Bellini duct carcinoma of the kidney--a case report.

Bellini duct carcinoma (BDC), originating from the principal cells of the collecting duct of the kidney, accounts for only 1% of renal carcinomas. This lesion is a histogenetically, morphologically and cytogenetically defined entity. We report an eosinophilic variant of BDC. A 71-year old male presented with a tumor in the right kidney. Nephrectomy was performed. BDC (stage pT3a N0 M0 G2, based on UICC classification) was diagnosed. Electron microscopy revealed an abundance of mitochondria. Intranuclear rodlets were seen. Neuroendocrine differentiation was not demonstrated. After a three-year observation period, there are no signs of recurrence or metastases.

Diagnostic and prognostic relevance of malignancy-associated changes in cervical smears.

OBJECTIVE: For approximately 15 years, malignancy-associated changes (MACs) have been consistently found by means of high-resolution cytometry in different tissues, especially in visually normal appearing cervical cells. Their biologic nature is not yet fully understood. The aim of this investigation was to assess the expression of MACs in cervical smears and to evaluate their prognostic relevance. STUDY DESIGN: This study was performed on normal intermediate cells obtained from 53 cytologically positive and 78 cytologically negative cervical smears. From a second sample, 31 cases showing negative cytology were selected for a prospective longitudinal study. Densitometric and texture features were generated, and MACs were described on the basis of multivariate discriminant analysis. RESULTS: Discrimination between positive and negative cases was possible, with a correct classification rate of approximately 80%. After a mean period of 29.5 months, we noted no statistically significant increase in the incidence of cervical intraepithelial neoplasia in the group of healthy but MAC-positive women as compared to those who were MAC negative. CONCLUSION: MACs were constantly expressed in the epithelium of the cervix. Although their prognostic relevance remains unclear, MACs play an important role in the effort to automate cervical cytology.

Expression of the adhesion molecules ICAM, VCAM, and ELAM in the arteriosclerotic plaque.

Subject of investigation was the demonstration of the different expression of ICAM-1, and VCAM in 13 complete cross sections of the Aorta abdominalis and the Arteria iliaca immediately available post operationem during the process of arteriosclerosis. With increasing severity of the arteriosclerosis, ELAM showed a decreasing expression of the endothelium of the main vessel and a moderate one of the cells in the adventitia. On the endothelium of the main vessel and the vasa vasorum, we detected ELAM-1, showing a decreasing tendency when the arteriosclerosis was aggravating. In case of severe arteriosclerosis, a focal expression was also found on cells in the intima and media. In the adventitia, the expression was relatively constant during the process of arteriosclerosis. The more serious the disease, the more provable was VCAM on endothelial cells. In cases of severe arteriosclerosis, we also detected a focal staining in deeper layers of the wall. This study supports the concept of the active role of endothelial cells in the recruitment of leucocytes during the early steps of the arteriosclerotic process. In this process, with the disease aggravating, the endothelium seems to be less significant than the smooth muscle cells, which obviously play an important role in the course of arteriosclerosis.

Intratumoral application of standardized mistletoe extracts down regulates tumor weight via decreased cell proliferation, increased apoptosis and necrosis in a murine model.

The cytotoxic in vitro activity of standardized mistletoe extracts (ME) was examined by established assays towards the human ductal breast carcinoma cell line BT474. A dose-dependent (optimum 25 mg/mL medium) and significantly (p < 0.05) enhanced cytotoxic activity towards the BT474 cells was demonstrated. In vivo experiments on the antitumor activity of ME-A and ME-M were performed in a BALB/c-mouse / BT474 ductal breast carcinoma model. ME-A and ME-M were intratumorally administered according to an application schedule which was found to be optimal concerning dosage and time of administration. Standardized intratumoral application of ME-A and ME-M induced a significantly (p < 0.05) decreased tumor weight in experimental mice. Histological investigations were performed comprising analysis of mitosis and proliferation rates (Ki67 expression), as well as necrosis and apoptosis induction (ssDNA detection). As compared to tumors of control mice with intratumoral phosphate-buffered saline (PBS) injections, tumors of the ME-A and ME-M treated groups showed a decreased cell proliferation rate, as well as an increased cell necrosis and apoptosis rate. Standardized mistletoe extracts, interfering with defined tumor cell functions, e.g., proliferation, necrosis and apoptosis, may have an impact on local cancer treatment.
