Defective expression of CD23 and autocrine growth-stimulation in Epstein-Barr virus (EBV)-transformed B cells from patients with Wiskott-Aldrich syndrome (WAS).

WAS is an X-linked, recessive, immune deficiency syndrome, characteristically associated with lymphocyte and platelet dysfunction. Peripheral B lymphocytes from WAS patients are nonresponsive to polysaccharide antigens and show reduced numbers of cells expressing the integral membrane glycoprotein, CD23. The release of CD23 proteolytic fragments, so-called soluble CD23 (sCD23), by B lymphoblasts and EBV-transformed B cell lines has previously been described, and these fragments have been shown to stimulate autocrine growth of these cells. We have found that the surface expression of CD23 is reduced on WAS compared with control EBV-B cells. Surface CD23 levels were reduced two-fold in four WAS cell lines (group I) and nine-fold in four other lines (group II). Group II WAS cell lines also showed reduced growth rates in serum-free medium when compared with group I cell lines and EBV-B cell lines from eight normal subjects. In contrast to the group II WAS lines, group I and EBV-B cells from normal individuals produced an autocrine-growth factor activity which could be absorbed by anti-CD23 antibodies. Immunoprecipitation of sCD23 from culture supernatants confirmed that group I WAS cell lines produced less sCD23, particularly the 37K fragment which was prevalent in control EBV-B cells. Northern analysis showed that CD23 mRNA levels were increased three-fold in group I and unchanged in group II WAS compared with normal EBV-B cell lines, suggesting that decreased surface expression in WAS EBV-B cells reflects post-transcriptional events. Together these results suggest that reduced cell surface expression and aberrant proteolysis of CD23 occurs in WAS patients' B lymphocytes and may contribute to impaired immune function in these patients.

Evidence for defective transmembrane signaling in B cells from patients with Wiskott-Aldrich syndrome.

B lymphocytes from patients expressing the X chromosome-linked immune deficiency disorder, Wiskott-Aldrich syndrome (WAS), fail to produce antibodies in response to stimulation with polysaccharides and other type-2 T cell-independent antigens. To investigate whether this abnormality reflects a defect in the signal transduction cascade normally triggered by ligation of surface immunoglobulin (sIg) on B cells, we have examined early signaling events induced by anti-Ig antibody stimulation of EBV B lymphoblastoid cell lines from WAS patients and healthy controls. Despite the expression of comparable levels of sIg and sIgM on WAS and control EBV B cells, WAS cells failed to manifest the increased proliferation in response to anti-Ig treatment observed in the control cell lines. WAS and control EBV B cells also differed in the magnitude of the change in cytosolic free calcium ([Ca2+]i) induced by sIg ligation; WAS cells showed either markedly diminished or no changes in [Ca2+]i levels whereas control EBV B cells consistently showed increases in [Ca2+]i. Anti-Ig-induced changes in inositol phosphate release were also markedly reduced in WAS compared with control cells. As protein tyrosine phosphorylation is thought to represent a proximal event in the activation of B cells, inducing increases in [Ca2+]i by virtue of tyrosine phosphorylation of phospholipase C (PLC)-gamma, profiles of protein tyrosine phosphorylation and expression of tyrosine-phosphorylated PLC-gamma 1 were compared between WAS and normal EBV B cells before and after sIg cross-linking. These studies revealed that in addition to defective mobilization of Ca2+, the WAS cells manifested little or no increase in tyrosine phosphorylation of PLC-gamma 1 or other intracellular proteins after sIg ligation. Together these results indicate the association of WAS with a defect in the coupling of sIg to signal transduction pathways considered prerequisite for B cell activation, likely at the level of tyrosine phosphorylation. The abnormalities observed in these early transmembrane signaling events in WAS EBV B cells may play a role not only in the nonresponsiveness of WAS patient B cells to certain T independent antigens, but also in the genesis of some of the other cellular deficits exhibited by these patients.

Ocular toxoplasmosis: the role of cellular immune defense in the development of recurrences. Results from animal experiments.

Five toxoplasmosis-free animals and ten animals that had been immunized intraocularly against toxoplasma organisms were used in a rabbit model of ocular toxoplasmosis. Five of the ten immune animals were immunosuppressed by cyclosporin A at a cellular level. Experimental toxoplasmic retinochoroiditis occurred in all animals, a process that in complete contradiction to experience reported previously. In the rabbit model, immunity after a past history of toxoplasma infection has thus far appeared to be a safe form of protection from experimental ocular toxoplasmosis. The concept of "immunological privilege of the eye" is discussed as the cause for the unexpected results.

Lack of effect of diacetyl-splenopentin (Berlopentin) on the pharmacokinetics of caffeine.

