[Arterial anastomosis in the angiogram following kidney transplantation].

In 26 patients who had undergone renal transplantation, renal arteriograms were obtained to analyze morphologic vascular changes in the region of the anastomosis. The donor kidney had been implanted in the right side of the pelvis in eight and the left side in 18 patients. The renal artery was anastomosed to the external iliac artery in 15 patients and to the internal iliac artery in twelve. Fourteen of 26 patients (54%) were found to have contour irregularities in the region of the anastomosis, either in the external iliac artery (or internal iliac artery, respectively), in the renal artery or both. Well-delineated membrane-like stenoses were seen in seven patients, long segmental stenoses, frequently with poststenotic dilation, in four and the anastomosis was completely occluded in three patients. Arteriography or digital subtraction angiography, respectively, enabled detection of vascular changes in the region of the anastomosis after renal transplantation, thus, rendering guidelines for subsequent interventions to maintain graft viability such as revascularization or percutaneous transluminal angioplasty.

[Roentgenologic observation of the course following sternotomy].

The course of healing after medial sternotomy can be controlled by means of tomography of the sternum. If healing progresses in a normal way, restitution can be seen in the region of the corpus sterni after about 20 days, whereas restitution in the area of the manubrium sterni is evident after 8 weeks only. Osseous absorption is a leading x-ray sign of disturbed restitution, the other leading sign being persistence of dehiscence of the sternum following sternotomy.

[Computer tomogram of changes in pancreatic calcification during pancreatitis].

In 3 patients an increase of pancreatic calcification was observed in the course of 2 months. The increase of the calcification was combined with an improvement of the patient and a decrease of pseudocysts. Therefore the increase of pancreatic calcification was considered as a good sign. After surgical drainage of a pseudocyst one patient showed a decrease of pancreatic calcification in the course of 1 year. In this patient the clinical picture indicated a favourable development. This observation demonstrates that a recovering pancreas may show diminishing calcifications.

[Radiation exposure and radiation risk in angiocardiography in adults].

In 100 patients, the X-ray exposure during routine angiocardiography was measured from which calculations were performed for the integral dose (the energy transferred to the body by the exposure), the mean body dose and the radiation risk (according to ICRP 26, 1977). Fluoroscopy contributed 21% and angiocardiography 79% of the total exposure. The mean body dose was 0.9 rd. The average of the area-dose product was 10,200 Rcm2 with a skin exposure in the central beam of 36.1 rd. The genetic risk (probability of X-ray-induced mutation in progeny) is 1 : 75,000 after average exposures and 1 : 7,700 at maximal doses. The somatic risk (probability of X-ray-induced lethal disease) in the patients studied is 1 : 11,000 after average exposures and approximately 1 : 4,400 at maximal doses. The genetic and somatic risks, thus, are small as compared with those of cardiac catheterization and contrast angiocardiography.

[Pancreatic calcinosis as shown on the computer tomogram and its general image].

The abdominal roentgenograms of 20 patients with pancreatic calcifications, demonstrated by CT, were evaluated retrospectively. The calcifications of 15 patients could be seen. In 2 patients with ERCP and in 1 patient with pneumocolon only, the calcifications could not be observed. In the remaining 2 patients with negative abdominal roentgenograms the calcifications were prevertebral and of small size. Possibly they could have been found with special projections.

[Roentgen findings in gastroenterostomy].

Gastroenterostomy is a palliative operation for drainage. It can be represented by retrocolic and antecolic, isoperistaltic and anisoperistaltic, laterolateral and Y-Roux anastomosis. Impressions by the colic ligament are normal findings. pathological findings are anastomosis - obstruction, high position, ulcer, blind loops, gastroileostomy, volvulus of the anastomosed loop, herniation, invagination, and changes in the underlying disease.

The orientation of melittin in lipid membranes. A polarized infrared spectroscopy study.

