Reconstructive pelvic operations for in vitro fertilization.

Lysis of adhesions, bilateral salpingectomy, and ovarian suspension were carried out in 54 normal ovulatory patients with long-standing infertility that was associated with severe pelvic adhesions after multiple laparotomies for reimplantation of the fallopian tubes, salpingostomy, lysis of adhesions, or severe endometriosis. Ovulation was induced in 39 patients after laparotomy for in vitro fertilization, with the use of human menopausal gonadotropin, pure follicle-stimulating hormone, and human chorionic gonadotropin. Oocyte retrieval by laparoscopy was accomplished in 37 patients, and embryo transfer was carried out in 36. Pregnancy after in vitro fertilization and embryo transfer occurred in 14 patients. Although severe adhesions recurred in four patients, a significant improvement was obtained after the procedure in the others.

Analysis of human spermatozoa before and after processing for in vitro fertilization.

Washed incubated spermatozoa processed for in vitro fertilization were compared to the spermatozoa from the same unprocessed ejaculate in 31 randomly selected normal males (Group 1) and 7 subfertile males (Group 2). In Group 1 there was a significant increase in motility (P = 0.001) as well as a significant increase in normal morphology (P = 0.001). All categories of morphologically abnormal sperm decreased, with a significant decrease in those exhibiting small heads (P = 0.01), tapered heads (P = 0.01), cytoplasmic droplets (P = 0.001), bent midpieces (P = 0.01), or coiled tails (P = 0.02). There was also a significant decrease in immature germ cells (P = 0.001) as well as white blood cells (P = 0.02). Group 2 also showed a significant increase in motility (P = 0.01) and a significant increase in normal morphology (P = 0.01). There was a decrease in various categories of abnormal sperm, with a significant decrease in sperm with cytoplasmic droplets (P = 0.02).

The role of in vitro fertilization in infertile patients with endometriosis.

Thirty-nine cycles were studied in patients with a history of endometriosis who went through in vitro fertilization. In 15 cycles, there was no evidence of endometriosis; in 10 cycles, the patients had mild or moderate disease; in 14 cycles, severe or extensive endometriosis was found. The pregnancy rates per cycle were 33%, 60%, and 7%, respectively (groups I and II, no significant difference; groups II and III, P less than 0.01). The difference was due to the different number of oocytes aspirated at laparoscopy because of technical problems in the cases with severe and extensive disease. There was also a significant difference in the number of pregnancies per transferred cycles. There was no difference in the luteal phase in the three groups. The reproductive potential, which seemed to be similar in groups I and II, was severely impaired in the group with severe endometriosis.

Correlation of follicular fluid volume with oocyte morphology from follicles stimulated by human menopausal gonadotropin.

The maturity of human oocytes was correlated with corresponding follicular fluid volumes in 547 cycles stimulated with human menopausal gonadotropin, with or without the supplementation of follicle-stimulating hormone. Mature oocytes were found to be associated with larger follicles (average volume, 2.7 ml). Immature or degenerating oocytes were found to be associated with smaller follicles (average volume, 1.0 and 0.8 ml, respectively). Follicles without oocytes were generally quite small (average volume, 1.0 ml). We studied follicular fluid volumes associated with mature oocytes that were responsible for the establishment of pregnancy after single conceptus transfer. Analysis of these data demonstrated that the rate of spontaneous abortion was very high with conceptuses derived from smaller follicles, suggesting that oocytes from larger follicles may be of better quality.

Male factor evaluation in in vitro fertilization: Norfolk experience.

Thirty-three patients from the in vitro fertilization (IVF) program at Norfolk are critically reviewed. A battery of tests was designed and an endocrine investigation was carried out on these patients. The fertilization rate for preovulatory oocytes was lower than in the normal male population (39.6% versus 88.6%). When total concentration of sperm with rapidly progressive motility was less than 6 X 10(5), to fertilize several eggs together the fertilization rate was zero. No fertilization was obtained when the number of sperm with rapidly progressive motility recovered after the separation was less than 1.5 X 10(6). The hamster zona-free oocyte penetration test correlated well with the human IVF system. The other parameters investigated did not show good correlation. When fertilization was achieved, the results of the IVF procedure in the series reviewed rendered a 30.8% pregnancy rate per transfer in 26 transfers. Fifty percent of the pregnancies were normal (either ongoing or delivered). Thirty-seven percent were preclinical miscarriages, and 12.5% were clinical abortions. In the abnormal male population, higher concentrations of sperm per egg should be used for insemination for achievement of optimum fertilization rates. Once fertilization is obtained, the results do not differ substantially from the IVF population at large.

Renal pelvi-ureteric injury in traumatic paraplegia.

Three cases of pelvi-ureteric injury associated with traumatic paraplegia are described. The diagnosis of such injuries is usually late and the consequences can seriously affect the outlook of the patient. The problems of late diagnosis are discussed and recommendations made concerning earlier diagnosis.

