not yet been evaluated. The reduction of both prostatic volume and PSA was monitored in 37 patients with Stage C and Stage D prostatic cancer. The change of prostatic volume was measured frequently by transrectal ultrasonography (TRS) after castration, as was the change of PSA. A kinetic analysis of both prostatic volume and PSA was made by comparing the reduction time tau(PSA) (speed of the reduction of PSA) and tau prostatic volume (PV). There was a statistically significant correlation between tau(PSA) and tau(PV) in patients with a tau(PV) of less than 30 days. However, no correlation was observed in patients with a tau(PV) of more than 30 days, because the tau(PSA) in this group fell into a relatively low range similar to the group with a tau(PV) of less than 30 days. It was assumed that the change of PSA after castration reflected mainly the response of the hormone dependent portion of the total cancer volume. However, all data of tau(PV) with a tau(PSA) less than 10 days were within 30 days. In conclusion, tau(PSA) might be promising as a prognostic factor after castration in patient with advanced prostatic cancer, although tau(PSA) was not a direct substitute for tau(PV).

The protective effect of a persistent trigeminal artery on brain stem infarctions: a follow-up case report.

A persistent trigeminal artery (PTA) represents an embryonic vascular anastomosis connecting the carotid and basilar arterial systems. Little is known about its protective role in cases of basilar artery occlusion. We followed up a 63-year-old man who had suffered a brain stem infarction due to basilar artery stenosis and was found to have a PTA. Although a second brain stem infarction due to basilar artery occlusion developed, the circulation to the brain stem was well maintained via collateral flow from the PTA, and the patient demonstrated good recovery. A PTA may function as an anastomosis between the carotid and basilar systems, thus preventing a more serious infarction.

Studies on lead-binding protein and interaction between lead and selenium in the human erythrocytes.

To clarify the function of Pb-binding protein and the interaction between lead and selenium in human erythrocytes, the following experiments were performed. (a) blood from a man who has not been exposed to lead occupationally, (b) blood from lead-exposed workers and (c) blood from control subjects working in the same plant were used. (a) was incubated with Pb and/or Se. Hemolysates of rinsed erythrocytes were applied onto a Bio-gel A 1.5 m column and eluted by 40 mM Tris-HCl buffer (pH 7.0). As the results, (1) Pb in erythrocytes was found in ALAD fraction. When Pb was added in vitro at 1.25 microM of final concentration, Pb content of ALAD fraction increased and the other Pb-containing fraction appeared at the position of a smaller molecular size than that of Hb fraction. When Pb dose added was increased to 2.5 or 10 microM, Pb contents in the newly appeared fraction was larger than that in ALAD fraction. The affinity of Pb with the protein in the new fraction was weak, and Pb was released from the protein by ultrafiltration. (2) There were main and sub-peaks containing Pb in erythrocytes from Pb-workers; the former was ALAD fraction and the others were smaller molecules than ALAD. The peak of Pb observed with the blood from a worker who has worked at the same plant for more than 20 years (chronic exposure) corresponded with the new peak which appeared in vitro Pb exposure. (3) As to the interaction between Pb and Se, there was no effect of Se added in vitro on the position of Pb-containing fraction and on Pb amounts in the chromatographic profile. When Se was incubated with blood, however, coexistence of Pb made Se concentration in erythrocytes high compared with the case of Se alone.

Cyfra 21-1 as a marker of lung cancer.

The utility of cytokeratin fragment (Cyfra) 21-1, a new tumor marker, was investigated in 100 patients with lung cancer. Sandwich enzyme immunoassay detected Cyfra 21-1 in the sera of 60% of patients. Sensitivity of this marker was especially high (86.4%) for squamous cell carcinoma, exceeding that of a similar marker, squamous cell carcinoma antigen (SCC, 54.5%). In contrast, sensitivity of Cyfra 21-1 was relatively low for adenocarcinoma (52.6%) and for small cell carcinoma (50%). We conclude that Cyfra 21-1 is of value in diagnosis of lung cancer, particularly squamous cell carcinoma.

Molecular cloning of Arabidopsis photolyase gene (PHR1) and characterization of its promoter region.