Polarized infrared spectra of melittin incorporated into macroscopically oriented lipid membranes are reported. From the linear dichroism of the amide I and amide II vibrational bands, the spatial orientation of the melittin helices was determined as being preferentially parallel to the membrane normal, under our experimental condition of low water content and an ordered lipid phase. Considering the various models for the orientation of melittin in lipid membranes proposed in the literature, we conclude that our data are in accord with an arrangement whereby the hydrophobic part of the polypeptide either spans the bilayer in the form of two bent helix segments, or is folded back within one monolayer in the form of a wedge.

[Preoperative localization diagnosis of parathyroid adenomas in 72 cases of primary hyperparathyroidism by selective catheterization of neck veins and parathyroid hormone determination].

In 72 cases of primary hyperparathyroidism, selective venous catheterization was performed preoperatively to localize parathyroid adenomas. Diagnosis of primary hyperparathyroidism was proved by operation in all but 5 cases, in which no adenomas could be found intraoperatively. By venous catheterization of the neck and upper region of the thorax, an average of 10 blood samples per patient were taken. Parathyroid hormone was estimated by radioimmunoassay with high sensitivity against the intact PTH-molecule and carboxy-terminal fragments. Only in 24 out of 72 cases was localization of parathyroid adenomas prognosticated correctly with respect to side and height, while in a total of 38 cases localization on the right or left side only could be determined preoperatively. These unsatisfactory results can be improved only by a much more extensive catheterization technic which would be justified only in patients already operated on before without success. The final evaluation of non-invasive methods, e.g. ultrasonics and computerized tomography, is still under discussion.

[Cavernosography].

Cavernosography can be useful in the investigation and follow-up of induratio penis plastica, tumours, trauma, priapism, impotence and in planning surgery. The examination is easily carried out. In the small series so far undertaken, no complications have been reported.

[Computer tomographic findings in destructive sternal lesions].

Computer tomographic findings in 25 patients with destructive lesions of the sternum have been analysed. In 14 cases there was, in addition to bone destruction, extensive or ill-defined soft tissue infiltration. With two exceptions. CT demonstrated the bone changes more clearly than conventional tomography. The value of CT lies in its ability to demonstrate the exact extent of the bone destruction with simultaneous demonstration, or exclusion, of soft tissue infiltration. This is of value when resection or radiotherapy is planned.

[Perforations during contrast enema].

During contrast enema, perforation into the retroperitoneal space can be differentiated from perforation into the peritoneum and perforation into the intestinal wall associated with formation of barium granulomas or submucosal spreading of the contrast medium. Other special forms are perforation with contrast medium embolism of diverticula; of the processus vermiformis; penetration of contrast medium into fistulous systems and from the operated area. Risk factors are: balloon catheter, intestinal tubes with a hard tip, preternatural anus, excessive enema pressure, contrast medium additions, preceding manipulations, intestinal diseases, advanced age and delegation of manipulations to assistants and unskilled staff. Children are particularly at risk.

Total and ionized calcium levels during open heart surgery with two different pump priming solutions.

UNLABELLED: Changes of total (Ca) and ionized Ca-levels (Ca2+) were studied during open heart surgery in 18 patients. 9 patients (group A) had only crystalloid solutions as pump priming, the other 9 patients (group B) had 500 ml of citrated whole blood in the priming solution. For assessment of hemodilution hct and total plasma protein and plasma albumin were determined. Measurements were done preoperatively, before and after systemic heparinisation, during and after extracorporeal circulation (ECC) and on the first postoperative day. RESULTS: 1. Anticoagulation with 375 U/kg BW heparin had no effect on ionized calcium levels. 2. With onset of ECC (Ca) decreased by 30% in group A and by 20% in group B. (Ca2+), however, decreased by 20% in group A and by 35% in group B. The percentage of ionized calcium increased by 15% in group A whereas it decreased by the same amount in group B. This difference is contributed to the calcium binding effect of citrate added to pump priming in group B. Mechanisms of the relative increase of ionized calcium during ECC in group A, however, remain to be studied.