Studies of parallel barrier performance by acoustical modeling.

An investigation is presented into the performance of parallel barrier configurations, using acoustical scale modeling. A realistic geometry is investigated, with the source being positioned over a paved roadway and the receiver over grass-covered ground. The grass-covered ground surface was properly modeled in terms of its impedance. Results were obtained for a range of barrier types, and demonstrate that frequency dependent effects are evident in barrier insertion loss data. In most cases, the barrier on the far side of the source did not significantly affect sound levels at the receiver. The most effective barrier design was found to be that of a gradual grass-covered slope up to an upright, thin barrier.

Eosinophilic cystitis: pleomorphic manifestations.

Because eosinophilic cystitis is rare, the authors present two cases of this inflammatory condition. In one, eosinophilic ureteritis presented as ureteral obstruction; this is only the second such case to be reported. The literature on eosinophilic cystitis is reviewed. The condition affects people of all ages and its cause is unknown. The clinical presentation varies, but hematuria is a constant feature with urinary frequency and dysuria being common. Management is also variable, ranging from treatment for symptoms only to aggressive surgical therapy.

Abdominopelvic computed tomography: evaluation in patients undergoing second-look laparotomy for ovarian carcinoma.

Preoperative abdominopelvic computed tomography results and operative findings were compared in 52 patients undergoing second-look laparotomy to confirm tumor status. Seventeen true-negative, 22 false-negative, and 13 true-positive scans were found. The sensitivity was 0.38, specificity was 1.0, and diagnostic accuracy was 0.58. Negative studies were associated with positive findings at laparotomy in 42% of all cases. Fourteen patients were identified who had computed tomography that would have enabled an attempt at the diagnosis of persistent cancer by computed tomography-directed needle aspiration or biopsy, thus avoiding laparotomy. Assuming 80% accuracy of needle aspiration, the cost of computed tomography in all 52 patients is considerably outweighed by the savings that could have been realized by eliminating the need for second-look surgery in these 11 women.

The 'milk-alkali' syndrome: two case reports with discussion of pathogenesis.

The milk-alkali syndrome is the association of hypercalcaemia and renal failure, with or without alkalosis, in the presence of absorption of excessive quantities of calcium, alkali, or both. Two patients with the milk-alkali syndrome are described, one representing an acute, reversible disorder and the other demonstrating a chronic syndrome with only partially reversible renal disease. Differential diagnosis is not difficult and is usually aided by the initial clinical evaluation as well as rapid response to conservative therapy. Because the initial stages of renal insufficiency are often fully reversible, the early identification and treatment of the milk-alkali syndrome can prevent progression to irreversible, chronic renal failure. Although non-absorbable antacids, H2 blockers, and sucralphate are the basis of modern treatment of peptic ulcer disease, the syndrome may still occur, especially in patients who self-treat symptoms of dyspepsia.

Problems experienced by residents in internal medicine training.

A review of the literature and the experience of a residency program in internal medicine indicate that house officers have special problems during training. Some are shared by all residents, whereas others are unique to certain groups. These problems are caused by historical and cultural factors that have led to the current structure of many residency programs and often interfere with the parallel development of professional, personal and family growth. Program directors and chiefs of service need to be flexible and humane and should negotiate clear expectations with house staff to allow efficient functioning of the residency program and insightful personal growth.

Pregnancy related to infertility diagnosis, number of attempts, and age in a program of in vitro fertilization.

Three hundred nineteen infertility patients who did not become pregnant with standard therapies presented to Norfolk for in vitro fertilization between January 1981 and December 1983. There were 560 laparoscopic cycles with 105 pregnancies. The pregnancy rate was 18.8% by cycle, 24.5% by transfer, and 33% by patient. The overall pregnancy rate was found to be independent of infertility diagnosis, age, and number of attempts.

In vitro fertilization in Norfolk, Virginia, 1980-1983.

Three years of progress of the Vital Initiation of Pregnancy (VIP) Program in Norfolk is reported. No conception resulted from 41 oocyte aspirations during spontaneous menstrual cycles in 1980. An average of 3.7 oocytes per cycle, or a 73.5% recovery rate, resulted in 362 human menopausal gonadotropin/human chorionic gonadotropin-induced cycles from January 1981 to March 1983. Forty percent of the oocytes recovered from these cycles were preovulatory, 35% atretic, and 25% immature. Immature oocytes were often matured in vitro, fertilized, and found to produce pregnancies. A total of 62 pregnancies occurred, which represents a 17 or 23% pregnancy rate, based on laparoscopies or embryo transfers, respectively. There were 11 preclinical and 7 clinical miscarriages. Twenty-nine normal babies have been delivered, including a set of twins. The remainder appears to be normally progressing pregnancies. Polyspermia was observed in 8.8% of the fertilizable oocytes.