The influence of Berlopentin on caffeine clearance was measured after a 6 week i.v. treatment or a 4 week s.c. administration within a phase I trial. It was demonstrated that therapy with Berlopentin did not affect significantly caffeine elimination. Thus, clearance of drugs which are biotransformed by hepatic microsomal oxidation are unlikely to be affected by the coadministration of Berlopentin.

Diacetyl-splenopentin (BCH 069) did not affect the concentrations of various hormones in men.

We investigated the effect of BCH 069 (similar structure as thymopentin) on concentrations of hormones of the hypophysis, adrenals, gonads and thyroid glands in men after a therapy period of 4 or 6 weeks. No significant effects of BCH 069 three times weekly 50 mg s.c. for 4 weeks or i.v. for 6 weeks on serum concentrations of prolactin, LH, TSH, cortisol, testosterone, progesterone, estradiol and ETR were observed.

[Immunological status of patients with recurrent herpes simplex infections. In vitro effect of thymopoietin and splenin-derived and partially modified peptides on peripheral blood lymphocytes in comparison to a thymus extract].

Immunological studies on 20 patients with frequently recurrent Herpes simplex labialis were performed. Disorders of the cell-mediated immunity were often observed which do not only occur during the relapses. The number of T cells and their function were especially reduced. On the other hand there were no alterations of the CD4/CD8 ratio and the suppressor-activity. In vitro studies with peptides of thymopoietin and splenin justify the assumption that these peptides have therapeutic effects in recurrent herpes simplex infections.

Phase-I study of diacetyl-splenopentin (BCH 069).

BCH 069 is a new synthetic pentapeptide with thymic hormone-like activity. Two groups of patients received 50 mg BCH 069 or placebo by subcutaneous injection for 4 weeks 3 times weekly. The third group of patients received 50 mg BCH 069 by intravenous injection for 6 weeks 3 times weekly. The therapy was carried out during the peak of the pollen season. We observed the following clinical and biochemical activities of BCH 069: - flush symptom with a feeling of heat during intravenous administration of 50 mg/10 ml but not during 50 mg/20 ml, - decrease of triglyceride serum level, - decrease of SGOT- and SGPT serum levels, - increase of lipase serum level, - decrease of hemoglobin. - All changes of the biochemical parameters were not outside of the normal laboratory values. The disease-specific inflammatory changes were not seen in the intravenously treated group. The administration route is very important for the clinical side effect but not for drug induced biochemical changes. It can be concluded that BCH 069 is a well tolerated drug.

[Production and characterization of a high affinity monoclonal antibody with digoxin and digitoxin specificity].

A monoclonal antibody with a high affinity for digoxin (KA = 5.5 x 10(10) M-1) and digitoxin KA = 5.0 x 10(10) M-1) was produced by somatic cell fusion. This antibody, designated 2A3, displayed little cross reactivity with other glucosides and no cross reactivity with endogenous steroids. It was shown that 2A3 is a suitable tool in an enzyme immunoassay for digoxin and digitoxin.

[The lymphocyte transformation test using mononuclear cells of elderly humans as a suitable in vitro method for testing of structure activity relationships of splenin peptides].

More and more new immunostimulatory substances are developed. Besides old and new drugs have to be examined with regard to their side effects. Hence it follows demands for effective test possibilities. The lymphocyte transformation test with mononuclear cells of old men is a suitable in vitro method for testing of substances with thymic hormone-like activity in order to investigate the structure-effect-relationships. Thymopentin, human splenopentin and human diacetyl-splenopentin increase the phytohaemagglutinin induced lymphocyte transformation, on the other hand in the case of bovine splenopentin and human splenotritin this effect was not observed. By means of a patient with hypogammaglobulinaemia is demonstrated that the lymphocyte transformation test could also be a suitable method for the estimation of pharmacodynamics such peptides after in vivo treatment.

[Age-dependent sensitivity of human lymphocytes to the immunomodulating effect of bovine and human diacetyl splenopentin].

Both newborns and elderly adults suffer from physiological immunodeficiency. The molecular mechanisms responsible for this immunodeficiency are currently investigated by many laboratories. The aim of our investigations was to answer the question wether these immunodeficiencies could be influenced by bovine and/or human diacetyl-splenopentin, two newly developed immunostimulatory peptides. The in vitro effects of these peptides were studied using the lymphocyte transformation test and the detection of the immunoglobulin production (IgG, IgM) by lymphocytes. Thymopentin was used as standard for these investigations. The age dependence of lymphocyte sensitivity was estimated using cells of the following groups of blood donors: newborns (cord blood); young donors (20-30 yr); old donors (over 70 yr). All peptides were shown to have the same effects. The stimulated lymphocyte proliferation (PHA, anti-CD3) was inhibited in young donors and further increased in old donors. There was no influence in the case of newborns. The biological activity of human diacetyl-splenopentin was shown to be higher in comparison with bovine diacetyl-splenopentin.