Photolyase is an enzyme that repairs ultraviolet (UV)-damaged DNA by photoreactivation. In higher plants, accumulation of photolyase (PHR1) mRNA is induced by either UV or visible light. In order to know the molecular mechanism by which PHR1 gene expression is induced by light, we have determined the genomic structure and the 5'-flanking sequence of the Arabidopsis PHR1 gene. The PHR1 gene spans approximately 2.5 kb of genomic DNA and consists of 9 exons. In the promoter region of PHR1, there are two pairs of inverted repeats spanning more than sixty base pairs. The promoter also contains DNA motifs similar to the GT-1 box or G-box found in many light-inducible gene promoters. EMSA analysis showed that several proteins in Arabidopsis nuclear extract bound to the G-box-like motifs. These results raise the possibility that the Arabidopsis PHR1 gene is regulated by transcription factors which interact with these motifs.

Nucleotide sequence of the GLABROUS1 gene of Arabidopsis thaliana ecotype Columbia.

The GLABROUS1 (GL1) gene from Arabidopsis thaliana ecotype Columbia was isolated and sequenced. Comparison of the nucleotide sequence with that of the gl1-2allele revealed that two sites were changed in gl1-2. One was a 14-bp deletion in exon 3, which seemed most likely to be the cause of the mutated phenotype. The other was a change from TC to CT in the 5' untranslated region. Since the 5' upstream sequence required for the GL1 gene expression has not been clearly specified, one cannot rule out the possibility that this change may contribute to the phenotype of gl1-2. A comparison was also made with the GL1 gene of ecotype Wassilewskija and revealed some lack of sequence conservation. The changes observed may have little influence on the function or the expression of the GL1 gene product.

Inhibition in a microgravity environment of the recovery of Escherichia coli cells damaged by heavy ion beams during the NASDA ISS phase I program of NASA Shuttle/Mir mission no. 6.

We participated in a space experiment, part of the National Space Development Agency of Japan (NASDA) Phase I Space Radiation Environment Measurement Program, conducted during the National Aeronautics and Space Administration (NASA) Shuttle/Mir Mission No. 6 (S/MM-6) project. The aim of our study was to investigate the effects of microgravity on the DNA repair processes of living organisms in the in orbit. Heavy ion beam radiation- or ç-irradiation-damaged biological samples of Escherichia coli and the radioresistant bacterium Deinococcus radiodurans were prepared and placed in a biospecimen box, which was loaded into the RRMD III sensor unit of the Space Shuttle. Two identical sets of samples were left in the Spacehab's Payload Processing Facility (SPPF) in Florida, USA, as a control. (flight No. STS-84) was launched from NASA John F. Kennedy Space Center (KSC) in Florida, USA, on May 15, 1997. The mission duration was 9.22 days. An astronaut activated the biological samples in the biospecimen box in the Spacehab during orbit in order to start repair of the DNA damaged by heavy ion beams or ç-irradiation and the samples were incubated for 19 h 35 min at about 22ûC, the cabin temperature. The control specimens in the SPPF were subjected to the same treatment under terrestrial gravity. After returned to earth, we investigated cell recovery by comparing the repair of the radiation-damaged DNA of E. coli and D. radiodurans in the microgravity environment in space with that on Earth. The results indicated that the DNA repair process of E. coli, but not of D. radiodurans, cells was inhibited in a microgravity environment.

Death activation does not stop tumor growth: quantitative evidence for concomitant activation of apoptosis in tumor growth in vitro.

