Complicated retinal migraine.

Retinal migraine is not uncommon, but permanent sequelae are rare. We describe the case of a 23-year-old woman who had suffered from retinal migraine for five years. After a typical migraine attack a left nasal quadranopsia persisted. Fluorescein angiography showed a branch retinal arterial occlusion. Other causes were ruled out by appropriate laboratory techniques. Her visual field defect has persisted for eleven months.

Acute febrile neutrophilic dermatosis (Sweet's syndrome) in neuro-Behçet's disease.

A case of Behçet's disease associated with Sweet's syndrome is presented. The onset was neurological symptoms and oral aphthae. Subsequently, genital ulcers developed. Coexistence of Sweet's syndrome and Behçet's disease has been reported only in few patients. On reviewing the literature, we propose that Sweet's syndrome is one of the associated diseases of Behçet's disease.

Persistent primitive proatlantal intersegmental artery (proatlantal artery I) results in 'top of the basilar' syndrome.

BACKGROUND: Persistent primitive proatlantal intersegmental artery (proatlantal artery I) is an anastomotic channel between the carotid and vertebrobasilar arterial systems. Persistence of this embryonic anastomosis is very rare. The "top of the basilar" syndrome is usually due to occlusion of thalamoperforating arteries by emboli that rise from the vertebrobasilar arterial system. In this case, however, the emboli stemmed from atherosclerotic plaques in the carotid system. CASE DESCRIPTION: A 55-year-old woman was hospitalized with top of the basilar syndrome. A brain scan showed hypodense lesions extending from the mesencephalon to both thalami. Digital subtraction angiography of the cerebral circulation was performed, which revealed a proatlantal artery I on the left side. In addition, atheromatous plaques were seen along the internal and common carotid arteries on the same side. CONCLUSIONS: The occurrence of proatlantal artery I is extremely rare. Top of the basilar syndrome is also rarely encountered. To our knowledge, this is the first report of a patient with both conditions.

Salmonella encephalopathy with seizure and frontal intermittent rhythmic delta activity.

A 54-year-old woman was admitted to our clinic with frequent convulsions. Subsequently she developed a toxic look, rigidity, confusion and severe encephalopathy. Salmonella typhii was isolated from the blood cultures and she was diagnosed with Salmonella encephalopathy. The prominent electrophysiological finding was the frontal intermittent rhythmic delta activity (FIRDA) in electroencephalograph (EEG). After treatment EEG revealed normal activity. The presence of FIRDA is not a specific finding but this kind of EEG change has not been reported before. Diffuse encephalopathy due to endotoxins might be the reason. We propose that S. typhii encephalopathy was the cause of FIRDA which is a sign of diffuse encephalopathy.

Fatal hydrothorax due to misplacement of a nasoenteric feeding tube.

Blind nasoenteric intubation was attempted in a patient with chronic parkinsonism. The tube was inadvertently misplaced and penetrated the left pleural cavity. The next day, the patient developed cardiopulmonary arrest during dietary supplement infusion. This complication ultimately led to the patient's death. We have reviewed the known complications of nasoenteric tube placement and conclude that difficult insertion in patients at risk from tube misplacement should be followed by chest radiography to confirm the correct placement of the tube before nutritional support is started.

Efficacy of the intramobile connector in implant tooth-supported fixed prostheses: an experimental stress analysis.

An experimental model was designed to evaluate the effects of the intramobile connector in reducing stress concentration in an IMZ implant in fixed connection with a natural tooth abutment. The study analyzed the stresses produced at the cervical level of the implant when the superstructure arm was deflected within the limits of physiologic mobility under static axial loads. The designed experimental model was sensitive to changes in strain data with the applied loads and different internal elements. The intramobile connector resilient component reduced strain up to 60% compared to the rigid internal element.

An experimental analysis of the stresses on the implant in an implant-tooth-supported prosthesis: a technical note.

When a natural tooth connected to an implant abutment by a framework is displaced within physiologic limits under functional loads, the superstructure acts as a cantilever moment arm and the implant is loaded additionally during that moment effect. The effects of a deflecting support connected to an IMZ implant, with either the IMC resilient or the titanium rigid element, were evaluated through an experimental model. The strain values measured when the moment arm was not allowed to deflect were only 30% lower than the strains measured when it was deflected within the limits of physiologic tooth mobility. The IMC was effective and achieved a reduction in the cervical strains up to 60% when compared with the rigid connector.
