[Fibrous dysplasia of the skull--a radiological, computer tomographic and scintigraphic study (author's transl)].

The diagnosis of fibrous dysplasia (Jaffé-Lichtenstein) of the skull can generally be made by plain films, occasionally supplemented by tomography. The differential diagnosis from sclerosing meningiomas in the anterior fossa can usually be made by cranial computer tomography. Cerebral scintigraphy does not play an important part in the differentiation from a meningioma. Total body scintigraphy, on the other hand, is useful, particularly in distinguishing between monostotic and polyostotic bone conditions. Invasive neurological methods, such as cerebral angiography, are in our opinion unnecessary unless there are definite clinical focal cerebral symptoms in the presence of a normal computer tomogram.

Diagnosis, Differential↗

[Thoracic myelography with water-soluble contrast medium (metrizamide) (author's transl)].

The use of water-soluble contrast media for the thoracic myelography has become possible with the introduction of Metrizamide (Amipaque). A modification of the technique used with oily contrast media results in excellent demonstration of detail of intraspinal structures and of pathological changes. The medium is well tolerated and does not cause irritation of the spinal cord; the results must be regarded as excellent.

[Cerebral and spinal angiography with metrizamide (amipaque) (author's transl)].

Metrizamide (Amipaque) has been used as the contrast medium for angiography in neuroradiological investigations. Solutions containing 280 mgI/ml. are well tolerated and produce good contrast. The most important difference from previously used contrast media is the lack of pain when injections are made selectively or super-selectively into the external carotid artery and its branches, or into the spinal arteries.

[Xeroradiography for cervical myelography using metrizamide (amipaque) (author's transl)].

Xeroradiography, particularly in addition to conventional tomography, for cervical myelography using water-soluble Metrizamide has been tested. Xerography was done in 30 of a total of 96 examinations and could be compared with conventional X rays. In the sagittal plane there was no advantage, in the lateral plane detail was clearer, especially at the cervico-dorsal junction and in establishing the extent of osteochondrotic changes narrowing the subarachnoid space. Xeroradiography should not be used routinely, but has its merits for certain problems, when higher radiation will be justified.

Neuroradiological diagnosis of intraspinal tumors in children.

In the diagnosis of spinal and intraspinal space-occupying processes great importance attaches to radiologic plain film diagnosis, including special projections and tomography. Using refined evaluation methods, direct and indirect signs of tumors can be demonstrated in 30-50% of cases. Of decisive importance is the myelographic examination. It permits the determination of the exact localization and the size of a tumor. Contrast studies frequently allow an identification of the tumor type. Spinal arteriography and phlebography, on the other hand, are of secondary importance in the diagnosis of tumors of both spinal canal and spinal cord. In patients with bone infiltration they accomplish a valuable contribution to determine whether this expansion is directed towards the epidural space or the soft tissue structures of the back.

[Cervical myelography with water-soluble contrast medium (Metrizamide). Technique and preliminary results (author's transl)].

Use of a water-soluble contrast medium for cervical spinal diagnosis has become possible with the introduction of Metrizamide (Amipaqque). Injection of contrast at the C.1/2 level by a lateral approach results in excellent visualisation of the cervical theca with previously unobtainable detail. This technique, which is so far not well known, allows a rapid examination and when used with the new, well tolerated contrast media provides optimal diagnostic results.

Cervical Vertebrae↗

[Experience with a new hydro-soluble contrast medium (author's transl)].

Report about a very well tolerated new contrast medium Myelografin. The side-effects are being decoded and compared with the side-effects of other contrast-media for lumbar myelography. Spinal or cerebral irritating effects have never been recorded. Due to the low neurotoxicity the patient can be put in supine position immediately after the examination. Therefore the side-effects are diminished and the examination is more tolerable for the patient.

Chemical Phenomena↗

[Experience with e new water soluble contrast medium for lumbar myelography (author's transl)].

Meglumin-Iodrinat has been synthesized as the contrasting part of Myelografin by Schering AG. and tested for lumbar Myelography in animal experiments. The excellent neural and general compatibility justified a first clinical trial. 200 patients were examined, 100 each with following upright or lying down positions. There were no local symptoms, no signs of spinal or cerebral irritation. This is seen to be the main advantage compared with drugs used so far. Intensity of contrast corresponded with that of Conray 60 and Dimer X. The most common complaints like headache and nausea were interpreted as CSF hypotension and can be clearly reduced by lying down. Also the concentration of the contrastmedium diminishes more rapidly with the patient lying down.

Experience with Dimer-X 1-ventriculography.

Positive contrast ventriculography was carried out in 72 cases, using the water-soluble contrast medium Dimer-X. The examinations were performed 1--3 days after ventricular drainage had been carried out to relieve raised intracranila pressure. After injection of 2--7.5 ml. of the medium into the non-anaesthetised patients -- including 37 children and juveniles -- the ventriculographs were taken under visual control using Mimer III. This method is simple and can be rapidly carried out. Radiographs taken in two planes, supplement by tomography if required, are sufficient for an accurate diagnosis. During the examination only two children suffered from vomiting, while another patient sustained a generalised convulsion because the contrast agent came into contact with the surface of the brain.

[Angiographic findings in juvenile angiofibroma of the nasopharynx (author's transl)].

Juvenile angiofibroma of the nasopharynx has a specific tendency to grow through anatomical fissures and foramina with invasion of the surroundings. Angiography by selective techniques, detail angiography and the use of subtraction permits accurate delineation of the tumour and of its vascular supply. The angiographic appearances are highly characteristic and equal in value to a histological diagnosis. Consequently a possibly difficult biopsy can be avoided and radical surgery carried out. As a result, the recurrence rate can be reduced. Post-operative control and the demonstration of recurrences is also possible with angiography.

[Angiography in the diagnosis of nasopharyngeal tumours (author's transl)].

The angiography of tumours of the nasopharynx is an important contribution to preoperative diagnostis. Applying modern, improved techniques (superselective catheterisation, magnification angiography, subtraction methods) it is possible to have an idea of the size and the localization of the tumour as well as of afferent and efferent vessels. Often a differential diagnosis is achieved; in some tumours (angiofibromas, haemangiomas, cysts, vascular malignant tumours) the angiographic findings are pathognomonic.

[Angiographic diagnosis of hemangiomas in the orbital region (author's transl)].

The polymorphus hemangiomas-especially those of the cavernous type in the orbital region, the face, the scalp or the rhinopharynx are not demonstrable neither by arterial nor venous angiography. If direct puncture of the angioma is possible, the injection of radioopaque medium shows a characteristic picture. The smooth limitation, partial septation and the delayed outflow of contrast medium are important criterions for the differential diagnosis. Moreover the delimitation of the hemangioma from the surrounding tissue is possible and the best surgical access can be selected. If the expanding process is situated in the orbit, and has reached a certain size, the venography of the orbit indicates the extent and localization of the orbital mass.
