Human psoriatic skin lesions improve with local hyperthermia: an ultrastructural study.

Electron microscopy of psoriatic skin prior to and after local hyperthermia revealed both temporary and gradual changes following treatment. The former occurred almost simultaneously with initiation of hyperthermia, and were characterized by an intracellular edema, the aggregation of tonofibrils in the basal half of the epidermis and an amorphous substance in the epidermal interspaces. The latter occurred during the course of treatment and were represented by narrowing of the intercellular spaces, a decrease in the number of mitochondria and free ribosomes, and an increased number of desmosomes and tonofibrils, with a consequent increase of keratohyaline granules. No such abnormal cells and structures were found in this study, as were observed in PUVA therapy.

Past and current dermatological status of yusho patients.

Skin symptoms of "yusho" and their change thereafter are described. In the early stage, skin eruptions were the most prominent and impressive features of yusho. Acneiform eruptions and pigmentation were most conspicuous. With the passage of time, there is some improvement and most patients now have few eruptions, except for the severely intoxicated group. As for blood PCB analysis, the severely intoxicated group who showed intensive skin symptoms showed "pattern A"--a typical specific yusho pattern. The blood PCB patterns of each patient were stable with no alteration after many years and, on the other hand, the blood PCB concentrations have diminished year by year.

Purification and characterization of a new peptide antigen extracted from dermatophyte mycelia.

A highly purified peptide antigen was produced from the cultured mycelia of Trichophyton mentagrophytes. Mycelia were extracted with 0.1 N-HCl at 37 degrees C repeatedly. The protein fraction was precipitated with picric acid and the final product was tentatively designated as Trichophyton mentagrophytes peptide (TMP). Total yield of TMP was about 3% of the original mycelia. When skin tests were done on sensitized guinea pigs, TMP showed a delayed-type reaction. The minimal dose which gave a positive reaction was 3 micrograms (0.1 ml of 30 micrograms/ml solution). Chemical analyses showed that TMP was almost exclusively of a peptide nature and digestion with bacterial peptidase (pronase) completely destroyed its skin test activity, thereby suggesting that protein fraction carried the entire antigenic activity. With chemical and/or physical processings, TMP showed firm stability; the skin test activity was unchanged after heating to 120 degrees C or after processing with HCl or NaOH. Fractionation of TMP by gel filtration demonstrated a considerable heterogeneity in its molecular size, while the antigenic activity was virtually the same in all the fractions. Disc electrophoresis also showed a wide-spread heterogeneous pattern. In the MIF test using peritoneal exudate cells from sensitized guinea pigs, 100 micrograms/ml of TMP had definite inhibitory effect on cell migration.

Detoxification of ammonia by immobilized urea cycle enzymes.

Detoxication of ammonia was performed by dialysis of ammonia through a hollow fiber unit and by conversion to urea in a column reactor packed with immobilized urea cycle enzymes. Enzymes in the urea cycle could be immobilized by covalently binding on CNBr-activated Sepharose 4B. Ammonia was sequentially converted to urea by immobilized urea cycle enzymes with an ATP-regenerating system. Optimum conditions for the cyclic reaction were obtained theoretically and experimentally for performing continuous urea synthesis in a packed column reactor. Detoxication of ammonia of similar concentration to that in the blood in hepatic insufficiency was analyzed and performed experimentally in an extracorporeal model.

Hyperthermia in the treatment of psoriasis.

The effectiveness of commercially available, chemically generated, topical exothermic pads that elevate skin surface temperature from 42 to 43 degrees C was stressed in 22 patients with psoriasis. Control sites were treated with conventional modalities such as Goeckerman's regimen, as well as with occlusion with nonexothermic pads. Skin lesions in 19 patients disappeared after the use of hyperthermia. The average time required for complete regression in the treated areas was 27 days with hyperthermia, compared with 44 days with Goeckerman's regimen. There were no hyperthermic side effects. Seventeen patients whose skin lesions disappeared with the use of both hyperthermia and Goeckerman's regimen were subsequently reexamined. The hyperthermia produced an equal or longer duration of remission than did Goeckerman's regimen.

Simplified determination of lorazepam and oxazepam in biological fluids by gas chromatography-mass spectrometry.

Lorazepam and oxazepam in plasma and urine were measured by gas chromatography-mass spectrometry. Oxazepam was used as an internal standard in the assay of lorazepam and vice versa. After removal of interfering substances with n-hexane, the drugs were extracted with benzene and converted to N1,O3-bistrimethylsilyl derivatives. Glucuronide forms of the drugs were extracted after hydrolysis with beta-glucuronidase. A common fragment ion at m/e 429 was used to monitor the two drugs. The sensitivity was 2 ng/ml for both drugs, which was sufficient to determine plasma and urine concentrations after therapeutic doses to humans.