A protein-independent fibrosarcoma, Gc-4 PF, grows exponentially in a protein-free medium. The doubling time (approximately 26 h) was similar to that of the serum-dependent parental clone, Gc-4 SD cultivated in the presence of fetal calf serum (FCS). We demonstrated here that the protein-free cultivation of Gc-4 PF cells concomitantly activates apoptotic phenotypes (one third of total cell population), including typical morphology, high uptake of Hoechst 33342 dye, and cleavage of DNA to large fragments, as observed in protein-deprived Gc-4 SD cell previously. Gc-4 SD cells arrested in the G0/G1-phase in response to the protein-free condition. In contrast, Gc-4 PF cells did not reach G0/G1 arrest in the protein-free condition; instead the durations of both G0/G1 and G2-phases were markedly reduced. The estimation of one cell cycle duration revealed that the cell division cycle was accelerated to 1.7 (27 h/15.4 h)-fold. Then the growth kinetics was able to be verified quantitatively by both the cell division rate and apoptotic cell loss. Protein-free cultivation resulted in slight down-regulation of c-myc protein in both cell types, while the down-regulation of p34cdc2, shown clearly in Gc-4 SD cells, was avoided in Gc-4 PF cells. Interestingly, while the expression of p53 was not affected in Gc-4 SD cells in response to the protein-free condition, the suppressor gene product expression was suppressed markedly in Gc-4 PF cells. These results suggest that Gc-4 PF cells may have acquired an ability to accelerate cell division by shortening the cell cycle duration to maintain a proper growth rate in response to intrinsic apoptosis activation with, at least in part, a suppression of p53 expression as well as an escape of down-regulation of p34cdc2.

[Experimental studies of the protective effect of deferoxamine mesilate on cisplatin induced toxicity].

Cisplatin (CDDP) is an antitumor agent against several types of neoplasms. It has, however, various side effects such as nephrotoxicity and ototoxicity. Several efforts have been made to prevent these toxic side effects. On the other hand, deferoxiamine mesilate (DFO), an iron-chelating agent, has been used for iron-overloaded patients. Since the 1980s, many authors have reported DFO-associated hearing impairment. Some experimental data suggest that DFO itself was responsible for ototoxicity. In addition, it has also been recognized that DFO can act as a free radical scavenger. Experimental trials using DFO are also expected to prevent CDDP-induced toxicity because its generation mechanism is thought to be associated with free radical formation. The present study was planned to investigate whether DFO, which might be an ototoxic agent, had a protective effect against various CDDP-induced toxicities including ototoxicity. Fisher rats were used in this study and were divided into four groups: 1) Group I, a vehicle control, 2) Group II, animals receiving 100 mg of DFO per kg, 3) Group III, animals administered 0.9 mg of CDDP per kg alone and 4) Group IV, animals receiving 100 mg of DFO per kg 60 min before 0.9 mg of CDDP per kg for 10 days. First, the protective effect of DFO against CDDP-induced ototoxicity was studied. The auditory threshold was determined by using the compound action potential (CAP) from the round window membrane. CAPs were recorded on the 5th day after completion of drug administration. Then CAPs recording, cochlear sensory epithelia were observed over all the turns of the cochlea by scanning electron microscopy. There were no significant differences in CAP thresholds between Group I and IV, though the thresholds in Group III were significantly higher at 16 kHz and 20 kHz than those in Group IV. The rate of missing outer hair cells in Group IV was significantly lower than that in Group III. The results clearly demonstrated that DFO had a protective effect against CDDP-induced ototoxicity. Second, the protective effect of DFO against CDDP-induced nephrotoxicity was studied. Renal function was evaluated by measuring blood urea nitrogen (BUN) and serum creatinine (Cr) levels. Both BUN and Cr levels in Group IV were significantly lower than those in Group III. The data suggested that DFO preadministration prevented CDDP-induced nephrotoxicity. Third, the influence of DFO on the antitumor activity of CDDP was investigated in rats inoculated with squamous cell carcinoma cells (SCC-131) subcutaneously. The influence of drugs was determined by measuring the tumor growth rate. There was no difference in the tumor growth rate between Group III and IV. The result revealed that DFO had no influence on CDDP antitumor activity. In conclusion, the above results demonstrating that DFO preadministration can prevent both CDDP-induced ototoxicity and nephrotoxicity without attenuation of CDDP antitumor activity, suggest the usefulness of CDDP antitumor chemotherapy.

[Acupuncture for urinary incontinence in patients with chronic spinal cord injury. A preliminary report].

