Expression of p8 in human pancreatic cancer.

The p8 gene is a recently identified gene with mitogenic activity. p8 expression is induced in acute pancreatitis, pancreatic development, and regeneration. However, the expression of p8 in pancreatic cancer is not reported. We investigated p8 expression in 72 human pancreatic tissues, including 38 pancreatic cancers (PCs), by immunohistochemistry. p8 was overexpressed (positive cells >25% in 1,000 cells) in 71% (27 of 38) of PCs, but in only 17% (3 of 18) of chronic pancreatitis cases. There was no overexpression in mucinous cystadenoma or in normal pancreas. The p8 overexpression rate in PC was significantly higher than that in other conditions (P < 0.05). Reverse transcription-PCR analysis confirmed p8 mRNA overexpression (tumor/nontumor ratio >2) in 75% (3 of 4) of PCs. p8 was overexpressed also in human pancreatic cancer cell lines (MIA PaCa-2 and PANC-1). These results suggest that p8 is involved in the development of pancreatic cancer, reflecting its mitogenic activity.

[Pitfall in the choice of the in situ arterial grafts in OPCAB].

Flow capacity with arterial grafts which have been used in off-pump CABG has been discussed. To analyze the hypoperfusion syndrome when in situ arterial grafts are selected, we made the simple mathematical model which consisted of two vessels with in parallel. We speculated the flow capacity of the various grafts, and flow distribution in the distal coronary artery, based on Poiseuille's law. This theoretical model clearly demonstrated that hypoperfusion syndrome occurred in reoperative coronary bypass grafting had a close relation with the diameters and lengths of bypass grafts.

[Epithelial scraping for corneal epithelial ingrowth after laser in situ keratomileusis].

BACKGROUND: Laser in situ keratomileusis (LASIK) is an effective method for correcting a wide range of myopia. However, complications sometimes occur. We report here a case treated with epithelial scraping two years and 4 months after surgery for epithelial ingrowth in the central cornea after LASIK. CASE: A 32 year-old woman who had undergone LASIK one year and 4 months before visited our hospital because of visual loss. At the first examination in our clinic, her best-corrected visual acuity was 20/60 in the right eye and 20/15 in the left eye. Epithelial ingrowth was observed in both eyes, but in the right eye, the epithelial ingrowth had reached the central cornea leading to the visual loss. The increased corneal stromal opacity decreased the visual acuity to 20/400. The patient decided to undergo epithelial scraping. The corneal flap was dissected again and epithelial scraping was performed on the stromal bed and back of the corneal flap. The visual acuity was improved from 20/400 to 20/40. However, the stromal opacity and the corneal astigmatism still remained. CONCLUSION: This case suggests that epithelial scraping should be performed immediately when ingrowing epithelium after LASIK begins to invade the central cornea.

[Comparison of quantitative coronary arteriography using cinefilm and digital images].

OBJECTIVES: Quantitative coronary arteriography using both analog data (cinefilm) and digital data was evaluated with a stenotic vessel phantom and clinical data. METHODS: A stenotic vessel phantom using contrast medium with diameters of 2, 3, 4 and 6 mm was imaged using an X-ray image intensifier. Quantitative coronary arteriography was performed to measure reference and stenosis diameters in the phantom and under clinical conditions to measure coronary artery diameters using cinefilm and digital images. RESULTS: Both cinefilm and digital images were distorted at the peripheral image area, so errors were caused when the reference and stenosis were not in the same area. Measurements of diameters in the horizontal direction were larger than in the vertical and diagonal directions (p < 0.05). Reference diameters measured on digital images were larger than those on analog images (p < 0.0001). The difference in the real and measured reference diameter (2 and 3 mm) was less than 1% on digital images. The mean error in measurement of stenotic diameter measured on cinefilm images was 0.4-6.3% smaller for 0.5-4.5 mm diameter, but on digital images was 13% larger for 1 mm diameter, 18% larger for 0.75 mm diameter and 28% larger for 0.5 mm diameter. Under clinical conditions, the reference vessel diameter measured at the stenosis and the percentage stenosis were well correlated between the analog and digital images. CONCLUSIONS: Reference diameters of stenotic vessels are almost the same on cinefilm and digital images, but stenotic diameters under 1 mm are larger than actual size on digital images and on cinefilm images.

The effect of TGF-beta1 on differential gene expression profiles in human corneal epithelium studied by cDNA expression array.

PURPOSE: TGF-betas regulate cell proliferation and differentiation, and they play important roles in maintenance of corneal epithelium. However, the precise function of TGF-betas in the corneal epithelium remains unclear. In this study, cDNA expression array technology was used to demonstrate the effect of TGF-beta1 on the simultaneous expression of a large number of genes in cultured human corneal epithelial cells (HCECs). The change in protein level expression of the specific genes influenced by TGF-beta1 was also investigated. METHODS: Human cDNA expression array technology was used to study the simultaneous expression of 1176 specific cellular genes in HCECs incubated with TGF-beta1 (10 ng/ml). Moreover, gene-specific semiquantitative reverse transcription-polymerase chain reaction (RT-PCR) was used to confirm the gene expression pattern measured by the cDNA expression array. Western blot analysis was used to examine protein expression of the specific genes in the presence or absence of TGF-beta1. RESULTS: TGF-beta1 significantly upregulated the expression of 19 genes and significantly downregulated ras-related protein, caspase10, and beta4-integrin in the treated HCECs. The expression of 277 genes including alpha3-integrin, PAI-2, transferrin receptor, and cyclin-D1 was studied. Semiquantitative RT-PCR analysis confirmed the TGF-beta1-mediated changes in expression patterns of these genes. Furthermore, Western blot analysis revealed that TGF-beta1 remarkably decreased PAI-2, transferrin receptor, and integrin alpha3, and increased caspase10 on the protein level. CONCLUSIONS: TGF-beta1 regulates the expression of specific types of genes in HCECs. These results strongly suggest that TGF-beta1 is critically involved in the maintenance of the corneal epithelium through the control of a network of various signal-transduction pathways.