[Disease-specific and non-specific seasonal parameter changes in hayfever patients].

10 patients have been treated by subcutaneous injections of placebo three times weekly for 4 weeks within a phase-I trial of BCH-069. Immunological, hematological and biochemical parameters were observed at different times: preseasonal, seasonal and postseasonal. --Some new observations are reported, which are induced by the seasonal inflammation. For example there was a strong increase of C-reactive protein and also an increase in the triglyceride level. --The parameters could be useful for trials of new antiallergic drugs in hay fever in the future.

[Determination of caffeine and metamizole elimination in men and women with and without hormonal contraceptives as an in vivo method for characterization of various cytochrome P-450 subspecies].

Caffeine is mainly metabolized by 3-methylcholanthrene-inducible cytochrome P-450 (P-450MC) and noramidopyrine-methanesulfonate sodium (metamizol, Analgin) is mainly metabolized by phenobarbital-inducible cytochrome P-450 (P-450PB). We investigated the elimination of caffeine by the use of plasma concentration curves (HPLC) and the elimination of metamizol by spectrophotometric determination of the metabolites in urine in 10 healthy young males, in 10 healthy young females using no OL-steroids and in 10 healthy young females using OC-steroids. No influence of sex on the microsomal drug metabolism activity of these two drugs has been observed. There was a significantly decreased microsomal drug metabolism of both drugs in females under hormonal contraception. We conclude that OC-steroids decrease the demethylation activity of both P-450MC and P-450PB.

[Determination of caffeine and metamizole elimination in pregnancy and after delivery as an in vivo method for characterization of various cytochrome p-450 dependent biotransformation reactions].

The elimination of caffeine from plasma and the elimination of the main metabolites of metamizol in urine were measured in 20 pregnant women, 19 puerperants, and 10 young healthy female non-pregnant volunteers. It was found a significant prolongation of caffeine elimination in pregnancy (t 1/2 = 9.4 +/- 0.6 h) with a correlation between duration of pregnancy and elimination half-life of caffeine (v = 0.72). The amount of the main metabolites of metamizol eliminated 0-9 h after application of the drug decreased from 82.6 +/- 8.2 mg in nonpregnant volunteers to 45.6 +/- 5.7 mg in pregnant women. Whereas caffeine elimination is restored within the first week after delivery, the rate of metamizol elimination does not reach the value of the non-pregnant volunteers in this period.

[Elimination of caffeine and metamizol in in vivo characterization of cytochrome P-450 dependent biotransformation reactions in aged humans].

Elimination of caffeine from plasma and excretion of main metabolites of metamizol (noramidopyrine methanesulphonate sodium) in urine were determined in young healthy (age 18-27 years) and in old aged volunteers (older than 65 years). From the elimination velocity of these model substances conclusions concerning the activity of 3-methylcholanthrene inducible (caffeine elimination) and of phenobarbital inducible (metamizol elimination) isoenzymes of cytochrome P-450 are drawn. Whereas in old age caffeine metabolism is unchanged, there is a strong delay in renal excretion of main metabolites of metamizol in old volunteers. It is concluded that different hepatic cytochrome P-450 isoenzymes are differentially influenced with increasing age in men.

[The influence of chronic renal insufficiency on the demethylating activities of various cytochrome P-450 subspecies in humans].

Caffeine is mainly metabolized by 3-methylcholanthrene-inducible cytochrome P-450 (P-450MC) and metamizol (noramidopyrine methanesulfonate sodium) is mainly metabolized by phenobarbital-inducible cytochrome P-450 (P-450PB). That's why the half life of caffeine and the elimination of the main metabolites of metamizol were used as parameters in vivo characterizing both groups of the cytochrome P-450-complex. The influence of chronic renal insufficiency was investigated (8 patients in the stage of compensated retention, creatinine in serum 380-1280 mumol X l-1, and 6 hemodialyzed patients, here only metamizol elimination was established). Both the caffeine and the metamizol eliminations of patients with chronic renal insufficiency are not different from those of healthy volunteers. We concluded that the demethylation activity of the cytochrome P-450-complex is not distinctly influenced by chronic renal insufficiency.

Functional antagonism by a monoclonal antibody to digoxin in a test system of cultured rat heart myocytes.

A monoclonal antibody with a high affinity for digitoxin (KA = 0.50 nM) and digoxin (KA = 0.55 nM) was produced by somatic cell fusion. This antibody, designated 2A3(47), displayed little cross reactivity with other glycosides. In cultured rat heart myocytes, 2A3(47), antagonized the positive chronotropic effect exerted by digitoxin but did not alter that of ouabain. Our results suggest that this monoclonal antibody may prove to be useful in treating digoxin and digitoxin intoxication.