PURPOSE: We investigated the possible use of acupuncture for the treatment of urinary incontinence caused by detrusor hyperflexia in patients with chronic spinal cord injury. METHOD: A total of 8 male chronic spinal cord injured patients with urinary incontinence were treated by acupuncture. Their ages ranged from 20 to 33 years (mean 27). The level of lesion was cervical in 4 and thoracic in 4. Detrusor hyperreflexia with uninhibited bladder contraction was confirmed by urodynamic studies in all of them. Acupuncture was performed using a disposable stainless needle (0.3 mm in diameter, 60 mm in length), which was inserted into bilateral BL-33 (Zhongliao) points and was rotated manually for 10 minutes. The treatment was conducted every week for 4 weeks. Urodynamic studies were repeated, immediately after the beginning of and a week after the completion of the treatment. Urinary symptoms were also checked before and after the treatment. RESULTS: No side effects were recognized throughout the treatment period. Among 8 patients, incontinence was controlled completely in 3 (38%) and partially in 3 (38%). The average maximum cystometric bladder capacity increased significantly, from 42.3 +/- 37.9 ml to 148.1 +/- 101.2 ml by the treatment (p < 0.05), while the average maximum bladder pressure was not changed. CONCLUSIONS: These data suggest that acupuncture could be a promising alternative for conventional therapies for urinary incontinence caused by detrusor hyperreflexia in patients with chronic spinal cord injuries.

[Predictability of Gleason score and reduction time (tau) of prostatic volume after castration for the prognosis of prostatic cancer].

PURPOSE: The efficacy of the reduction time (tau) after castration as a prognostic factor was examined by comparing to Gleason score. MATERIALS AND METHODS: The change of prostatic volume after castration was observed from the castration to 3 months after in 24 cases of prostatic cancer. Prostatic volume was examined by transrectal ultrasonography of the prostate. Survival curves was calculated by Kaplan-Meier method. Differences among survival curves were analyzed using Cox-Mantel test. RESULTS: tau had a close relationship to the prognosis of each case (Wilcoxon test: p < 0.05, Cox-Mantel test: p < 0.05). Gleason score had a weak relationship to prognosis (Wilcoxon test: N. S., Cox-Mantel test: p < 0.05). CONCLUSIONS: tau was efficient as prognostic factor compared to Gleason score.

[A case-control study on the efficacy and the optimum interval of mass screening for prostatic cancer].

PURPOSE: The efficacy and the optimum interval of mass screening for prostatic cancer were investigated by a case-control study. MATERIALS AND METHODS: A matched pairs analysis by 1:5 was conducted between 31 cases of advanced prostatic cancer in stage C and D detected by mass screening and 155 controls of normal subjects selected at random. In all cases and controls, the history of taking part of the mass screening for last 3 years, then the estimates of odds ratios (OR) and 95% confidence intervals (95% CI) were calculated on discordant pairs of the matched analysis. RESULTS: OR to suffer from advanced prostatic cancer was 0.22 (95% CI: 0.07-0.70) in the group to submit to mass screening 1 year ago in comparison with the group not to submit once for 3 years. According to the analysis on the interval of submission to mass screening, the effect of screening to prevent the progression to advanced stages might be valied for 1 year. CONCLUSION: Mass screening for prostatic cancer every year might be efficient to reduce the number of patients progressing to advanced stages.

Chronic cerebral hypoperfusion-induced neuropathological changes in rats.

We investigated the time courses of histopathological changes in various brain regions following permanent occlusion of the bilateral common carotid arteries (2VO) in rats. 2VO rats exhibited rarefaction in the white matter, shrinkage of neurons in the cerebral cortex and hippocampus CA1-3 and dentate gyrus areas 1-3 days after the operation. These histological changes in the cortex and hippocampus were accompanied by a decrease in immunoreactivity for microtubule-associated protein 2 (MAP2), a marker protein of neuronal dendrites. Immunoreactivity for glial fibrillary acid protein (GFAP) was observed at 3-7 days after the 2VO operation. A marked increase in GFAP staining of the astrocytes in the cerebral cortex and hippocampus was found 30 days after ligation. Eight-arm radial maze performance was tested from 14 days to 60 days after the operation. The 2VO rats showed fewer initial correct responses than sham-operated control rats during a repeated training period. These findings suggested that the loss in dendritic MAP2 immunoreactivity and an increase in astroglial staining and/or rarefaction of the white matter may cause neuronal death, infarction and learning impairment under conditions of chronic hypoperfusion.