[Long-term results of valve-retaining homograft and xenograft patch for transannular reconstruction of the right ventricular outflow tract in tetralogy of Fallot].

Thirteen patients with tetralogy of Fallot underwent reconstruction for hypoplastic right ventricular outflow tract using a preserved homograft or xenograft patch with single or double cusps. Homografts and xenografts preserved in 70% ethanol were implanted in 10 patients, and xenografts treated with glutaraldehyde were used in 3 patients. All the patients are doing well in their profession without one who died 18 years after operation. Valve-retaining homograft and xenograft patches provided excellent early hemodynamic function, but did not prevent pulmonary insufficiency late after the operation. Both homograft and xenograft cusps tended to degenerate several years after implantation, but cusp failure was not accompanied by outflow stenosis.

[A successful surgical case of incomplete atrioventricular septal defect with pulmonary dysfunction].

A 58-year-old female with incomplete atrioventricular septal defect was treated successfully with surgery. We repaired the defect by closure of atrioventricular septal defect, suture of cleft and tricuspid valve annuloplasty. We adopted method of minimally invasive cardiac surgery (MICS) expecting to preserve pulmonary function and to perform tracheostomy on early stage. After the operation she recovered with no major complications. It is suggested that MICS is effective method for patients with pulmonary dysfunction.

Molecular analysis of Salmonella Enteritidis isolates resistance to ampicillin and streptomycin from three outbreaks of food poisoning in Shiga prefecture.

In 1998 and 1999, there were three outbreaks caused by Salmonella Enteritidis in Shiga Prefecture. One outbreak was suspected to be a diffuse outbreak, caused by frozen cream puffs that had been sold in chain stores throughout Shiga Prefecture between the beginning of September and the beginning of October, 1998. The other outbreaks occurred in May and in August, 1999. All isolates of the three outbreaks showed an identical lysis pattern against the typing phage, though this pattern did not conform to the current scheme, so-called RDNC. In addition all isolates were resistant to ampicillin and streptomycin. However, the patterns of pulsed-field gel electrophoresis strongly indicated that the three outbreaks were actually independent.

Effects of CYP3A4 inhibition by diltiazem on pharmacokinetics and dynamics of diazepam in relation to CYP2C19 genotype status.

Diazepam is metabolized by CYP2C19 and CYP3A4 in the liver. CYP2C19 shows genetic polymorphism associated with the poor metabolizer (PM) and extensive metabolizer (EM) phenotypes. The aim of this study was to assess the effect of diltiazem, a CYP3A4 inhibitor, on pharmacokinetics and dynamics of diazepam in relation to CYP2C19 genotype status. Thirteen healthy volunteers (eight EMs and five PMs) were given placebo or diltiazem (200 mg) orally for 3 days before and for 7 days after the oral 2-mg dose of diazepam in a double-blind, randomized, crossover manner. The pharmacokinetics and pharmacodynamics of diazepam were assessed with and without diltiazem. Plasma concentrations and area under the plasma concentration-time curves (AUCs) of diazepam and N-desmethyldiazepam were significantly greater in the PM compared with the EM group during the placebo phase. Diltiazem significantly increased AUC and prolonged elimination t(1/2) of diazepam in both the PM and EM groups. These pharmacokinetic changes, however, caused no significant difference in the pharmacodynamics between the two trial phases. Diltiazem affects the pharmacokinetics of diazepam in the PM and EM groups of CYP2C19. Inhibition of CYP3A4 by a concomitant substrate drug like diltiazem may cause a pharmacokinetic interaction with diazepam irrespective of CYP2C19 genotype status, but whether this interaction would reflect a pharmacodynamic change of diazepam remains unconfirmed by our study.

[Splenectomy in 5 patients with idiopathic myelofibrosis].

Between 1992 and 1999, 5 patients with chronic idiopathic myelofibrosis (MF) underwent splenectomy at our hospital. The median age at the time of splenectomy was 63 years (range, 58-69 years), while the median interval from diagnosis of MF to splenectomy was 38.2 months (range, 3.7-87.2 months). Reasons for splenectomy included symptomatic splenomegaly in 5 patients, transfusion-dependent anemia in 5, and refractory thrombocytopenia in 3. Although most of the patients with transfusion-dependent anemia and thrombocytopenia showed some improvement at 1 month after splenectomy, the response was durable in only 2 patients at 6 months. Post-surgical thrombocytosis of 1020 x 10(9)/l was observed in one patient. Blast cell counts in peripheral blood increased after splenectomy in 4 patients. Leukemic transformation occurred in one patient 5 months after splenectomy. Four patients eventually died (2 of infection, 1 of acute myelocytic leukemia, and one of heart failure). Overall median survival was 54.7 months (range, 10.9-110.0 months) and 10.2 months (range, 6.0-33.6 + months) from diagnosis and time of splenectomy, respectively. We confirmed the palliative role of splenectomy in advanced-stage MF, but sufficient consideration should be given to late complications including blastic transformation.
