[Trichomoniasis. Physiopathology and therapy].

Based on many years' experience with trichomonas vaginalis the author comes to the conclusion that trichomonads are much more common than expected. Therefore the possibility of trichomoniasis shoudl be taken into account not only when discharge is found in females or persistent urethritis in males, but also in other affections. Remote reactions may occur in either sex, e.g. prostatis or adnexitis. Sterility is sometimes due to trichomonas vaginalis. Apart from classical pictures in either sex arthritis and erythema nodosum may develop. The unstained preparation is usually sufficient for demonstrating the organism. The clinical diagnosis in males and females is discussed. The new single dose therapy with tinidazole is of special interest for three reasons: low minimum trichomonacidal concentrations, 72-hour-action, high active excretion in the urine. No effect on the blood picture was observed. It appears that tinidazole (Simplotan) has a secondary effect on the pathological mixed flora. Candida, too, is often eliminated, but in some cases nystatin therapy is still necessary. The relations to diabetes mellitus, antibiotics, pregnancy and hormonal contraception are discussed.

[Recovery from apparently hopeless fresh-water drowning (author's transl)].

Report of 3 cases of drowning (children). 2 patients survived without severe injury, 1 patient died after 13 days. The protective action of hypothermia to the brain and heart in hypothermic drowning is emphasized. Even in apparently hopeless cases resuscitation should be started immediately and should not be discontinued too early. Especially in hypothermic drowning efforts can be successful after more than 60 min. of resuscitation. In cases of drowning an emergency physician should be called immediately (emergency-ambulance, rescue-helicopter). A short survey of pathologic physiology, emergency therapy and intensive care of drowning is given.

[Pharmacokinetics of sulfaclomide in man under therapeutically relevant conditions].

Pharmacokinetic parameters of the long-term sulfanilamide Sulfaclomid (4-[4-aminobenzensulphonamido]2,6-dimethyl-5-chlorpyrimidine) were determined in 25 female subjects. Both the elimination half-life of Sulfaclomid in the serum and the maximum acetylation rate in the urine are genetically determined. They show a polymodal behavior. The half-life is unrelated to the protein binding, which shows considerable individual differences in the therapeutic relevant range of about 200 microgram/ml serum. This confirms the human pharmacological postulate according to pharmacokinetical dosage schedules including genetic factors. An individual therapy service is necessary in certain drugs and in highly endangered groups of patients.

Calculation of specific energies of incorporated 239Pu and 131I in accordance with the concept of the critical cell.

A better understanding of the effects of energy deposited in cells by incorporated isotopes can be expected from an analysis of differential cell doses. With this thought in mind, the distributions of specific energies in tissue were calculated for 239Pu and 131I. A program written in Fortran IV makes use of a matrix of spherical cells and cell nuclei of 10 and 8 mum diameter, respectively. The cells are arranged in close-packed structure. The radioactivity is considered as being compiled to point sources of 1 dps activity. The sources are located in the common centers of cells and nuclei. The calculations yield discrete values of specific energy using the mass of the nuclei for mass of reference. The numbers of cell nuclei receiving given amounts of specific energy are functions of the specific activity of the isotopes in the tissue. The specific activity is varied by changing the number of sources per g of tissue. The program also allows to calculate the numbers of cell nuclei with zero energy deposition. From the 1 dps point sources an average specific energy of 831 rads/h results for plutonium for cell nuclei within the range of the 5.14 MeV alpha-particles. For iodine, the value is 35 mrads/h within the range of the beta-particles of 188 KeV mean energy. If the volumes irradiated by the sources begin to overlap these values begin to increase accordingly.

[Isolated rupture of the interventricular septum after blunt chest trauma (author's transl)].

The mechanism, symptoms and treatment of traumatic ventricular septal defect are discussed on the basis of an own observation. To the best of our knowledge this is the first report of a rupture of the membranous septum, involving the portion between the left ventricle and the right atrium. An attempt is made to explain the mechanism of isolated traumatic ventricular septal rupture on the basis of embryological considerations. The patient died on the 11th postoperative circulatory status and inspite rather prompt diagnosis and operative treatment.

[Distribution of endogenous inhibitors of the respiratory chain in plants].

70 phosphate buffer extracts of various plant tissues of 40 species as well as of 2 bacteria were tested for the presence of endogenous inhibitors of the respiratory chain. Electron transfer particles (ETP) from beef heart mitochondria served as test object. The NADH oxidase (spectrophotometrically) and the succinate oxidase activity (manometrically) were measured. Inhibitory activities could be detected in all the plant species tested, but there were quantitative differences by orders of magnitude. The inhibitory effects were more frequent and higher in the NADH oxidase system than those in the succinate oxidase system. The highest inhibitory activities were observed with blossoms of Forsynthia intermedia, male blossoms of Corylus avellana, inflorescences of Brassica oleracea, fronds of Pteridium aquilinum and gallnuts of Quercus. The specific inhibitory activities (related to the dry mass of the extracts) suggest very efficient inhibitors having concentrations of half-inhibition in the muM-range. With 6 extracts the inhibitory activity on the NADH oxidase system was completely destroyed by boiling (Brassica oleracea, Amoracia rusticana, leaves of Digitalis purpurea, roots of Allium cepa, fruit pips of Malus domestica and mushrooms of Lactarius vellereus). The results with some plant species (Bryophyllum daigremonteanum, Allium cepa, male blossoms of Corylus avellana, Pteridium aquilinum) suggest a biological dynamics of the inhibitory activity. The inhibitor from Bryophyllum was partially characterized with regard to its mode of action. The following supposed biological functions of endogenous respiratory inhibitors of plants are discussed: 1. Involvement in the degradation of mitochondria in the course of differentiation, maturation and involution processes as well as in biologically controlled senescence processes; 2. A switch-over to the alternative mitochondrial respiratory pathway; 3. Induction and maintenance of a resting metabolism, e.g. in dormancy, by action as growth inhibitors; 4. Action as phytoncides (phytoallexines) for the defense against parasites.