Gait analysis before or after varus osteotomy of the femur for hip osteoarthritis.

SUMMARY OF BACKGROUND: Intertrochanteric varus osteotomy of the femur for hip osteoarthritis is expected to bring improved congruity and increased weight-bearing surface area of the hip joint. Before surgery, we usually make a tracing on a paper using an anteroposterior roentgenogram of the hip to simulate the operating procedures and obtain better conditions in the hip. Nevertheless, there have been some cases in which this procedure does not provide satisfactory postoperative results in terms of relief of pain or locomotion, with long-term limp due to weakness of the hip abductor muscle group and shortening of the leg. OBJECTIVES: To confirm if the improved congruity and increased covering of the weight-bearing surface area obtained by varus osteotomy can be actually reproduced during walking, and to evaluate the kinetic mechanism of the effects of this procedure using gait analysis. METHODS: We measured the strength ratio of the hip abductor muscles as the percent of the opposite side, and analyzed the pelvic movement and data of dynamic electromyography (EMG) recorded during locomotion in 24 female patients who underwent unilateral intertrochanteric varus osteotomy of the femur for hip osteoarthritis, 30 non-surgically-treated female patients with hip osteoarthritis, and 54 healthy women. In the operated patients, the roentgenograms obtained during one-legged stance on the affected side disclosed that 10 were positive for Trendelenburg's phenomenon (T(+) group) and 14 were negative for Trendelenburg's phenomenon (T(-) group). All subjects walked with bare feet at a comfortable pace on a walkway containing a force plate. Photo switches were placed in a walkway to measure the gait velocity and to determine the stance phase time of one gait cycle. Frontal and sagittal trajectories of body surface markers for computerized joint-angle motion analysis were acquired using the Quick-MAG system. EMG data for the gluteus maximus, gluteus medius, tensor fascia latae, and lateral hamstrings recorded using surface electrodes were integrated to quantify as the percent of maximum voluntary contraction (% MVC). RESULTS: This study disclosed that the stance phase time was shorter and the strength ratio of the hip abductor muscles was lower in the operated patients than those in the other 2 groups, and the change of the pelvic obliquity was smaller and the % MVC of the gluteus medius and tensor fascia latae were greater than those in the normal subjects. The change of the pelvic tilt showed the same pattern with those of the pelvic obliquity. The T(+) group showed decreased strength ratio of the hip abductor muscles and increased % MVC for the gluteus medius and the tensor fascia latae compared to the T(-) group, but the changes of both the pelvic obliquity and pelvic tilt did not significantly differ in the 2 groups. CONCLUSION: This study showed the postoperative reappearance of the simulated conditions in the hip before varus osteotomy of the femur, providing evidence that the pelvis was horizontally maintained during walking due to decreased stance phase time and increased performance of the hip abductor muscles after this procedure.

[The usefulness of carotid artery ultrasonography in patients with Binswanger's encephalopathy].

Carotid artery ultrasonography (B-mode and Duplex method) was conducted in 49 cases of Binswanger's encephalopathy (group B) to compare with 189 cases of lacunar infarction (group L). With B-mode method, there was no significant difference in the amount of plaque between group B and group L, but the inner diameter of the common carotid artery was greater in group B. Using Duplex method, we measured maximum systolic flow velocity (Max), minimum diastolic flow velocity (Min) and time velocity integral (TVI). We found that those data were consistently lower in group B than in group L, with a correlation to the severity of mental impairment. The decrease in Max in group B, which has diffuse arteriosclerosis, indicates a decline in cardiac function, while the decrease in Min indicates an increase of intracranial small vessel resistance. The lower TVI in group B is thought to reflect a loss of velocity in cerebrovascular circulation due to the decrease in Max and Min. Carotid artery ultrasonography thus proved useful in evaluating the pathogenesis and progression of the disease.
